KEY POINTS: The causal relationship between serum osmolality and kidney outcome remained unclear in patients with autosomal dominant polycystic kidney disease. Serum osmolality may be a possible surrogate marker for the clinical monitoring of autosomal dominant polycystic kidney disease patients, especially when access to copeptin level is limited. High serum osmolality conveys possible detrimental effect on the kidney outcomes.
BACKGROUND: Current treatment of autosomal dominant polycystic kidney disease (ADPKD) is mainly focused on inhibiting cystogenesis through arginine vasopressin suppre

Machine learning (ML) is a valuable tool in healthcare, enabling the prediction of disease progression through data-driven regression and nonlinear modeling. Unlike traditional statistical methods, ML can identify complex interactions among explanatory variables. Autosomal dominant polycystic kidney disease (ADPKD) is a common cause of chronic kidney disease (CKD), often progressing to end-stage renal failure. Accurately predicting CKD progression in ADPKD patients is essential for personalized treatment strategies. This study analyzed data from 2,737 patients with ADPKD enrolled in the Japane

Obturator hernia, a rare condition that accounts for only 0.07-1% of all hernias, typically occurs in older adult, thin, multiparous women owing to weakening of the obturator membrane and increased intra-abdominal pressure. Conditions such as chronic constipation, chronic obstructive pulmonary disease, ascites, peritoneal dialysis (PD), and autosomal dominant polycystic kidney disease (ADPKD) are considered risk factors for an obturator hernia. Diagnosis is often delayed because of nonspecific symptoms, and computed tomography imaging is essential for early detection. We report the case of a 6

Autosomal dominant polycystic kidney disease (ADPKD) is a common monogenic genetic disease characterized by bilateral renal cyst formation and enlargement. The purpose of this study was to explore the therapeutic effects of emodin, which is a key active constituent of traditional Chinese herbal medicine Rhubarb, on ADPKD and its underlying mechanisms. Herein, we found emodin significantly inhibited renal cyst progression in ADPKD. RNA-sequencing analysis and network pharmacological analysis indicated that emodin may exert its therapeutic effects by modulating cell proliferation and oxidative s

We enrolled 553 patients with autosomal dominant polycystic kidney disease (ADPKD) to investigate the association between hemoglobin (Hb) levels and kidney outcomes. Renal outcome was defined as a 50% reduction in estimated glomerular filtration rate or the initiation of renal replacement therapy. Survival analyses and logistic regression were used to generate time-series McFadden’s pseudo-R-squared (pseudo-R²) values as a goodness-of-fit index to compare the relative contribution of Hb cut-offs over follow-up time, and the mean pseudo-R² values from the first to the 17th year (1–17Y mean) wer

Autosomal dominant polycystic kidney disease (ADPKD) is the most common genetic cause of chronic kidney disease, resulting in substantial morbidity and mortality globally. Advances in molecular genetics and deep-phenotype imaging techniques have refined existing diagnostic and prognostic tools. The strong evidence-base for tolvaptan as a disease-modifying treatment supports its early use in groups at high risk of kidney failure. Screening and management of potentially serious complications including cyst infection, intracranial aneurysms, and polycystic liver disease are important components o

Endoplasmic reticulum glycosyltransferase ALG8 controls metabolic fate in autosomal dominant polycystic kidney disease (ADPKD). In this paper, we summarize human genetics, cell-based, and organ-based evidence to investigate whether ALG8 variants affect cyst initiation and metabolic states of ADPKD. Population screening showed ALG8 variant enrichment in ADPKD cohorts (OR = 9.75, P0.001); loss-of-function alleles interact with PKD1 mutations to accelerate cystogenesis. ALG8 deficiency leads to metabolic collapse by several mechanisms. Impaired polycystin-1 glycosylation disrupts ER-to-cilium tra

Ubiquitination serves a critical role in regulating both inflammatory responses and kidney injury. Among inherited renal disorders, autosomal dominant polycystic kidney disease (ADPKD) has demonstrated associations with disrupted ubiquitin signaling that exacerbates inflammation and cyst progression. In this study, we demonstrate that the E3 ligase Pellino1 (Peli1) acts as an essential contributor to the pathogenesis of ADPKD amid inflammatory conditions. In individuals with clear cell renal cell carcinoma (ccRCC), Peli1 exhibits markedly elevated expression, and this upregulation is associate

We experienced a 45-year-old male with ADPKD who developed spontaneous retroperitoneal hemorrhages with hypertension in the right subcapsular renal space without trauma. After his condition worsened with conservative management, he was treated by renal transarterial embolization (TAE). Over the next eight years, he experienced bilateral bleeding at three different sites and underwent repeated endovascular treatment with renal TAE to stop the bleeding. After the fourth occurrence, the patient underwent hemodialysis. Although renal cysts and urinary tract bleeding occur frequently in ADPKD, retr

INTRODUCTION: For patients with autosomal dominant polycystic kidney disease (ADPKD), the only approved treatment is tolvaptan, a twice-daily medication that works to slow kidney function decline. Patients on tolvaptan report a considerable burden related to the drug's aquaretic effects which can intensify immediately following a dose of the medication.
METHODS: A targeted literature review and individual concept elicitation (CE) interviews with nephrologists and adult patients with ADPKD taking tolvaptan were conducted to inform the development of a hybrid diary, programmed as a smartphone a

