PURPOSE: Unlike colonic atresia, concomitant jejuno-ileal atresia (JIA) and Hirschsprung disease (HSCR) is an under-recognized association. We used a multi-institutional patient registry to describe the incidence, clinical characteristics, management, and outcomes of patients with both conditions.
METHODS: We performed a retrospective review of patients with HSCR in the Pediatric Colorectal and Pelvic Learning Consortium (PCPLC) registry. Additional data was collected from participating sites regarding atresia management and outcomes.
RESULTS: Of 1459 eligible patients with HSCR, 15 (1.0%) a

INTRODUCTION: Congenital colorectal conditions including anorectal malformations (ARM) and Hirschsprung's disease (HD) can cause chronic issues with constipation and incontinence leading to reduced quality of life. Bowel management programs (BMP) are critical to addressing these challenges. There is a paucity of literature on intermediate-term outcomes of BMPs for patients in low and middle-income countries (LMICs).
METHODS: In June 2022, a BMP for patients with ARM or HD was initiated at a tertiary referral hospital in Mbarara, Uganda. Local staff were trained by visiting staff with experien

INTRODUCTION: We evaluated whether Trisomy 21 (T21) affected bowel management use, ability to toilet train for stool, and rates of Hirschsprung-Associated Enterocolitis (HAEC) in children with Hirschsprung disease (HD). We hypothesized that T21 would be associated with increased rates of HAEC, bowel management program (BMP) use, and delayed toilet training for stool.
METHODS: Children with HD in the Pediatric Colorectal and Pelvic Learning Consortium (PCPLC) database who underwent primary pull-through were included and stratified by T21 status: HD only versus HD + T21. Children requiring a re

Hirschsprung disease (HSCR) is a congenital intestinal motility disorder characterized by the absence of enteric neurons in the distal bowel. Compared with bulk RNA sequencing, single-cell RNA sequencing (scRNA-seq) enables gene expression profiling at the single-cell level, resolving cellular heterogeneity that is masked in population-averaged analyses. A standardized scRNA-seq workflow was established to characterize the transcriptional landscape of ganglionic and aganglionic segments from patients with HSCR. Colon tissues were collected, and hematoxylin and eosin staining was performed to i

BACKGROUND: Incomplete migration of enteric neural crest cells (ENCCs) into the colon underlies the pathogenesis of Hirschsprung disease, resulting in aganglionosis and disordered intestinal motility. Although activation of the glial cell line-derived neurotrophic factor (GDNF) pathway has been implicated in neurosphere differentiation, specific regulatory genes governing ENCC differentiation and proliferation remain poorly characterized. This study seeks to elucidate regulatory genes and pathways involved in neural crest cell differentiation, furthering our understanding of GDNF's role in ent

Hirschsprung disease (HSCR) is a life-threatening congenital disorder characterized by defective migration and proliferation of enteric neural crest cells (ENCCs), a process in which the receptor tyrosine kinase RET plays a central regulatory role. However, the regulatory mechanisms governing RET expression remain incompletely understood. In the present study, we identify the circular RNA circRET (hsa_circ_0093651) as a critical translational regulator of its parental gene RET and explore its role in HSCR pathogenesis. We demonstrate that circRET is significantly downregulated in the colonic t

Congenital central hypoventilation syndrome (CCHS) and Hirschsprung's disease (HD) belong to neurocristopathies - autonomic nervous system defects caused by abnormal neural crest cells migration. Their co-occurrence defines Haddad Syndrome (HS) associated also with increased risk of neural crest origin tumors, mainly neuroblastoma (NBL), and PHOX2B gene mutation. This case concerns a male newborn presenting respiratory failure minutes after birth. Due to sleep apnea and increasing hypercapnia, he required intubation. Genetic testing revealed an anomaly in PHOX2B gene. In subsequent days, the p

The formation of the enteric nervous system (ENS) primarily involves the migration of enteric neural crest-derived cells (ENCCs) and the subsequent maturation of enteric neurons. The developmental dysfunction of ENCCs and enteric neurons can result in ENS disorders, such as hypoganglionosis (HG). Although neurite outgrowth is fundamental to neuronal maturation, the mechanisms by which neurite outgrowth influences neuronal maturation remain poorly defined. Here, we identified EMB as a critical regulator of enteric neuronal maturation. In EMB mutant patients, the expression of EMB is reduced in

OBJECTIVE: HSCR is caused by disruption of complex signaling pathways within the gene regulatory network (GRN) during enteric nervous system (ENS) development, including glial cell line-derived neurotrophic factor (GDNF) and GDNF family receptor alpha-1 (GFRα1). However, pathogenic variants in all GRN genes account for only ~ 80% cases; therefore, the epigenetic role remains to be elucidated. We compared GDNF and GFRα1 expression between HSCR patients and controls.
RESULTS: qPCR revealed an upregulated GDNF expression in both ganglionic (206.37-fold) and aganglionic (126.35-fold) HSCR compare

