A pilot study of magnetic resonance fingerprinting and radiomics analysis in autosomal dominant polycystic kidney disease.

INTRODUCTION: This study investigated radiomic features extracted from magnetic resonance fingerprinting (MRF)-acquired T1 and T2 quantitative maps in children and young adults with autosomal dominant polycystic kidney disease (ADPKD) with normal kidney function and compared these features to those extracted from MRF images of healthy volunteers. METHODS: Mean T1 and T2 relaxation times, along with additional radiomic features, were derived from MRF T1 and T2 maps in 11 healthy volunteers and 18 patients with ADPKD. Two radiomic feature schemes were investigated to differentiate between patie

Fingerprinting the polycystic kidney: radiomics of the noncystic parenchyma for earlier ADPKD detection.

Autosomal dominant polycystic kidney disease risk stratification relies on total kidney volume, a validated marker of cyst burden and progression. Kremer et al.'s pilot study used rapid breath-hold magnetic resonance fingerprinting with radiomics to evaluate noncystic renal parenchyma in children and young adults. Quantitative T1/T2 texture features distinguished autosomal dominant polycystic kidney disease from controls independently of total kidney volume, cyst fraction, age, or body size, suggesting early tissue changes. Small, cross-sectional, unmatched data require longitudinal validation

Real-world epidemiology and renal outcomes of ADPKD in Türkiye: the PKD-TURK cohort.

INTRODUCTION: Real-world epidemiological and longitudinal outcome data on autosomal dominant polycystic kidney disease (ADPKD) remain limited in underrepresented populations, including Türkiye. We aimed to characterize the epidemiology, regional distribution, and renal outcomes of ADPKD in a large multicenter national cohort. METHODS: In this retrospective multicenter study, adult patients with ADPKD were identified across tertiary nephrology centers in six regions of Türkiye (2010-2025). Study-based prevalence was calculated using Turkish Statistical Institute population data. Longitudinal r

Genetic Spectrum and Family-Based Cascade Screening in Vietnamese Families With Autosomal Dominant Polycystic Kidney Disease.

AIM: Autosomal dominant polycystic kidney disease (ADPKD) is one of the most common inherited causes of kidney failure. We performed a whole-exome sequencing study that aimed to characterize the spectrum of PKD1 and PKD2 variants in a Vietnamese ADPKD cohort and to evaluate the clinical utility of family-based cascade genetic testing. METHODS: This cross-sectional descriptive study included 40 unrelated patients with clinically diagnosed polycystic kidney disease. Variants were identified in probands using whole-exome sequencing. Family-based genetic testing was subsequently performed using S

The effect of tolvaptan on total kidney volume and inflammatory markers in autosomal dominant polycystic kidney disease.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is characterized by progressive cyst growth, kidney enlargement, and decline in renal function. Increasing evidence suggests that inflammatory processes may contribute to disease progression. This study aimed to evaluate the association between tolvaptan use, systemic inflammatory markers, and total kidney volume (TKV) progression in patients with ADPKD. METHODS: This retrospective study included 67 patients with ADPKD, including 40 receiving tolvaptan and 27 untreated controls. Neutrophil-to-lymphocyte ratio (NLR), platelet-to-

The impact of nephrectomy on liver and contralateral kidney volume in patients with ADPKD; a case series.

PURPOSE: A substantial number of patients with autosomal dominant polycystic kidney disease (ADPKD) undergo nephrectomy. A decrease in intra-abdominal pressure after nephrectomy might hypothetically accelerate the growth of other polycystic organs and accelerate the rate of eGFR decline of the remaining kidney. In this case-series, we investigated the change in kidney and liver volume in patients before and after nephrectomy, and in a control group without nephrectomy. METHODS: In this retrospective case series kidney and liver volumes were measured using an automated stereological approach (

Less is More: Robotic Kidney Transplantation in Polycystic Kidney Disease Without Preemptive or Simultaneous Native Nephrectomy.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) often prompts pre-transplantation or simultaneous native nephrectomy. METHODS: We reviewed adult patients with ADPKD who underwent RKT without native nephrectomy at one transplant centre. Native kidney volume (NKV) and estimated abdominopelvic cavity volume (EACV) were estimated using ellipsoid approximations from cross-sectional imaging, and a Spatial Occupancy Ratio (SOR = combined NKV/EACV) was derived to quantify anatomical crowding. RESULTS: Sixteen patients underwent RKT for ADPKD without preemptive native nephrectomy, wi

A new conceptual framework for PKD1 in ADPKD; integrating DNA structures and inflammation.

