Spondyloepimetaphyseal dysplasia with joint laxity (SEMDJL): clinical and radiological findings in a Guatemalan patient.

We report on a patient of Guatemalan descent whose physical and radiological findings are consistent with a diagnosis of spondyloepimetaphyseal dysplasia with joint laxity (SEMDJL). This is a rare, autosomal recessive skeletal dysplasia with short limbs, severe scoliosis, high dorsal kyphoscoliosis, and joint hypermobility. Most described patients with SEMDJL are from the Afrikaans-speaking communities of South Africa. Patients with SEMDJL have an oval face, prominent eyes, and blue sclerae. Our patient's height and weight were below the fifth centile. She had prominent eyes with blue sclerae,

Dominant mutations in the type II collagen gene, COL2A1, produce spondyloepimetaphyseal dysplasia, Strudwick type.

The chondrodysplasias are a heterogeneous group of disorders characterized by abnormal growth or development of cartilage. Current classification is based on mode of inheritance as well as clinical, histologic, and/or radiographic features. A clinical spectrum of chondrodysplasia phenotypes, ranging from mild to perinatal lethal, is due to defects in the gene for type II collagen, COL2A1. This spectrum includes Stickler syndrome, Kniest dysplasia, spondyloepiphyseal dysplasia congenita (SEDC), achondrogenesis type II, and hypochondrogenesis. Individuals affected with these disorders exhibit ab

Iatrogenic pectus carinatum. A case report.

A boy underwent cardiac surgery when he was 27 months old; prior to that his anterior chest wall had been normal. He later developed a progressive pectus carinatum deformity. Thoracic surgeons are cautioned to be mindful of the sternal and costal growth plates in any surgical approach to intrathoracic structures. Special care is needed when such deformities are corrected in children and adolescents.

Repair of pectus deformities with sternal support.

During the past 25 years, 252 children underwent repair of pectus deformities. There were 195 male and 57 female patients, of whom 227 had pectus excavatum and 25 had pectus carinatum. Of the 252 patients, 113 underwent repair at between 2 and 5 years of age. Exercise limitation was reported by 51%, and 32% had frequent respiratory infections or asthma. Repair was performed through a transverse incision with subperiosteal resection of the lower four or five costal cartilages, from sternum to costochondral junction bilaterally. A transverse wedge osteotomy was made through the anterior table of

A case of trichorhinophalangeal syndrome, type I.

A single case of trichorhinophalangeal syndrome, type I, is reported. The patient manifested the complete triad of the syndrome: thin, sparse, slow-growing scalp hairs; a pear-shaped nose; and cone-shaped epiphysis in some interphalangeal articulations of both hands. In addition, tooth decay, pectus carinatum, lumbar scoliosis, and polycystic ovary were observed. This multisystemic disorder is apparently rare, but its prevalence is probably underestimated in the dermatologic literature.

[Surgical treatment of thoracic wall malformations].

Anterior chest wall deformities in children are the expression of some congenital malformation such as Poland's syndrome and pectus excavatum. The surgical treatment of these malformations affecting children is aimed at correcting large wall defects and stabilizing the chest wall, assuring at the same time chest harmonious growth and postoperative functional assessment. We present the results of the treatment of 2 cases of Poland's syndrome, 20 cases of pectus excavatum and 2 cases of pectus carinatum, with a follow-up of 6 months--9 years.

Spondylometaphyseal dysplasia (Kozlowski type): case report.

Kozlowski syndrome is the most common type of spondylometaphyseal dysplasia (SMD). It is characterized by short stature (130 to 150 cm), pectus carinatum, limited elbow and hip movement, mild bowleg deformity, and curvature of the spinal column. Children with Kozlowski dwarfism usually are not recognized at birth, since they have normal clinical features, weight, and size. This article reports the dental treatment and oral findings of a 14-year-old female patient with Kozlowski dwarfism.

Prevalence of congenital abnormalities in Turkish school children.

