Reflex anuria: an old concept with new evidence.

Reflex anuria (RA) was defined by Hull as cessation of urine output from both kidneys due to irritation or trauma to one kidney or its ureter, or severely painful stimuli to other organs. This is not a common concept among urologists or nephrologists even though it has been proposed for more than half a century. The phenomenon has not been thoroughly understood. But intrarenal arteriolar spasm and ureteral spasm have gained wide acceptance as the mechanisms of RA. The present review summarized papers published up to now on RA, in order to depict the general profile of the disease and to furthe

Spina bifida occulta: not to be overlooked in children with nocturnal enuresis.

OBJECTIVE: Previous reports have suggested that the incidence of spina bifida occulta in patients with nocturnal enuresis is higher than in the general population. However, the effect of spina bifida occulta on the response to nocturnal enuresis treatment is controversial. The purpose of this study was to investigate the relationship between spina bifida occulta and response to treatment of nocturnal enuresis. METHODS: Between 2006 and 2009, the records of 160 children with nocturnal enuresis were reviewed. Children with other organic urological disease or symptoms suggestive of spinal dysrap

Renal pelvis rupture in a kidney with ureteropelvic junction obstruction and extrarenal calyces.

The extrarenal calyx (ERC) is a rare congenital anomaly, associated with various other abnormalities of the urogenital system. We report a unique case of ERC in a solitary functioning kidney with a massively dilated pelvis that developed spontaneous rupture. A dismembered pyeloplasty was performed at the time of rupture. The patient did well post-operatively with a stable creatinine and stable SFU grade 2 hydronephrosis.

Ultrasonography reveals a high prevalence of lower spinal dysraphism in children with urogenital anomalies.

BACKGROUND: Lower spinal dysraphism is frequently reported in anorectal anomaly combined with urogenital anomalies. The prevalence of the spinal dysraphism has not been comprehensively studied in children with simple urogenital anomalies. We evaluated the prevalence of the spinal dysraphism using ultrasound data of the lumbosacral area in children with urogenital anomalies. METHODS: Lumbosacral ultrasound images of 259 children who underwent urological surgery with simple urogenital anomalies were reviewed by an ultrasound-specialized radiologist. The primary outcome measures were the conus m

Congenital arteriovenous fistula of the horseshoe kidney with multiple hemangiomas.

INTRODUCTION: Congenital renal arteriovenous fistulas (AVF) are rare, especially if they are associated with other developmental renal anomalies. CASE OUTLINE: A 34-year-old female was hospitalized due to total painless hematuria and bladder tamponade. Excretory urography revealed a horseshoe kidney with normal morphology of pyelocaliceal system and ureters. Aortography and selective renovasography detected a cluster-like vascular formation with multiple arteriovenous fistulas (AVF). Due to a large AVF gauge and poor flow of the efferent vein to the inferior vena cava, a surgical procedure of

Laparoscopic pyeloplasty: a comparison between the transperitoneal and retroperitoneal approach during the learning curve.

PURPOSE: To compare the transperitoneal and the retroperitoneal approach in the laparoscopic management of pelviureteric junction (PUJ) obstruction, and to find out which approach is preferable during the learning curve period at two different urologic centers. MATERIALS AND METHODS: We retrospectively evaluated 50 consecutive laparoscopic pyeloplasties performed by two urologists from different urologic centers during their learning curve period in laparoscopy. Each surgeon used a different approach: transperitoneal (group A) or retroperitoneal (group B). Timing for patient positioning, acce

Penile duplication and two anal openings; report of a very rare case.

BACKGROUND: Penile duplication (diphallus) is an extremely rare disorder. It is almost always associated with other malformations like double bladder, exstrophy of the cloacae, imperforate anus, duplication of the rectosigmoid and vertebral deformities. Meanwhile anal canal duplication, the most distal and least common duplication of the digestive tube and is a very rare congenital malformation. CASE PRESENTATION: A 21 days old Egyptian neonate is reported with complete penile duplication and two scrotums with each one carrying two palpable testes. Both penises have normal shaft with normally

Prenatal diagnosis of bladder exstrophy by fetal MRI.

PURPOSE: To review our experience with prenatal diagnosis of bladder exstrophy by fetal magnetic resonance imaging (MRI). Bladder exstrophy can be diagnosed by ultrasonography (US) evaluation of the fetus based on absence of bladder filling, low-set umbilicus, small genitalia and lower abdominal mass, although in some instances more accurate anatomical information is desired. MATERIAL AND METHODS: We studied three patients at mean gestational age of 27.3 weeks. The fetal MRI exam was performed on axial, sagittal, coronal planes and echo gradient in the best plan for acquisition of fetus. Imag

Perinatally discovered complete tubular colonic duplication associated with anal atresia.

