Microcirculatory, micro-rheological, metabolic and histomorphological investigation of experimental mesoblastic nephroma in rats treated with Vincristine.

The development of malignant tumors and their abnormal vasculature can lead to severe changes in tissue perfusion. In this pilot study, we aimed to investigate changes in renal microcirculation and parenchymal abnormalities without treatment and after Vincristine treatment in a rat model of mesenchymal mesoblastic nephroma (Ne/De) tumor cell implantation of rat origin, which closely resembles Wilms' tumor. On the 1st, 4th, and 9th post-operative days, blood samples were collected to test for hematological parameters, erythrocyte deformability, and aggregation. In the Vincristine-treated group

A new conceptual framework for PKD1 in ADPKD; integrating DNA structures and inflammation.

In ADPKD renal function is corrupted by the accumulation and growth of fluid-filled cysts. Disruption of the PKD1 gene product, polycystin-1, is the most frequent cause of ADPKD, but the mechanisms that predispose PKD1 to recurrent somatic mutation remain poorly understood. Because current evidence for sequence- and structure-dependent mutational susceptibility is strongest at the human PKD1 locus, we will focus here on mechanisms that may promote somatic PKD1 inactivation. Experimental evidence for polycistin-1 inactivation supports a two-hit pathway, with the first hit being an inherited ger

Generation of human induced pluripotent stem cell line (CMCi015-A) from a patient with nephronophthisis harboring compound heterozygous loss-of-function variants in the NPHP4 gene.

Nephronophthisis, an autosomal recessive ciliopathy, is a rare genetic kidney disorder characterized by progressive tubulointerstitial fibrosis, corticomedullary cyst formation, and progression to end-stage renal disease. Here, we established a human induced pluripotent stem cell (hiPSC) line from peripheral blood mononuclear cells of an 18-year-old female patient with nephronophthisis harboring compound heterozygous variants in the NPHP4 gene. This hiPSC displayed features similar to human embryonic stem cells, including pluripotency-associated markers expression, normal karyotype, and the ab

Focal double-lumen ureter causing upper ureteric obstruction in a pelvic ectopic kidney: A rare case report.

A 49-year-old woman presented with right flank pain and hematuria. Computed tomography demonstrated a nonfunctioning pelvic ectopic kidney with severe hydronephrosis and cortical thinning. Nephrectomy revealed an abnormally attached, markedly dilated ureter coursing anteriorly along the renal surface and abruptly narrowing distally. Histological examination of the transition zone demonstrated two distinct urothelial-lined lumina within a shared lamina propria and common muscularis propria and adventitia, with distal reconstitution into a single lumen. The findings support a rare focal double-l

Laparoscopic Anderson-Hynes pyeloplasty with Butler flap for pelviureteric junction obstruction in a pelvic ectopic kidney presenting as a giant palpable abdominal mass: A case report.

Pelvic ectopic kidney with pelviureteric junction obstruction is rare and poses significant reconstructive challenges. We report a 35-year-old man presenting with a giant palpable abdominal mass caused by congenital obstruction in a malrotated pelvic ectopic kidney. Diuretic renography showed 11.6% split renal function. The patient underwent successful transperitoneal laparoscopic Anderson-Hynes pyeloplasty with Butler flap reconstruction and double-J stenting. Recovery was uneventful. Six-month renography demonstrated improved drainage with split renal function increasing to 22%, while the pa

Heterogeneity in the response to the metabolic environment drives fosfomycin heteroresistance.

Heteroresistance is a form of antibiotic resistance in which a phenotypically unstable subpopulation of resistant cells coexists with a majority susceptible population. Many mechanisms of heteroresistance are unclear and the impact of the extracellular environment on this phenomenon is unknown. Studying fosfomycin heteroresistance in Enterobacter cloacae complex, we observed that glucose availability greatly increased the frequency of the resistant subpopulation. Glucose downregulated the glycerol and fosfomycin importer GlpT via catabolite repression. Heteroresistance was generated by heterog

Renal blood flow and progression to kidney failure in ADPKD.

BACKGROUND: A decrease in renal blood flow (RBF) precedes estimated glomerular filtration rate (eGFR) decline among patients with autosomal dominant polycystic kidney disease (ADPKD) and inversely associates with disease severity and progression. However, it is unknown whether RBF independently predicts progression to kidney failure in patients with ADPKD. METHODS: A total of 379 participants with early-stage ADPKD (mean age 36.6 ± 8.5 years; eGFR: 92.9 ± 17.1 ml/min/1.73 m2) who participated in the Halt Progression of Polycystic Kidney Disease Study A (HALT PKD A; NCT00283686) with baseline

Severe bilateral ocular coloboma and microphthalmia in a neonate with WDR37-associated neurooculocardiogenitourinary syndrome.

