Hepatic methionine-cycle dysregulation is associated with immune-inflammatory and fibrogenic remodeling in biliary atresia.

BACKGROUND: Biliary atresia (BA) is a progressive fibroinflammatory cholangiopathy leading to liver failure in infancy. The contribution of methionine metabolism to BA remains incompletely defined. This study investigated hepatic methionine-cycle alterations and the potential therapeutic effects of S-adenosylmethionine (SAMe). METHODS: Public bulk transcriptomic and single-cell RNA sequencing datasets were analyzed to characterize methionine-cycle-related alterations in BA. Selected genes were validated in clinical liver tissues. Methionine-cycle metabolites, oxidative stress indicators, and

Delayed Thoracoscopic Repair Following Abdominal Esophageal Banding in a 539-g Infant With Triple Atresia: A Case Report.

Triple atresia, comprising esophageal atresia, duodenal atresia, and anorectal malformation, has no established management strategy in extremely low-birth-weight (ELBW) infants. We report successful staged treatment in a 539-g infant with severe respiratory failure. Transabdominal esophageal banding and gastrostomy on Day 0 controlled airflow through the tracheoesophageal fistula without thoracic intervention. Duodeno-duodenostomy and transverse colostomy were performed on Day 18 (680 g), permitting enteral feeding. After growth to 1924 g, thoracoscopic repair of Gross type C esophageal atresi

Between technique and virtue: a bioethical reflection on the management of patients with anorectal malformations.

Anorectal malformations (ARM) present complex clinical and ethical challenges that extend from neonatal surgical decision-making to long-term functional rehabilitation. This review explores three bioethical issues that emerge in the care of patients with ARM: first, the application of Beauchamp and Childress's principles of biomedical ethics to surgical management and rehabilitation; second, the identification of an ethical framework to navigate the intersection between clinical and ethical decision-making in rare diseases; and third, the ethical challenges related to acquiring professional co

Integrative transcriptomic and single-cell analysis reveals a convergent cholangiocyte-associated host inflammatory program in biliary atresia and SARS-CoV-2-associated hepatobiliary injury.

SARS-CoV-2-associated hepatobiliary injury and biliary atresia (BA) differ markedly in etiology and biological context, but both involve injury to the biliary epithelial compartment. Here, we performed integrative transcriptomic and single-cell analyses to investigate whether these distinct hepatobiliary injury contexts converge on a shared cholangiocyte-associated inflammatory host-response program. Bulk transcriptomic datasets from SARS-CoV-2-infected human liver organoids and BA liver tissues were integrated to identify shared transcriptional alterations, followed by functional enrichment,

Fetal Structural Anomalies and Vasa Previa Newly Identified After an Unremarkable Mid-Trimester Systematic Ultrasound Screening: Clinical Significance and Follow-Up Findings in 47 Cases.

BACKGROUND: Some fetal structural anomalies and cases of vasa previa may not be detectable until after mid-trimester ultrasound screening. This study evaluated the spectrum, sonographic features, and perinatal outcomes of these newly detected abnormalities. METHODS: We retrospectively reviewed 14 579 singleton pregnancies that underwent standardized mid-trimester ultrasound screening with no structural abnormalities detected at that time. Among these, 47 fetuses had anomalies newly identified before delivery. One additional pregnancy, in which multiple malformations were already detected at m

Adult intestinal malrotation: imaging findings.

Intestinal malrotation is the most common congenital anomaly of the small intestine. However, it is associated with delayed diagnosis due to the lack of specificity of its symptoms, which can lead to devastating consequences such as intestinal volvulus or massive intestinal necrosis. We present a clinical case in which we highlight the importance of abdominal computed tomography and the detection of its characteristic signs for the early identification of this pathology.

How to Deal with Median Arcuate Ligament during Pancreaticoduodenectomy? Management Strategies and Impact on Perioperative Outcomes.

