Obesity and MASLD (NAFLD) are chronic, low-grade systemic inflammatory diseases resulting from complex interactions among diet, adipose tissue, gut microbiota, liver, muscle, pancreas, and the brain. Central to this integrative framework are AA and other polyunsaturated fatty acids (PUFAs), which regulate inflammation, insulin sensitivity, neuroendocrine signaling, and energy homeostasis.Excess consumption of energy-dense foods rich in saturated and trans fats leads to adipocyte hypertrophy, altered membrane fluidity, increased antigenicity, and enhanced recruitment of macrophages and T cells

Pancreatic hamartoma (PH) is an exceedingly rare benign non-neoplastic lesion of the pancreas. Owing to its nonspecific clinical and radiological features, it is prone to misdiagnosis. Among its various subtypes, functionally active pancreatic hamartomas with distinct endocrine activity have only been sporadically reported in the literature as individual cases. We report the case of a 58-year-old female patient whose initial presentation was characterized by paroxysmal behavioral abnormalities during nocturnal sleep, manifesting as sudden agitation, arm-flinging, and purposeless stereotyped mo

BACKGROUND: Diabetes mellitus constitutes a major global health burden. L. sativum has long been employed in traditional medicine for glycemic management, an effect attributed to its rich phytochemical constituents, including flavonoids, phenolic acids, and glucosinolates. This study investigated the ameliorative effects of L. sativum oil against streptozotocin-induced type 1 diabetes in rats, with particular emphasis on pancreatic and duodenal homeobox 1 (PDX-1) and glucokinase (GCK) gene expression.
METHODS: A total of 48 young male Wistar rats were randomly divided into four experimental g

BACKGROUND AND OBJECTIVES: Univentricular heart is a complex congenital heart disease with rare survival into adulthood. It could be part of a heterotaxy syndrome when, apart from the cardiac malformations, there are associated abnormal arrangements of extra-cardiac organs like the lungs, spleen and pancreas. The syndrome is said to be rarer than the univentricular heart and to have a worse clinical outcome.
METHODS: After clinical assessment, the patient was investigated with Chest X-ray, Electrocardiography, Echocardiography, and contrast-enhanced CT scan of the chest and abdomen to make th

INTRODUCTION: Maternally inherited diabetes and deafness (MIDD) is a multisystem mitochondrial disorder most commonly caused by the m.3243A>G pathogenic variant in the MT-TL1 gene. Kidney transplantation in affected individuals presents unique perioperative and immunosuppressive management challenges due to underlying metabolic, neurologic, cardiac, and neuromuscular vulnerability.
CASE REPORT: We report a 64-year-old woman with end-stage renal disease secondary to insulin-dependent diabetes and MIDD (m.3243A>G) who underwent unrelated living donor kidney transplantation. The immediate postop

Background/Objectives: Diabetic cardiomyopathy is an under-recognized complication of type 1 diabetes mellitus (T1DM), often co-existing with diabetic nephropathy. This study investigates whether patients with T1DM and end-stage renal disease (ESRD) undergoing simultaneous pancreas-kidney transplantation (SPKT) show echocardiographic signs of cardiomyopathy prior to surgery and explores whether cardiac changes are more attributable to diabetes itself or to chronic kidney disease. Methods: We retrospectively analyzed preoperative echocardiographic data of 16 SPKT patients with T1DM and ESRD, us

OBJECTIVE: Diabetes is a frequent complication of Cushing's disease, but metabolic differences between normal glucose tolerance, prediabetes, and diabetes remain incompletely characterised. We aimed to characterise glucose metabolism across these groups, assess abnormalities in patients with normal glucose tolerance, and identify clinical, hormonal, and body-composition factors associated with glycaemia.
DESIGN: Cross-sectional analysis of a prospective cohort with matched healthy controls.
MATERIAL AND METHODS: The study included 50 patients with Cushing's disease and 52 age- and sex-matche

RATIONALE: Lymphangioma is a benign lymphatic malformation that occurs predominantly in children and at superficial sites. Intra-abdominal lymphangioma is uncommon, and involvement of the pancreatic tail-splenic hilum region is particularly rare. Such lesions may mimic pancreatic or peripancreatic cystic disease, making preoperative diagnosis challenging.
PATIENT CONCERNS: We report a 43-year-old woman who presented with persistent dull left upper abdominal pain for 2 days. Laboratory tests showed nonspecific inflammatory and mild hepatobiliary abnormalities.
DIAGNOSES: Ultrasonography and c

Pancreaticobiliary maljunction (PBM) is a rare congenital anomaly in which the pancreatic and bile ducts unite outside the duodenal wall, forming an abnormally long common channel that permits reciprocal reflux of pancreatic juice and bile. This leads to a high risk of recurrent pancreatitis, cholangitis, biliary stone formation, and biliary tract malignancy. Type D PBM, particularly when associated with complete pancreas divisum and aberrant bile duct drainage, represents an exceptionally rare and complex variant. We report the case of a 20-year-old female with a long-standing history of recu

