Identifying Cardiac Autoantibody Biomarkers that Predict the Severity of Chagas Cardiomyopathy.

Chagas cardiomyopathy (ChCM) affects up to 30% of patients with chronic Chagas disease and lacks reliable biomarkers for severity and conduction abnormalities. We profiled cardiac autoantibodies using 2-dimensional gel electrophoresis and mass spectrometry, followed by enzyme-linked immunosorbent assay validation and machine learning analysis. Six autoantibodies linked to glycolysis, adenosine triphosphate metabolism, and sarcomere structure distinguished ChCM stages (area under the curve: 0.87; 95% CI: 0.777-0.977) and a mean classification accuracy (0.84; 95% CI: 0.705-0.935). These findings

Chagas cardiomyopathy.

Chagas cardiomyopathy, caused by chronic infection with Trypanosoma cruzi, is a leading cause of non-ischaemic heart failure in Latin America and is increasingly recognised worldwide due to migration. Its pathogenesis is multifactorial, involving persistent parasitism, immune-mediated myocardial injury, microvascular dysfunction and progressive fibrosis, leading to a heterogeneous clinical spectrum. Four main phenotypes are recognised, including subclinical, arrhythmic, heart failure and thromboembolic, which often overlap and evolve throughout the disease course.Diagnosis relies on serologica

Treatment with low-dose bicistronic immunotherapy mitigates parasite persistence and improves serological and cardiac outcomes in adult mice infected with Trypanosoma cruzi: comparative analysis with benznidazole.

Chagas disease (CD), caused by Trypanosoma cruzi (Tc), remains difficult to treat in adults, as benznidazole (BZN) has limited curative efficacy and notable adverse effects. This study evaluated a newly-designed immunotherapy without (BCV) and with RIG-I adjuvant (BCVR). Adult C57BL/6 mice were infected and treated with BCV/BCVR (0.5 mg/kg, two doses, 21-day interval) or BZN (50 mg/kg for 6-weeks). Outcomes included parasite burden, biomarkers of inflammation, fibrosis, and heart disease, and left ventricular (LV) function. Infected/untreated mice developed high acute parasite burden, and pers

[Chagas cardiomyopathy].

Chagas disease is a chronic parasitic infection caused by Trypanosoma cruzi. Its most severe complication is chronic Chagas cardiomyopathy, which may lead to heart failure, malignant ventricular arrhythmias, and sudden cardiac death. Because the disease often evolves silently for decades, early detection of myocardial involvement remains challenging. Cardiovascular imaging plays a key role in diagnosis and risk stratification. Echocardiography provides a functional assessment and detects structural abnormalities, whereas cardiac magnetic resonance allows for tissue characterization and identif

Cardiac imaging in Chagas cardiomyopathy for phenotypic characterisation and risk stratification.

Chagas disease, caused by Trypanosoma cruzi, remains one of the leading infectious causes of non-ischemic cardiomyopathy worldwide. Although an estimated 6-7 million individuals are chronically infected, 20-30% will develop Chagas cardiomyopathy (CCM), a progressive myocardial disease characterised by conduction abnormalities, ventricular arrhythmias, focal aneurysm formation, diffuse fibrosis, and heart failure. Cardiac involvement often evolves silently over decades, and conventional clinical markers frequently underestimate early myocardial injury. Advances in cardiac imaging have fundament

Auxiliary-conditioned cross-attention with physiologically interpretable features for chagas disease detection from 12-lead ECGs.

Objective.Chagas disease remains a major public-health concern in endemic regions, and chronic cardiac involvement often manifests as conduction abnormalities detectable on standard 12-lead electrocardiograms (ECGs). Reliable automated screening remains challenging because of dataset heterogeneity and label uncertainty, particularly when combining strongly labeled cohorts with large weakly labeled repositories.Approach.We propose a hybrid architecture that integrates a 1D ResNet encoder for local ECG morphology, a bidirectional GRU for long-range temporal context, and handcrafted physiological

Heterotopic Ossification: A Rare Complication of Lateral Condyle Humerus Fracture Treatment in Children.