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is characterized by progressive cystic enlargement and an increased risk of renal cell carcinoma. Early diagnosis and treatment are often hindered by severe distortion of renal architecture and by the limitations of contrast-enhanced computed tomography or magnetic resonance imaging in patients with impaired renal function. Contrast-enhanced ultrasound offers a nephrotoxic-free alternative for lesion characterization, treatment planning, and post-ablation monitoring. CASE PRESENTATION: A 44-year-old woman was referred for evaluat

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is characterized by progressive cyst formation, inflammation, and metabolic dysregulation. The endocannabinoid system (ECS), particularly the cannabinoid-1 receptor (CB1R), regulates renal metabolism and inflammatory signaling, yet its role in ADPKD remains largely unexplored. METHODS: We analyzed publicly available human kidney transcriptomic datasets (bulk microarray GSE7869; single-nucleus RNA-sequencing from ADPKD GSE185948, and diabetic kidney disease cohorts GSE195460) and validated findings in ADPKD patient kidney tissue v

Autosomal dominant polycystic kidney disease (ADPKD) is a common inherited disorder caused predominantly by heterozygous mutations in the PKD1 gene, leading to progressive renal cyst formation. While PKD1 mutant mouse models have provided mechanistic insights, PKD1 heterozygous mice fail to replicate the early cystogenesis observed in human patients. To address this gap, we conducted a longitudinal study using PKD1 heterozygous cynomolgus monkeys. Serial renal ultrasonography from birth to five years of age-corresponding to human childhood-revealed progressive cyst development. Remarkably, a s

PKD1 variants are risk factors for autosomal dominant polycystic kidney disease (ADPKD). However, the detailed mechanisms by which PKD1 variants cause ADPKD remain unclear. Currently, there are no curative treatments for ADPKD. We generated induced pluripotent stem cells (iPSCs) from a patient with ADPKD and a PKD1 variant. These iPSCs expressed stem cell markers and could differentiate into the three germ layers in vitro. The iPSCs will serve as useful tools for understanding the pathophysiology of ADPKD and for screening therapeutic approaches.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is associated with distinct vascular remodeling; however, its impact on arteriovenous fistula (AVF) outcomes in hemodialysis patients remains unclear. This study aimed to compare AVF patency and survival between patients with and without ADPKD.
METHODS: We conducted a retrospective observational cohort study including 38 patients with ADPKD and 38 age-matched non-ADPKD patients who underwent first-time AVF creation between 2013 and 2021. Patients were followed for 24 months. Primary failure, primary patency, and assisted primary

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is the leading monogenic cause of end-stage kidney disease (ESKD). Because disease-modifying therapy is costly and can cause adverse effects, identifying patients most likely to benefit is essential. We developed and internally validated a model to predict ESKD by age 80 in class 1 ADPKD, hypothesizing that combining baseline eGFR with an exponential estimate of kidney growth would improve forecasting.
METHODS: We retrospectively analyzed 142 adults with class 1 ADPKD followed at a tertiary center in Mexico City (2012-2023). Tot

The Kidney Disease Outcomes Quality Initiative (KDOQI) convened a work group to review the 2025 KDIGO (Kidney Disease: Improving Global Outcomes) clinical practice guideline for the evaluation, management, and treatment of autosomal dominant polycystic kidney disease (ADPKD). The KDOQI work group reviewed the KDIGO guideline statements and practice points and provided perspective for implementation within the context of clinical practice in the United States. In general, the KDOQI work group concurs with several recommendations and practice points proposed by the KDIGO guidelines regarding the

Cheng et al. identify aldehyde dehydrogenase 1A1 as a novel risk factor for autosomal dominant polycystic kidney disease, by both activating proliferative pathways and acting as a transcription factor. Targeting aldehyde dehydrogenase 1A1 with disulfiram delays cyst growth in autosomal dominant polycystic kidney disease mice. Combining low-dose disulfiram with anti-programmed death ligand 1 antibody synergistically attenuates cyst progression and improves the immune microenvironment. Although aldehyde dehydrogenase 1A1 upregulation occurs in other kidney diseases and its transcriptional mechan

RATIONALE & OBJECTIVE: Autosomal dominant polycystic kidney disease (ADPKD) is characterized by a broad range of extrarenal complications, yet epidemiologic data on its association with aortic dissection (AD) or aortic aneurysm (AA) remain limited. In the present study, we assessed whether ADPKD is associated with an increased risk of developing these aortic conditions.
STUDY DESIGN: Observational cohort study.
SETTING & PARTICIPANTS: 2,568,283 individuals without a history of AD or AA, enrolled between April 2014 and August 2023 in a nationwide Japanese epidemiological database provided by

Autosomal dominant polycystic kidney disease (ADPKD) is a genetic disorder characterized by progressive kidney enlargement by cyst formation. Endothelial dysfunction significantly contributes to chronic kidney disease (CKD). The Mediterranean diet (Med-diet) may reduce endothelial dysfunction in ADPKD patients, but its effect was not investigated in these patients. Our aim was to assess the relationship between Med-diet adherence and endothelial function biomarkers such as nitric oxide (NO) and endothelin-1 (ET-1). We enrolled ADPKD patients aged 18-70 years with CKD stages G2-G4. Adherence to
