A comparative meta-analysis and systematic review of gubernaculum sparing versus gubernaculum excision orchidopexy.

BACKGROUND: Cryptorchidism is one of the most common congenital abnormalities and it is associated with impaired fertility, increased malignancy risk, and testicular atrophy. Orchidopexy is the standard treatment for intra-abdominal testes, but the optimal surgical approach remains debated, particularly regarding the role of gubernacular sparing. OBJECTIVES: To compare gubernaculum-sparing and gubernaculum-excision orchidopexy for abdominal cryptorchidism in pediatric patients, focusing on postoperative testicular atrophy rates. MATERIALS AND METHODS: A systematic review and meta-analysis we

Persistent Mullerian duct syndrome.

Persistent Müllerian Duct Syndrome is a rare genetic disorder, consisting in the persistence of the müllerian structures in males. We report the case of a 29-year-old male patient presented with an abdominopelvic mass. Radiological examination showed a 30cm abdominopelvic mass that was biopsied, and the pathological examination concluded to a testicular seminoma. The patient underwent surgery after neoadjuvant chemotherapy, where a uterus with right adnexa were discovered. This case highlights the therapeutic challenges of persistent Müllerian duct syndrome and the importance of the early diag

The HIF-Mediated PI3K-AKT Signaling Pathway Is a Key Signaling Pathway Triggering Testicular Spermatogenic Disorders in Yaks with Cryptorchidism.

Cryptorchidism is one of the major reproductive diseases affecting testicular function in yaks. However, the mechanisms underlying its impact on testicular spermatogenesis remain unclear. In this study, high-throughput transcriptomics (RNA-seq) and proteomics technologies were employed to analyze the key signaling pathways involved in cryptorchidism-induced spermatogenic dysfunction in yak, and a mouse model was established for validation. The results indicate that differentially expressed genes (DEGs) in the testes of yak with cryptorchidism are primarily enriched in the hypoxia-inducible fac

Clinical Outcomes of Orchiopexy and the Risk of Malignancy in Postpubertal Cryptorchid Patients.

BACKGROUND: Postpubertal cryptorchidism remains clinically challenging, as patients often present with long-standing maldescent, impaired spermatogenesis, and an increased risk of testicular malignancy. However, the optimal management of this population remains controversial, and comprehensive data on clinical characteristics, surgical outcomes, endocrine changes, and spermatogenic damage are still limited. METHODS: We conducted a single-center retrospective cohort study of 194 patients (210 undescended testes) undergoing primary orchiopexy between January 2018 and June 2024. Clinical charact

The Impact of Congenital Anomalies of the Male and Female Reproductive Organs on Infertility and Recurrent Pregnancy Loss: A Review.

Congenital anomalies of the reproductive system represent a heterogeneous group of structural and functional abnormalities affecting both male and female genital organs. These anomalies typically arise during embryogenesis and may remain asymptomatic until they are incidentally identified during evaluation for infertility, recurrent pregnancy loss, or disorders of sexual development. In females, abnormalities include Müllerian duct anomalies and congenital malformations of the uterus, cervix, vagina, and ovaries, such as Mayer-Rokitansky-Küster-Hauser syndrome, septate, unicornuate, bicornuate

Giant seminoma of the testis (2.51 kg) in a setting of severe occupational and lifestyle exposures: a case report and pathophysiological insights.

We report a case of a giant testicular seminoma in a late 30s male with a two-year history of progressive left scrotal enlargement, who presented with a 22 × 14 × 12 cm mass weighing 2.51 kg. The patient had a significant occupational history of working as a barbecue vendor with daily exposure to high-temperature cooking oil fumes for 3 years, accompanied by extreme lifestyle disruption including approximately 20 working hours per day, less than 4 h of sleep, and heavy smoking and alcohol consumption. Although the exact timing of tumor initiation cannot be determined, these exposures may have

Incidental discovery of persistent Müllerian duct syndrome with transverse testicular ectopia during adult inguinal hernia repair: A case report.

