New phenotype of adult alpha-L-iduronidase deficiency (mucopolysaccharidosis I) masquerading as Friedreich's ataxia with cardiopathy.

Clinical, ultrastructural and biochemical studies are reported in a 42-year-old woman presenting with congenital pes cavus who, at the age of 23 years, developed slowly progressive distal amyotrophies, hypesthesia, bilateral hearing loss and severe cardiopathy leading to death. There were skeletal anomalies, mild reduction of motor NCVs, but no corneal opacity, retinitis pigmentosa, organomegaly or vacuolated lymphocytes. Autopsy disclosed severe thickening of fibrous tissues (endocardium, cerebrospinal dura) with accumulation of vacuolated cells containing glycosaminoglycans in numerous membr

X-linked dominant hereditary motor and sensory neuropathy.

Modern techniques have defined the hereditary motor and sensory neuropathies (HMSN) as a genetically heterogeneous group of disorders. This includes a rare variant with X-linked dominant inheritance. We have traced this disorder through 6 generations of a large Canadian kindred; neurological and electrophysiological examinations were performed in 57 family members and nerve biopsies were studied in 2 affected males, early and late in the disease; 42/83 family members were affected. No male-to-male transmission was encountered in 19 sons of affected fathers, whereas all their daughters expresse

Conservative versus neurosurgical treatment of tethered cord patients.

We compared the outcome of neurosurgical release of a tethered spinal cord in 20 children with the neurological evolution of 21 other children known iwth a tethered spinal cord on nuclear magnetic resonance scan (NMR). Neurosurgery yielded stabilisation of the symptoms without any lasting improvement. Pre-operative conservative medical treatment had to be continued in all operated children. Three of the 20 operated children are showing signs of retethering despite appropriate neurosurgery, indicating that retethering should be considered as a major and frequent complication. In the majority of

Hereditary induced peripheral neuropathies.

The hereditary neuropathies, representing up to 20 per cent of the total of peripheral neuropathies, present a considerable problem in neurology, rehabilitation medicine, and orthopedics. Patients have predominant foot abnormalities such as pes cavus and talipes equina varus. The largest section is the Charcot-Marie-Tooth syndrome or peroneal muscular atrophy, followed by the leucodystrophies and adrenal myloneuropathy. Information is burgeoning with the advent of molecular genetics, and we anticipate therapeutic options when gene products are discovered.

[Familial idiopathic vitamin E deficiency associated with cerebellar atrophy].

Sibling cases of familial vitamin E deficiency accompanied by ataxia, polyneuropathy and mental retardation were reported. Case 1 was a 37-year-old male who developed progressive gait disturbance, deformity of the feet and head tremor from childhood, after normal delivery and development of early childhood. On physical examination, he had cataract, high arched palate and pes cavus. Neurological examination revealed mental retardation (WAIS 68), scanning speech, muscular atrophy of the face and extremities with predominance in the lower limbs, absent Achilles tendon reflex, disturbance of super

[Two siblings of distal hereditary motor neuropathy with choroideremia].

Two brothers, 31 and 25 years of age, with distal hereditary motor neuropathy (distal HMN) and choroideremia are reported. Their parents were second cousins. During the first decade, their visual acuity gradually diminished with night blindness, accompanied by weakness of the legs. From the third decade, they noticed weakness of the hands. The neurological signs of both cases could be summarized as follows: bilateral pes cavus, Charcot-Marie-Tooth type distal atrophy and weakness of the four limbs, no sensory deficit, hyperreflexia in the upper limbs and knees, absent ankle jerks and plantar r

[Chronic peripheral neuropathies with corticosensitive heredodegenerative aspects: 2 cases].