19750 school children, ages 6 to 15 years, were examined by the authors of this study, 1,220 (6.18%) had congenital abnormalities. In this group, 4.23% were boys and 1.88% were girls. Case histories revealed inbreeding amongst the parents (families) of children with congenital malformation to be 8.9% and 8.2% for the rest of the families in this study. There were 27 different congenital abnormalities identified, with prevalence rates of 0.05/1,000 to 15.85/1,000. The most prevalent abnormalities were umbilical hernia (15.85/1000), inguinal hernia (14.50/1,000), pectus carinatum and excavatum (

[Surgical correction of pectus carinatum: a case report].

A 12-year-old girl with a keel chest, underwent cosmetic repair of the severe deformity. The deformity was corrected by subperichondrial resection of the all deformed costal cartilages, transverse osteotomy through the anterior cortex of the sternum, detachment of the xiphoid process, bending of the posterior cortex of the sternum, and resection of the lower end of the sternum. The sternum was maintained in its corrected position by traction force of the rectus muscles through the reattached xiphoid and the shortened perichondrial tissues and by reapproximating the pectoral flaps in the midlin

Schwartz-Jampel syndrome: an atypical form?

The Schwartz-Jampel syndrome (SJS) is an autosomal recessive disorder of myotonia, short stature, "mask-like" face, blepharophimosis, stiff joints, spinal malalignment, and pectus carinatum. The authors report on a 10-year-old girl with expressionless face, blepharophimosis, mild thoracic asymmetry, and myotonic electromyographic pattern, but without osteoarticular, spinal, and statural involvement. This appears to be one of the mildest form of SJS described so far. Contiguous genes could be an explanation for this and other atypical cases of SJS.

[Surgical treatment of pectus excavatum and pectus carinatum].

The authors present their experience with surgery of deformities of the chest wall in childhood. In the course of 1986 to 1992 they operated 93 children with the diagnosis of pectus excavatum. This number comprised 66 boys and 27 girls. The mean age of the operated children was 10.5 years. During the same period 17 patients (12 boys and 5 girls) were operated on account of pectus carinatum. The mean age of the latter group of patients was 13.5 years. 23% of the patients with a pectus excavatum had before operation mild scoliosis, two patients suffered from severe scoliosis. 76 patients after o

Pectus carinatum successfully treated with bracing. A case report.

We report the case of a 14 year old girl who desired treatment for a cosmetically objectionable pectus carinatum deformity. She was initially managed with a corrective underarm body cast for six weeks followed by full-time bracing for seven months. Subsequently, she was braced only at night for another eight months. Seven years after the onset of treatment she had an excellent result without recurrence of deformity. In a well-motivated, skeletally immature individual bracing can be an effective treatment for cosmetically displeasing pectus carinatum.

Classification of chest wall diseases.

Several disorders of the thoracic cage are known to cause respiratory failure, by means of relatively simple mechanisms, such as the increased work of breathing, which results in alveolar hypoventilation. A variety of pathogenic mechanisms may be considered, as functions of the types of thoracic disorders present. As causes of these additional potential mechanisms, we considered the following: 1) ventilation-perfusion (V/Q) inhomogeneity; 2) inability to cough; 3) malformation or acquired defect of the respiratory centres; and 4) excess blood volume and fluid retention, which aggravate work of

Currarino-Silverman syndrome (pectus carinatum type 2 deformity) and mitral valve disease.

Currarino-Silverman syndrome is a rare disorder characterized by premature fusion of manubrio-sternal joint and the sternal segments, resulting in a high carinate chest deformity; it is frequently associated with congenital heart disease. Among the various heart lesions reported in this syndrome, mitral valve disease and coarctation of the aorta have not yet been described (to our knowledge). Our report consists of five children with this syndrome, four of whom had mitral valve disease, with an associated coarctation of the aorta in one patient. The fifth patient had an innocent heart murmur.

Preliminary results of orthotic treatment of pectus deformities in children and adolescents.

Fifty-five children and adolescents with different types of pectus deformities and a minimum follow-up of 1 year were assessed to determine the effectiveness of nonoperative treatment with a dynamic chest compressor orthosis. Thirty-seven patients were treated and compared with 18 untreated patients. The treated patients had a better outcome. Reshaping of the contour of the chest wall was observed in 21 patients with inferior and lateral types of pectus carinatum. The dynamic chest compressor has minimal morbidity and is a valuable treatment option of pectus deformities in the growing period.
