Urinary heme oxygenase-1 in children with congenital hydronephrosis due to ureteropelvic junction obstruction.

CONTEXT: Heme oxygenase-1 (HO-1) is implicated to be correlated with renal function in oxidative stress. OBJECTIVE: To determine whether urinary (u) HO-1 is associated with the progression of congenital obstructive hydronephrosis (HN). METHODS: A total 50 children with HN (study group and control 1) and 30 healthy children were enrolled in this study. RESULTS: The uHO-1/cr levels increased significantly and negatively correlated with split renal function in study group before and during surgery. One month after surgery, it decreased significantly. CONCLUSION: Increased uHO-1 levels could b

Urinary NGAL, cystatin C, β2-microglobulin, and osteopontin significance in hydronephrotic children.

BACKGROUND: Ureteropelvic junction obstruction (UPJO) accounts for 35 % of all congenital hydronephrosis cases. The challenge in managing childhood hydronephrosis is to distinguish obstructive from nonobstructive cases and, thereby, identify patients requiring surgical intervention. This study aimed to examine four urinary proteins as potential biomarkers of obstruction in hydronephrosis. METHODS: Urine samples from 24 children with UPJO were collected pre-, peri-, and postoperatively, together with urine samples from healthy children. Neutrophil gelatinase-associated lipocalin (NGAL), cystat

[The features of sclerotic development in junction-ureteral segment of children with congenital hydronephrosis].

A comprehensive clinical, laboratory, instrumental and morphological study of children with congenital hydronephrosis has been carried out. We compared the clinical features of pyelonephritis at children with primary pyelonephritis and congenital hydronephrosis. Bacterial inflammation was dominated symptom at the children with primary bacterial pyelonephritis, whereas children with hydronephrosis had low-grade clinical changes and significant renal functional impairment. Clinically and prognostically more frequent severe unilateral disease was left-side hydronephrosis. For the first time the m

Simplified diagnostic algorithm for evaluation of neonates with prenatally detected hydronephrosis.

INTRODUCTION: The management of neonates with congenital hydronephrosis (CHN) diagnosed antenatally is still controversial. MATERIALS AND METHODS: A prospective study was performed in all newborn infants with CHN born over a 2-year period in order to identify which neonates require a full radiologic investigation including investigation with invasive tests such as voiding cystoureterography (VCUG) and diuretic-enhanced renography. Data on kidney ultrasonography, VCUG, and diuretic renography were collected. The ultrasound grading of hydronephrosis was determined according to Society of Fetal

A translational approach to congenital non-obstructive hydronephrosis.

Congenital hydronephrosis, defined as congenital dilatation of one or more components of the renal collecting system, is detected in 1-2% of all pregnancies. The majority of antenatally detected hydronephrosis is non-obstructive and either resolves or stabilizes. Management of persistently severe cases may include surgical intervention, the only available treatment. Recent data demonstrate that hedgehog signaling plays a critical role in regulating the structure and function of the collecting system in a manner dependent on the formation of GLI3 repressor. Here, we review the pathobiology and

Decrease of renal aquaporins 1-4 is associated with renal function impairment in pediatric congenital hydronephrosis.

BACKGROUND: Renal aquaporins (AQP1-4) concentration is downregulated and is in proportion to the degree of hydronephrosis graded by ultrasound in pediatric congenital hydronephrosis (CH). However, the relationship between the expression of AQP1-4 with the changes of renal function impairment (RFI) evaluated by (99m)Tc-DTPA renal dynamic imaging is still unclear. This study aimed to investigate the relationship between AQP1-4 expression and degree of RFI in children with CH. METHODS: The expression of AQP1-4 was evaluated in 45 children with unilateral ureteropelvic junction obstruction (28 bo

KIM-1 and NGAL: new markers of obstructive nephropathy.

