Contralateral condylar degenerative changes in patients with unilateral mandibular condylar hyperplasia.

Unilateral mandibular condylar hyperplasia (UCH) is one of the causes of mandibular asymmetry, leading to both aesthetic concerns and temporomandibular joint (TMJ) disorders. This observational study investigated the association between active UCH and degenerative changes in the contralateral TMJ. Using a standardized radiographic scoring system, 105 patients with active UCH, 23 with non-UCH facial asymmetry, and 23 healthy controls were compared. Significantly higher degenerative scores were found in the contralateral TMJs of UCH patients compared to their ipsilateral (affected) side (median:

Investigating smiling asymmetries in Parkinson's disease through the whistle-smile reflex.

Hypomimia is a hallmark of Parkinson's disease (PD), but whether emotional expressions show lateralized abnormalities remains unclear. While asymmetry has been described for posed smiles, no study has examined true spontaneous smile asymmetry in PD. The whistle-smile reflex (WSR), an involuntary smile elicited after voluntary whistling, offers an opportunity to investigate a form of smiling that closely resembles typical spontaneous expressions of joy. In the present study, we aimed to assess the frequency, intensity, and asymmetry of smiling elicited by the WSR in PD compared with healthy con

Cerebrofacial arteriovenous metameric syndrome: Case report and brief literature review.

Cerebrofacial arteriovenous metameric syndrome (CAMS) is a rare, noninherited vascular malformation characterized by an arteriovenous malformation of the cerebral, orbital, and facial structures. Patients with CAMS often present with visual loss, progressive neurological deficits, seizures, and or bleeding. They may also present with cosmetic concerns such as skin discoloration, proptosis and facial asymmetry. Diagnosis of CAMS is based on history, clinical presentation, and findings on cerebral computed tomography and/or magnetic resonance imaging. We share a case of a young man who presented

Posterior quadrant disconnection with selective posterior corpus callosotomy for drug-resistant epilepsy in Dyke-Davidoff-Masson syndrome.

Dyke-Davidoff-Masson syndrome (DDMS) is a rare condition classically characterised by seizures with contralateral hemiparesis (often with facial asymmetry and variable cognitive impairment) and unilateral cerebral hemiatrophy on neuroimaging, frequently accompanied by compensatory calvarial thickening and paranasal sinus hyperpneumatisation. We report a woman in her early 20s with DDMS and medically refractory epilepsy who underwent a combined posterior quadrant disconnection with a selective posterior corpus callosotomy. Postoperatively, she experienced transient transcortical motor aphasia a

Styloid Process Elongation as an Incidental Finding in Adult Orthodontic Patients: Prevalence, Morphology and Diagnostic Implications from Panoramic Radiography.

Background: The styloid process exhibits considerable anatomical variability, and its elongation is frequently identified as an incidental finding on panoramic radiographs. However, limited evidence exists regarding its prevalence and possible association with craniofacial skeletal characteristics in adult orthodontic patients. Therefore, the aim of this study was to evaluate the prevalence, morphological characteristics, mineralisation patterns, and possible association of styloid process elongation with skeletal class and facial pattern in an adult orthodontic population. Methods: A retrospe

Left-Sided Mandibular Condylar-Ramus Deficiency Within the Craniofacial Microsomia Spectrum in a 10-Year-Old Patient: A Case Report.

We present the case of a 10-year-old female patient with a progressive limitation of mouth opening since early childhood, worsening at eight years of age, with difficulty tolerating solid food and impaired mastication. Physical examination revealed facial asymmetry predominantly affecting the lower third of the face, with a maximal interincisal opening of less than 8 mm and mandibular deviation toward the left during mouth opening. Computed tomography with three-dimensional reconstruction demonstrated a marked hypoplasia of the left mandibular condylar-ramus unit, a severe shortening of the ra

Correction of Mandibular Asymmetry With Bilateral Asymmetric Custom-Made 3D Implant.

BACKGROUND: Unilateral mandibular hypoplasia affects not only the involved side but also the contralateral hemimandible. Thus, treatment must address bilateral deformities. The authors present a novel bilateral CAD/CAM titanium onlay implant design for unilateral mandibular hypoplasia and evaluate outcomes using objective and patient-reported measures. PATIENTS AND METHODS: Thin-slice CT scans were processed with Mimics® software. The unaffected hemimandible was repositioned to the midline using the TMJ as a pivot, and its mirror image was used to create the contralateral side. Bilateral cust

Condylar hyperactivity, an anatomical variation: a critical integrative review bridging biology, diagnosing, and decision-making in orthognathic surgery.

Unilateral condylar hyperactivity (CH) is a craniofacial condition characterised by excessive growth of one mandibular condylar process that leads to progressive facial asymmetry. Patients commonly require combined orthodontic and surgical interventions to restore function, facial symmetry, and quality of life, posing substantial financial burdens on individuals and healthcare systems. Diagnosis has traditionally relied on clinical and imaging findings, including visible facial asymmetry, enlargement of the mandibular ramus and/or condyle on two-dimensional (2D) and three-dimensional (3D) imag

Three-dimensional volumetric analysis of mandibular condyle changes in active growing condylar hyperplasia.

Condylar hyperplasia is characterized by excessive mandibular condylar growth, causing progressive facial asymmetry and occlusal discrepancies. This study investigates the transition from active to inactive growth stages, a critical determinant for optimizing surgical timing and selecting appropriate intervals for corrective orthognathic surgery. A retrospective study was conducted involving 30 patients with unilateral condylar hyperplasia (mean age 21.90 ± 4.16 years) at a university hospital. Assessment of condylar hyperplasia involved measuring metabolic activity through bone scintigraphy,

Structural Patterns of the Mandibular Condyle in Facial Asymmetry and Dentofacial Deformities.

