Outcomes at 1 Year After Total Hip Arthroplasty Combined With Low Femoral Neck Osteotomy vs Subtrochanteric Osteotomy in 73 Patients With Crowe Type IV Developmental Dysplasia of the Hip: A Retrospective Study.

BACKGROUND Crowe type IV developmental dysplasia of the hip (DDH) is characterized by complete proximal subluxation/dislocation of the femoral head, a small hypoplastic true acetabulum, and marked proximal femoral deformity. Total hip arthroplasty (THA) in Crowe IV DDH often requires femoral osteotomy for safe reduction. This retrospective study compared THA combined with low femoral neck osteotomy (LRNO) vs subtrochanteric osteotomy (SO). MATERIAL AND METHODS We reviewed 73 patients with Crowe IV DDH who underwent unilateral cementless THA from January 2017 to June 2024 (LRNO, n=37; SO, n=36)

Comprehensive assessment of acetabular morphology in developmental dysplasia of the hip: introducing novel parameters for pediatric evaluation.

Developmental dysplasia of the hip (DDH) represents a wide range of abnormalities affecting the acetabulum and femoral head. Traditional diagnostic tools such as anteroposterior pelvic radiographs are limited in evaluating the three-dimensional morphology of the acetabulum, especially in pediatric populations. This study introduces novel measurement parameters that include both angular and dimensional metrics for a comprehensive assessment of acetabular morphology. It also compares these parameters between normal and dysplastic hips in pediatric population. Computed tomography scans of 35 chil

Calcitonin-negative 'MTC-like' thyroid paraganglioma revealing an SDHD-related hereditary paraganglioma syndrome: functional confirmation of a non-canonical splice-site variant.

MATERIAL AND METHOD: Acetabular anteversion angle (AAA) and orientation angle of the iliac bone (AOOI) determined by use of a CT scan were studied in CDH. 55 children with CDH were selected on hip arthrography for this study. Subluxated hips were excluded (i.e. opposite hip of a unilateral CDH is normal). 10 boys and 45 girls with a mean of age of 2 years 1 month (extremes from 1 to 4 years 3 months) were studied. CT scan was performed before any orthopaedic treatment in 3 cases of bilateral luxation and 14 cases of unilateral. In the other cases, time between the end of orthopaedic treatment

Risk factors for delayed gait recovery after primary total hip arthroplasty in patients with crowe type I/II developmental dysplasia of the hip.

BACKGROUND: The gait recovery process after total hip arthroplasty (THA) in patients with developmental dysplasia of the hip (DDH) is complex. However, systematic evidence regarding risk factors for delayed early postoperative gait recovery in patients with Crowe type I/II DDH remains limited. METHODS: This retrospective study included 283 patients with Crowe type I/II DDH who underwent primary THA at our institution between January 2020 and December 2024. The primary outcome was delayed clinical gait recovery at 3 months after surgery, defined as persistent limping or Trendelenburg gait afte

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Does Open Reduction of Arthrogrypotic Hips Cause Stiffness?

BACKGROUND: Concerns of potential hip stiffness following open reduction for congenitally dislocated hips in arthrogryposis multiplex congenita (arthrogryposis) persist, despite published accounts of positive outcomes. We retrospectively compared preoperative and postoperative hip motion and evaluated the ambulatory abilities of patients who underwent the procedure. METHODS: Over a 10-year period, 52 patients with arthrogryposis had 75 open reductions for congenitally dislocated hips with a minimum follow-up of 2 years. The mean patient age at surgery was 23 months. Hip range of motion data w

O'Brien's Line Revisited: Does a Park-Harris Line of the Proximal Femur Predict Proximal Femoral Growth Disturbance in DDH?

BACKGROUND: O'Brien originally described a descending Park-Harris line of the proximal femur in patients treated for developmental dislocation of the hip (DDH), which reportedly predicted the absence of avascular necrosis/proximal femoral growth disturbance (PFGD). However, these findings have not been validated in a modern population using an objective classification system of PFGD. The purpose of this study is to evaluate the diagnostic utility of "O'Brien's Line" as a predictor of PFGD. METHODS: We retrospectively reviewed patients treated with closed or open reduction for unilateral DDH.

Femoral Trochlear Dysplasia Is Common in Lower Limbs With Hartofilakidis C2 Hip Dysplasia.

BACKGROUND: Patients with developmental dysplasia of the hip (DDH) can also present with deformities of the patellofemoral joint, such as femoral trochlear dysplasia. Although previous studies have reported that trochlear dysplasia is clinically important in some patients with DDH, the association between DDH and trochlear dysplasia is still unclear. QUESTIONS/PURPOSES: (1) Is trochlear dysplasia more common among lower limbs with bilateral DDH, with unilateral DDH, or without DDH (specifically, in the contralateral limb of patients with unilateral DDH)? (2) Is having more severe DDH (as meas

Is step-cut shortening osteotomy a better choice than transverse osteotomy for total hip arthroplasty for Crowe type III-IV hip dysplasia?

