A Modified Subcondylar Endaural Approach for Proportional Condylectomy in Condylar Hyperplasia: Clinical Report.

BACKGROUND: Condylar hyperplasia is characterized by excessive unilateral growth of the mandibular condyle, resulting in facial asymmetry, occlusal disturbances, and functional impairment. It is usually managed by proportional condylectomy. Different surgical approaches have been described, each associated with functional and aesthetic implications. METHODS: A systematic review was conducted following PRISMA guidelines using PubMed, Scopus, Web of Science, and Cochrane databases. Studies reporting extraoral approaches for proportional condylectomy in patients with condylar hyperplasia were in

Profiles of power: lateral posing asymmetries of emotional expression in political portraits.

Facial appearance plays a key role in social and political judgments, yet the mechanisms linking facial pose and emotional expression remain unclear. One robust phenomenon is a left-cheek posing bias, often attributed to asymmetries in emotional expression. The current study examined this issue using facial photographs of all candidates (N = 1,107) in a Japanese national election. Head orientation and smile intensity were estimated using image-based measures. A reliable left-cheek bias was observed, with a stronger bias among conservative politicians. Left-cheek posing was not associated with

Efficacy and Safety of Vacuum Bell Therapy in Pectus Excavatum: A Systematic Review of Clinical Outcomes.

INTRODUCTION: Pectus excavatum (PE) is the most common congenital chest wall deformity. The vacuum bell (VB) is a noninvasive alternative to surgical correction, increasingly used in pediatric and adolescent patients. However, evidence on its efficacy, safety, and predictors of success remains fragmented. OBJECTIVE: To systematically review and synthesize available evidence on the efficacy, safety, and clinical outcomes of VB therapy in patients with PE. METHODS: A systematic review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 g

Contralateral condylar degenerative changes in patients with unilateral mandibular condylar hyperplasia.

Unilateral mandibular condylar hyperplasia (UCH) is one of the causes of mandibular asymmetry, leading to both aesthetic concerns and temporomandibular joint (TMJ) disorders. This observational study investigated the association between active UCH and degenerative changes in the contralateral TMJ. Using a standardized radiographic scoring system, 105 patients with active UCH, 23 with non-UCH facial asymmetry, and 23 healthy controls were compared. Significantly higher degenerative scores were found in the contralateral TMJs of UCH patients compared to their ipsilateral (affected) side (median:

Investigating smiling asymmetries in Parkinson's disease through the whistle-smile reflex.

Hypomimia is a hallmark of Parkinson's disease (PD), but whether emotional expressions show lateralized abnormalities remains unclear. While asymmetry has been described for posed smiles, no study has examined true spontaneous smile asymmetry in PD. The whistle-smile reflex (WSR), an involuntary smile elicited after voluntary whistling, offers an opportunity to investigate a form of smiling that closely resembles typical spontaneous expressions of joy. In the present study, we aimed to assess the frequency, intensity, and asymmetry of smiling elicited by the WSR in PD compared with healthy con

Cryoanalgesia in Thoracic Surgery.

Cryoanalgesia is a technique for reducing postoperative pain that involves freezing peripheral nerves while preserving their ability to regain function over time. The technique has been used for decades, and randomized trials have shown a reduction in pain and opioid use in some patients undergoing thoracotomy and pectus excavatum repair. In these trials, outcomes were highly dependent on the control groups. Randomized studies of patients undergoing minimally invasive thoracic surgery have failed to show the same results. Furthermore, several studies have reported chronic pain and neuropathic-

Cerebrofacial arteriovenous metameric syndrome: Case report and brief literature review.

Cerebrofacial arteriovenous metameric syndrome (CAMS) is a rare, noninherited vascular malformation characterized by an arteriovenous malformation of the cerebral, orbital, and facial structures. Patients with CAMS often present with visual loss, progressive neurological deficits, seizures, and or bleeding. They may also present with cosmetic concerns such as skin discoloration, proptosis and facial asymmetry. Diagnosis of CAMS is based on history, clinical presentation, and findings on cerebral computed tomography and/or magnetic resonance imaging. We share a case of a young man who presented

Clinical outcomes of sub-millisievert CT evaluation of congenital chest wall deformity using photon counting CT: a new standard of care?

BACKGROUND: Pectus excavatum is the most common congenital chest wall deformity and is typically treated surgically. Disease severity is commonly assessed using the CT-derived Haller Index (HI), creating opportunities to reduce radiation exposure in pediatric patients. Photon-counting detector CT (PCD-CT) enables substantial dose reduction; however, the clinical impact of sub-millisievert (sub-mSv) imaging has not been well established. OBJECTIVE: To evaluate the impact of sub-millisievert (sub-mSv) PCD-CT scans on clinical outcomes in the preoperative assessment of congenital pectus deformit

Posterior quadrant disconnection with selective posterior corpus callosotomy for drug-resistant epilepsy in Dyke-Davidoff-Masson syndrome.

