Laparoscopic Distal Pancreatectomy for Lymph Node Metastasis around Splenic Artery from Hepatocellular Carcinoma in a Patient with Portal Annular Pancreas.

INTRODUCTION: Lymph node metastases after hepatocellular carcinoma (HCC) resection exist, although they are not common. However, solitary metastasis to the splenic artery lymph node with suspected pancreatic invasion after HCC resection is rare. In certain cases, surgical resection is performed to improve patient outcomes. We report a case of lymph node metastasis resected by laparoscopic distal pancreatectomy (LDP) in a patient with a unique anatomical anomaly known as portal annular pancreas (PAP). CASE PRESENTATION: A 79-year-old Japanese man underwent laparoscopic left lateral segmentecto

Challenges and outcomes of pancreato-duodenectomy in portal annular pancreas: A single center experience with a systematic review of the literature.

BACKGROUNDS/AIMS: Portal annular pancreas (PAP) is an anomaly where pancreatic tissue surrounds the portal vein (PV). We present a case series from our institution and a systematic review of PAP patients who underwent pancreatoduodenectomy (PD). METHODS: We conducted a retrospective review of patient records from a tertiary referral center, from January 2014 to June 2024, who underwent PD to identify those with PAP. Additionally, a literature search was performed and articles discussing PAP patients who underwent PD were included. RESULTS: The incidence of PAP was 0.4% (7 out of 1,750 PD cas

Clinical insights from a rare case of neonatal upper gastrointestinal obstruction complicated by H-Type tracheoesophageal fistula: case report and review of the literature.

We present a rare case of a 3-day-old male infant presenting with both annular pancreas (AP) and H-type esophageal atresia (H-TEF). Initially, our focus was solely on the upper gastrointestinal malformation in the child, and the misdiagnosis of a concurrent Tracheoesophageal fistula (TEF) was corrected only after a secondary surgery. Therefore, this article aims to discuss methods for accurately diagnosing this rare anomaly and performing timely surgery to improve the prognosis of the child and enhance their quality of life.

Successful Laparoscopic-Assisted Pancreaticoduodenectomy for a Neuroendocrine Tumor of the Papilla of Vater in Type 1 Portal Annular Pancreas.

INTRODUCTION: Portal annular pancreas (PAP) is a rare anomaly of pancreatic embryology that is classified into three types according to the position of the main pancreatic duct. PAP type 1, in which the main pancreatic duct runs dorsal to the pancreas, is extremely rare. Herein, we describe a case of successful laparoscopic-assisted pancreaticoduodenectomy in a patient with type 1 PAP. CASE PRESENTATION: A 72-year-old Japanese woman with neck swelling was referred to our hospital. CT at admission showed dilation of the main pancreatic duct. After a thorough examination, a preoperative diagnos

Annular Pancreas: Insights Into the Diagnosis of a Rare Anatomical Malformation in Pediatric Patients.

Annular pancreas is a rare congenital abnormality, detected in both pediatric and adult patients. Normally, the pancreas arises from the fusion of two endodermal buds of the caudal foregut, the dorsal and ventral pancreatic buds, during the first weeks of gestation. However, defects in embryonic development can lead to the formation of an annular pancreas. Being an uncommon clinical entity, a high level of suspicion is crucial for early diagnosis. We present a case of an annular pancreas in a neonate who was initially referred to our department with a prominent diagnosis of esophageal atresia.

Management of Pancreatectomy for Pancreatic Cancer in a Patient With Annular Portal Pancreas: A Case Report.

Portal annular pancreas (PAP) is one of the rare pancreatic anomalies in which the pancreatic parenchyma surrounds the portal vein (PV) or superior mesenteric vein (SMV), accounting for only around a few proportions of all patients. PAP is thought to be associated with the high risk of postoperative pancreatic fistula (POPF) after pancreatectomy. We describe our experience of a case with PAP and review the literature on pancreatectomy in patients with PAP. A 72-year-old male presented to our department with a pancreatic body mass with a history of previous abdominal surgeries, who underwent di

Robotic versus laparoscopic approaches to neonatal annular pancreas: a comparative analysis of surgical outcomes and innovation trade-offs.

OBJECTIVE: To evaluate the safety and to efficacy of robotic-assisted duodenoduodenostomy (RAD) and laparoscopic-assisted duodenoduodenostomy (LAD) for neonatal annular pancreas and analyze its associated learning curve. METHODS: A retrospective analysis of 59 neonates undergoing minimally invasive DS (2019-2024) was performed. Patients were stratified into robotic (RAD, n = 25) and laparoscopic (LAD, n = 34) cohorts. Perioperative outcomes (operative time, anastomosis time, blood loss, drainage duration, complications) were compared. Learning curves for console docking and intracorporeal man

Cross-sectional imaging features of duodenal obstruction: A case series highlighting etiologic diversity.

