Nonsurgical Repositioning of the Premaxilla Using a "Lasso" Elastic Band Technique in Conjunction With Presurgical Lip, Alveolus, and Nose Approximation (PLANA).

This case report presents a patient with bilateral complete cleft lip and palate with a severely deviated and rotated premaxilla. The infant was treated with a combination of lip taping and presurgical lip, alveolus, and nose approximation (PLANA) therapy for 3 months before primary lip and nose repair. Derotation and centering of the premaxilla was accomplished using a "Lasso" elastic band system, facilitating PLANA therapy. This treatment protocol promoted an esthetic lip and nose repair, while minimizing soft tissue tension and avoiding a surgical premaxillary setback.

'This is the Test of Our Faith': An Exploratory Study of the Religio-spiritual and Cultural Construction of Cleft Lip and Palate Among Muslim Parents in Punjab, Pakistan.

One of the most common congenital defects affecting the orofacial areas worldwide is cleft lip and/or cleft palate (CL/P), which causes a range of social, psychological, and medical challenges for affected children and their families. Several socio-cultural beliefs and superstitions are attached to this condition, but little is known about how CL/P is framed by Muslim parents. The religious, spiritual, and cultural construction of CL/P is a significant yet often overlooked aspect of the subject covered in this study. It also discusses how parents of children with CL/P use spirituality and reli

The role of Rho GTPases in facial morphogenesis.

Facial morphogenesis relies on the coordinated regulation of cellular behaviors that sculpt the face from simple facial prominences to the fully confluent lip and nose. Disruptions to migration of cranial neural crest cells and/or the directed growth of the prominences will lead to abnormalities such as orofacial clefts. Here we review what is known about the roles of the actin cytoskeleton and its key regulators, the small RHO GTPases, during neural crest cell migration and facial development. Although small RHO GTPase signaling has been studied in the context of cancer and vascular pathologi

Long-Term Osteogenic Outcomes of Deproteinized Bovine Bone in Alveolar Cleft Reconstruction: A 7-Year Histopathological and Radiographic Comparative Analysis.

Various bone substitutes have been used in alveolar cleft repair, with the advantages of reduced donor-site morbidity and reported desirable bone formation. In the majority of these studies, radiographs were considered to be an effective means of assessing the bone repair rate. However, in the present case report of a 27-year-old man who underwent alveolar cleft repair with deproteinized bovine bone particles (Bio-Oss®) for 7 years, inconsistencies in osteogenic findings were observed between histopathology and radiography. This case suggests that radiographic assessment may overestimate the e

Multisystem Comorbidities Associated With Orofacial Dysfunction in the Appalachian Region: A Retrospective Analysis.

BACKGROUND: Cleft lip and/or palate are among the most common congenital anomalies, often accompanied by systemic comorbidities. In rural regions such as Appalachia, barriers to healthcare access may delay comprehensive diagnosis and multidisciplinary care. METHODS: A retrospective review of 83 patients with cleft anomalies treated between 2017 and 2021 at a rural academic hospital in West Virginia was conducted. Demographics, cleft type, functional impairments, comorbidities, and syndromic associations were analyzed. χ2 tests were used to assess associations. RESULTS: Forty-one out of 83 pa

Genetic Associations with Non-Syndromic Cleft Lip/Palate and Dental Caries in Kuwaiti Patients: A Case-Control Study.

Background: Non-syndromic cleft lip/palate (NCL/P) is a prevalent congenital anomaly. Despite an unclear epidemiological link between orofacial clefts and dental caries, genetic studies suggest that polymorphisms in taste receptor genes may influence caries risk. Objectives: This study had two primary objectives: (1) to compare SNPs in NCL/P-associated genes (IRF6, FOXE1) between Kuwaiti NCL/P cases and controls, and (2) to explore whether variants in caries-associated (KLK4, DSPP) and taste receptor (TAS1R2, TAS2R38) genes are associated with dental caries susceptibility in individuals with N

Prophylactic Antibiotic Therapy in Cleft Surgery-A Scoping Review.

Background/Objectives: Cleft lip and/or palate are common craniofacial anomalies whose surgical repair is classified as clean-contaminated and may be complicated by surgical site infection or palatal fistula. Despite widespread perioperative antibiotic use, there are no standardized, evidence-based recommendations, and rising antimicrobial resistance underlines the need for rational prescribing. This systematic scoping review aimed to map current evidence on prophylactic antibiotic therapy and related perioperative measures in cleft surgery. Methods: A scoping review was conducted using the Ar

Reliability of Auditory-Perceptual Analysis in the Study of Speech Function in Patients with Unilateral Cleft and Palate.

Background/Objectives: Multidisciplinary outcome studies are carried out to evaluate long-term treatment in patients with cleft lip and palate. Speech function as one of the key outcomes of the treatment is examined by means of an auditory-perceptual analysis. For scientific and global studies it is essential to reduce the risk of bias as much as possible. The aim of the present study was the examination of auditory-perceptive analyses on the basis of an outcome study. Reliability was evaluated. Methods: Twenty patients were examined to evaluate their speech function. The speech sample was obt

Virtual Surgical Planning (VSP) in Orthognathic Surgery for Non-Syndromic Cleft Patients: A Scoping Review of Trends and Clinical Outcomes.

Background/Objectives: Isolated cleft lips and/or palates often require orthognathic treatment. Traditional planning based on 2D images and plaster models limits precision; therefore, virtual surgical planning (VSP) and Computer-Aided Design and Computer-Aided Manufacturing (CAD/CAM) technologies are increasingly being used. The aim of this scoping review was to analyze the techniques, outcomes, and gaps in research on VSP in orthognathics for patients with isolated (non-syndromic) clefts. Methods: Searches were conducted in July 2025 in seven databases (including PubMed, Scopus, and Cochrane)

BACKGROUND: Botulinum Toxin Type A (BoNT/A) injection for glabellar lines is popular due to its minimally invasive nature, safety, and efficacy. However, injection techniques and factors influencing outcomes need further study. This study compares three-point and five-point injection techniques and constructs a predictive model based on five-point injection results. METHODS: 444 patients were divided into three-point (219) and five-point (225) injection groups. Wrinkle improvement was assessed using the Facial Wrinkle Scale (FWS) and Patient-Rated Facial Wrinkle Severity Scale (PFWS), and com

A Technique for Repositioning the Posteriorly Displaced Premaxilla Following Prior Repair of Complete Bilateral Cleft Lip.

It is well known that osteotomy of the premaxilla is an effective surgical procedure for the correction of a displaced premaxilla in patients with bilateral cleft lip and palate. In cases with a posteriorly displaced premaxilla, it is not easy to move the premaxilla forward because of scarring of the palatal mucosal attachment, narrowing of the adjacent maxillary segments, and the stable fixation of this bone segment after its movement. This fixation is also important in cases without secondary bone grafting. We propose a new method that combines osteotomy and a method such as bone distraction

Notch signaling in the embryonic ectoderm promotes periderm cell fate and represses mineralization of vibrissa hair follicles.

The Notch signaling pathway is a critical means to regulate cell fate choice in animals. Appropriate regulation of this pathway is also required for human face formation as both loss and gain of function mutations of Notch signaling can cause syndromes with craniofacial abnormalities. Here we examine the consequences of manipulation of Notch signaling in the early mouse embryonic ectoderm by either removing the transcriptional effector Rbpj or expressing a constitutively active form of the Notch1 intracellular domain. Loss of Rbpj resulted in cleft secondary palate but strikingly was also asso
