What's in a name: Imaging nomenclature in variations of sex development.

STUDY OBJECTIVE: In patients with variations of sex development (VSD), phenotype may not match with karyotype and/or gonadal tissue. The neutral term "gonad" is often preferred (instead of "ovaries" or "testes") to avoid confusion. This study describes the variability of nomenclature in imaging reports. METHODS: This is a retrospective cohort study of patients from 2007 to 2021 with a diagnosis of complete androgen insensitivity syndrome (CAIS), 45X/46XY or 46XX/46XY sex chromosomal mosaicism (SCM), and 46XY gonadal dysgenesis (GD). RESULTS: 69 reports were analyzed, including 18 pelvic ultr

Virilization and Hyperandrogenism in Turner Syndrome without Y Mosaicism: A Case Report and Review of the Literature.

BACKGROUND: Up to 10%-12% of individuals with Turner syndrome (TS) have Y chromosome material (eg, 45,X/46,XY), which can lead to virilization. We present the case of a patient with TS who developed virilization due to ovarian hilus cell (OHC) hyperplasia. CLINICAL CASE: An 11-year-old with TS presented with clitoromegaly, hyperandrogenism, and primary ovarian insufficiency (POI). Thorough workup was unable to identify the source of hyperandrogenism. She underwent laparoscopic bilateral gonadectomy and was diagnosed with OHC hyperplasia on pathologic evaluation. Hyperandrogenism resolved post

Parietal Lobe Epilepsy Associated With 2q13 Duplication: Expanding the Neurogenetic Spectrum.

Copy number variations (CNVs) involving the 2q13 region have been associated with a wide range of phenotypes, including developmental delay, dysmorphic features, hypotonia, and congenital heart defects. However, parietal lobe epilepsy has not yet been reported in association with 2q13 duplication. We describe the case of a 20-year-old woman with a duplication of the 2q13 region identified during childhood, who later presented with recurrent, brief episodes of right upper limb paresthesia spreading to other limbs, followed by transient pain. Her past medical history included dyspraxia, polycyst

Is it possible to separate the testicular and ovarian components of an ovotestis?

UNLABELLED: The presence of an ovotestis is a rare etiology of differences of sex development. Histologically, ovotestis come in two forms: 1) Mixed or 2) Bipolar. OBJECTIVE: We hypothesize that it is surgically impossible to cleanly separate and preserve either the testicular or ovarian component of an ovotestis. STUDY DESIGN: Twenty human gonads with a previous diagnosis of ovotestis were re-sectioned in entirety and re-stained with markers for testicular (SOX9, TSPY, SALL4, DDX4, cP450, AR, α-actin) and ovarian tissue (FOXL2, SALL4, DDX4). Histologic sections were photographed at low powe

Frequency of Polycystic Ovary Syndrome and “Being at Risk for Polycystic Ovary Syndrome” in Obese Adolescent Girls in Light of Current Definitions.

OBJECTIVE: Obesity is associated with an increased risk of polycystic ovary syndrome (PCOS). It can be difficult to differentiate between PCOS and physiological oligomenorrhoea/anovulation in adolescent girls. To date, studies of the prevalence of PCOS in adolescents have predominantly used diagnostic criteria validated primarily in adult women. The aim of this study was to investigate the prevalence of PCOS in obese girls using the current diagnostic criteria for adolescents. METHODS: The diagnosis of PCOS was based on the presence of menstrual irregularity, clinical hyperandrogenism and hyp

Alterations of insulin sensitivity, clearance, and secretion, either alone or in combination, in women with PCOS: impact on metabolic profile and androgenemia.

STUDY QUESTION: What is the role of insulin resistance (IR), increased insulin secretion, and impaired insulin clearance, either alone or in combination, on metabolic features and androgen levels in women with PCOS? SUMMARY ANSWER: IR, reduced insulin clearance and increased insulin secretion independently contribute to predicting hyperandrogenemia, whereas metabolic abnormalities may differ according to the mechanisms underlying hyperinsulinemia. WHAT IS KNOWN ALREADY: Hyperinsulinemia is a common finding in women with PCOS, and it is closely associated with the metabolic and endocrine alte

Update on diagnosis of polycystic ovary syndrome during adolescence.

Polycystic ovary syndrome (PCOS) is a common endocrine and metabolic condition beginning during the peripubertal years. The international evidence-based guidelines, updated in 2023, have clearly defined the diagnostic criteria during adolescence with the goal of decreasing controversies and challenges with the diagnosis during this critical life stage. In this narrative review, we aimed to examine the recent available data related to the diagnosis of PCOS during adolescence. Specifically, we evaluated publications from August 2022 (date when the last searches were run for the update of the int

Impact of combined hormonal contraceptives and metformin on metabolic syndrome in women with hyperandrogenic polycystic ovary syndrome and obesity: The COMET-PCOS randomized clinical trial.