Duodenal obstruction is an uncommon but potentially life-threatening cause of upper gastrointestinal obstruction. Its clinical presentation is often nonspecific, necessitating prompt and accurate imaging for diagnosis and therapeutic planning. We present a series of 6 cases of duodenal obstruction evaluated in our radiology department, highlighting a broad spectrum of underlying etiologies, including congenital, inflammatory, neoplastic, and vascular-mechanical origins. Imaging modalities included contrast-enhanced abdominopelvic computed tomography (CT) in 5 patients and magnetic resonance im

Multiseptate gallbladder accompanied by an incomplete annular pancreas in a 19-year male patient: A case report.

Multiseptate gallbladder (MG) is a rare congenital malformation that may occur as an isolated anomaly or in conjunction with other abnormalities of the pancreaticobiliary, urinary, and gastrointestinal systems. The coexistence of MG with an annular pancreas is an exceedingly rare phenomenon. The diagnosis is typically established using abdominal ultrasound; however, further imaging modalities are often required to delineate the detailed anatomy and to identify any associated anomalies within the pancreaticobiliary system. We present a case of a symptomatic MG associated with an incomplete annu

Clinical Analysis of Congenital Duodenal Obstruction and the Role of Annular Pancreas.

Background and Objectives: Congenital duodenal obstruction (CDO) is a very rare anomaly with an incidence of 1 in 5000-10,000 live births. Annular pancreas is one of the reasons for CDO and is defined as the pancreatic tissue encircling the duodenum. The exact cause of annular pancreas remains unclear. Materials and Methods: A retrospective study was conducted on all prenatally diagnosed cases of CDO, with the diagnoses and ethiology confirmed by surgery after birth from 1 January 2018 to 1 January 2024. The cases suspected of having CDO in their fetuses underwent ultrasound evaluations on a w

Mayer-Rokitansky-Küster-Hauser syndrome type II. Case report.

Mayer-Rokitansky-Küster-Hauser syndrome is a rare congenital disorder characterized by Müllerian agenesis while preserving secondary sexual characteristics and a normal 46XX karyotype. It is classified into two types, with type II involving additional multisystemic anomalies affecting the skeletal, renal, and cardiovascular systems. Due to its rarity, epidemiological data on prevalence and incidence are lacking, and no cases have been previously reported in our country. We present the case of a 48-year-old female patient diagnosed with Mayer-Rokitansky-Küster-Hauser syndrome type II at the age

"Partial Annular Pancreas Causing Obstruction of the First Part of the Duodenum: An Exceedingly Rare Conundrum-A Rare Case Report and Comprehensive Literature Review".

The annular pancreas is one of the very few congenital anomalies associated with gastric outlet obstruction. Usually, it is associated with obstruction of the second part of the duodenum, but to the authors' knowledge, this is the first reported case of the partial annular pancreas obstructing the first part of the duodenum, leading to late presentation of gastric outlet obstruction. As clinicians, we should be vigilant about the causes of abdominal fullness and pain and consider the annular pancreas an important differential diagnosis. It also challenges our predetermined concept of obstructi

Annular Pancreas in a 13-Year-Old Boy: A Delayed Clinical Presentation of a Congenital Anomaly Highlighting Challenges of Diagnosis.

INTRODUCTION: The annular pancreas is a rare congenital anomaly that typically results in duodenal obstruction during the neonatal period. However, its presentation can be variable, with some cases remaining undiagnosed until adolescence and posing diagnostic challenges, especially in low-resource settings where advanced imaging may be limited. CASE PRESENTATION: We report the case of a 13-year-old boy with a three-year history of recurrent non-bilious projectile vomiting and epigastric pain. Despite normal laboratory findings, plain computed tomography (CT) and post-intravenous (IV) contrast

A Case of Robot-Assisted Pylorus-Preserving Pancreatoduodenectomy for Branch-Duct Intraductal Papillary Mucinous Neoplasms Complicated With an Annular Pancreas.

Annular pancreas is a rare congenital anatomical anomaly, in which the pancreatic parenchyma surrounds the descending duodenum. Generally, annular pancreas is diagnosed on the basis of symptoms associated with complications of peptic ulcer, pancreatitis, cholelithiasis, and rarely, malignant tumors. Herein, we report an 84-year-old man for whom, during hospitalization for a urinary tract infection, pancreatic cystic lesions and an annular pancreas were noted incidentally on computed tomography. These findings led to a diagnosis of intraductal papillary mucinous neoplasms on further examination
