The impact of nephrectomy on liver and contralateral kidney volume in patients with ADPKD; a case series.

PURPOSE: A substantial number of patients with autosomal dominant polycystic kidney disease (ADPKD) undergo nephrectomy. A decrease in intra-abdominal pressure after nephrectomy might hypothetically accelerate the growth of other polycystic organs and accelerate the rate of eGFR decline of the remaining kidney. In this case-series, we investigated the change in kidney and liver volume in patients before and after nephrectomy, and in a control group without nephrectomy. METHODS: In this retrospective case series kidney and liver volumes were measured using an automated stereological approach (

Less is More: Robotic Kidney Transplantation in Polycystic Kidney Disease Without Preemptive or Simultaneous Native Nephrectomy.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) often prompts pre-transplantation or simultaneous native nephrectomy. METHODS: We reviewed adult patients with ADPKD who underwent RKT without native nephrectomy at one transplant centre. Native kidney volume (NKV) and estimated abdominopelvic cavity volume (EACV) were estimated using ellipsoid approximations from cross-sectional imaging, and a Spatial Occupancy Ratio (SOR = combined NKV/EACV) was derived to quantify anatomical crowding. RESULTS: Sixteen patients underwent RKT for ADPKD without preemptive native nephrectomy, wi

A new conceptual framework for PKD1 in ADPKD; integrating DNA structures and inflammation.

In ADPKD renal function is corrupted by the accumulation and growth of fluid-filled cysts. Disruption of the PKD1 gene product, polycystin-1, is the most frequent cause of ADPKD, but the mechanisms that predispose PKD1 to recurrent somatic mutation remain poorly understood. Because current evidence for sequence- and structure-dependent mutational susceptibility is strongest at the human PKD1 locus, we will focus here on mechanisms that may promote somatic PKD1 inactivation. Experimental evidence for polycistin-1 inactivation supports a two-hit pathway, with the first hit being an inherited ger

Renal blood flow and progression to kidney failure in ADPKD.

BACKGROUND: A decrease in renal blood flow (RBF) precedes estimated glomerular filtration rate (eGFR) decline among patients with autosomal dominant polycystic kidney disease (ADPKD) and inversely associates with disease severity and progression. However, it is unknown whether RBF independently predicts progression to kidney failure in patients with ADPKD. METHODS: A total of 379 participants with early-stage ADPKD (mean age 36.6 ± 8.5 years; eGFR: 92.9 ± 17.1 ml/min/1.73 m2) who participated in the Halt Progression of Polycystic Kidney Disease Study A (HALT PKD A; NCT00283686) with baseline

Comparison of genetic profile and kidney outcomes between very early-onset and non-very-early-onset autosomal dominant polycystic kidney disease in children: a systematic review and meta-analysis.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is the most common inherited kidney disorder and a leading cause of kidney failure. Although typically considered an adult-onset condition, a subset of patients present in utero or within the first 18 months of life (very early-onset ADPKD; VEO-ADPKD). This subgroup may experience a more severe disease course. METHODS: A systematic search of MEDLINE was conducted to identify studies reporting clinical, genetic, radiologic and kidney outcome data in children (< 18 years) with VEO-ADPKD. Case series including fewer than three VEO-

Mediation Analysis of Urine Osmolality and Response to Tolvaptan in Autosomal Dominant Polycystic Kidney Disease: A Post Hoc Assessment.

AIM: Tolvaptan delays growth in total kidney volume (TKV) and decline in estimated glomerular filtration rate (eGFR) among individuals with autosomal dominant polycystic kidney disease (ADPKD) at elevated risk of rapid progression. We evaluated changes in urine osmolality (Uosm) after treatment initiation as a potential biomarker of long-term response. METHODS: This was a post hoc analysis of TEMPO 3:4, a 3-year, randomized, placebo-controlled trial of tolvaptan. Causal mediation analysis assessed if the effect of tolvaptan on longer-term outcomes of change in eGFR and TKV at month 36 was med

Targeted analysis of salicylate binding partners in renal epithelial cells reveals major AMPK dependency.

