Incidental Right Aortic Arch in a 55-Year-Old Male With Cerebral Palsy: A Radiologic Pitfall Mimicking Pulmonary Pathology.

We present the case of a 55-year-old male with a past medical history of cerebral palsy, obstructive sleep apnea (OSA), seizures, and generalized anxiety disorder who presented with recurrent episodes of respiratory distress and was incidentally found to have a right aortic arch (RAA) on radiologic imaging. Although this anatomical variation is rare, this case serves as a teaching point to raise awareness of the existence of a RAA and how it can be mistaken for a lung lesion, mass, or malignancy. Studies have shown that this anatomical abnormality can also give rise to subtle esophageal and re

Case Report: Adult liver transplantation using a long-standing mesocaval shunt as portal inflow in congenital absence of the portal vein with atypical variceal bleeding and refractory shunt encephalopathy.

Congenital absence of the portal vein (CAPV) is an exceedingly rare vascular malformation in which mesenteric and splenic venous blood bypasses the liver and drains into the systemic circulation. Because congenital portosystemic drainage usually decompresses the splanchnic venous bed, classical portal hypertension is uncommon, and CAPV typically presents with metabolic and vascular complications rather than variceal bleeding. We report the case of a 36-year-old woman with CAPV that presented atypically in early childhood with esophageal variceal hemorrhage, suggesting functional inadequacy or

Antimicrobial resistance and vaccine-preventable infections: concurrent health security challenges after the double earthquake in Venezuela.

SCIENTIFIC BACKGROUND: Meningiomas are the most common benign intracranial neoplasms with a slow growth presented as the intracranial lesion. These tumors are without any symptoms for a long time. At the time of diagnosis it is frequently an asymptomatic tumor. In that case the therapist may well suggest a wait-and-see strategy. The therapy of meningiomas focuses firstly on the microsurgical treatment. Volume reduction can be achieved immediately after treatment. Stereotactic radiosurgery is an important non-invasive treatment option for recurrent tumors or meningiomas with partial resection.

Lyell´s syndrome associated with the use of topical agents: case report.

UNLABELLED: Pseudomonas aeruginosa AG1 (PaeAG1) is a Costa Rican strain that was isolated in 2010 in a major Hospital. This strain has resistance to multiple antibiotics such as β-lactams (including carbapenems), aminoglycosides, and fluoroquinolones. PaeAG1 is considered critical (Priority 1) due to its resistance to carbapenems, and it was the first report of a P. aeruginosa isolate carrying both VIM-2 and IMP-18 genes encoding for metallo-β-lactamases (MBL) enzymes (both with carbapenemase activity). Owing to these traits, we have studied this model for 10 years using diverse approaches inc

Iturin A disrupts Cryptococcus neoformans membrane integrity, attenuates virulence, and synergizes with amphotericin B.

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or

Continuous clamping facilitates distal fistula dissection in thoracoscopic repair of type C esophageal atresia: a novel surgical technique.

PURPOSE: To determine whether continuous clamping improves distal fistula dissection in thoracoscopic type C esophageal atresia repair. METHODS: Between January 2019 and May 2024, 31 patients with type C esophageal atresia underwent thoracoscopic repair. Patients were randomly assigned to receive either conventional surgery (control group, n = 16) or surgery with continuous clamping during distal fistula dissection (study group, n = 15). This study compared intraoperative parameters (fistula dissection time, total surgical duration, etc.) and postoperative outcomes (anastomotic leak, anastomo

Outcome of Esophageal Atresia: What Choice of Suture Material?

BACKGROUND: Esophageal atresia (EA) is a congenital esophageal malformation frequently associated with a tracheoesophageal fistula (TEF). The treatment relies on surgical intervention, but postoperative complications such as anastomotic stricture, leakage, and recurrent fistula can impact recovery and long-term outcomes. Suture material may influence healing and fibrosis, but its role in EA repair outcomes remains unclear. The aim of the present study was to assess any relationship between use of absorbable or not absorbable suture and further complications. MATERIALS AND METHODS: We conducte

Thoracoscopic management of recurrent tracheoesophageal fistula: a single-center outcome analysis.