Facial asymmetry is defined as a clinical discrepancy between the 2 sides of the face and is considered multifactorial in origin. It may affect any facial region, although it is most frequently observed in the lower third of the face. In this context, the mandibular condyle represents a primary growth center whose variations in behavior may lead to changes in volume and morphology. The aim of this study was to analyze the morphologic characteristics of the mandibular condyle in subjects with facial asymmetry associated with sagittal class II deformity (AF-CII) and in subjects with facial asymm

Facial Retaining Ligaments Influence Basal Cell Carcinoma Growth in the Midface and Periorbital Region.

OBJECTIVES: Facial retaining ligaments (RLs) are well-characterized for their role in soft-tissue support and in rhytidectomy. This study investigates whether RLs also act as anatomical barriers that influence the spread of basal cell carcinoma (BCC). We hypothesized that the dermal insertion of RLs impedes the dermal spread pattern of BCCs, specifically causing them to grow along the RL. METHODS: We conducted a retrospective cohort study of 163 patients who underwent Mohs micrographic surgery (MMS) for BCC. Post-MMS defects were used as a proxy for the BCC size and shape. The defects were ca

A systemic review of facial expression recognition (FER) in stroke: diagnosis and emerging applications in rehabilitation.

BACKGROUND: Stroke remains a leading cause of long-term disability and mortality worldwide, the early identification and evaluation of rehabilitation effectiveness are of great significance. Facial expression recognition (FER), a computer vision technology, offers potential for early screening and monitoring in many conditions. However, its application in stroke remains underexplored. AIMS: To offer insights into how the FER can be used in stroke identification and rehabilitation monitoring. METHODS: We systematically searched four databases, including PubMed, Web of Science, CNKI, and WanFa

Facial Asymmetry as a Universal Signature: A Morphometric, Perceptual, and Artistic Approach.

BACKGROUND: Facial asymmetry is universal, yet its directionality and its relationship with patients' self-perception remain underexplored. This study evaluates the association between 3-dimensional-measured malar asymmetry and the patient's declared "better side" in an aesthetic population. METHODS: One hundred twenty consecutive patients were photographed in standardized frontal view using the Vectra H2 system. Malar width was assessed by measuring the zygion-midline distance on each side, recorded twice and averaged. Patients completed a 10-item ad hoc questionnaire addressing their prefer

X Suture Technique for Alar Base Contouring and Nostril Symmetry in Rhinoplasty.

Alar base surgery plays a critical role in achieving both aesthetic harmony and functional integrity in rhinoplasty. Despite appropriate alar base resection, residual asymmetry and undesired nostril shape variations may persist, particularly in patients with intrinsic facial asymmetry or prior surgery. In this study, we describe a novel X suture technique designed to refine alar base contour and optimize nostril shape. This clinical observational study included 32 patients (18 primary and 14 revision rhinoplasty cases) with varying degrees of nostril asymmetry or shape irregularities. All proc

The Exoscope in the Excision of Medial Orbital Dermoid Cysts.

Orbital dermoid cysts are the most common orbital tumors in childhood. Although typically superficial and lateral, uncommon medial lesions may pose surgical challenges due to proximity to critical structures. The exoscope offers high-definition, 3-dimensional visualization and ergonomic advantages to surgeons in these complex anatomical cases. A 4-year-old patient presented with progressive globe displacement and fullness of the right upper lid. Imaging revealed a 1.7 × 1.3 cm circumscribed lesion in the superior medial orbit, displacing the globe and abutting the superior rectus muscle. The p

Asymmetrical Correction of Facial Rotation Using Unilateral Thread Insertion.

BACKGROUND: Facial asymmetry, particularly rotational asymmetry, represents a significant aesthetic concern that impacts facial harmony and perceived attractiveness. Traditional approaches to facial rejuvenation often employ symmetrical interventions, which may fail to address the underlying rotational patterns. This study investigates the efficacy of asymmetrical thread lifting using JETEMA threads (Epiticon, Bi-thread, JETEMA, Korea) for correction of facial rotation. OBJECTIVE: To evaluate the effectiveness of standardised unilateral thread placement in the nasolabial fold area for correct

Primary intraosseous cavernous hemangioma of the zygomatic bone.

PURPOSE: We present an uncommon case of primary intraosseous cavernous hemangioma involving the zygoma which displayed CD31 positivity on pathologic analysis. OBSERVATIONS: Macroscopic examination showed a bulging purple-red mass intrinsic to bone measuring 2.1 × 1.8 × 1.6 cm (Fig 2A). Histopathology demonstrated the lesion was comprised of thin-walled vascular channels lined by a monolayer of endothelial cells interspersed among bony trabeculae (Fig 2B) consistent with intraosseous cavernous hemangioma.7,8 Vascular walls lacked both venous and arterial features. Morphometric analysis and spatial localization of the infraorbital foramen in adult dry human skulls: bilateral symmetry, sexual dimorphism, and accessory foramina.

BACKGROUND: The infraorbital foramen (IOF) is a key anatomical landmark for infraorbital nerve block, maxillofacial surgery, and orbital reconstruction. However, its location and morphology exhibit considerable individual and population-based variation, emphasizing the need for population-specific morphometric data. METHODS: This anatomical study evaluated 103 adult dry human skulls (206 hemimaxillae).