Autosomal dominant polycystic kidney disease (ADPKD) is the most prevalent genetic kidney disease and is characterized by the formation and growth of fluid-filled renal cysts. With the current treatment tolvaptan not suited for the majority of patients, an urgent need remains for novel therapeutics. We have shown before that activation of adenosine monophosphate activated protein kinase (AMPK) with salsalate ameliorates disease progression in vivo to a similar extent as tolvaptan. Salsalate achieves this by affecting multiple disease mechanisms known to be involved in ADPKD pathogenesis, such

High systolic blood pressure and stroke: evidence from the NHANES 1999-2023 and global burden of disease 2021.

High systolic blood pressure (HSBP) is a major modifiable risk factor for stroke, but trends in disease burden and causal associations related to HSBP in the United States require further investigation using multidimensional approaches. This study aims to elucidate this relationship by utilizing data from the Global Burden of Disease (GBD) database, the National Health and Nutrition Examination Survey (NHANES). This study integrated data from the GBD 2021 database and the NHANES. The GBD data provided macro-level estimates of stroke-related mortality and disability-adjusted life years (DALYs)

Typical and atypical ADPKD: predicted pathogenic genetic variants and population frequencies.

BACKGROUND AND HYPOTHESIS: The population frequency of AD polycystic kidney disease (ADPKD) has not been known with certainty. We hypothesized that more accurate population frequencies for typical (PKD1, PKD2) and atypical (GANAB, ALG9, DNAJB11 ALG5, IFT140, NEK8) ADPKD were possible using a computational approach. METHODS: Initially, this study calculated the number of predicted pathogenic structural, null, and missense variants in the ADPKD-associated genes in gnomAD v.2.1.1, and compared this with disease-causing changes according to the ClinVar, HGMD, or LOVD databases. However, because o

Multi-Target Effects of Novel Synthetic Indanedione-Based Spirotransdicalinol Compound 4l in Autosomal Dominant Polycystic Kidney Disease.

Autosomal dominant polycystic kidney disease (ADPKD) is a common genetic disorder characterized by the development of fluid-filled cysts in the kidneys. Tolvaptan is the only FDA-approved drug to treat ADPKD, which has significant side effects, prompting the need for safer novel treatments. Given the pathophysiological similarities between ADPKD and malignant tumors, repurposing anti-cancer compounds signifies a promising strategy. In this study, we explored the therapeutic potential of a novel spirocyclic compound 4l with anti-cancer effects in ADPKD. Our preliminary results demonstrated that

The Renal Micro-RNA Expression Profile of Pkd1RC/RC Mice Changes Longitudinally with the Progression of the Disease.

KEY POINTS: Experimental autosomal dominant polycystic kidney disease is associated with longitudinal changes in the kidney micro-RNA and mRNA profiles. Alterations in cellular transport, a feature of CKD, and a common biological process within polycystic kidney disease models and human autosomal dominant polycystic kidney disease, is present from early stages. Early versus late micro-RNA expression changes in autosomal dominant polycystic kidney disease could be targeted for a more tailored therapeutic intervention in the disease. BACKGROUND: Autosomal dominant polycystic kidney disease (ADP

PAX gene expression in autosomal dominant polycystic kidney disease contributes to cyst expansion and regulates a gene network associated with cyst growth.

Autosomal dominant polycystic kidney disease (ADPKD) is a common inherited disorder caused by mutations in PKD1 or PKD2. The PAX2 and PAX8 genes encode transcription factors required for kidney development, but their role in ADPKD remains unclear. We hypothesized that PAX2 and PAX8 contribute to ADPKD cystogenesis through distinct and overlapping mechanisms. Using immunofluorescence, PAX2 and PAX8 expression was assessed in human ADPKD kidney tissues. Nuclear PAX2 expression, and cytoplasmic PAX8 expression were found to be up-regulated in cyst-lining epithelial cells. Furthermore, using siRNA

Robotic-Assisted Simultaneous Bilateral Native Nephrectomy and Living Donor Kidney Transplantation.