Congenital obstructive nephropathy is the primary cause of chronic renal failure in children. Rapid diagnosis and initiation of the treatment are vital to preserve function and/or to slow down renal injury. The aim of our study was to determine whether urinary (u) kidney injury molecule-1 (KIM-1) and neutrophil gelatinase-associated lipocalin (NGAL) may be useful non-invasive biomarkers in children with congenital hydronephrosis (HN) caused by ureteropelvic junction obstruction. The study cohort consisted of 20 children with severe HN who required surgery (median age 2.16 years) and two contro

[Coupling between the hemodynamic parameters and the morphological changes in the kidney in children with congenital hydronephrosis].

The data of renal ultrasonographic and Doppler studies were comparatively assessed in children with congenital hydronephrosis. The degree of renal B-mode ultrasound hemodynamic parameters was used as the basis for grouping of the children. The resistance index (RI) of all branches of renal arteries receives attention. Morphological studies were carried out on renal biopsy specimens from 29 children from different groups and on 12 removed kidneys; the expression of TGF beta1 and alphaSMA was revealed using the streptavidin-biotin-peroxidase method. Morphological changes as hypoplasia or dysplas

Etio-pathogenic and morphological correlations in congenital hydronephrosis.

Hydronephrosis, "distension in varying degrees of pelvis and calyces, accompanied by progressive atrophy of renal parenchyma due to obstruction in urinary flow", is an apparently simple reno-urinary disease, but, in reality, by association of its own characters with those conferred by causative lesions, becomes of a significant complexity. The purpose of this paper is to demonstrate the plurivalent character of this entity on a batch of cases with congenital hydronephrosis, by identification of lesional features correlated with cause of disease. The etiology of hydronephrosis in analyzed cases

Parental age as a risk factor for isolated congenital malformations in a Polish population.

Currently available data on the relationship between the prevalence of isolated congenital malformations and parental age are inconsistent and frequently divergent. We utilised the data from the Polish Registry of Congenital Malformations (PRCM) to accurately assess the interplay between maternal and paternal age in the risk of isolated non-syndromic congenital malformations. Out of 902 452 livebirths we studied 8683 children aged 0-2 years registered in the PRCM. Logistic regression was used to simultaneously adjust the risk estimates for maternal and paternal age. Our data indicated that pat

[Choice of a drainage method in plastic reconstructive operations for congenital hydronephrosis in boys].

We analysed surgical outcomes in 41 boys aged 2.5 months to 17 years with congenital hydrohephrosis who were treated with external drainage (nephrostoma, intubation drainage, Gibbon's drainage) and by means of stenting. We discovered that stent drainage produces urethral traumas, orchitis, orchoepididymitis. Our study made us to recommend external drainage methods for boys with congenital hydronephrosis undergoing reconstructive plastic surgery to prevent injuries and inflammation in the urogenital system.

[Program of early nephro-urologic management of children with congenital hydronephrosis].

Congenital anomalies of kidney and urinary tract (CAKUT) are the main cause of end stage renal disease in childhood. Early prenatal detection with planned postnatal diagnosis and therapy are the mainstay of management of neonates with CAKUT which is aimed at the conservation of renal tissue. The above assumptions led to the establishment of the Pommeranian Program for Management of Children with CAKUT. The strategy of the program is to coordinate prenatal diagnosis performed by obstetricians, postnatal care by neonatologists and early management by pediatric nephrologists and urologists. It wi

Ureter myogenesis: putting Teashirt into context.

After the basic shape of the mammalian ureter is established, its epithelia mature and a coat of smooth muscle cells differentiate around nascent urothelia. The ureter actively propels tubular fluid from the renal pelvis to the bladder, and this peristalsis, which starts in the fetal period, requires coordinated smooth muscle contraction. Teashirt-3 (Tshz3) is expressed in smooth muscle cell precursors that form the wall of the forming mammalian ureter. The Teashirt gene family was first identified in Drosophila where Teashirt (Tsh) protein acts as a transcription factor directing embryonic an
