Comparing Provider and Adolescent Estimates of Postoperative Opioid Use.

INTRODUCTION: Most adolescents report unused opioids after surgery. Current interventions compare opioid prescribing between surgeons without capturing actual patient-reported use. METHODS: We recruited pediatric surgery residents, fellows, advanced practitioners, and surgeons from four surgical divisions at a tertiary care children's hospital. Providers reviewed clinical vignettes based on adolescent-reported postoperative opioid use data from our institution. Afterward, providers were asked to select the number of opioid pills they would prescribe and compare their responses to adolescent-r

Minimally invasive surgical correction technique of Pectus Arcuatum, case report of an innovative method for a rare chest wall malformation (Jerusalem technique).

Pectus Arcuatum (PA) is a rare anterior chest wall deformity characterized by a deformation with potential psychological impact on patients and combining features of both pectus carinatum and pectus excavatum. Traditional corrective methods involve open surgical techniques, such as sternotomy, which often result in significant surgical scars. This case report presents two patients who underwent successful minimally invasive correction of PA using a modified sandwich method, referred to as the Jerusalem Technique. The innovative approach involved bilateral thoracoscopic-guided osteotomies and p

Surgical adjuncts in minimally invasive repair of pectus excavatum and carinatum deformities: techniques for internal visualization, sternal elevation, and depression.

Over the past two decades, the repair of pectus excavatum (PE) and pectus carinatum (PC) has evolved significantly, driven by innovations in surgical adjuncts and device development. Originally limited by high risks due to blind mediastinal dissection and force-based correction, pectus repair techniques now prioritize safety, precision, and minimal invasiveness. This paper introduces a series of adjunctive techniques and custom-designed tools that address major limitations in PE and PC surgery. The pectoscope, with its curved shaft and touch-view functionality, allows safe internal visualizati

Idiopathic hypoplastic right breast in a 60-year-old female: A case report.

Breast hypoplasia may be acquired or congenital as well as unilateral or bilateral, it may also be isolated or associated with other pathologies such as anomalies and syndromes of the chest wall like Jeune and Poland syndromes, pectus excavatum and pectus carinatum. Usually, nonsyndromic breast hypoplasia is idiopathic though some have known causes like direct effect from radiation treatment of cutaneous hemangiomas in childhood. Other iatrogenic causes may be due to thoracotomy, previous tube thoracostomy, impaired breast development postsurgical removal of benign or malignant breast tumors d

Congenital sternal cleft in a DSH cat: A case report.

The sternum, essential for protecting intrathoracic structures and enabling thoracic stability and breathing motion, is a vital part of small animal thoracic anatomy. Evaluating the sternum is crucial in small animal practice, with typical anatomy including eight to nine sternebrae and species-specific variations. Congenital sternal anomalies, such as pectus excavatum, pectus carinatum, and sternal cleft, though rare, can have significant clinical implications. Sternal cleft, characterized by incomplete fusion of sternebrae, is particularly uncommon in veterinary literature, with limited docum

Whole-exome sequencing identifies novel truncating ALPK3 variants in a compound heterozygous state associated with pediatric hypertrophic cardiomyopathy and phenotypic heterogeneity.

BACKGROUND: Pathogenic biallelic variants in ALPK3 are associated with recessively inherited early-onset and severe cardiomyopathies in children, often accompanied by extracardiac manifestations such as facial deformities and skeletal malformations. However, the number of reported cases remains limited. In this study, we report two additional pediatric patients with hypertrophic cardiomyopathy (HCM) having (likely) pathogenic heterozygous ALPK3 variants in a compound heterozygous state, together with a comprehensive review of the literature. METHODS: Whole-exome sequencing (WES) was performed

Innovative surgical repair of severe pectus carinatum using 3D surgical planning and advanced rib fixation techniques.

This case describes an early adolescent boy with severe pectus carinatum, managed with a complex surgical approach after bracing was deemed unfeasible. A multidisciplinary team collaborated, using advanced three-dimensional (3D) imaging, digital segmentation and a patient-specific 3D-printed model to plan and rehearse the surgery. The operation involved sternum repositioning and rib plating with polyether ether ketone plates. The procedure resulted in favourable chest stability and minimal blood loss. Postoperatively, the patient's pain was well-controlled, and he was discharged on day 7. The

Impulse oscillometry for evaluating pulmonary function in pediatric pectus deformities: a prospective controlled study.

UNLABELLED: Pectus deformities (PDs), including pectus excavatum and pectus carinatum, may affect pulmonary mechanics; however, spirometry often fails to detect subtle abnormalities. This study aimed to evaluate pulmonary function in children with PDs using both spirometry and impulse oscillometry (IOS). In this prospective cross-sectional study, 73 children with PDs and 80 age- and sex-matched healthy controls underwent spirometry and IOS testing. z-scores were calculated for all parameters using established reference equations. A possible restrictive ventilatory defect was defined as zFVC <

Innovation in the correction of pectus carinatum: A combination of orthopedic and surgical treatment (the "3-Cs" technique).

Pectus carinatum is a chest wall deformity characterized by a protrusion. For its correction, the flexibility of the chest wall is essential, as it will determine the possibility of correction through orthopedic or surgical treatment. Orthopedic treatment has experienced a boom in recent years thanks to the development of a dynamic compression system (vest) that applies pressure to the area. This effect can be limited by the rigidity of the chest wall (which increases with age) or by the presence of pronounced depressions of abnormally positioned cartilage (limiting cartilage). In these cases,