In ADPKD renal function is corrupted by the accumulation and growth of fluid-filled cysts. Disruption of the PKD1 gene product, polycystin-1, is the most frequent cause of ADPKD, but the mechanisms that predispose PKD1 to recurrent somatic mutation remain poorly understood. Because current evidence for sequence- and structure-dependent mutational susceptibility is strongest at the human PKD1 locus, we will focus here on mechanisms that may promote somatic PKD1 inactivation. Experimental evidence for polycistin-1 inactivation supports a two-hit pathway, with the first hit being an inherited ger

Renal blood flow and progression to kidney failure in ADPKD.

BACKGROUND: A decrease in renal blood flow (RBF) precedes estimated glomerular filtration rate (eGFR) decline among patients with autosomal dominant polycystic kidney disease (ADPKD) and inversely associates with disease severity and progression. However, it is unknown whether RBF independently predicts progression to kidney failure in patients with ADPKD. METHODS: A total of 379 participants with early-stage ADPKD (mean age 36.6 ± 8.5 years; eGFR: 92.9 ± 17.1 ml/min/1.73 m2) who participated in the Halt Progression of Polycystic Kidney Disease Study A (HALT PKD A; NCT00283686) with baseline

Targeted VNTR long read sequencing resolves a diagnostic bottleneck in ADTKD and detects de novo ADTKD-MUC1.

INTRODUCTION: The molecular diagnosis of autosomal dominant tubulointerstitial kidney disease due to MUC1 variants (ADTKD-MUC1) using high-throughput (short-read) sequencing methods remains challenging due to the presence of a coding long variable-number tandem repeat (VNTR) region wherein most known pathogenic variants are located. METHODS: Here, we used targeted amplicon long-read sequencing to study the MUC1 VNTR in a retrospective cohort study of 78 individuals. Using a bioinformatic pipeline including newly developed specialized software, VNTRtools, we reconstructed patient-specific comp

Comparison of genetic profile and kidney outcomes between very early-onset and non-very-early-onset autosomal dominant polycystic kidney disease in children: a systematic review and meta-analysis.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is the most common inherited kidney disorder and a leading cause of kidney failure. Although typically considered an adult-onset condition, a subset of patients present in utero or within the first 18 months of life (very early-onset ADPKD; VEO-ADPKD). This subgroup may experience a more severe disease course. METHODS: A systematic search of MEDLINE was conducted to identify studies reporting clinical, genetic, radiologic and kidney outcome data in children (< 18 years) with VEO-ADPKD. Mediation Analysis of Urine Osmolality and Response to Tolvaptan in Autosomal Dominant Polycystic Kidney Disease: A Post Hoc Assessment.

AIM: Tolvaptan delays growth in total kidney volume (TKV) and decline in estimated glomerular filtration rate (eGFR) among individuals with autosomal dominant polycystic kidney disease (ADPKD) at elevated risk of rapid progression. We evaluated changes in urine osmolality (Uosm) after treatment initiation as a potential biomarker of long-term response. METHODS: This was a post hoc analysis of TEMPO 3:4, a 3-year, randomized, placebo-controlled trial of tolvaptan. Causal mediation analysis assessed if the effect of tolvaptan on longer-term outcomes of change in eGFR and TKV at month 36 was med

Targeted analysis of salicylate binding partners in renal epithelial cells reveals major AMPK dependency.

Autosomal dominant polycystic kidney disease (ADPKD) is the most prevalent genetic kidney disease and is characterized by the formation and growth of fluid-filled renal cysts. With the current treatment tolvaptan not suited for the majority of patients, an urgent need remains for novel therapeutics. We have shown before that activation of adenosine monophosphate activated protein kinase (AMPK) with salsalate ameliorates disease progression in vivo to a similar extent as tolvaptan. Salsalate achieves this by affecting multiple disease mechanisms known to be involved in ADPKD pathogenesis, such

High systolic blood pressure and stroke: evidence from the NHANES 1999-2023 and global burden of disease 2021.