BACKGROUND: Pancreatic exocrine insufficiency (PEI) is a maldigestion syndrome leading to symptoms and nutritional deficiencies. Prospective real-world data on pancreatic enzyme replacement therapy (PERT) dose requirements across aetiologies are limited.
OBJECTIVE: To define PERT dose requirements and treatment outcomes across PEI aetiologies in specialist European practice.
METHODS: EURO-PEI was a multicentre observational registry with predominantly prospective data collection conducted at eight pancreatic centres in five European countries between June 2021 and September 2025. Adults with

Type 2 diabetes (T2D) subgroups defined by clinical variables differ in disease progression and treatment response. To uncover potential molecular drivers of this heterogeneity, we performed a multi-omics analysis of 826 drug-naïve T2D patients from two phase 3 trials of the insulin sensitizer chiglitazar. Here we show that severe insulin-resistant diabetes (SIRD) is characterized by distinct miRNA profiles (e.g., miR-122-5p) correlated with liver injury, and metabolic shifts in amino acids and primary bile acids. Mild obesity-related diabetes (MOD) showed the lowest level of phenylacetylgluta

PURPOSE: Human embryological studies have documented the transient presence of paired ventral pancreatic buds during early development. The persistence of the left ventral bud into adulthood could theoretically result in duplicated ventral pancreatic ducts in adulthood. Here, we report a case of duplicated ventral ducts incidentally observed in a human pancreas retrieved from an organ donor.
ANATOMIC VARIATION: The pancreas was obtained from a 46-year-old male donor after brain death for the purpose of islet transplantation. During the islet isolation procedure, we unexpectedly identified two

Diabetes-associated auditory dysfunction may extend beyond cochlear threshold elevation and involve broader metabolic and auditory neural abnormalities. This study investigated whether Castanopsis echinocarpa extract (CAE) attenuates peripheral-to-central auditory electrophysiological dysfunction in streptozotocin (STZ)-induced diabetic mice and modulates related auditory and pancreatic stress responses. HEI-OC1 auditory cells were exposed to neomycin or inflammatory cytokines, and Tnf, Il1b, Trpv1, and Gjb2 mRNA expression was analyzed. Post-exposure otic hair cell recovery was assessed in ne

Nanotargeted drug delivery systems (nanotargeted DDS) have emerged as promising solutions to improve treatment precision and reduce toxicity. However, achieving efficient delivery from the bloodstream to the tumor site remains challenging due to the complex tumor microenvironment (TME). To address these issues, a novel pH-responsive surface-switching DDS, UiO@CeO2/IR@(bPEI/HA)-A6, was designed. This system features CeO2 immobilized on zirconium metal-organic frameworks (UiO-66-NH2) to create a rough surface, which is then further modified with bPEI and HA-A6 polymers, facilitating its transpor

We report the case of a 59-year-old woman with a history of smoking, hypertension, and prior surgeries. She presented with acute right upper quadrant abdominal pain radiating to the back. Laboratory tests revealed elevated liver enzymes and imaging studies (CT and MRCP) showed a dilated common bile duct. Endoscopic ultrasound confirmed an incomplete pancreas divisum and a Todani type 1C bile duct cyst. The patient underwent hepaticojejunostomy anastomosis, with resolution of symptoms at follow-up. This is the first documented case of a choledochal cyst and incomplete pancreas divisum coexistin

BACKGROUND: Type 2 diabetes (T2D) is a disease of high prevalence that is expected to continue increasing despite the pharmacological treatments available; in most cases, it is difficult to control. Therefore, more research on experimental drugs is necessary to propose better treatments.
OBJECTIVE: This study aimed to identify the molecular alterations of pancreatic islets in type 2 diabetes through multi-omics data integration and possible pharmacological targets using bioinformatics methods.
METHODS: In this study, the OmicsNet tool was used to integrate the multi-omics data associated wit

UNLABELLED: Vascular dysfunction is considered a consequence of diabetes. However, in pancreatic islets, some hemodynamic changes occur before the onset of symptoms. The underlying mechanisms driving islet vascular abnormalities have not been fully characterized, but islet pericyte dysfunction seems to be an early event in the pathogenesis of type 1 diabetes in humans. It remains to be investigated, however, how abnormal pericyte physiology affects their ability to regulate islet blood flow and vascular permeability. To address this issue, we treated mice with multiple subdiabetogenic doses of

BACKGROUND: Adjuvant chemotherapy (AC) provides an improved long-term survival chance for those with R0/R1 resection, following limited survival improvement space, as well as intolerable side effects. There is currently a gap in the use of macromolecule-targeted drugs (nimotuzumab) added to AC, so we developed this study.
MATERIALS AND METHODS: Resectable pancreatic cancers (RPC) were treated with surgery and postoperative chemotherapy with or without nimotuzumab. Demographic and clinical data were collected from the electronic medical records of Ruijin Hospital from May 2016 to July 2022. Th