Transverse testicular ectopia (TTE) associated with persistent Müllerian duct syndrome (PMDS) is a rare congenital anomaly that may remain undiagnosed until adulthood. We report a 27-year-old man with a right inguinal hernia, non-palpable left testis, and primary infertility. During hernia repair, exploration revealed a uterus, bilateral fallopian tubes, and an atrophic ectopic left testis within the right inguinal canal. Müllerian structures were excised, left orchidectomy performed, and the right testis preserved by orchidopexy. Histopathology showed no malignancy. Postoperative evaluation d

Testicular remnant phenotype, histology, and ultrasonographic findings in non-palpable testes: implications for surgical management.

BACKGROUND: In boys with non-palpable testes (NPT), the clinical significance of excised testicular remnants-and whether gross phenotype predicts histologic viability-remains uncertain. We evaluated remnant phenotype, histology, and ultrasonographic (US) findings to inform surgical management. METHODS: We retrospectively reviewed boys undergoing surgical exploration for NPT (2010-2025) following examination under general anesthesia. Testes were classified as intra-abdominal (IAT) or excised remnants; remnants were categorized as nodular versus non-nodular and stratified by internal ring patte

A Review on the Effects of Atrazine on Male Rat Reproductive System Cytoarchitecture, Steroidogenesis and Oxidative Pathway.

Atrazine (2-chloro-4-ethylamino-6-isopropylamino-s-triazine) is an active component of herbicides used worldwide in agriculture. Atrazine is an endocrine disruptor, capable of interfering with the delicate hormonal balance necessary for normal reproductive function. Despite numerous studies on atrazine's impact on male reproduction, significant gaps remain, particularly concerning the male genital tract beyond the testes. Therefore, we reviewed the effects of atrazine on the entire male reproductive system to understand the complexity of its actions. Adverse effects of atrazine on the morphoph

Progressive Pseudorheumatoid Dysplasia Presenting with Cauda Equina Compression: A Case Report.

PURPOSE: Fresh vegetables are essential human dietary components but are increasingly implicated in foodborne infections caused by Escherichia coli. Contamination may occur during cultivation, irrigation, handling, and retail display, and the presence of pathogenic or antimicrobial-resistant E. coli poses significant public health risks. Despite widespread vegetable consumption in Ogun State, Nigeria, limited data exist on the resistance and virulence characteristics of E. coli in these foods. This study therefore assessed antimicrobial resistance profiles and virulence genes in E. coli isolat

Injury and recovery patterns of the blood-testis barrier and spermatogenic function in a surgical unilateral cryptorchidism model in rats.

BACKGROUND: Cryptorchidism is one of the most common congenital disorders of the pediatric urogenital system. In addition to regulating the passage of substances into and out of the seminiferous epithelium, the blood-testis barrier (BTB) plays a critical role in maintaining a stable microenvironment for spermatogenesis. BTB alterations in cryptorchidism remain poorly characterized. This study aimed to establish a surgical unilateral cryptorchidism model in rats (SUCMR) and to investigate the injury and recovery patterns of both the BTB and spermatogenic function within this model. METHODS: Th

All-in-one exome sequencing approach for genetic testing of unexplained premature ovarian insufficiency.

STUDY QUESTION: Is exome sequencing (ES) an efficient approach for simultaneous analysis of causative single gene defects and copy number variants (CNV) in unexplained premature ovarian insufficiency (POI)? SUMMARY ANSWER: Among 51 idiopathic POI cases, a conservative molecular diagnostic yield of 12% was achieved with equal contribution of pathogenic or likely pathogenic (P/LP) monogenic causes and pathogenic microdeletions. WHAT IS KNOWN ALREADY: POI is a clinically and genetically heterogeneous condition, yet most cases remain idiopathic. Although numerous monogenic causes and CNVs have b

Case Report: Pseudo-prune belly syndrome after early fetal bladder decompression: implications for the pathogenesis of prune belly spectrum disorders.

