[Multiple endocrine neoplasia type 2b. Clinical, diagnostic and therapeutic aspects].

Multiple Endocrine Neoplasia type 2b (MEN 2b) is a rare syndrome. The principal features are: medullary thyroid carcinoma (MTC), dysmorphism, a ganglioneuromatosis and pheochromocytomas. Eight cases of MEN 2b have been observed at the Institute Gustave Roussy, between 1968 and 1983. Seven involved children under 15 years of age. Eight had a bilateral MTC; six had dysmorphism; six had mucosal tongue neuromas. Six were troubled with visceral ganglioneuromatosis of whom two had intestinal obstruction and one urinary chronic retention. One patient had pheochromocytoma with hypertension. From this

The Golabi-Rosen syndrome--report of a second family.

Golabi and Rosen (1984) have reported on a new X-linked mental retardation/multiple congenital anomalies (XLMR/MCA) syndrome of pre- and postnatal overgrowth, characteristic "coarse" facial appearance with macrostomia, midline groove of tongue, lower alveolar ridge and lip, submucous cleft of palate, supernumerary nipples, intestinal anomalies, supernumerary pair of ribs, anomalies of sacrum and tailbone, hypoplastic index fingernails, postaxial polydactyly and other digital anomalies. This was an incompletely recessive trait with some manifestations evident in an obligatory carrier. Here we r

A "new" lethal multiple congenital anomaly syndrome: joint contractures, cerebellar hypoplasia, renal hypoplasia, urogenital anomalies, tongue cysts, shortness of limbs, eye abnormalities, defects of the heart, gallbladder agenesis, and ear malformations.

Three cases of a lethal malformation syndrome with severe visceral anomalies were seen in two families and include one pair of sibs. The predominating external manifestations are mesomelic dwarfism, micrognathia, V-shaped upper lip, microglossia, thick alveolar ridges, ambiguous genitalia, webbed neck, highly arched palate, clubfeet, fused fontanelles, inclusion cysts of the tongue, four-finger creases, digital anomalies, apparently low-set ears, widely spaced nipples, and dislocated thighs and forearms. The internal findings include oligopapillary renal hypoplasia, severe congenital heart def

A case of blue rubber bleb nevus syndrome with coronary artery fistula to left ventricle.

A 69-year-old female was admitted to the hospital because of tarry stool and dyspnea on exertion. Multiple hemangiomas were noticed on the tongue, right arm and abdomen. Gastrointestinal examination revealed multiple polypoid lesions in the stomach and small intestine. In addition, coronary artery-left ventricular fistula formation and pulmonary arteriovenous malformation were shown by angiography. Compared with other reported cases of blue rubber bleb nevus syndrome, the present case was unique because of the coronary artery-left ventricular fistula, heart failure and the familiar incidence o

Median cleft lip, polydactyly, syndactyly and toe anomalies in a non-Indian infant.

A case is reported of a child who presented with a median cleft of the lip and alveolus with polydactyly, a complex form of syndactyly and multiple toe anomalies. This is the fifth case to be reported with these features and the second case to be reported in a non-Indian patient. Our case is characterised by unique features, consisting of a complexity of hand anomalies in association with other facial anomalies such as median alveolar cleft, epicanthus, fusion of the tongue to the floor of the mouth and rare big toe anomalies.

Monoclonal antibodies to anchoring fibrils for the diagnosis of epidermolysis bullosa.

Murine monoclonal antibodies to human anchoring fibrils reacted with human and monkey cervix, tongue, esophagus, and vagina. Rat, mouse, and guinea pig tissues were negative. In 11 patients with dystrophic recessive epidermolysis bullosa there was no reaction by immunofluorescence and immunoelectron microscopy. Other forms of epidermolysis bullosa had normal reactivity.

Craniopagus parasiticus: a case report of a parasitic head protruding from the right side of the face.

An extremely rare case of parasitic head protruding from the right side of the face is presented. It differs from all the other cases previously reported in the literature. The patient had a parasitic head with an abnormal brain, two eyebrows, two underdeveloped eyes, nose, mouth, 12 teeth, a tongue and plenty of hair. The parasitic head was excised. The nature of the embryological abnormalities is discussed and the literature of polygnathism reviewed.

Severe congenital hypoglossia and micrognathia with other multiple birth defects.

Severe congenital hypoglossia with the anterior two-thirds of the tongue missing is a rare finding. This case report describes an infant 14 months of age who manifests this tongue anomaly along with other congenital malformations including an underdeveloped mandible, partial anodontia, enamel hypoplasia, situs inversus, dextrocardia, and asplenia. Only one other case similar to this case has been reported in the literature. There is no report of any case of severe congenital hypoglossia with asplenia. The etiology of this birth defect is only speculative at this time, but the mother of our pat

The distal arthrogryposes: delineation of new entities--review and nosologic discussion.