INTRODUCTION: Total hip arthroplasty for developmental hip dysplasia is a challenging surgery due to anatomic abnormalities. Crowe III and Crowe IV hip dysplasia generally necessitates a subtrochanteric shortening osteotomy. Transverse and step-cut osteotomy are the most common procedures for shortening of femur although there is still no consensus which one is a superior method. The objective of this study was to demonstrate whether transverse or step-cut osteotomy is superior in hips who undergo arthroplasty for high riding hip dysplasia. HYPOTHESIS: Our hypothesis was that higher rates of

Identifying Risk Factors for Disease Progression in Developmental Dysplasia of the Hip Using a Contralateral Hip Model.

BACKGROUND: Developmental dysplasia of the hip (DDH) can cause pain and premature osteoarthritis. The risk factors and timing for disease progression in adolescents and young adults have not been fully defined. This study aimed to determine the prevalence of and risk factors for contralateral hip pain and surgery after periacetabular osteotomy (PAO) on a dysplastic hip. METHODS: Patients undergoing unilateral PAO for DDH were followed for at least 2 years and categorized into contralateral pain and no-pain groups and contralateral surgery and no-surgery groups. Pain was defined with the modif

Hip reconstruction in children with cerebral palsy: does magnitude of surgery influence complications and outcomes?

Children with cerebral palsy (CP) and neuromuscular hip dysplasia (NMHD) frequently require hip reconstruction including femoral and pelvic osteotomies. Outcomes and complications in CP patients with NMHD who underwent varying degrees of hip reconstruction surgeries were examined. Ninety-nine patients with a diagnosis of CP who underwent surgical management of NMHD over a 15-year period (2005-2020) were reviewed. Three patient cohorts with varying degrees of surgery were analyzed including: (1) unilateral femoral osteotomy with pelvic osteotomy; (2) bilateral femoral osteotomy with unilateral

When to Combine Acetabular Osteotomy in Patients With Proximal Femoral Deformity Causing Residual Hip Dysplasia/Subluxation After Reduction of Developmental Dysplasia of the Hip.

BACKGROUND: After the successful reduction of developmental dysplasia of the hip, residual hip dysplasia may persist and lead to early osteoarthritis. Femoral and/or acetabular osteotomy has been used to address this problem. The purpose of this study is to determine the indication of femoral versus combined femoral-acetabular osteotomy in the management of residual hip dysplasia. METHODS: Fifty-five patients with unilateral dislocated-type dysplasia of the hip, who had residual hip dysplasia after reduction, underwent femoral osteotomy with or without acetabular osteotomy before 8 years of a

Gait pattern analysis before and after periacetabular osteotomy in unilaterally affected dysplastic patients.

BACKGROUND: Patients with acetabular dysplasia have an abnormal acetabular geometry which results in insufficient coverage of the femoral head. This coverage deficiency reduces contact surfaces within the joint, accelerating the wear of the articular cartilage and predisposing patients to early osteoarthritis. Periacetabular osteotomy is a surgical treatment of acetabular dysplasia that aims to reorient the acetabulum relative to the femoral head, increasing coverage. METHODS: Pelvic kinematics and lower limbs joint kinematics and kinetics during the stance phase of gait were recorded using a

Clinical and radiological outcomes of open reduction alone versus open reduction with pelvic osteotomy for developmental dysplasia of the hip in children over 1.5 years of age.

Developmental dysplasia of the hip (DDH) is commonly addressed through surgical intervention, usually performed in a specialized tertiary care facility. The purpose of this study was to evaluate the surgical outcomes in patients with DDH who had open reduction alone or in conjunction with bone surgery at our facility. We retrospectively reviewed the medical records of patients with DDH, categorizing them into two groups: Group OR underwent open reduction (OR) alone, and group ORBO underwent OR in conjunction with femoral or pelvic osteotomies. The modified McKay classification was used to eval

Outcomes for Patients With Developmental Hip Dysplasia Who Present After Six Months of Age.

PURPOSE: Late presentation of hip dysplasia persists despite robust screening methods. After 6 months of age, treatment with a hip abduction orthosis becomes challeng-ing, and all other treatment modalities have higher reported rates of complications. METHODS: We performed a retrospective review of all patients from 2003 to 2012 who had the sole diagnosis of de-velopmental hip dysplasia, who presented before 18 months of age, and who had at least 2 years of follow-up. The cohort was then grouped based on their presentation before (BSM) or after (ASM) 6 months of age. The groups were compared
