Long-term pulmonary morbidity in survivors of congenital diaphragmatic hernia.

Our objective was to study long-term respiratory outcomes of congenital diaphragmatic hernia (CDH) treated in the perinatal period. This was a cohort study with 26 adolescent survivors and age- and gender-matched controls. Medical histories were retrieved from hospital charts and questionnaires. Pulmonary function testing included measurement of maximum inspiratory and expiratory pressures (MIPS and MEPS) and maximum voluntary ventilation (MVV). Unpaired two-tailed t-test and nonlinear regression were used for statistical analysis. Significant differences were found in forced expiratory volume

[The outcome of modified procedure for pectus carinatum correction].

OBJECTIVE: To improve the surgical procedure to correct pectus carinatum. METHODS: From 1990 to 2003, 9 patients with pectus carinatum were treated, whose ages ranged from 3 years and 6 months to 16 years. The conventional operation was performed on 4 patients, the modified procedure on 5 patients. The modified procedure included: (1) the shortening rib periosteum was not sutured transversally; (2) the corrected position of the sternum was stabilized with the metal strut. RESULTS: In 4 patients corrected by the conventional procedure, the sternum depression as pectus excavatum occurred in 1

[A minimally invasive technique to repair pectus carinatum. Preliminary report].

The present report deals with a new technique for the correction of pectus carinatum. Although less frequent than pectus excavatum, this deformity leads to numerous consultations, as it is difficult to conceal and affects the personality and behavior of many young patients. We report the case of a 13-year-old boy with asymmetrical pectus carinatum. The chest wall was compressible, so the protrusion was corrected by means of intrathoracic compression. In this technique, the sternochondral region is compressed by implanting a metal bar in the presternal region and securing it bilaterally to the

Phenotypic and molecular characterisation of a de novo 5q deletion that includes the APC gene.

We report on a 12-year-old female patient with mild dysmorphic signs, including bilateral epicanthal folds, low-set dysplastic ears, a short nose with anteverted nostrils, conically shaped fingers, generalised increase of subcutaneous fat, multiple fine venous teleangiectasia on her back, mild pectus carinatum, and a general muscular hypotonia. Cytogenetic analysis and fluorescence in situ hybridisation (FISH) studies using region-specific BAC and YAC clones indicated a de novo interstitial deletion of the long arm of chromosome 5, resulting in monosomy 5q21.1-q23.1. Rickets among children of a coastal area of Bangladesh.

Many children with rachitic deformities have been reported in southern coastal area of Bangladesh but the actual rate of prevalence was not known. A survey was conducted to determine the magnitude of rachitic problem among the children of Chakaria thana of Cox's Bazar district of Bangladesh. Nine hundred children between 1-15 years selected randomly from 30 villages of total 340 villages. Face to face interview of the parents was taken and the children were examined for evidences of rickets. Serum calcium, phosphorus, alkaline phosphatase (ALP) were estimated and radiology of limbs were done i

Less extensive techniques for repair of pectus carinatum: the undertreated chest deformity.

BACKGROUND: Although patients with pectus carinatum (PC) often experience moderate to severe symptoms, there are sparse published data about the indications for correction, the newer techniques of surgical repair, and the results. This study reviews clinical experience with new, less extensive, open operative techniques for repair of PC. STUDY DESIGN: Since 1970, 154 patients (119 men and 35 women) with symptomatic PC (mean severity index 1.76) underwent correction at the UCLA Medical Center using modifications of the Ravitch repair. The last 60 consecutive patients had a less extensive open

Repair of the pectus deformity: results of the Ravitch approach in the current era.

BACKGROUND: Recent publications have advocated a minimally invasive approach to repair of the pectus deformity. Efforts to evaluate this new approach have been hampered by lack of comparative information regarding outcomes of the standard Ravitch approach. We use a modified Ravitch procedure, and present our series as a basis for comparison. METHODS: Records of 69 consecutive patients undergoing repair of the pectus deformity were retrospectively reviewed. Modifications included a minimal incision and a new technique to address sternal angulation. A patient satisfaction survey evaluated the p

Chest wall anomalies: pectus excavatum and pectus carinatum.