BACKGROUND: The risk-to-benefit ratio of using combined oral contraceptive pills (COCPs) and/or metformin for comprehensive management of polycystic ovary syndrome (PCOS) in women with obesity is unclear. As there is a lack of robust evidence on the impact of these first-line medications on cardiovascular disease (CVD) risk, we compared the effect of COCPs, metformin or both on prevalence of metabolic syndrome (MetS) in participants with hyperandrogenic PCOS and hypothesized that COCPs would increase prevalence of MetS while metformin would decrease prevalence of MetS. Expression Patterns of Escape Genes in Turner Syndrome Fibroblasts and Induced Pluripotent Stem Cells.

Turner syndrome (TS) is an X monosomy-related disorder caused by X chromosome nondisjunction during embryonic development. Patients with TS have only one intact X chromosome, with the other either completely or partially lost. TS affects various tissues, including the liver, kidneys, brain, cardiovascular system, and ovaries. These abnormalities are suggested to involve an altered dosage of escape genes that evade X chromosome inactivation. However, the mechanisms and roles of these escape genes in the TS phenotype remain unclear. We hypothesized that the expression levels of escape genes diff

A Rare Autopsy Finding of Müllerian and Renal Agenesis Linked to Suspected AMHR2 Pathway Disruption.

We present a rare incidental finding of Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome identified during the autopsy of a reproductive-aged woman who died in a motor vehicle accident. In situ examination showed the absence of both the uterus and cervix, with the vaginal canal ending in a blind pouch that measured 7.0 × 6.0 cm, with a depth of 4 cm. Both ovaries were present and positioned high near the iliac vessels, demonstrating multiple cysts upon cut surfaces. The bilateral fallopian tubes were absent in their entirety. The right kidney, along with its hilar vessels and ureter, was also ab

Hyperandrogenism in polycystic ovary syndrome and adrenal hyperplasia: finding differences to make a specific diagnosis.

PURPOSE: Polycystic Ovary Syndrome (PCOS) and Adrenal hyperplasia (CAH) are two pathologic conditions sharing several clinical features (hirsutism, acne, polycystic ovary morphology, metabolic alterations, ovulatory dysfunctions) and especially hyperandrogenism as a common clinical hallmark. Therefore, making a differential diagnosis of the two conditions still remains a great medical challenge. METHODS: In particular, the comparison discussed in this review referred to non-classical form of adrenal hyperplasia (NCAH), which regards the adult population, and the Endocrine Metabolic Syndrome (

[Hirsutism: a common problem; when to consider rare causes?].

Excessive hair growth is a common and distressing complaint in women. It is imperative to differentiate excessive hair growth from hirsutism with possible other signs of virilization. Hirsutism is commonly attributed to polycystic ovary syndrome (PCOS). Hirsutism with rapid onset or progression, potentially combined with other signs of virilisation or secondary amenorrhea, should prompt evaluation of other underlying causes. In this clinical lesson, we present three cases of hirsutism to illustrate red flags to consider rare diagnoses. Diagnoses include non-classical congenital adrenal hyperpl

Analysis of Hirsutism as a Symptom of Ihtibās al-Tamth (Amenorrhea) and its Relation to Polycystic Ovary Syndrome.

BACKGROUND: Hirsutism, defined as excessive terminal hair growth in females in a male pattern, is a significant clinical manifestation of increased androgen. Unani scholars such as Hippocrates, Haly Abbas, Rhazes, and Avicenna linked hirsutism with menstrual irregularities, specifically Ihtibās al-Tamth (Amenorrhea). Modern medicine also associates hirsutism with hyperandrogenism, a hallmark of polycystic ovarian syndrome (PCOS), among other conditions. OBJECTIVE: This study aims to explore the understanding of hirsutism from both Unani classical literature and modern medical perspectives, pa

Metformin use in women with polycystic ovary syndrome (PCOS): Opportunities, benefits, and clinical challenges.

Metformin, a synthetic biguanide, is widely used to manage type 2 diabetes, and is commonly prescribed in polycystic ovary syndrome (PCOS) to address insulin resistance and associated metabolic and reproductive disturbances. PCOS is characterised by hormonal imbalances such as hyperandrogenism and anovulation, metabolic abnormalities including insulin resistance and increased cardiometabolic risk, and higher rates of pregnancy complications. However, the role of metformin in the multifaceted nature of PCOS remains debated. This review synthesises the mechanisms of action of metformin and its e

The Effects of Maternal Endocrinopathies and Exposure to Endocrine Disruptors During Pregnancy on the Fetus and Newborn.

Maternal health has a profound impact on fetal development, influencing the risk of pediatric endocrine disorders both directly and indirectly through various biological and environmental mechanisms. Throughout pregnancy, several endocrine disorders can arise or be exacerbated due to the physiological changes that occur. An in-depth review of articles with evidence-based research discussing the significant effects of maternal endocrinopathies and endocrine disruptors on fetal development and infant health was conducted in this review paper. The most common endocrine disorder during pregnancy i
