Secondary Unilateral Cleft Lip Repair Using a Novel Neurovascular Protection Method: A Three-Dimensional Evaluation.

OBJECTIVES: We proposed a novel neurovascular protection technique based on the intramuscular neurovascular partitions of the upper lip in nasolabial muscle biomechanical bionic surgery for secondary unilateral cleft lip repair. The aim of this study was to describe this modification and evaluate the short-term and long-term outcomes compared with the results of traditional repair. METHODS: A total of 44 patients with secondary unilateral cleft lips were included in this study, and were divided into 2 groups: 22 patients treated with the traditional repair, and the other 22 treated with the m

Pathologic myopia as a concurrent condition in Pierre Robin sequence: a case report and literature review.

INTRODUCTION: Pierre Robin sequence (PRS) is a rare congenital disorder often associated with multisystem abnormalities, yet cases combined with pathologic myopia and medial upper eyelid entropion with trichiasis in whom no pathogenic genetic variants are detectable are rare. CASE PRESENTATION: This article reports on a 4-year-old male child with syndromic PRS, whose parents verbally stated that no pathogenic variants were identified in previous genetic testing. Since birth, he has undergone multiple surgeries for micrognathia, cleft palate, and secretory otitis media. He presented for ophtha

Maternal Fish Intake in the Year Prior to Conception and Birth Defects, National Birth Defects Prevention Study, 1997-2011.

BACKGROUND: Epidemiologic data on the association between maternal fish intake and birth defects are sparse. Our objective was to assess associations between maternal fish intake and 52 different birth defects, most of which have not been assessed previously. METHODS: Using logistic regression, data was analyzed for 29,242 mothers of infants with birth defects and 10,973 mothers of control infants who participated in the National Birth Defects Prevention Study (NBDPS) and delivered between 1997 and 2011. We focused on associations between mothers who reported high fish intake of 2 or more ser

Craniofacial development: genetics, signaling pathways, teratogenic mechanisms, and clinal significance.

Craniofacial development is a highly coordinated and stage-specific process involving neural crest cell migration, proliferation, differentiation, and epithelial-mesenchymal transition. Genetic mutations, signaling pathway abnormalities, and exposure to environmental teratogens can disrupt normal development. These anomalies can lead to cleft palate, craniosynostosis, and other common congenital craniofacial malformations. Although significant progress has been made in understanding craniofacial development in recent years, the systematic integration of these factors remains limited. Here, we

First-trimester nonsteroidal anti-inflammatory drugs exposure and risk of major congenital malformations: A retrospective register-based cohort study.

BACKGROUND: Pain and fever are common in early pregnancy, yet their management poses a major clinical dilemma. Although not confirmed, recent studies have raised safety concerns regarding acetaminophen. Evidence on the use of nonsteroidal anti-inflammatory drugs (NSAID) in the first trimester remains inconclusive. This uncertainty has left clinicians with limited evidence to guide treatment decisions. This study evaluated the association between first-trimester NSAID exposure and the risk of major congenital malformations (MCMs) in a large, population-based cohort of pregnancies. METHODS AND

Differentiating the Clinical and Variant Spectrum of Hardikar Syndrome From Other MED12 -Related Developmental Disorders.

The rare X-linked female-restricted Hardikar syndrome (HDKR, OMIM # 301068) is characterized by multiple congenital anomalies including orofacial clefts, gastrointestinal, genitourinary, and cardiac anomalies, but cognitive and neurobehavioral development is rarely impaired. HDKR is caused by heterozygous frameshift, splice or nonsense variants in the MED12 gene. Besides HDKR, MED12 pathogenic variants cause a broad spectrum of developmental disorders, collectively referred to as MED12-related disorders, including Opitz-Kaveggia syndrome or FG syndrome type 1 (OKS, OMIM #305450), Lujan-Fryns s

Novel compound heterozygous FAM20C variants cause Raine syndrome - retrospective prenatal diagnosis and literature review.