Complete tubular colonic duplication (CTCD) is exceedingly rare. The association of CTCD with an anorectal malformation is unusual. This malformation may be found unexpectedly at laparotomy. We present 3 cases of surgically proven neonate CTCD discovered at laparotomy for anal atresia. We reviewed the mode of clinical presentation, the imaging, and laparotomy findings. Our series illustrates that this rare disease presents perinatally in association with anal atresia, with or without other associated anomalies.

Abnormal insertion of right ureter into the left ureter associated with absence of normal right ureterovesical junction.

AIM: There are lots of factors which cause the dilatation of one or both ureteres. Congenital absence of ureterovesical junction was reported as very rare cause of megaureter. Early detection of congenital absence of normal ureterovesical junction offers utilities for reconstruction and prevents the complications such as megaureter and vesicoureteral reflux. MATERIAL AND METHODS: A male patient 16 months old was referred for renal DTPA scintigraphy confirmed previously with diagnosis as megaureter of left kidney associated with consecutive stasis grades III of the left ureter. Dynamic renal s

Renal and urological abnormalities occurring with Mullerian anomalies.

OBJECTIVE: To describe the spectrum and frequency of renal and urological abnormalities in a cohort of patients with uterine anomalies. MATERIAL AND METHODS: We conducted an ethics committee approved review of 164 women referred to the imaging department from a specialist gynaecological anomaly clinic. Imaging acquired over 8 years was assessed and this was sufficient for assessment of the morphology of the gynaecological and renal tracts in 113 patients. RESULTS: Absent unilateral kidney was the most common abnormality (31.8%), most frequent in Type 3 uterine anomalies. Absent kidneys occur

Mutation screening of BMP4 and Id2 genes in Chinese patients with congenital ureteropelvic junction obstruction.

UNLABELLED: Ureteropelvic junction obstruction (UPJO) is the most common congenital anomaly of the urinary tract. Evidence has shown that BMP4 and Id2 play crucial roles in nephrogenesis, alterations of which may cause ureteral developmental anomalies. Here, we directly sequenced the coding sequences in BMP4 and Id2 genes of 108 unrelated Chinese patients with ureteropelvic junction stenosis. One missense mutation c.485G> A (p.R162Q) in BMP4 and two synonymous mutations (c.1167T> C in BMP4 and c.108A> G in Id2) were detected in three cases. None of these variations were present in the 150 norm

Ureteral triplication: a rare anomaly with a variety of presentations.

Ureteral triplication remains a very rare congenital malformation of the urinary tract with a wide spectrum of presentation. The sporadic nature of this condition and its association with other anomalies makes evidence-based management difficult. We report two cases of triplication in association with the VACTERL syndrome, one developing pelvi-ureteric junction obstruction and the other vesico-ureteric reflux.

Etio-pathogenic and morphological correlations in congenital hydronephrosis.

Hydronephrosis, "distension in varying degrees of pelvis and calyces, accompanied by progressive atrophy of renal parenchyma due to obstruction in urinary flow", is an apparently simple reno-urinary disease, but, in reality, by association of its own characters with those conferred by causative lesions, becomes of a significant complexity. The purpose of this paper is to demonstrate the plurivalent character of this entity on a batch of cases with congenital hydronephrosis, by identification of lesional features correlated with cause of disease. The etiology of hydronephrosis in analyzed cases

Vesicoureteral reflux and other urinary tract malformations in mice compound heterozygous for Pax2 and Emx2.

Congenital anomalies of the kidney and urinary tract (CAKUT) are the most common cause of chronic kidney disease in children. This disease group includes a spectrum of urinary tract defects including vesicoureteral reflux, duplex kidneys and other developmental defects that can be found alone or in combination. To identify new regulators of CAKUT, we tested the genetic cooperativity between several key regulators of urogenital system development in mice. We found a high incidence of urinary tract anomalies in Pax2;Emx2 compound heterozygous mice that are not found in single heterozygous mice.

Environmental factors in genitourinary development.

PURPOSE: In the last century the world has experienced an increase in the use of industrial chemicals as well as possible increases in the prevalence of hypospadias and cryptorchidism. Because hormones regulate the fetal development of many organs, numerous investigations have explored the role of environmental factors in genitourinary growth. We summarize the data regarding endocrine disruptors in human genitourinary development. MATERIALS AND METHODS: A PubMed literature search was performed for human studies from 2004 to 2009. RESULTS: Few data exist on environmental influences on the kid