We describe a neonate with severe bilateral ocular abnormalities associated with WDR37-related neurooculocardiogenitourinary syndrome. Ocular findings identified at birth included bilateral microphthalmia, microcornea, corneal edema, marked anterior segment dysgenesis, and bilateral colobomatous posterior pole abnormalities. The patient also had multiple systemic anomalies, including complex congenital heart disease, renal dysplasia, skeletal abnormalities, and cerebellar malformation. Clinical exome sequencing identified a de novo pathogenic variant in the WDR37 gene, confirming the diagnosis

Uremic Optic Neuropathy as the Initial Manifestation of Advanced Chronic Kidney Disease.

Uremic optic neuropathy (UON) is a rare but potentially reversible complication of advanced kidney failure characterized by optic disc edema and visual decline that improves with dialysis. We report the case of a 45-year-old man with no prior diagnosis of chronic kidney disease who presented with 3 months of progressive bilateral blurry vision, eye pain, and exertional fatigue. Ophthalmologic examination revealed bilateral optic disc edema with reduced visual acuity. Laboratory evaluation showed profound azotemia (blood urea nitrogen 182.9 mg/dL; creatinine 29.5 mg/dL), hyperkalemia, anemia, m

Targeted VNTR long read sequencing resolves a diagnostic bottleneck in ADTKD and detects de novo ADTKD-MUC1.

INTRODUCTION: The molecular diagnosis of autosomal dominant tubulointerstitial kidney disease due to MUC1 variants (ADTKD-MUC1) using high-throughput (short-read) sequencing methods remains challenging due to the presence of a coding long variable-number tandem repeat (VNTR) region wherein most known pathogenic variants are located. METHODS: Here, we used targeted amplicon long-read sequencing to study the MUC1 VNTR in a retrospective cohort study of 78 individuals. Using a bioinformatic pipeline including newly developed specialized software, VNTRtools, we reconstructed patient-specific comp

Prevalence of Fabry disease GLA variants among patients with parapelvic cysts: a Multicenter Italian Cohort Study.

BACKGROUND: Fabry disease is a rare X-linked disorder characterized by the accumulation of sphingolipids in various vital systems, leading to multiple organ damage. As previous data showed the association between Fabry disease and parapelvic cysts, the aim of our work was to evaluate the prevalence of GLA variants in patients with parapelvic cysts. METHODS: From November 2022 to September 2024, we conducted a multicenter cohort study on patients referred to 15 Italian Nephrology Units. The prevalence of GLA variants among patients with parapelvic cysts was compared to the prevalence of Fabry

Necrotizing Myopathy Presenting With Guillain-Barré-Like Areflexic Paraparesis, Severe Rhabdomyolysis, and Dialysis-Requiring Acute Kidney Injury.

Necrotizing myopathy typically presents with subacute symmetric proximal weakness and marked creatine kinase elevation. Acute areflexic paraparesis with sensory abnormalities may instead suggest Guillain-Barré syndrome and create uncertainty regarding neuromuscular localization. A man in his 50s presented after collapse with abrupt bilateral lower-extremity paralysis. Examination demonstrated 0/5 strength across proximal and distal lower-extremity muscle groups, marked rigidity and hypercontracture, areflexia, and sensory impairment, with preserved upper-extremity function. Comparison of genetic profile and kidney outcomes between very early-onset and non-very-early-onset autosomal dominant polycystic kidney disease in children: a systematic review and meta-analysis.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is the most common inherited kidney disorder and a leading cause of kidney failure. Although typically considered an adult-onset condition, a subset of patients present in utero or within the first 18 months of life (very early-onset ADPKD; VEO-ADPKD). This subgroup may experience a more severe disease course. METHODS: A systematic search of MEDLINE was conducted to identify studies reporting clinical, genetic, radiologic and kidney outcome data in children (< 18 years) with VEO-ADPKD. Case series including fewer than three VEO-

Diagnostic Accuracy of Bosniak v2019 for Predicting Cancer and Unfavorable Histology in Class III-IV Cystic Renal Masses.