BACKGROUND: Raine syndrome, RS, (OMIM 259775) is a rare autosomal recessive disorder with prevalence of less than 1:1 000 000, caused by homozygous or compound heterozygous variants in FAM20C gene. A retrospective genetic investigation was performed on DNA extracted from amniotic cell cultures previously used for cytogenetic studies. METHODS : Extracted DNA was used for aCGH (array comparative genomic hybridization) and Sanger sequencing. Parental blood samples were tested for karyotype (GTG – G-banding using trypsin and Giemsa) and molecular karyotype (aCGH). Additionally, paternal sample was

Characterization of a Familial Goldenhar Syndrome Case Using Whole-Exome Sequencing.

BACKGROUND: Goldenhar syndrome (oculo-auriculo-vertebral spectrum, OAVS) is a rare congenital disorder characterized by craniofacial malformations, systemic anomalies, and significant phenotypic variability. Although it is the second most common craniofacial malformation after a cleft palate, the genetic etiology of Goldenhar syndrome remains largely unexplored. This study aimed to identify genetic variants contributing to Goldenhar syndrome in a Lebanese family with three affected individuals, using whole-exome sequencing and complementary genomic approaches. METHODS: Whole-exome sequencing

Genetic variation near ROBO1 is associated with craniofacial microsomia and related phenotypes in the Finnish population.

Craniofacial microsomia (CFM) encompasses a phenotypic continuum of congenital anomalies ranging from isolated microtia to more complex manifestations within the oculo-auriculo-vertebral spectrum, including Goldenhar syndrome, reflecting abnormal development of first and second pharyngeal arch-derived structures. While several genetic susceptibility loci have been reported, population-based evidence in individuals of European ancestry remains limited. Using nationwide data from FinnGen in the Finnish founder population, we identified a genome-wide significant association at a conserved interge

Super-Passive Alveolar Correcting Equipment (SPACE): a novel presurgical cleft device with 5-year outcomes following one-stage repair.

In patients with unilateral cleft lip, alveolus, and palate (U-CLAP), wide clefts negatively affect facial aesthetics, alveolar alignment, and speech. Although various presurgical orthopedic devices have been used, none reliably achieve substantial cleft narrowing. This study introduces the Super-Passive Alveolar Correcting Equipment (SPACE), a molar-supported passive plate that significantly reduces cleft width, narrowing the mean alveolar gap to 1.87 mm before surgery (n = 83). SPACE enabled a one-stage repair - lip repair, gingivoperiosteoplasty, and palatoplasty - performed at approximatel

Simultaneous Orthognathic Surgery and Costal Bone Grafting in Hemifacial Microsomia: A Rare Case Report and Literature Review.

Hemifacial microsomia (HFM) is a congenital craniofacial anomaly frequently associated with mandibular hypoplasia. Achieving stable correction in severe cases remains challenging due to abnormal mandibular anatomy and deficient temporomandibular joint structures. We report a rare revision case of a 20-year-old female with type IIA HFM who developed progressive postoperative resorption following two-jaw surgery, resulting in a "floating" ramus-condyle segment and subsequent recurrent facial asymmetry. Revision surgery consisted of unilateral sagittal split ramus osteotomy on the unaffected side

OGT mediates O-GlcNAcylation of MEIS2 and affects palatal osteogenic development.

Post-translational modifications (PTMs) have been gradually elucidated in congenital malformations such as cleft palate. Among them, O-GlcNAcylation as a dynamic PTM of proteins regulates various critical biological processes including transcription, translation, and cell fate determination. In this study, a substantial decline in O-linked β-D-N-acetylglucosamine (O-GlcNAc) levels was detected within the palatine plates of all-trans retinoic acid (atRA)-induced cleft palate mice. The role of O-GlcNAc transferase (OGT), the sole enzyme responsible for catalyzing O-GlcNAcylation, was investigate

Clinical and zebrafish studies of truncating SF3B2-variants in craniofacial microsomia.

Craniofacial microsomia (CFM) exhibits significant phenotypic variability and degree of severity. While loss-of-function variants in SF3B2 have recently emerged as a genetic etiology, the molecular basis underlying this clinical heterogeneity remains poorly understood. Here, we report two probands harboring novel truncating SF3B2 variants and presenting with distinct clinical phenotypes. Proband 1, with a heterozygous p.(Gln60*) variant, exhibited characteristic CFM features, including mandibular hypoplasia, cleft palate, bilateral tragal abnormalities, microtia, external auditory canal stenos