Pictures certainly are worth a thousand words in the case of the structure of the connective tissue skeleton of normal and diseased myocardium. This report reviews the connective tissue matrix of the normal human myocardial tissue and the pathological myocardial fibrosis in left ventricular hypertrophy due to chronic arterial hypertension in humans and in human chronic chagasic myocarditis. The myocardial connective tissue matrix was studied employing a cell-maceration method that removes the myocardial tissue non-fibrous elements, and leaves behind a non-collapsed matrix, thus allowing a bett

Role of glucocorticoid receptor expression in Chronic Chagas Cardiomyopathy: implications for inflammation and cardiac hypertrophy.

INTRODUCTION: Chronic Chagasic Cardiomyopathy (CCC) has an infectious and inflammatory nature. Recent data also suggest an association with altered regulation of glucocorticoid (GC)-mediated circuits failing to control systemic inflammation. However, the involvement of glucocorticoid receptors (GR) and their isoforms have been unexplored. MATERIALS AND METHODS: The expression of GR-α/β isoforms, 11β-hydroxysteroid dehydrogenase type-1 (11β-HSD1), inflammatory cytokines, and the GC-regulated gene tristetraprolin (TTP) in peripheral blood mononuclear cells (PBMCs) as well as GR immunoreactivity

Profile of soluble cell adhesion molecules as potential biomarkers in the cardiac stages of chronic Chagas disease.

Chagas cardiomyopathy is the most severe chronic manifestation and leading cause of mortality in the disease. Chronic inflammation, resulting from persistent infection by T. cruzi, leads to continuous immune system activation in patients with Chagas disease. The knowledge of immunological aspects can be important for the identification of biomarkers capable of indicating parasitological cure or clinical progression provides for physician's valuable tools for improved clinical evaluation. Soluble cell adhesion molecules (sCAMs) have been applied in other disease like potential biomarkers. Thus,

Beyond Trypanosoma cruzi: LINE-1 Activation as a Driver of Chronic Inflammation in Chagas Disease.

Chagas disease (CD) is endemic in Latin America, with its pathogenesis linked to Trypanosoma cruzi (Tc) persistence and autoimmune responses. This study investigates the role of LINE-1 (L1) activation in inflammation and loss of self-tolerance during Tc infection. In vitro assays evaluated the expression of genes involved in L1 regulation and interferon signaling under basal conditions and following L1 suppression via CRISPR/dCas9. In vivo analyses in a murine model included L1 and IFN expression profiling, autoantibody quantification, and histopathological assessments of liver, spleen, intest

Cardiac alterations induced by Trypanosoma cruzi extracellular vesicles and immune complexes.

BACKGROUND: Trypanosoma cruzi is a protozoan parasite responsible for American trypanosomiasis or Chagas disease (CD). This disease is characterized by the presence of cardiac or gastrointestinal symptoms in many patients during the chronic phase, with cardiac symptoms being the most common and severe, affecting approximately 30% of all patients. Although the origin of these pathologies remains unclear, several mechanisms have been proposed, involving factors related to T. cruzi and the host immune response. Extracellular vesicles (EVs) have been studied for their role in parasite-host cell co

Small-RNA sequencing identifies serum microRNAs associated with abnormal electrocardiography findings in patients with Chagas disease.

BACKGROUND: Chagas disease, caused by the parasite Trypanosoma cruzi (T. cruzi), affects around 6-7 million people in Latin America and hundreds of thousands in the United States and Europe. A main complication of chronic Chagas disease is cardiomyopathy, possibly manifesting as arrhythmias, heart failure, or sudden cardiac death. Understanding the link between T. cruzi infection and cardiomyopathy is essential for early diagnosis and adequate treatment. METHODS: We sequenced small RNAs in serum samples from 228 Chagas patients recruited in Chile, Bolivia and Italy. After bioinformatic proces

Serum Cytokine Levels and Heart Rate Variability in the Frequency Domain in Patients With Chronic Chagas Heart Disease.