Pectus excavatum, the concave depression of the breast bone, comprises most chest wall anomalies. The Nuss procedure, a minimally invasive procedure to correct pectus excavatum, has revolutionized the management of this disease over the past decade. The results and complications of this procedure are discussed. The surgical management of the less common pectus carinatum or "pigeon breast" also is reviewed.

[Noonan's syndrome with an unusual combination of hypertrophic cardiomyopathy, congenital bicuspid aortic valve, coarctation of the aorta and hypoplastic aortic arch].

Noonan's syndrome is characterized by craniofacial anomalies, i. e. ptosis, webbing of the neck and a deep nuchal hairline, as well as skeletal deformities such as short stature, clinodactyly, pectus carinatum and funnel chest and other organ anomalies, mainly cardiac valve disease, less often testicular retention or kidney malformations. Noonan syndrome presents with aspects similar to Ullrich-Turner syndrome, but can be found in both male and female patients. In about one half of the patients with Noonan's syndrome cardiovascular anomalies occur, mostly anomalies of the right heart (mainly v

Fetal warfarin syndrome.

Fetal warfarin syndrome (FWS) or warfarin (coumadin) embryopathy is a rare condition as a result of fetal exposure to maternal ingestion of warfarin during pregnancy. A male infant, whose mother was treated with the anticoagulant (warfarin) because of a mechanical heart valve replacement after rheumatic heart disease, presented with signs of warfarin embryopathy. The facial dysmorphism included hypoplasia of nasal bridge, laryngomalacia, pectus carinatum, congenital heart defects (atrial septal defect and patent ductus arteriosus), ventriculomegaly, stippled epiphyses, telebrachydactyly, and g

Morgagni hernia and thoracic deformities.

We report two cases of Morgagni hernia associated with pectus carinatum. This association is exceptional; only two other cases have been reported so far. In one of our patients, an abdominal surgical approach was used to repair the Morgagni hernia and to perform a Nissen-Rossetti procedure (for an associated endobrachyesophagus); the patient did not require correction of the pectus carinatum. In the other patient, both thoracic deformity and Morgagni hernia were repaired using the same thoracic approach.

Recurrent chest wall anomalies.

Chest wall anomalies are a heterogeneous group of malformations requiring repair. Recurrence and the need for secondary repair may occur. Congenital anomalies, including bifid sternum, pentalogy of Cantrell, Jeunes's syndrome and Poland's anomaly, rarely recur. Pectus carinatum may recur in the original surgical area or an adjacent area and most often recurs in patients who undergo repair before completion of teenage growth. Pectus excavatum may recur in approximately 5% of patients. Simple recurrence, floating sternum, or Acquired Jeune's syndrome may result. All of these would require reoper

Pectus deformities of the anterior chest wall.

Pectus deformities affect about 1% of the population. Despite a subjective reduction in exercise tolerance, objective physiological disturbance is very uncommon. However, there is no doubt that cultural changes have significantly altered perception and tolerance of these deformities and increasing numbers of children are actively seeking surgery. Pectus excavatum is readily amenable to correction using minimally invasive techniques and pectus carinatum can be corrected using safe established conventional techniques. Good or excellent results can be expected in over 90% of cases, and children s

[Correction of the congenital malformations of the front chest wall by the modelling technique of sternochondroplasty: technique and results on a series of 14 cases].

In the congenital malformations of the front chest wall showing depression or protrusion, the cardiorespiratory disorders are inconstant and often not linked with the distortion. On the other hand psychoaffective repercussions are major in adolescents and young adults justifying recourse to surgery with morphological and aesthetic aiming. The authors present the technique of modelling sternochondroplasty by raising with osteosynthesis by slide fastener-handle of Martin-Borrelly, applied to the correction of congenital malformation of the front chest wall in young adult, both of protrusion (pec

Giant intrathoracic extrapulmonary hydatid cyst manifested as unilateral pectus carinatum.

Liver and lung are the most common sites of hydatid disease, but it can also be seen elsewhere in the body. Extrapulmonary intrathoracic location of the disease is rare. This case of giant intrathoracic extrapulmonary hydatid cyst manifested as unilateral pectus carinatum serves to illustrate that hydatid disease can produce various symptoms and that it may also exist in locations apart from lung and liver.
