Radiological features of the oesophagus of 7 normal horses and 13 with oesophageal lesions are described. The use of barium sulphate as a contrast agent and the techniqes of its administration are discussed. It is suggested that chronic intermittent oesophageal impaction with food material was a predisposing factor in the development of a localised oesophageal dilation in 3 horses and that one other probably resulted from an injury. A similar dilatation was seen which resulted from oesophageal constriction by a vascular ring. Megaoesophagus was seen in 2 ponies associated with grass sickness a

Obstruction of the left subclavian artery is one of the rarest causes of the cogenital subclavian steal syndrome with a right-sided aorta. Radiological diagnosis can be made from a standard film which shows the aorta on the right, and a barium examination of the esophagus which demonstrates the absence of any compression posteriorly. Angiographic exploration (aortography, pulmonary angiography) is essential for positive confirmation of diagnosis and to exclude other types of congenital subclavian steal with a right-sided aorta. Certain cardiac malformations may be associated, but forms have be

The birthweights of patients with congenital anomalies of the intestinal tract have been examined. Cases of esophageal and duodenal atresia, malrotation of the midgut and anal anomalies have been chosen only if they were uncomplicated by any other major anomaly, and their weights plotted on the centile charts when the gestational age was known. The birthweights of cases of esophageal and duodenal atresia are clearly less than in cases of malrotation and anal malformation. This difference may be related to the function of the intestinal canal. Despite low birthweights in cases of esophageal atr

Ten years' experience with pertechnetate (Tc-99m) abdominal scintigraphy has shown that the method is specifically indicated for the diagnosis and location of ectopic gastric mucosa in a Meckel's diverticulum, in enteric or gastric duplications and cysts, and in Barrett's esophagus. Careful patient preparation is essential, and sequential gamma camera imaging for 1 hr in the anterior projection is advisable, with complementary lateral, upright, and postvoiding views. Nonspecific accumulations of the tracer within tumours, intestinal obstructions, arteriovenous malformations, and the urinary tr

Laryngotracheoesophageal cleft (LTE) is a rare disorder that is still associated with a high mortality. Often the mortality is associated with other coexisting severe congenital anomalies, but even in those infants in whom the LTE cleft is the only anomaly, the mortality rate is still unacceptably high. This paper explores two avenues aimed at reducing the mortality. There are two main factors responsible for the high mortality. First, the diagnosis may be difficult and hence delayed; and second, once the diagnosis is made, management reported to date does not adequately prevent aspiration of

Twenty-one cases of congenital tracheal stenosis seen at the Royal Alexandra Hospital for Children 1971 through 1980 were reviewed with regard to the clinical features, associated anomalies, endoscopic findings and radiological evaluation. Congenital tracheal stenosis was usually a serious problem often associated with other major anomalies of the respiratory tract, esophagus, or skeleton. There was no set pattern of presentation. The presenting features included: persistent wheeze or stridor, atypical "respiratory distress," and atypical "croup" or "bronchiolitis." Documentation of the nature

The progress of esophageal surgery from the beginning of medical history through five historical eras, up to the second half of the 20th century, is reviewed. Progress was slow from the first surgical repair of the esophagus in ancient Egypt in 2500 B.C. until the end of the 19th century, when scientific discoveries made possible the solid beginnings of abdominal surgery. Thoracic surgery followed in the 20th century, with rapid strides in World War II. These wartime advances stimulated an interest in esophageal surgery in the postwar era, when operative techniques became well standardized and

Between January 1976 and March 1979, esophageal atresia was successfully repaired in six infants with circular myotomy (Livaditis technique) and end-to-end anastomosis. Children were 1 day-6 mo in age and weighed 2.4 to 8.0 kg at the time of repair. Four children required one and two children required two circular myotomies in the proximal esophagus to bridge gaps between esophageal ends, which varied from 1.2-3.0 cm. Follow-up has been 1-4 yr (mean-3 yr). Operative complications included: opening of the mucosa at myotomy (2) and anastomotic leak (2). Subsequently, three children developed ana

Fifty-seven babies were surgically treated for esophageal atresia and tracheoesophageal fistula between 1968 and 1978. Forty-eight (84%) had proximal esophageal atresia and a distal tracheoesophageal fistula, 2 (4%) had proximal and distal esophageal atresia and no tracheosophageal fistula, and 7 (12%) had a tracheosophageal fistula without esophageal atresia. Primary repair was accomplished in 43 patients (75%), colon interposition was required in 5 (9%), while the remainder had staged or palliative reapirs. Forty-six (81%) survived surgical treatment. All 21 babies in Waterston Category A, 9

Successful surgery for esophageal atresia and tracheoesophageal fistula is a relatively recent development, and progress has been rapid over the past 10 years. Because the surgical technique is still controversial, the authors reviewed their experience in treating 38 infants with the condition. Transpleural end-to-end repair was carried out in all cases. In 21 cases a two-layer repair was done and in 17 a one-layer repair. After 10 days, if no anastomotic leak was detected radiologically, esophagoscopy and dilatation at the anastomotic site were performed; dilatation was carried out routinely