The serum cytokine profile is well established in patients with chronic Chagas heart disease (CCHD). Little is known, however, about the relationship between plasma cytokine levels and frequency-domain parameters of heart rate variability (HRV) in patients with CCHD. We included patients with CCHD with ECG and/or echocardiographic abnormalities; people with no cardiac history matched by sex and age served as controls. ELISA performed cytokine quantification. The HRV in the frequency domain was used to detect parasympathetic and sympathetic dysfunction. Serum cytokine levels were higher in pati

In Vivo Imaging of Cardiac Attachment of TcI and TcII Variants of Trypanosoma cruzi in a Zebrafish Model.

Trypanosoma cruzi, the etiological agent of Chagas disease, is a parasite known for its diverse genotypic variants, or Discrete Typing Units (DTUs), which have been associated with varying degrees of tissue involvement. However, aspects such as parasite attachment remain unclear. It has been suggested that the TcI genotype is associated with cardiac infection, the most common involved site in chronic human infection, while TcII is associated with digestive tract involvement. Traditional models for T. cruzi infection provide limited in vivo observation, making it challenging to observe the dyna

Intermittent suppressive posaconazole therapy is ineffective at mitigating cardiac and digestive tract pathologies in an experimental model of chronic Chagas disease.

Infections with Trypanosoma cruzi cause Chagas disease, a chronic condition that can give rise to debilitating cardiac and/or gastrointestinal damage. However, it is unclear why only ~30% of individuals progress to symptomatic pathology, why this can take decades to become apparent, and why there is such a wide range of disease outcomes. Disease pathology is a long-term cumulative process resulting from collateral damage caused by inflammatory immune responses that continually eliminate transient parasite infections in the heart and/or gastrointestinal tract. The guiding principle behind anti-

Chagas disease, caused by the protozoan parasite Trypanosoma cruzi (T. cruzi), is a neglected disease affecting around 6 million people, with no effective antiparasitic drugs or vaccines. About 40% of Chagas disease patients develop symptomatic forms in the chronic phase of infection, chronic Chagas cardiomyopathy (CCC) or digestive forms like megaoesophagus and megacolon, while most infected patients (60%) remain asymptomatic (ASY) in the so-called indeterminate form (IF). CCC is an inflammatory cardiomyopathy that occurs decades after the initial infection. Death results from heart failure o

II - Evaluation of Autonomic Control of Heart Rate in Various Clinical Conditions.

Through the comparison of responses of normal volunteers, assessment of autonomic function under abnormal clinical conditions focused on the detection of dysautonomia involving both the parasympathetic and adrenergic limbs in patients with Chagas heart disease, post-cardiac surgery, chronic heart failure, mitral valve prolapse, and hyperthyroidism. In particular, the autonomic impairment observed in Chagas disease patients involved predominantly the parasympathetic control of heart rate at the sinus node level, and the adrenergic innervation at the myocardial ventricular level. The autonomic d

Nasal vaccination against Trypanosoma cruzi: a dual approach for prevention and treatment of chronic Chagas cardiomyopathy.

Chagas disease, caused by the parasite Trypanosoma cruzi (T. cruzi), is a neglected life-threatening disease. Given that available pharmacologic treatments are effective only in the acute phase, and that diagnosis typically occurs during the chronic phase when cardiac damage is already present, current efforts should aim to mitigate cardiac pathology during chronic infection. This study evaluates the effectiveness of a nasal vaccine based on trans-sialidase (TS) plus c-di-AMP in both prophylactic and therapeutic settings against chronic Chagas cardiomyopathy (CCC) in a mouse model of T. cruzi