INTRODUCTION: Simultaneous bilateral nephrectomy and living donor kidney transplantation (LDKT) is often indicated for autosomal dominant polycystic kidney disease (ADPKD) with end-stage kidney disease (ESKD). Robotic-assisted surgery offers a minimally invasive alternative to open approaches and may reduce perioperative morbidity and length of stay. METHODS: Four adults with ADPKD/ESKD underwent fully robotic-assisted simultaneous bilateral nephrectomy/LDKT (RASBN/LDKT) at a single high-volume transplant center. All procedures used an intra-abdominal approach with a Pfannenstiel incision and

Minocycline infusion sclerotherapy for infected hepatic cysts in a patient with autosomal dominant polycystic kidney disease: a case report and literature review.

We report the case of a Japanese man in his 60 s undergoing hemodialysis for end-stage renal disease due to autosomal dominant polycystic kidney disease, who had experienced repeated hepatic cyst infections. Despite treatment with oral antibiotics, including minocycline and trimethoprim-sulfamethoxazole, he presented with fever and was admitted to our hospital. Diffusion-weighted magnetic resonance imaging demonstrated a high signal intensity in a large hepatic cyst, and percutaneous transhepatic drainage was performed. Enterococcus faecium was isolated from the cyst fluid. Three intracystic m

Abnormal purinergic signaling contributes to development of renal cysts in autosomal dominant polycystic kidney disease.

Polycystic kidney diseases (PKD) are a group of inherited nephropathies marked by the formation of fluid-filled cysts along the nephron, which cause renal failure. Bulk RNA sequencing of microdissected renal cysts from Pkd1RC/RC mice identifies 4,970 differentially expressed genes in comparison to intact collecting ducts. A poorly understood factor of cyst growth is the high ATP level in the cyst fluid, probably released by pannexin-1 hemichannels. Here, we evaluate a novel pannexin-1 inhibitor SIL14 as a potential therapeutic agent for cyst reduction and investigate proteins involved in cysto

Disruption of a six-nucleotide miRNA motif improves PKD1 dosage and ameliorates polycystic kidney disease.

Disrupting microRNA interactions to restore protein expression from haploinsufficient genes offers a promising precision-therapy strategy for monogenic disorders. PKD1 heterozygosity underlies autosomal dominant polycystic kidney disease (ADPKD), a disorder affecting nearly 12 million people worldwide, where reduced PKD1 dosage drives progressive cyst formation and kidney failure. We previously identified a 55-bp cis-repressive element in the PKD1 3'UTR. Here, we define a six-nucleotide miR-17 seed match within this element that is sufficient to reproduce PKD1 repression. In vivo base substitu

The Role of Immune Cells as Modulators of Progression in Polycystic Kidney Disease: A Systematic Review.

Autosomal dominant polycystic kidney disease (ADPKD) is a genetic disorder caused by mutations in genes encoding ciliary proteins. Increasing evidence suggests that immune cell infiltration, chronic inflammation, and dysregulated immune signaling pathways contribute to cyst growth, fibrosis, and progressive kidney function decline. Notably, features of chronic inflammation are already detectable in early stages of the disease, including infiltration by immune cells, elevated levels of proinflammatory cytokines, and immunologic crosstalk between immune and epithelial cells. These immune respons

Urinary interleukin-37 as a suggested protective biomarker of renal prognosis in autosomal dominant polycystic kidney disease.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is characterized by progressive cyst expansion and chronic inflammation. Interleukin-37 (IL-37) is an anti-inflammatory cytokine, but its role in ADPKD remains unclear. This study aimed to evaluate IL-37 expression in ADPKD patients and assess its clinical relevance in disease severity and renal prognosis. METHODS: IL-37 expression in kidney tissues and cell lines was examined by real-time PCR, Western blot, and immunofluorescence. IL-37 levels were measured by ELISA. This retrospective observational cohort study analyzed longit

2025 KDIGO Executive Summary: Diagnosis and Management of Pediatric Autosomal Dominant Polycystic Kidney Disease.

Autosomal dominant polycystic kidney disease (ADPKD) is the most common inherited kidney disorder, affecting 1 in 400 to 1,000 people worldwide. These significant care requirements reflect the importance of early diagnosis and prevention for affected patients and the need for evidence-based guidelines for treatment protocols and best practice measures. The 2025 KDIGO guidelines build on earlier frameworks such as the 2019 International consensus statement on the diagnosis and management of ADPKD in children and young people to improve screening, monitoring, and treatment methodology for pediat