Two cases of peripheral neuropathy beginning in childhood are reported. Both had features suggestive of HMSN with pes cavus and kyphoscoliosis. Familial history of HMSN type I was present in 9 members of the kinship in case 1. In case 2, a mild neuropathy without pes cavus was present in the father. Case 1 developed four relapses of ascending sensory-motor deficit with conduction blocks suggestive of a superimposed chronic inflammatory polyneuropathy (CIP) which improved under prednisone. In case 2, the course was progressive with a severe proximal and distal motor weakness and sensitive distu

Neurologic findings in men with isolated hypogonadotropic hypogonadism.

We studied the neurologic abnormalities in 41 men with isolated hypogonadotropic hypogonadism. Findings included anosmia, hyposmia, mirror movements, ocular motor abnormalities, cerebellar dysfunction, and pes cavus foot deformity. One-third of the subjects had a family history of delayed sexual maturation. Patients with a family history of delayed sexual maturation had a significantly higher incidence of olfactory dysfunction, mirror movements, and pes cavus foot deformity. Our data suggest that isolated hypogonadotropic hypogonadism and its accompanying neurologic abnormalities may arise fro

[Changes in the skeleton of the middle foot caused by professional classical dancing].

UNLABELLED: On half and three-quarter points, the most frequent foot positions in dance, the metatarsals form an extension of the lower leg. In 54% of 56 professional male and female dancers whose feet were examined, this extraordinary stress leads to pronounced thickening of the cortex of the second metatarsal and in 6%, of the third metatarsal as well. There are no symptoms. Hypertrophy of the bone occurs in response to the additional stress imposed by dancing. Dancing on points is not the triggering factor, since male and female dancers are equally affected. COMPLICATIONS: Under intensive

[Clinical and radiologic comparison of "idiopathic" and "neurogenic" pes cavus].

41 patients with 66 cavus feet were reviewed clinically and radiologically according to the parameters of Dennemann at an average age of 41.3 years. The etiology was a so-called "idiopathic pes cavus" in 51 per cent of the cases, in the others, a neurological basic disease (15 per cent condition after poliomyelitis, 3 per cent meningocele, 6 per cent Friedreich's ataxia, 10.5 per cent other neurological disorders) or condition after congenital club foot or trauma. Exact neurological and electrodiagnostic examination of the cavus feet primarily staged as idiopathic revealed sensory or motor los

Fiber type disproportion in nemaline myopathy.

A 21-year-old female with nemaline myopathy and concomitant dysmorphism characteristic of elongated face, high arched palate, scoliosis and pes cavus is reported. Neurological examination revealed mild bilateral facial weakness, moderate proximal muscle weakness, and generalized absence of deep tendon reflexes in all limbs. Electromyography disclosed a moderate amount of fibrillations and positive, and short duration polyphasic waves. A biopsied specimen, obtained from the vastus lateralis muscle, showed abundant nemaline bodies from pathological study, and electron dense particles on Z-line b

Familial spastic paraparesis with amyotrophy of the hands was reported, and its significance in the literature was reviewed. Case 1: An 18 year-old boy, who had been suffering from spastic gait since 12 years old, noticed his hand muscle wasting distributed bilateral first interosseous muscle, thenar and hypothenar muscle at age 17. Case 2: A 20 year-old man, elder brother of case 1, who also walked in spastic manner from his childhood, developed bilateral hand muscle atrophy similar to case 1 at age 19. Clinical features of these two cases could be summarized as familial spastic paraparesis w

[A case of MELAS (mitochondrial myopathy, encephalopathy, lactic acidosis and stroke-like episodes) with progressive cytochrome c oxidase deficiency].

We report a 9 year-old boy with MELAS. High dosed oral thiamine administration and high fat diet induced remarkable neurological and biochemical improvement. His mother had episodic headaches and hemiplegia, probably MELAS. He complained muscle weakness and repeated episodes of vomiting started from 2 years of age. High levels of serum lactate and pyruvate were recognized, but with no metabolic acidosis. He developed generalized muscle weakness, growth retardation, generalized convulsions and stroke-like episodes at 5 years old. Optic nerve atrophy and mental retardation gradually appeared. A

[A case of motor and sensory neuropathy with elevated serum lactate and pyruvate which responded to large dose of coenzyme Q10 therapy].

