Non-Invasive Clinical Assessment of Medial Longitudinal Arch Morphology and Its Interrelationships in Foot and Ankle Musculoskeletal Pain: a Cross-Sectional Analytical Study.

OBJECTIVES: Accurate non-radiological assessment of the medial longitudinal arch (MLA) is essential for diagnosing foot deformities and guiding musculoskeletal pain management, yet no consensus exists on which method most reliably reflects MLA morphology in populations with ankle-foot pain, and comparative data across methods within the same clinical sample remain limited. This study evaluated and compared four non-radiological MLA measures and examined their interrelationships with ankle-foot musculoskeletal pain, physical activity and ankle-foot range of motion. MATERIALS AND METHODS: A cro

Exploring EPHB4 in gait disorders: VUS detection and phenotypic characterization in a toe-walking cohort.

BACKGROUND: Idiopathic toe walking (ITW) is a gait abnormality with a suspected genetic contribution, although its underlying mechanisms remain poorly understood. While ephrin type-B receptor 4 (EPHB4) is primarily recognized for its role in vascular development, this study explored whether EPHB4 variants identified in children with ITW were associated with specific clinical characteristics. METHODS: We retrospectively analyzed the clinical and genetic data of 20 children with toe walking who harbored EPHB4 variants identified using a targeted 49-gene next-generation sequencing (NGS) panel.

Combined Endoscopic SpeedBridge™ Reinsertion and Proximal Medial Gastrocnemius Lengthening for Chronic Insertional Achilles Tendinopathy in an Elite Sprinter: A Case Report.

INTRODUCTION: Insertional Achilles tendinopathy (IAT) is common in running athletes and, when refractory to conservative treatment, may require surgery. Open debridement and reinsertion are associated with wound-healing complications and prolonged recovery, which are particularly problematic in sprinters, whose performance depends heavily on the gastrocnemius-soleus-Achilles complex. Endoscopic double-row (SpeedBridge™) Achilles reinsertion may reduce soft-tissue morbidity, while concomitant gastrocnemius tightness is increasingly recognized in the pathogenesis of IAT. Pediatric toe-walking cohort with heterozygous SBF1 variants: A phenotypic description.

PURPOSE: Persistent toe walking is frequently labeled idiopathic; however, targeted genetic testing in selected cohorts can identify variants in genes implicated in neuromuscular disease. SBF1 is a known cause of autosomal recessive Charcot-Marie-Tooth disease type 4B3 (CMT4B3), whereas the clinical relevance of heterozygous SBF1 variants-particularly variants of uncertain significance (VUS)-remains unclear. We aimed to describe, in an exploratory manner, the clinical features of children with persistent toe walking in whom heterozygous SBF1 variants were identified, and to contextualize these

Clinical Features in Children with Persistent Toe Walking Who Carry Heterozygous PYGM Variants: A Cross-Sectional Study.

OBJECTIVES: Persistent toe walking (TW) in children is often idiopathic but may conceal subtle neuromuscular or metabolic causes. This cross-sectional, retrospective study investigated clinical and genetic characteristics of children carrying PYGM variants to explore potential subclinical phenotypes among heterozygous carriers. METHODS: Seventy-two (72) children meeting criteria for idiopathic TW were evaluated using a standardized clinical protocol and a validated 49-gene next-generation sequencing neuromuscular panel. Variants were classified as pathogenic, likely pathogenic, or of uncertai

Enhancing Podiatry Students' Understanding of Foot Arch Through Footprint Activity.

BACKGROUND: Understanding the structural anatomy and biomechanics of the foot arches is essential for podiatry students due to its clinical relevance in diagnosing conditions such as Pes Planus and Pes Cavus. The foot arches form a flexible base that supports body weight, absorbs shock and functions as a spring lever during activities such as walking and jumping. Therefore, understanding this anatomical knowledge is fundamental for podiatry students for maintaining foot health and supporting clinical assessment and treatment planning. Traditional pedagogical methods often face limitations due

Enlarging the phenotypical spectrum of DYNC1H1-related epilepsy.