INTRODUCTION: The study aimed to explore the causal effect of pre-pregnancy waist-to-hip ratio adjusted for body mass index (WHRadjBMI) on Hirschsprung's disease (HD) in offspring.
MATERIALS AND METHODS: Genome-wide association data for WHRadjBMI and HD were obtained from the Mendelian randomization (MR) base platform. Significant single nucleotide polymorphisms associated with WHRadjBMI in females were identified as instrumental variables. Bidirectional, two-sample MR analysis was then conducted to assess the causal relationship between female WHRadjBMI and HD, using three statistical method

BACKGROUND & AIMS: Low weight-for-age z-score (WAZ) is prevalent in children with Hirschsprung's disease (HD), yet its prognostic value for surgical outcomes is unclear. This study evaluated the impact of preoperative WAZ on perioperative course complexity and postoperative recovery in children undergoing pull-through surgery.
METHODS: A retrospective cohort study was conducted on 256 children aged ≤10 years who underwent pull-through. Patients were stratified by preoperative WAZ: underweight (WAZ < -2), at risk of underweight (-2 ≤ WAZ < -1), and well-nourished (WAZ ≥ -1). Demographic, clini

PURPOSE: Hirschsprung-associated enterocolitis (HAEC) remains a major cause of morbidity, with poorly defined triggers and limited modifiable risk factors. Given the roles of immune immaturity and mucosal dysfunction in HAEC pathogenesis, this study evaluated whether age is associated with HAEC risk, and whether seasonal variation consistent with viral exposure is present.
METHODS: A retrospective cohort study was conducted including patients with HD treated at Children's Hospital Colorado (2008-2021). A piecewise exponential Poisson regression model estimated age-specific HAEC incidence rate

BACKGROUND: Early and accurate diagnosis of Hirschsprung disease (HSCR) prevents life-threatening complications, improves patient outcomes, and reduces healthcare utilization. Contrast enema (CE), anorectal manometry (AMAN), and rectal biopsy (RB) are commonly used diagnostic tests, with RB being the gold standard. However, the most cost-effective initial diagnostic strategy remains unclear.
METHODS: A decision-analytic model was used to compare CE-first, AMAN-first, and RB-first strategies for the evaluation of HSCR in children between 1 and 18 years of age presenting to specialty care for c

Hirschsprung disease (HSCR) is a congenital malformation characterized by the absence of the enteric nervous system (ENS) in the distal colon, resulting from defective colonization of enteric neural crest cells (ENCCs). The underlying pathogenesis of HSCR remains incompletely understood. Here, we report that c-Cbl-associated protein (CAP), also known as sorbin and SH3 domain-containing protein 1 (SORBS1), is upregulated in the aganglionic colon tissues of children with HSCR. Functional studies revealed that CAP overexpression suppresses ENCC colonization by binding the lipid raft protein floti

The management of septal formation after the Duhamel procedure in Hirschsprung disease remains unclear. We report two cases treated with an endoscopic-assisted approach. Case 1 is a 5-year-old girl with Hirschsprung disease who underwent the Duhamel procedure. Persistent constipation led to a contrast enema, confirming septal formation. Transanal endoscopic-assisted division with an endoscopic linear stapler improved symptoms initially. However, constipation and enterocolitis with bleeding recurred 5 months later, requiring redo surgery. Case 2 is a 21-year-old female with a history of Hirschs

Hirschsprung disease (HSCR) is characterized by absence of enteric ganglia (aganglionosis) along variable lengths of the distal intestine. This disorder results from deficient colonization of fetal intestine by enteric neural crest-derived cells (ENCDCs). HSCR exhibits complex, multifactorial inheritance with penetrance and severity varying widely even within families. SOX10 is among causal genes that predispose to aganglionosis. Yet, how gene interactions influence severity of HSCR aganglionosis is not understood. Prior mapping of aganglionosis modifiers was achieved in a standard F1-intercro

The intestinal muscularis is a multifunctional tissue that coordinates motility, immune responses, and tissue homeostasis through interactions among enteric neurons, smooth muscle cells (SMCs), and immune cells. Beyond its classical role in peristalsis, emerging evidence identifies the intestinal muscularis as an active regulatory hub integrating neural, immune, and microbial signals. Here, we review the anatomical organization, physiological functions, and pathological alterations in inflammatory bowel diseases, irritable bowel syndrome, infection, and Hirschsprung disease. We further highlig
