The impact of pathological cyst wall changes on congenital biliary dilatation in infancy.

PURPOSE: This study aimed to evaluate the impact of pathological changes in bile duct cysts on the clinical and liver pathological findings in early infantile patients with congenital biliary dilatation (CBD). METHOD: In patients with CBD who underwent definitive surgery ≤ 4 months of age, the clinical and liver pathological findings were analyzed according to the presence or absence of inflammatory and fibrotic changes on the luminal side of the bile duct cyst. RESULTS: Among the 14 patients with infantile CBD, 8 had inflammatory cell infiltration (iCBD), and 4 of these 8 patients showed th

Serial carbohydrate antigen 19-9 monitoring reflects occult biliary abnormalities after surgery for choledochal cyst.

BACKGROUND: Carbohydrate antigen 19-9 is widely used as a pancreatobiliary biomarker; however, its value after choledochal cyst surgery remains unclear. Because long-term benign biliary complications predominantly occur after surgery for choledochal cyst requiring extrahepatic bile duct excision, we investigated the longitudinal behavior of carbohydrate antigen 19-9 in this patient population. We evaluated whether carbohydrate antigen 19-9 levels predict postoperative biliary complications or reflect concurrent biliary abnormalities during follow-up. METHODS: We retrospectively analyzed 233 p

Repair of Main Pancreatic Duct Injury during Surgery for Congenital Biliary Dilatation in a Child: A Case Report.

INTRODUCTION: In surgery for congenital biliary dilatation, it is essential to minimize residual intrapancreatic bile duct tissue to prevent future complications, such as intrapancreatic stones and carcinogenesis. However, dissection of the intrapancreatic bile duct is technically demanding and may result in injury to the main pancreatic duct. As a result, very few cases have been reported regarding the management of this complication in pediatric patients. We present a pediatric case of intraoperative main pancreatic duct injury and describe its surgical management. CASE PRESENTATION: A 3-ye

Integrated single-cell and bulk-tissue transcriptomic analysis reveals an epithelial-stromal inflammatory microenvironment in biliary atresia.

BACKGROUND: Biliary atresia is a severe fibroinflammatory cholangiopathy of infancy. Its pathological process involves biliary epithelial injury, immune activation, and intrahepatic stromal remodeling, but the cellular interactions linking these processes remain incompletely defined. Using public single-cell and bulk-tissue transcriptomic cohorts, this study systematically characterized disease-associated alterations across liver cell populations in biliary atresia and further evaluated their consistency in independent cohorts. METHODS: Public single-cell liver data (GSE176189) and three inde

Artificial intelligence-based phase recognition for workflow analysis in robotic choledochal cyst excision: a proof-of-concept study.

BACKGROUND: Automated surgical phase recognition may convert routinely recorded robotic surgery videos into objective workflow metrics, but its clinical interpretability in complex robotic pediatric and hepatobiliary procedures remains unclear. METHODS: In this retrospective proof-of-concept study, we developed and internally evaluated a video-based phase-recognition model for robotic choledochal cyst surgery using nine predefined operative phases. The model was trained on 20 robotic cases and evaluated on an independent internal dataset of 10 cases. AI-derived phase durations were applied to

A rare intersection: choledochal cyst (Todani IC) and incomplete pancreas divisum in an adult female. Diagnostic and therapeutic insights.

We report the case of a 59-year-old woman with a history of smoking, hypertension, and prior surgeries. She presented with acute right upper quadrant abdominal pain radiating to the back. Laboratory tests revealed elevated liver enzymes and imaging studies (CT and MRCP) showed a dilated common bile duct. Endoscopic ultrasound confirmed an incomplete pancreas divisum and a Todani type 1C bile duct cyst. The patient underwent hepaticojejunostomy anastomosis, with resolution of symptoms at follow-up. This is the first documented case of a choledochal cyst and incomplete pancreas divisum coexistin

Early (≤ 1 year) anastomotic complications after laparoscopic and robotic hepaticojejunostomy: initial experience from a single center.

BACKGROUND: Presently, experience with laparoscopic and robotic bilio-enteric anastomosis (BEA) has been increasing world-wide although widespread adoption remains limited. In this study, we report our initial experience with both these modalities. METHODS: A retrospective cross-sectional review of a single center prospective database of 130 consecutive laparoscopically and robotically constructed BEAs in hepatopancreatobiliary surgeries performed between 2013 and 2022 was conducted. A propensity-scored match analysis comparing results between both groups was performed. RESULTS: There were 5

Laparoscopic management of cystic duct cyst (Type VI): case report.

Cystic duct cyst (CDC) is a rare biliary anomaly that may occur in isolation or in association with other choledochal cysts. While individual variants have been described, reports focusing on dual biliary pathology managed by a minimally invasive approach are limited. Surgical resection is recommended to prevent complications, including malignant transformation. We report the case of a 68-year-old woman with a remote history of cholecystectomy who presented with acute pancreatitis. Magnetic resonance cholangiopancreatography identified a CDC associated with dilatation of the common bile duct.

Timing of definitive surgery after external biliary drainage for completely perforated congenital choledochal cysts: a single-center retrospective cohort study.