High systolic blood pressure (HSBP) is a major modifiable risk factor for stroke, but trends in disease burden and causal associations related to HSBP in the United States require further investigation using multidimensional approaches. This study aims to elucidate this relationship by utilizing data from the Global Burden of Disease (GBD) database, the National Health and Nutrition Examination Survey (NHANES). This study integrated data from the GBD 2021 database and the NHANES. The GBD data provided macro-level estimates of stroke-related mortality and disability-adjusted life years (DALYs)

Atorvastatin protects cardiomyocytes from doxorubicin toxicity by modulating YAP/TAZ signaling.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is the most common genetic renal disorder, with a prevalence of 1 : 500 to 1 : 1,000. ADPKD is genetically heterogeneous: the genes involved are PKD1 and PKD2. ADPKD occurs worldwide and in all ethnic groups and is an important cause of CKD Stage 5. Prevalence of ADPKD on renal replacement therapy (RRT) in Italy has been reported to be 8.2%. In the dialysis population of Vicenza, a province in Northeastern Italy, it accounts for 13.4%. The study aims to investigate reasons for the high prevalence of ADPKD in our region and to des

Typical and atypical ADPKD: predicted pathogenic genetic variants and population frequencies.

BACKGROUND AND HYPOTHESIS: The population frequency of AD polycystic kidney disease (ADPKD) has not been known with certainty. We hypothesized that more accurate population frequencies for typical (PKD1, PKD2) and atypical (GANAB, ALG9, DNAJB11 ALG5, IFT140, NEK8) ADPKD were possible using a computational approach. METHODS: Initially, this study calculated the number of predicted pathogenic structural, null, and missense variants in the ADPKD-associated genes in gnomAD v.2.1.1, and compared this with disease-causing changes according to the ClinVar, HGMD, or LOVD databases. However, because o

Multi-Target Effects of Novel Synthetic Indanedione-Based Spirotransdicalinol Compound 4l in Autosomal Dominant Polycystic Kidney Disease.

Autosomal dominant polycystic kidney disease (ADPKD) is a common genetic disorder characterized by the development of fluid-filled cysts in the kidneys. Tolvaptan is the only FDA-approved drug to treat ADPKD, which has significant side effects, prompting the need for safer novel treatments. Given the pathophysiological similarities between ADPKD and malignant tumors, repurposing anti-cancer compounds signifies a promising strategy. In this study, we explored the therapeutic potential of a novel spirocyclic compound 4l with anti-cancer effects in ADPKD. Our preliminary results demonstrated that

The Renal Micro-RNA Expression Profile of Pkd1RC/RC Mice Changes Longitudinally with the Progression of the Disease.

KEY POINTS: Experimental autosomal dominant polycystic kidney disease is associated with longitudinal changes in the kidney micro-RNA and mRNA profiles. Alterations in cellular transport, a feature of CKD, and a common biological process within polycystic kidney disease models and human autosomal dominant polycystic kidney disease, is present from early stages. Early versus late micro-RNA expression changes in autosomal dominant polycystic kidney disease could be targeted for a more tailored therapeutic intervention in the disease. PAX gene expression in autosomal dominant polycystic kidney disease contributes to cyst expansion and regulates a gene network associated with cyst growth.

Autosomal dominant polycystic kidney disease (ADPKD) is a common inherited disorder caused by mutations in PKD1 or PKD2. The PAX2 and PAX8 genes encode transcription factors required for kidney development, but their role in ADPKD remains unclear. We hypothesized that PAX2 and PAX8 contribute to ADPKD cystogenesis through distinct and overlapping mechanisms. Using immunofluorescence, PAX2 and PAX8 expression was assessed in human ADPKD kidney tissues. Nuclear PAX2 expression, and cytoplasmic PAX8 expression were found to be up-regulated in cyst-lining epithelial cells. Furthermore, using siRNA
