Secondary Unilateral Cleft Lip Repair Using a Novel Neurovascular Protection Method: A Three-Dimensional Evaluation.

OBJECTIVES: We proposed a novel neurovascular protection technique based on the intramuscular neurovascular partitions of the upper lip in nasolabial muscle biomechanical bionic surgery for secondary unilateral cleft lip repair. The aim of this study was to describe this modification and evaluate the short-term and long-term outcomes compared with the results of traditional repair. METHODS: A total of 44 patients with secondary unilateral cleft lips were included in this study, and were divided into 2 groups: 22 patients treated with the traditional repair, and the other 22 treated with the m

Risk Factors for Infection, Dehiscence, and Unplanned Reintubation Following Cleft Lip Repair.

ObjectiveWe sought to characterize factors associated with postoperative complications in patients undergoing primary cleft lip repair (CLR).DesignThe National Surgical Quality Improvement Program-Pediatric database was queried to identify patients under 12 months old undergoing CLR using CPT codes. Multivariable analysis was conducted to characterize complications and associated comorbidities.SettingA multicenter sample.Patients/ParticipantsA total of 11 957 cases (81.9% unilateral CLR and 18.1% bilateral CLR) were identified.InterventionsNone.Main Outcomes/MeasuresThe postoperative outcomes

Congenital double lower lip in a newborn with feeding difficulties.

We describe the case of a full-term male newborn admitted on the first day of life due to feeding difficulties and hypoglycemia, who presented with a protruding bifid mass along the mucosa of the lower lip that interfered with breastfeeding. Double lip is a rare congenital condition caused by hyperplasia of the lip mucosa, most commonly affecting the upper lip. Surgical removal is generally recommended if the condition interferes with speech, sucking, or, more commonly, for cosmetic reasons. In this case treatment was conservative, resolving feeding difficulties without the need for surgery, w

Super-Passive Alveolar Correcting Equipment (SPACE): a novel presurgical cleft device with 5-year outcomes following one-stage repair.

In patients with unilateral cleft lip, alveolus, and palate (U-CLAP), wide clefts negatively affect facial aesthetics, alveolar alignment, and speech. Although various presurgical orthopedic devices have been used, none reliably achieve substantial cleft narrowing. This study introduces the Super-Passive Alveolar Correcting Equipment (SPACE), a molar-supported passive plate that significantly reduces cleft width, narrowing the mean alveolar gap to 1.87 mm before surgery (n = 83). SPACE enabled a one-stage repair - lip repair, gingivoperiosteoplasty, and palatoplasty - performed at approximatel

Clinical effectiveness of a patient-tailored mouthpiece based on 3-dimensional (3D) scanner modeling and 3D printing technology for microstomia caused by burns: A pilot study.

BACKGROUND: Microstomia is defined as a small mouth size that interferes with daily activities. No protocol has been established for microstomia caused by facial burns. This study aimed to confirm the clinical usefulness of a patient-customized mouthpiece using 3D scanner modeling and printing technology. MATERIALS AND METHODS: Each participant's mouth was scanned using a 3D scanner and the scanned model was imported into modeling software. After modeling the mouthpiece to fit the maximum horizontal and vertical lengths of the mouth and the thickness of the lips, it was manufactured using a 3

Harlequin Ichthyosis in a Preterm Neonate: A Rare Case Report.

A condition known as harlequin ichthyosis appears rarely, marked by intense abnormalities in skin development due to inherited changes in the ABCA12 gene. This leads to major issues with the outer layer of skin, forming hard, plate-like coverings split by wide cracks. Features include turned-out eyelids, lips pulled back, and stiff joints in arms and legs; survival rates at birth remain low because of complications. A baby boy born before full term showed clear signs matching this diagnosis immediately after delivery. Earlier scans during pregnancy, specifically in the last 3 months, revealed

The orofacial cleft risk gene IRF6 is a target gene of SOX9 in cranial neural crest cells.

Orofacial clefts represent the most prevalent congenital anomalies affecting the craniofacial region. They can be evoked by a disturbed development of either oral epithelium or cranial neural crest-derived mesenchyme, or by disruptions of their interplay. IRF6 is a well-known risk gene associated with non-syndromic orofacial clefts and causative for Van der Woude syndrome, an autosomal dominant syndrome characterized by orofacial clefting and lower lip pits. Its expression in the oral epithelium depends on the transcription factor TFAP2A. We here show by immunofluorescence on mouse embryonic s

Partial craniofacial duplication in a resource-limited setting: a case report.

INTRODUCTION AND IMPORTANCE: Diprosopus, or craniofacial duplication, is an extremely rare congenital malformation, representing only 0.4% of conjoined twins with an estimated prevalence of 1 in 15 million births. Fewer than 40 cases have been reported worldwide. We describe a case of partial craniofacial duplication in a child from rural Ethiopia, highlighting the challenges of delayed diagnosis and management in a resource-limited setting. CASE PRESENTATION: A 7-year-old girl presented with a congenital mandibular mass that had remained untreated since birth due to a lack of antenatal care

Cleft Lip and Palate is Common in PORCN-Related Focal Dermal Hypoplasia in Asians: Three New Case Reports and Literature Review.

Focal dermal hypoplasia is an X-linked dominant genetic disorder predominantly affecting females, caused by loss-of-function variants in the PORCN gene, characterized by ectodermal, skeletal, craniofacial and ocular structural abnormalities. We report three cases with de novo novel variants, and performed a comprehensive review of the clinical features of this disorder focusing on Asian patients, and identified significant differences compared with previous literature focusing on Caucasian patients. Cleft lip and palate is a frequently reported feature in Asian patients, but not in Caucasians.

Prenatal Fentanyl Exposure Association With Characteristic Neonatal Anomalies.

OBJECTIVES: To investigate if previously described anomalies associated with fentanyl could be identified in our cohort of individuals with active fentanyl use in pregnancy. METHODS: Potential cases of a novel syndrome were identified from a prospectively collected database of obstetric patients with substance use disorder enrolled in a multispecialty treatment program from 2014 to 2024 in this case series study. Suspected Fetal Fentanyl Syndrome (FFS) was defined as small head circumference (<10%ile) and at least one other sign: cleft palate, clubfoot, rocker bottom feet, toe syndactyly, sin

Clinical and genetic basis of congenital gonadotropin deficiency.

STUDY QUESTION: What is the clinical and genetic overlap across subtypes of congenital gonadotropin (Gn) deficiency? SUMMARY ANSWER: This study reveals substantial clinical and genetic overlap among Gn deficiency disorders, with shared genetic and developmental features across congenital hypogonadotropic hypogonadism (CHH), combined pituitary hormone deficiency (CPHD), and syndromic forms of Gn deficiency. WHAT IS KNOWN ALREADY: Congenital Gn deficiency includes a subset of hypogonadotropic hypogonadism (HH) and can result from defects at the level of the hypothalamus or the pituitary. It in