A 16-year-old high school male student was admitted to our hospital with complaints of difficulty in walking and muscle atrophy of the lower legs. He noticed his gait disturbance when he was about 12 years old and his symptoms had gradually increased. On examination, he was unable to walk on his heels and on his toes. He had mild pes cavus and marked muscle wastes of the lower legs. The weakness was limited to the feet, lower legs, and hands. Mild sensory losses were demonstrated inside of the feet. Autonomic dysfunction was not present. The deep tendon reflexes were diminished. Nerves were no

Hereditary motor and sensory neuropathy type I and motor neuron disease. An unusual association.

A 34-year-old man experienced progressive muscle weakness and wasting in the shoulder girdle later spreading distally to the upper limbs. Neurological examination revealed signs of pyramidal and bulbar involvement as well as widespread fasciculations. Pes cavus and distal hypoesthesia were also observed. MCV and nerve biopsy findings were consistent with HMSN-I, while EMG pattern suggested a MND. The association of HMSN-I with MND has not been previously described in literature.

Assessment and management of pes cavus in Charcot-Marie-tooth disease.

The sequential approach to evaluating the cavus foot is integrated with a description and assessment of the various treatment options. Decision making in the treatment of these cases is complicated by the progressive neurologic condition that underlies many of these deformities. An effort is made to recommend the most appropriate surgical intervention based on the nature of the deformity and its rigidity. Although these principles apply to all cavus feet, the deformity in Charcot-Marie-Tooth disease is the most difficult to treat and the most prone to recurrence because of the progressive natu

[A family of hereditary motor and sensory neuropathy with spastic paraplegia (HMSN type V)].

Three siblings with hereditary motor and sensory neuropathy with spastic paraplegia (HMN V) were described. Their grandfather was suspected to have the similar symptoms. Their parents were normal and not consanguineous. The first case was a 54-year-old woman. She has suffered from difficulty in walking since the age of 10 years. Distal muscular weakness and wasting began at 20 years old. She was admitted to our hospital for investigation in 1988. Results of examination of the brain, cranial nerve, and cerebellar function were normal. The deep reflexes were hyperactive in the arm and knee, whil

[A misdiagnosed disorder: syndrome of the musculocutaneous nerve of the foot].

The syndrome of the pedal dorsal cutaneous nerve (described by one of us in 1979) is caused by irritation of the nerve in its course at the dorsum pedis. This is a rather frequent syndrome, often overlooked. It is manifested by a distinct association of atypical pain at the dorsum pedis and in the foot. After a review of 10 cases and a literature survey, the authors describe the characteristic signs of this syndrome. There are many factors at the origin of this pathology: static deformities (pes cavus anterior, valgus calcaneus, hallux valgus), local trauma or repeated microtrauma (ill sitting

[Ulcero-mutilating acro-osteopathy in hereditary neuropathies. Differential diagnosis and pathogenesis].

Two patients, aged 51 and 70 years, had indolent ulcerations at the sole of the foot with destructive osteolysis in the bones of the feet, lesions characteristic of mutilating ulcerative acro-osteopathy. Patient 1, who had pes cavus, developed ulcerations on the balls of the feet along the second metatarsal bones, and patient 2 with pes equinovarus developed ulcers in the area of the calcaneus. Sock-like hypesthesia/hypalgesia from the toes to the ankles was present in both patients, and electrophysiological tests confirmed the presence of axonal sensory-motor neuropathy. Following an anti-smallpox vaccination a 17 year-old boy developed spontaneous muscular activity with cramps and fasciculations in the lower limbs. EMG showed a significant spontaneous activity and was neurogenic in the lower limbs. Nerve conduction velocities were consistent with axonal damage. Muscular biopsy showed aspects of chronic denervation. Neuromyotonia disappeared with carbamazepine treatment. This case can be added to the reported associations of neuromyotonia and Cha