BACKGROUND: Median arcuate ligament (MAL) can cause coeliac trunk (CT) stenosis, which is often asymptomatic but may lead to severe hepatic, splenic, or digestive ischemia following pancreaticoduodenectomy (PD). Optimal diagnosis and management remain debated. The purpose of this study was to evaluate management strategies and perioperative outcomes of patients with MAL undergoing PD. PATIENTS AND METHODS: A retrospective multicenter study across ten high-volume French centers included patients diagnosed with MAL (hemodynamically significant or not) who underwent PD between 2005 and 2024. Serum Adiponectin and Fibroscan as Non‑invasive Post‑operative Prognostic Markers for Biliary Atresia.

BACKGROUND: Majority of patients with biliary atresia (BA), including those who undergo Kasai hepatico‑portoenterostomy, develop cirrhosis, hepatic fibrosis and eventually progress to end‑stage liver disease. Therefore, early assessment of hepatic status and fibrosis is essential. The study objective was to estimate serum adiponectin, liver function tests and to associate them with liver stiffness scores in post‑operative BA patients. MATERIALS AND METHODS: A total of 30children (20 post‑Kasai BA cases and 10 healthy controls) were enrolled in this prospective case-control study. Serum adipon

Redox Imbalance and Impaired Protein Clearance Drive Chronic Liver Injury in Biliary Atresia.

INTRODUCTION: Excessive or uncontrolled generation of reactive oxygen species (ROS) is closely associated with the development of liver diseases. However, direct evidence linking oxidative stress to impaired hepatic function in human biliary atresia (BA) remains limited. To address this, we performed bulk RNA sequencing to investigate (1) alterations in hepatic oxidative stress and antioxidant defense mechanisms and (2) transcriptomic changes associated with impaired hepatic function in BA. METHODS: Total RNA was extracted from liver samples of patients with BA and non-BA cholestasis. Bulk RN

Esophageal atresia type III? Intraoperative surprise and different approaches.

A newborn was referred due to clinical and radiological suspicion of esophageal atresia (EA) type III. Surgery revealed an esophagus without evident interruptions; however, intraoperative advancement of the nasogastric tube was unsuccessful, and the distal esophagus inflated with each ventilation, indicating the presence of a distal fistula. An intraoperative esophago-tracheobronchoscopy showed a proximal esophageal pouch with a tiny tracheoesophageal fistula and a large distal tracheoesophageal fistula. The esophageal ends were blind but overlapping, with no external discontinuity observed. W

A rare intersection: choledochal cyst (Todani IC) and incomplete pancreas divisum in an adult female. Diagnostic and therapeutic insights.

We report the case of a 59-year-old woman with a history of smoking, hypertension, and prior surgeries. She presented with acute right upper quadrant abdominal pain radiating to the back. Laboratory tests revealed elevated liver enzymes and imaging studies (CT and MRCP) showed a dilated common bile duct. Endoscopic ultrasound confirmed an incomplete pancreas divisum and a Todani type 1C bile duct cyst. The patient underwent hepaticojejunostomy anastomosis, with resolution of symptoms at follow-up. This is the first documented case of a choledochal cyst and incomplete pancreas divisum coexistin

Individuals with reported and novel KDM5C variants present with seizures, a feature recapitulated in a Drosophila model.

Variants that disrupt the function of the chromatin regulator KDM5C cause a rare neurodevelopmental disorder (KDM5C-NDD) characterized by intellectual disability, seizures, and a broad range of systemic features. To better understand this disorder, more detailed and standardized information is required regarding the association between these genetic variants and cognitive and behavioral traits. Utilizing data obtained by the RARE-X KDM5C Data Collection Program, we analyzed survey and genetic data from 31 newly reported individuals. In addition to the expected neurodevelopmental challenges, pa

Lower Thoracic Epidural Block in Diaphragmatic Plication: A Case Report.

Acquired diaphragmatic eventration (DE) is an uncommon condition characterized by all or a portion of the diaphragmatic muscle replaced by a thin fibroelastic sheath, resulting in an abnormal elevation of the hemidiaphragm. Currently, the treatment of choice for symptomatic DE is a minimally invasive diaphragmatic plication via a video-assisted thoracoscopic or robotic approach. Surgical diaphragmatic plication is often associated with increased intra-abdominal pressure (IAP) and associated complications. This report describes the usefulness of lower thoracic epidural block in the management o