BACKGROUND: The optimal timing of definitive surgery after external biliary drainage (EBD) for children with completely perforated congenital choledochal cysts (CCs) remains controversial. Prolonged EBD may increase the risk of drainage tube–related complications. This study compared the safety and efficacy of early (≤ 14 days) versus delayed (> 14 days) definitive surgery after EBD. METHODS: This study is a single-center retrospective cohort study that included all consecutive pediatric patients with complete perforation of choledochal cysts (CC) admitted between January 2018 and February 202

Arteriogenic hemorrhage of the anastomotic loop after choledochojejunostomy: case series and literature review.

Arteriogenic hemorrhage of the anastomotic loop after choledochojejunostomy is a life-threatening, and complex postoperative complication, necessitating a comprehensive diagnostic approach integrating clinical manifestations, imaging findings, and results from endoscopy or angiography. We present 3 clinical cases: a 22-year-old male who developed hemorrhagic shock 28 days after laparoscopic choledochal cyst resection, due to an anomalous communication between the right hepatic artery stump and the choledochojejunostomy, and achieving hemostasis via transarterial embolization; a 71-year-old fem

Transgenic mouse model of congenital choledochal cyst.

PURPOSE: Choledochal cyst is a rare, congenital dilation of the hepatobiliary tree. Due to the associated malignancy risk, complete resection is recommended. There remains a risk of metachronous cholangiocarcinoma despite resection necessitating lifelong surveillance. Choledochal cysts are increasingly prevalent with an incompletely understood connection to carcinogenesis. We sought to develop a mouse model to reliably mimic human disease process of choledochal cyst. METHODS: Experimental transgenic mice were bred with a genotype of Pdx-Cre, TGFα, LSL-Kras G12D Mu/Wt. Control C57 mice were use

Age-Dependent Dynamics of the Biliary Microbiome in Children with Choledochal Cysts: Functional Remodeling Underlying Taxonomic Conservation.

Choledochal cyst (CC), a congenital biliary anomaly, is associated with recurrent infections, chronic inflammation, and an increased risk of malignancy. Although emerging evidence implicates the biliary microbiome in disease pathophysiology, its developmental dynamics in pediatric CC remain unclear. Using deep metagenomic sequencing and comprehensive functional annotation, this study characterized age-dependent changes in the biliary microbiome of 201 pediatric CC patients stratified into infancy (<1 year), early childhood (1-5 years), and later childhood (5-12 years). We found that while the

Predictive value of cyst size for clinical symptoms in patients with prenatally diagnosed choledochal cysts.

BACKGROUND: Choledochal cysts (CDCs) are common congenital biliary malformations in children. However, current research on whether cyst size links to postnatal clinical symptoms in prenatally diagnosed CDCs remains scarce and unclear. This study aimed to explore the predictive value of cyst size for the occurrence of clinical symptoms in patients with prenatally diagnosed CDCs. METHODS: A retrospective review of medical records was conducted for patients with prenatally diagnosed CDCs who were admitted to Fujian Children's Hospital between January 2018 and May 2025. The patients were divided

Robot-assisted choledochal cysts resection in children across all ages: a larger cohort single-center experience with 318 Cases.

Robotic-assisted surgery (RAS) is an emerging alternative to laparoscopic surgery (LAS) for the treatment of choledochal cysts. We aim to evaluate the safety and efficacy of robotic-assisted choledochal cyst resection (RACC) in children from neonates to 16-year-olds. In this retrospective study, 318 patients undergoing RAS between April 2020 and July 2025 were compared with 489 patients undergoing LAS between January 2015 and July 2025. The robotic cohort was stratified into six age-based subgroups: R1 (neonates), R2 (> 1 to ≤ 3 months), R3 (> 3 to ≤ 12 months), R4 (> 12 to ≤ 36 months), R5 (>

Robot-assisted Cyst Excision, Ductoplasty, and Embedding Portoenterostomy for Choledochal Cysts with a Small Caliber Hepatic Duct less than 6 mm.

Anastomotic stricture is a primary concern following the surgical treatment of choledochal cysts (CC), particularly in pediatric patients with narrow bile ducts (<6 mm), and remains the main indication for re-operation. Robotic-assisted surgery (RAS), with its superior 3D visualization and instrument dexterity, offers an ideal platform for the complex reconstruction required to prevent this complication. This study presents a robot-assisted modified technique designed to mitigate the risk of stricture, as demonstrated in a 34-month-old female with a Type Ia CC. The procedure involves complete

Technical feasibility should not outweigh long-term outcomes in biliary reconstruction for children.

The choice of biliary reconstruction after excision of choledochal malformations in children should prioritize long-term physiological outcomes over operative convenience. In response to the article by Kanojia et al. on the management of aberrant bile ducts at the porta hepatis during minimally invasive surgery, we wish to highlight that while hepaticoduodenostomy is technically attractive (particularly with laparoscopic and robotic platforms) evidence suggests it is associated with increased biliary reflux and reflux-related morbidity, especially in cases with complex hilar anatomy requiring