Background The current Bosniak classification system (Bosniak Classification version 2019 [Bosniak v2019]) helps predict cancer, but it is unknown whether it also helps predict unfavorable histologic features. Purpose To determine whether Bosniak v2019 and nodule size help predict cancer and unfavorable histologic features. Materials and Methods This retrospective study included Bosniak III or IV cystic renal masses across 10 institutions and three countries (January 2007 to February 2024). Pretreatment renal CT or MRI scans obtained within 1 year of resection were independently reviewed by tw

Mediation Analysis of Urine Osmolality and Response to Tolvaptan in Autosomal Dominant Polycystic Kidney Disease: A Post Hoc Assessment.

AIM: Tolvaptan delays growth in total kidney volume (TKV) and decline in estimated glomerular filtration rate (eGFR) among individuals with autosomal dominant polycystic kidney disease (ADPKD) at elevated risk of rapid progression. We evaluated changes in urine osmolality (Uosm) after treatment initiation as a potential biomarker of long-term response. METHODS: This was a post hoc analysis of TEMPO 3:4, a 3-year, randomized, placebo-controlled trial of tolvaptan. Causal mediation analysis assessed if the effect of tolvaptan on longer-term outcomes of change in eGFR and TKV at month 36 was med

Multisystem developmental abnormalities and insulin resistance in ADTKD-HNF1B with the p.E105K variant: A clinical conundrum.

AIMS: HNF1B variants cause autosomal dominant tubulointerstitial kidney disease (ADTKD-HNF1B), but the phenotypic spectrum of variants of uncertain significance (VUS) remains incompletely defined. We aimed to characterise the multisystem phenotype associated with the HNF1B p.E105K variant and to examine the mechanism underlying insulin resistance in this setting. METHODS: A 15-year-old male proband with chronic kidney disease and multisystem developmental abnormalities underwent clinical, biochemical and genetic evaluation, including trio-based whole-exome sequencing. [SINGLE-PORT LAPAROSCOPIC NEPHRECTOMY FOR CONGENITAL HYDRONEPHROSIS WITH A DISC KIDNEY AFTER THE EPISODE OF TRAUMATIC URINE LEAKAGE : A CASE REPORT].

A 7-year-old boy who had been under observation after being diagnosed with a disc kidney, right hydronephrosis and poor right renal function was urgently transported to our hospital for treatment of a right perirenal urinary leakage caused by abdominal trauma. We performed interventional radiology-computed tomography (IVR-CT) before nephrectomy of the nonfunctioning right hydronephrotic kidney. Preoperative IVR-CT revealed five and four arterial branches supplying the right and left kidney, respectively, derived from the abdominal aorta, bilateral common iliac arteries, and the sacral artery.

Targeted analysis of salicylate binding partners in renal epithelial cells reveals major AMPK dependency.

Autosomal dominant polycystic kidney disease (ADPKD) is the most prevalent genetic kidney disease and is characterized by the formation and growth of fluid-filled renal cysts. With the current treatment tolvaptan not suited for the majority of patients, an urgent need remains for novel therapeutics. We have shown before that activation of adenosine monophosphate activated protein kinase (AMPK) with salsalate ameliorates disease progression in vivo to a similar extent as tolvaptan. Salsalate achieves this by affecting multiple disease mechanisms known to be involved in ADPKD pathogenesis, such

The population frequency of predicted pathogenic variants in the genes associated with Autosomal Dominant Polycystic Liver Disease (ADPLD) and kidney cysts.

Autosomal dominant polycystic liver disease (ADPLD) commonly results from pathogenic variants in one of 7 genes (GANAB, ALG8, ALG9, PRKCSH, SEC63 and possibly LRP5 and SEC61B). These genes are also included in kidney panels because pathogenic variants cause kidney cysts but rarely kidney failure. This study determined the population frequency of predicted pathogenic variants in the ADPLD genes in the general population. Variants for each gene were downloaded from gnomADv.2.1.1 and annotated with ANNOVAR. The population frequencies were calculated from the number of people with 'predicted patho

Solid Malignant Tumors in Infancy: A Single-Center Experience.

Infantile tumors are rare and different from those in older children in terms of anatomic site, histologic features, and behavior. Thus, they require adapted multimodality approaches for diagnosis and treatment. We aimed to evaluate the characteristics and outcomes of different tumor types in children under 1 year of age who were referred to the Hacettepe University Pediatric Oncology Department over 20 years. There were 515 infants, with a mean age of 6.1±3.4 months, diagnosed with malignant tumors. The male/female ratio was 1.15. Thirty-three (6.4%) of them were diagnosed in the newborn peri