BACKGROUND: Prune Belly Syndrome (PBS) is a rare congenital disorder characterized by abdominal wall deficiency, urinary tract dilatation, and bilateral cryptorchidism. Its pathogenesis remains controversial, with both developmental and mechanical theories proposed. Whether early fetal decompression influences the phenotypic expression of PBS has not been established. CASE PRESENTATION: We report a male fetus diagnosed at 12 weeks' gestation with severe megacystis and bilateral hydroureteronephrosis. A Somatex® vesicoamniotic shunt was placed at 16 weeks because of progressive lower urinary t

Prenatal and postnatal manifestations of WBP11-related disorder in Chinese patients: expanding the phenotypic and mutational spectrum.

BACKGROUND: Heterozygous pathogenic variants in WBP11, a spliceosome-associated gene, have recently been linked to VACTERL syndrome, yet prenatal manifestations and genotype–phenotype correlations remain poorly characterized. METHODS: Genomic DNA was extracted from fetal tissues, amniotic fluid cells, or peripheral blood samples for trio-based whole-exome sequencing (WES) to identify potential genetic etiologies. The structural stability of the WBP11 protein associated with missense variants was characterized using 3D protein structural modeling. The impact of splicing variant was further eval

Extensive handlebar narrow pedicle penile degloving injury in a child: A case report.

Bicycle handlebar trauma rarely causes extensive perineogenital degloving in children. A 13-year-old boy sustained an 11 × 11 cm degloving injury involving the suprapubic, penile, and perineal regions, with narrow-based skin flaps and disruption of superficial penile vessels. He underwent wound exploration, washout, debridement, and layered closure. The suprapubic flap subsequently necrosed, while the penile skin remained viable, and the wound healed progressively by secondary intention. Follow-up revealed scar-related acquired cryptorchidism and mild dorsal chordee. Prompt surgical management

Biliary Cirrhosis in Myhre Syndrome: The First Case Report of Liver Transplantation and a Review of Reported Hepatic Findings.

Myhre syndrome is a rare autosomal-dominant disorder caused by gain-of-function pathogenic variants in SMAD4 and is now recognized as a progressive multisystem fibrotic disease. Although transforming growth factor-β (TGF-β) signaling plays a central role in hepatic fibrogenesis, hepatobiliary involvement in Myhre syndrome has not been systematically evaluated. We report the first case of Myhre syndrome complicated by rapidly progressive biliary cirrhosis requiring liver transplantation in a 15-year-old male with a confirmed SMAD4 p.Ile500Val variant. Following an infectious episode, the patien

Persistent Müllerian Duct Syndrome on 18 F-FDG PET/CT.

A 32-year-old man presenting with limb weakness was diagnosed with Guillain-Barré syndrome. Abdominal distension was noted, and imaging revealed a large, heterogeneously enhancing abdominopelvic mass with bilateral cryptorchidism and uterine-like tissue. The mass showed high FDG uptake on 18 F-FDG PET/CT, while the uterine-like tissue did not. Following surgical biopsy, histopathologic examination demonstrated seminoma and rudimentary uterine tissue, confirming persistent Müllerian duct syndrome (PMDS). PMDS is a rare autosomal recessive disorder characterized by male phenotype with cryptorchi

Incidence of Gonadal and Extragonadal Germ Cell Tumours in Patients With Klinefelter Syndrome.

BACKGROUND: Klinefelter's syndrome (KS; 47, XXY) is associated with an altered risk profile for malignancies compared with non-KS males. In particular, several reports have noted a striking association between KS and extragonadal germ cell tumours (EGCTs), especially in the mediastinum, whereas the risk of testicular germ cell tumours (TGCTs) in KS remains unclear. KS patients also have a higher prevalence of cryptorchidism (undescended testes)-a known risk factor for TGCT in the general population-yet it is uncertain if cryptorchidism confers the same cancer risk in the KS population. Incomplete male puberty: a new nosographic entity.

PURPOSE: Incomplete male puberty is a recently proposed nosographic entity defined by the development of normal secondary sexual characteristics in boys who nonetheless fail to achieve adequate testicular volume (<12 mL) during puberty due to underlying Sertoli cell dysfunction. The condition typically goes unrecognised until the first spermiogram reveals azoospermia or severe oligozoospermia. This review examines the pathophysiology, available diagnostic tools, and clinical management strategies. METHODS: A comprehensive literature search was conducted across SciSpace, PubMed, and Google Sch