Dyke-Davidoff-Masson syndrome (DDMS) is a rare condition classically characterised by seizures with contralateral hemiparesis (often with facial asymmetry and variable cognitive impairment) and unilateral cerebral hemiatrophy on neuroimaging, frequently accompanied by compensatory calvarial thickening and paranasal sinus hyperpneumatisation. We report a woman in her early 20s with DDMS and medically refractory epilepsy who underwent a combined posterior quadrant disconnection with a selective posterior corpus callosotomy. Postoperatively, she experienced transient transcortical motor aphasia a

Styloid Process Elongation as an Incidental Finding in Adult Orthodontic Patients: Prevalence, Morphology and Diagnostic Implications from Panoramic Radiography.

Background: The styloid process exhibits considerable anatomical variability, and its elongation is frequently identified as an incidental finding on panoramic radiographs. However, limited evidence exists regarding its prevalence and possible association with craniofacial skeletal characteristics in adult orthodontic patients. Therefore, the aim of this study was to evaluate the prevalence, morphological characteristics, mineralisation patterns, and possible association of styloid process elongation with skeletal class and facial pattern in an adult orthodontic population. Methods: A retrospe

Cannula-assisted sternal displacement for VATS correction of pectus excavatum in three cats.

CASE SERIES SUMMARY: Three skeletally immature cats were treated for pectus excavatum using a modified video-assisted thoracoscopic surgery (VATS) technique that involved using a thoracoscopic cannula to intraoperatively ventrally displace affected sternebrae, thereby assisting the placement of circumsternal sutures while simultaneously protecting underlying viscera. All cases recovered well from their procedure, and the described technique was successful in allowing good radiographic improvement of pectus excavatum in all cases. No intraoperative complications were encountered and all cases w

Left-Sided Mandibular Condylar-Ramus Deficiency Within the Craniofacial Microsomia Spectrum in a 10-Year-Old Patient: A Case Report.

We present the case of a 10-year-old female patient with a progressive limitation of mouth opening since early childhood, worsening at eight years of age, with difficulty tolerating solid food and impaired mastication. Physical examination revealed facial asymmetry predominantly affecting the lower third of the face, with a maximal interincisal opening of less than 8 mm and mandibular deviation toward the left during mouth opening. Computed tomography with three-dimensional reconstruction demonstrated a marked hypoplasia of the left mandibular condylar-ramus unit, a severe shortening of the ra

Correction of Mandibular Asymmetry With Bilateral Asymmetric Custom-Made 3D Implant.

BACKGROUND: Unilateral mandibular hypoplasia affects not only the involved side but also the contralateral hemimandible. Thus, treatment must address bilateral deformities. The authors present a novel bilateral CAD/CAM titanium onlay implant design for unilateral mandibular hypoplasia and evaluate outcomes using objective and patient-reported measures. PATIENTS AND METHODS: Thin-slice CT scans were processed with Mimics® software. The unaffected hemimandible was repositioned to the midline using the TMJ as a pivot, and its mirror image was used to create the contralateral side. Bilateral cust

Condylar hyperactivity, an anatomical variation: a critical integrative review bridging biology, diagnosing, and decision-making in orthognathic surgery.

Unilateral condylar hyperactivity (CH) is a craniofacial condition characterised by excessive growth of one mandibular condylar process that leads to progressive facial asymmetry. Patients commonly require combined orthodontic and surgical interventions to restore function, facial symmetry, and quality of life, posing substantial financial burdens on individuals and healthcare systems. Diagnosis has traditionally relied on clinical and imaging findings, including visible facial asymmetry, enlargement of the mandibular ramus and/or condyle on two-dimensional (2D) and three-dimensional (3D) imag

Right Ventricular Compression From Pectus Excavatum: A Reversible Cause of Ventricular Dysfunction.

Pectus excavatum can rarely result in clinically significant right ventricular (RV) compression, leading to impaired diastolic filling, reduced stroke volume, and secondary biventricular dysfunction involving both the right and left ventricles. We report a 19-year-old male who presented with progressive exertional dyspnea and was found to have severe pectus excavatum with a Haller index of 5.8 on cardiac magnetic resonance imaging and 3.6 on computed tomography. Multimodal cardiac evaluation demonstrated focal RV compression, moderate RV systolic dysfunction on cardiac MRI with a right ventric

Three-dimensional volumetric analysis of mandibular condyle changes in active growing condylar hyperplasia.

Condylar hyperplasia is characterized by excessive mandibular condylar growth, causing progressive facial asymmetry and occlusal discrepancies. This study investigates the transition from active to inactive growth stages, a critical determinant for optimizing surgical timing and selecting appropriate intervals for corrective orthognathic surgery. A retrospective study was conducted involving 30 patients with unilateral condylar hyperplasia (mean age 21.90 ± 4.16 years) at a university hospital. Assessment of condylar hyperplasia involved measuring metabolic activity through bone scintigraphy,