We report on 44 patients (18 with additional affected family members), with congenital distal limb contractures identified from a large study of over 350 patients with congenital joint contractures. Fourteen propositi (seven familial cases, seven isolated cases) had a newly recognized form of arthrogryposis, which we have designated distal arthrogryposis type 1, with the predominant manifestations of autosomal dominant inheritance; tightly clenched fists at birth, with medially overlapping fingers, ulnar deviation, and camptodactyly in adults; and positional foot deformities. Contractures at o

Hypoglossia: case report and discussion.

A case of hypoglossia was observed between the ages of 3 and 15 years in a patient with micrognathia, congenital absence of three lower incisors, and a telescopic occlusion accompanied by an extremely narrow lower arch and severe overbite. Orthodontic intervention was begun when the patient was 8 years old. Craniofacial growth changes that occurred before and after orthodontic treatment are discussed. Cephalometric study of this case suggests the distinctive features of the postnatal growth pattern similar to other cases with the syndrome.

Anatomy and embryology in cephalothoracopagus twins.

Anatomical features are described in a case of cephalothoracopagus female twins with laterally fused heads. There was a single foregut, shared equally by the two individuals. The tracheae, lungs, hearts, livers, and gallbladders were also shared by the two individuals. They were located in the anterior and posterior regions of the joined bodies. These peculiarities occur in other cases of cephalothoracopagus twinning reported in the literature, and may have been caused by spatial problems in the developing twin embryos. In particular, each heart is considered to be a composite organ, formed by

Facial paralysis in children: differential diagnosis.

The differential diagnosis in 170 patients between birth and 18 years of age is reviewed. There are a number of obvious physical findings and historical features that allow one to make a diagnosis rather quickly. Pain, vesicles, a red pinna, vertigo, and sensorineural hearing loss suggest herpes zoster oticus. Slow progression beyond three weeks, recurrent facial paralysis involving the same side, facial twitching, weakness, or no return of function after six months indicate a neoplasm. Bilateral simultaneous facial paralysis indicates a cause other than Bell's palsy, such as Guillain-Barré sy

Ankyloglossum superius syndrome.

The case of a male infant with the ankyloglossum superius syndrome is documented. In addition to oromandibular and limb malformations, the patient had extensive jejunal and ileal atresia. The cause of this syndrome remains unknown, but both genetic and intrauterine environmental factors have been suggested. There was no family history of other affected relatives, but throughout her pregnancy the mother had received imipramine, diazepam, chlorpromazine, and meclizine.

Embryonic development of the head and neck: part 4, organs of special sense.

A synopsis of the embryonic development of the eye, the ear, and the olfactory organ and taste buds is presented. The eye is discussed in terms of the sensory and nonsensory protions of the eyeball and of such associated structures as the extrinsic eye muscles, eyelids, conjuctiva, and lacrimal apparatus. The embryology of the internal, middle, and external ear is reviewed, as well as positional changes of both the eye and ear relative to other sturctures of the head and neck. Various congenital defects affecting the eye and ear are also discussed.

The human materno-foetal relationship in malaria. II. Histological, ultrastructural and immunopathological studies of the placenta.

Histological and ultrastructural studies of four placentae heavily infectd with Plasmodium falciparum revealed large intervillous accumulations of erythrocytes containing parasites together with monocytes which had ingested pigment. These appearances were associated with focal syncytial necrosis, loss of syncytial microvilli and proliferation of cytotrophoblastic cells. In addition, marked irregular thickening of trophoblastic basement membranes and protrusion of tongue-like projections of syncytiotrophoblast into the basement membrane were observed. In six other placentae which contained scan

Beckwith-Wiedemann's syndrome--a report of an autopsied case--.

The case presented here is a three-month-old male infant with the Beckwith-Wiedemann's syndrome. Clinically, the patient was characterized by macroglossia, visceromegaly, umbilical hernia, microcephalus and other multiple malformations such as facial flame nevus or ear lobe grooves. The patient died of bronchopneumonia at the age of three months, and an autopsy was performed. Morphological examination revealed adrenal cytomegaly, hyperplasia and hypertrophy of the pancreatic islets, adrenal rest tissue in the right testis or hypertrophy of muscle fibers of the tongue associated with fibrous de

Phenotypic variation in two patients with a ring chromosome 22.

Two severely mentally retarded patients with a ring chromosome 22 presented with disparate phenotypes: one patient manifested only minimal dysmorphic features, whereas the other had a distinctive pattern of anomalies consisting of an abnormal skull configuration with mild maxillary hypoplasia, a large nose, thick full lips, a protruding tongue, lymphedema, hypotonia and an unsteady gait. The findings in these and previously reported patients indicate that a ring chromosome 22 is usually associated with moderate to severe mental retardation, with a range of dysplastic features from mild and non

[Arterial thrombosis induced by direct electric current. Clinical applications (author's transl)].

Authors have demonstrated that it is possible to induce arterial thrombosis by applying direct electric current (10 mA -V). By the use of this method several thromboses were formed in the rabbit. This technique is now well established and the first clinical applications on human arteries give rise to considerable hopes, since it is simple, reliable, and much less dangerous than other types of embolisation. Experiments are being done to apply this technique to aneurysms.