BACKGROUND: Although thoracoscopic treatment of congenital esophageal atresia is widely accepted among pediatric surgeons, its application in recurrent tracheoesophageal fistula (RTEF) remains rare because of its technical difficulty. This study retrospectively analyzed thoracoscopic RTEF repairs at a single center to evaluate the safety, feasibility, and outcomes of this approach, providing a clinical reference. METHODS: We retrospectively reviewed 22 patients with RTEF who underwent thoracoscopic repair at the Capital Center for Children's Health between October 2020 and July 2024. All patie

Pediatric benign esophageal strictures: current understanding from etiology to treatment.

Esophageal strictures (ES) are a significant clinical concern in pediatric esophageal diseases and substantially affect patients' quality of life. The causes can be classified as congenital, secondary, and dyskinetic types. While most ES cases are benign, clinical outcomes vary widely by etiology, necessitating personalized therapeutic strategies. Current approaches predominantly involve minimally invasive endoscopic interventions, including balloon or bougie dilation, stent placement, incisional therapy, and intralesional steroid injection. Refractory cases may require combining or alternatin

Thoracoscopic Congenital Esophageal Stenosis Repair and Thal Fundoplication by Right Thoracic Approach: A Case Report.

INTRODUCTION: Congenital esophageal stenosis (CES) is a rare clinical condition found in 1 in 25000-50000 live births. Surgical treatment is required when endoscopic balloon dilatation is ineffective. Laparoscopic and thoracoscopic approaches are selected based on lesion location. Gastroesophageal reflux (GER) is often observed as a postoperative complication that necessitates additional fundoplication. We report a case of CES in the lower third of the esophagus that was treated with simultaneous thoracoscopic resection and Thal fundoplication using the right thoracic approach. CASE PRESENTAT

Endoscopic remodeling of esophageal stenosis refractory to traditional endoscopic treatment.

Esophageal atresia (EA), with or without tracheoesophageal fistula, is the most common congenital anomaly of the esophagus. Surgical correction is the primary treatment, however, up to 80% of patients experience anastomotic stenosis, with esophageal balloon dilation (EBD) being the first-line treatment. Unfortunately, some patients develop refractory anastomotic stenosis (RAS), defined by the presence of strictures after three EBD sessions. The endoscopic management of RAS remains controversial. Although EA itself is rare, the incidence of postoperative stricture is significant, contributing t

Endoscopic Controlled Radial Expansion Balloon Dilatation in Infantile Achalasia: A Case Series of Four Infants.

Achalasia cardia is an uncommon esophageal motility disorder that is often misdiagnosed as gastroesophageal reflux disease (GERD). Although rare in infancy, it presents unique diagnostic and therapeutic challenges compared to adults. We describe four infants who presented with early-onset vomiting and feeding difficulties and were diagnosed with achalasia cardia based on characteristic findings on timed barium esophagogram (TBE). None of our patients had features of Allgrove (Triple-A) syndrome, which is characterized by achalasia, alacrimia, and adrenal insufficiency. Serial controlled radial

An Aberrant Right Subclavian Artery Causing Severe Esophageal Compression: A Case Report.

An aberrant right subclavian artery (ARSA), a congenital vascular anomaly, can cause significant esophageal compression, leading to a condition known as dysphagia lusoria (DL). We present the case of a 44-year-old man with progressively worsening dysphagia and odynophagia over the last six months, resulting in severe weight loss and dietary restrictions. Imaging techniques revealed esophageal stenosis caused by external compression from an ARSA arising from the posterior wall of the distal aortic arch, accompanied by a Kommerell's diverticulum. Computed tomography angiography confirmed the abe
