BACKGROUND: Apert syndrome is a rare congenital malformation caused by a mutation in the fibroblast growth factor receptor 2 (FGFR2) gene. Characteristic imaging findings include bilateral premature closure of the coronal suture, midfacial hypoplasia, and symmetrical syndactyly of the fingers and toes. These features may be accompanied by lateral ventricle dilation, absence of the corpus callosum, cervical vertebral fusion, and other anomalies. However, in the second trimester, syndactyly and extracranial anomalies may be the only detectable signs, which complicate early diagnosis. The purpose

Orofacial clefts represent the most prevalent congenital anomalies affecting the craniofacial region. They can be evoked by a disturbed development of either oral epithelium or cranial neural crest-derived mesenchyme, or by disruptions of their interplay. IRF6 is a well-known risk gene associated with non-syndromic orofacial clefts and causative for Van der Woude syndrome, an autosomal dominant syndrome characterized by orofacial clefting and lower lip pits. Its expression in the oral epithelium depends on the transcription factor TFAP2A. We here show by immunofluorescence on mouse embryonic s

Focal dermal hypoplasia is an X-linked dominant genetic disorder predominantly affecting females, caused by loss-of-function variants in the PORCN gene, characterized by ectodermal, skeletal, craniofacial and ocular structural abnormalities. We report three cases with de novo novel variants, and performed a comprehensive review of the clinical features of this disorder focusing on Asian patients, and identified significant differences compared with previous literature focusing on Caucasian patients. Cleft lip and palate is a frequently reported feature in Asian patients, but not in Caucasians.

OBJECTIVES: To investigate if previously described anomalies associated with fentanyl could be identified in our cohort of individuals with active fentanyl use in pregnancy.
METHODS: Potential cases of a novel syndrome were identified from a prospectively collected database of obstetric patients with substance use disorder enrolled in a multispecialty treatment program from 2014 to 2024 in this case series study. Suspected Fetal Fentanyl Syndrome (FFS) was defined as small head circumference (<10%ile) and at least one other sign: cleft palate, clubfoot, rocker bottom feet, toe syndactyly, sin

STUDY QUESTION: What is the clinical and genetic overlap across subtypes of congenital gonadotropin (Gn) deficiency?
SUMMARY ANSWER: This study reveals substantial clinical and genetic overlap among Gn deficiency disorders, with shared genetic and developmental features across congenital hypogonadotropic hypogonadism (CHH), combined pituitary hormone deficiency (CPHD), and syndromic forms of Gn deficiency.
WHAT IS KNOWN ALREADY: Congenital Gn deficiency includes a subset of hypogonadotropic hypogonadism (HH) and can result from defects at the level of the hypothalamus or the pituitary. It in

BACKGROUND: Presurgical nasoalveolar molding (NAM) has become an integral component of comprehensive cleft lip and palate (CLP) care. The aim of the present systematic review and meta-analysis is to assess the efficacy of digitally versus conventionally fabricated NAM appliances for presurgical approximation of complete CLP.
METHODS: The focused research question is "In patients with CLP, does the use of digitally-fabricated NAM appliances yield superior presurgical cleft approximation outcomes than conventionally-fabricated NAM appliances?" Literature search was conducted in accordance with

Cleft lip and palate (CLP) are congenital malformations of the craniofacial complex that disrupt the structure and function of the stomatognathic system, particularly articulation and phonation. These anatomical abnormalities affect pressure consonants and resonance balance, often resulting in impaired phoneme production and varying degrees of nasality. Understanding how CLP affects speech is essential for guiding rehabilitation and designing targeted early interventions. This systematic review aimed to identify the most frequently affected speech phonemes and determine the degree of nasality

Fistulas are the most consequential complication of cleft palate repair and have compounding effects on multiple functional domains. Successful reconstruction is critical to resuming the normal pathway of care. The nature of a fistula can be understood considering the multiple axes that define its presentation. Closure involves addressing nasal lining, oral lining, and velar muscle function and options can be organized according to a "reconstructive stepladder". Ultimately, the selection of a strategy considers the fistula, patient, and surgeon/resources; with the goal of robust closure that o

Ankyloblepharon-ectodermal dysplasia-clefting (AEC) lip/palate (Hay-Wells) syndrome is a rare ectodermal dysplasia characterized by ankyloblepharon, ectodermal abnormalities, and orofacial clefting. Congenital alveolar synechia is an extremely rare anomaly that may cause severe feeding and airway difficulties in infants and is most often associated with cleft lip and/or palate. This report describes 2 rare cases of AEC syndrome associated with bilateral congenital alveolar synechia. Both patients presented with markedly restricted mouth opening and significant feeding difficulties. Early surgi