BACKGROUND: Epilepsy is a frequent feature of genetic neurodevelopmental disorders and is increasingly recognized as a disorder of disrupted neuronal networks rather than isolated focal pathology. DYNC1H1, encoding the heavy chain of cytoplasmic dynein 1, is implicated in a broad spectrum of neuromuscular and central nervous system disorders, including intellectual disability, malformations of cortical development (MCDs), and epilepsy. CASE PRESENTATION: We report a female patient with drug-resistant epilepsy beginning at 9 years of age, normal brain MRI and multiple seizure types, including f

Compound Heterozygous COA7 Variants Presenting With Childhood-Onset Axonal Neuropathy in 2 Siblings.

OBJECTIVES: Variants in COA7 (cytochrome c oxidase assembly factor 7) are a rare cause of mitochondrial disease, with limited clinical descriptions and phenotypic variability. We describe 2 siblings carrying compound heterozygous COA7 variants, one of which (c.457C>T; p.Leu153Phe) is novel. Both presented with early-onset, slowly progressive axonal sensorimotor neuropathy, with differences in severity and associated features. METHODS: Patients were identified based on clinical presentation and evaluated through longitudinal neurologic, neurophysiologic, genetic, biochemical, and neuroimaging

Radiographic Angle-Based Machine Learning Models for the Diagnosis of Pes Planus and Pes Cavus: A Large-Scale Study Using Weight-Bearing Lateral Foot Radiographs.

Background/Objectives: Pes planus and pes cavus are common foot deformities, which may lead to pain, functional limitations, and impairment of foot biomechanics. While calcaneal pitch, talar declination, and Meary angles, commonly used in diagnosis, provide objective information, their lack of a gold standard and the observer's dependence on manual measurements limit their reliability. Therefore, in this study, these angles obtained from weight-bearing lateral foot radiographs were evaluated according to literature references, and the aim was to determine the model that provides the most accur

Toe walking: A regional analysis of TTN variants between c.10,000 and c.99,999 in a cohort of children.

BACKGROUND: Persistent toe walking is traditionally considered idiopathic when no neurological or orthopedic cause is identified. Emerging evidence suggests genetic contributors, including TTN, but genotype-phenotype relationships remain poorly understood in affected children. METHODS: This retrospective descriptive study evaluated children with persistent toe walking assessed between November 2022 and April 2026 at a specialized physiotherapy practice. All participants underwent standardized clinical assessment and targeted next-generation sequencing. Individuals carrying TTN variants within

Late-onset CMT neuropathy in the NGS era: a 10-year retrospective study.

BACKGROUND AND AIM: Late-onset Charcot-Marie-Tooth (CMT) disease remains under-recognized and may mimic acquired neuropathies in older adults. We aimed to determine the prevalence and clinical, electrophysiological, and genetic characteristics of genetically confirmed late-onset CMT and to identify features distinguishing it from late-onset non-CMT neuropathies. METHODS: We retrospectively reviewed all patients who underwent next-generation sequencing (NGS) for suspected hereditary neuropathy between 2015 and 2025 at a single neuromuscular referral center. Late-onset CMT was defined by sympto

Subclinical Facial Nerve Enlargement in a Case of Chronic Demyelinating Peripheral Neuropathy.

We report an archival temporal bone case from the Otopathology Archives at Johns Hopkins of a man with progressive distal-predominant weakness, sensory loss, areflexia, and bilateral pes cavus, with clinicopathologic features suggestive of a chronic demyelinating peripheral neuropathy. Bilateral temporal bone histopathology demonstrated diffuse enlargement of the facial nerve throughout its intratemporal course, with additional enlargement of the glossopharyngeal and vagus nerves. This case suggests that marked cranial nerve hypertrophy may be clinically unapparent in some hereditary polyneuro

Weightbearing CT Reveals No Anatomical Features Associated With Ligamentous Lisfranc Injuries: A Matched Case-Control Study.

BACKGROUND: Several radiographic and cadaveric studies have suggested that specific anatomical features of the Lisfranc joint complex, including a shallower medial mortise of the second metatarsal (M2), shorter M2 length, or pes cavus alignment, may be associated with ligamentous Lisfranc injuries. These associations have largely been derived from 2-dimensional imaging or post-injury assessments. Weightbearing computed tomography (WBCT) enables 3-dimensional evaluation of midfoot anatomy under physiologic load and allows a more robust reassessment of these proposed anatomical associations.