One hundred twenty-nine infants with esophageal atresia and tracheoesophageal fistula were reviewed from 1955 to 1978. The overall mortality was 38%. Factors associated with the increased mortality include prematurity, pneumonia, and congenital defects other than this anomaly. A classification based on these factors in introduced, which provides the clinician with a prognostic survival rate greater than 90% with only physical examination, chest and abdominal roentgenography, and intravenous pyelography. Postoperative mortality was reduced to 11% in the last five years of the study; this is att

A sixteen months old girl with Down's-Syndrome suffered from congenital oesophageal membrane and segmental oesophageal stricture. In the neonatal period, two other associated malformations of the gastrointestinal tract (malrotation of gut and duodenal web) had been operated successfully. The first endoscopic examination of the eosophagus was performed at the age of sixteen months because of increasing dysphagia. A solid membrane in the lower part of the oesophagus was incised by diathermy. The associated segmental stenosis necessitated repeated bougienages. After this treatment the child devel

The authors report six cases of congenital laryngeal stridor in which appearances of oesophageal compression had led to a diagnosis of abnormalities of the aortic arches. Two children underwent surgery, thoracotomy failing to reveal any abnormality. The other four were investigated by endoscopy which made it possible to eliminate the diagnosis of tracheal compression. All of these children were suffering from laryngomalacia. These cases indicate the need for complete routine studies, in particular endoscopic, in all cases of stridor, as well as for careful definition of the radiological criter

Comparative investigation of 92 cases of V.A.T.E.R. syndrome (4 personal cases) and 62 cases of caudal regression (Duhamel syndrome) (2 personal cases) are performed. There is much analogy between these two entities. Initial impairment would be an early dysfunction of mesoderm setting up located on esophagus in V.A.T.E.R. syndrome and on kidneys in Duhamel syndrome. Etiopathogenic factors remain unknown. Genetic counseling is good. Detection of only one mesodermal malformation leads to inquire other unnoticed anomalies (kidneys, heart, spine, alimentary duct).

Four cases of H-type tracheoesophageal fistula are reported. The patients all had chronic aspiration from the esophagus. Although serious symptoms were present in all, definitive diagnoses were not made until the patients had reached 1, 6, 12, and 50 years of age, because ordinary radiologic methods failed to establish the diagnosis. The angulation of the fistula usually prevents contrast medium in the esophagus from entering the trachea, especially with the subject upright. On the other hand, air easily passes from the trachea to the esophagus, eventually producing megaesophagus which may be

Radio-ulnar synostosis is a congenital bony union between radius and ulna, usually in the vicinity of the radial tuberosity. A family is described on whom this anomaly was inherited over three generations. The rare cases described in the literature were usually bilateral, in our family it was unilateral. The following aspects are discussed: the frequency of this condition, the anatomical localisation of the synostosis, their types, local soft tissue and other abnormalities, embrylogy and the hereditory pattern. Finally, methods of treatment are discussed briefly.

A five-year study of possible teratogenicity of exogenous female sex hormones included three case-control studies and one cohort study. The first case-control study disclosed an estimated relative risk of 8.41 and a highly significant difference in maternal hormonal exposure (P less than .001) between controls and infants with three major anomalies of the VACTERL group (V, vertebral; A, anal; C, cardiac; T, tracheal; E, esophageal; R, renal; and L,limb). Relative risk (RR) estimates of 5.58 (P = .017) and 3.35 (P less than .001) were found in two case-control studies involving maternal hormona

The present paper describes two rare cases of epidermolysis bullosa dystrophica (recessive), one with oesophageal stricture and another with laryngeal stenosis, along with other usual features of the disease. Different views on the line of treatment of the oesophageal stricture as a complication of this dreadful disease are discussed. Replacement of the strictured oesophagus by coloplasty may be contemplated in the first case, if the patient develops increasing dysphagia. In the second case, with cicatrical stenosis of the larynx and hoarseness of voice which is a rare complication of the dise

At the Royal Children's Hospital, Melbourne, 375 patients with oesophageal atresia and/or tracheo-oesophageal fistula have been admitted under the care of one surgical unit and, as with other reported series, there has been a predominance of the type with proximal atresia and distal tracheo-oesophageal fistula. With the object of evaluating the long-term follow-up, a review of the patients seen before 1963 has been made. During the period 1948 - 1962, 132 babies with oesophageal atresia and distal tracheo-oesophageal fistula were treated surgically with 72 survivals. A complete follow-up has b

Eleven patients with previous esophageal atresia repair (TEF) underwent esophageal motility studies and were compared to 10 normal patients. The upper sphincters (UES) in the two groups did not differ in resting pressure, contraction pressure, or coordination. UES relaxation, was 100% in normal patients and 83+/-8% in the TEF patients (p less than 0.05). In the proximal esophagus, 100% of the TEF group showed a normal peristaltic wave, coupled with an abnormal nonperistaltic wave. The normal group did not show this type of contraction. In the middle esophagus, the mean peak pressure of 30.5+/-
