Congenital knee hyperextension (CKH), or congenital genu recurvatum, is a rare orthopedic anomaly in neonates that requires prompt intervention to prevent long-term disability. This case report presents the successful non-surgical management of a one-day-old female neonate presenting with severe unilateral CKH (Leveuf Type I/II) in a resource-limited setting. The novelty of this case lies in the application of an accelerated, two-stage serial casting protocol spanning just 14 days, followed by targeted physiotherapy. This approach resulted in the complete resolution of the deformity and full f

An infant with equino-varus deformity of her right foot and flexion deformity of the knee presented to our centre. Based on clinical and radiological examination findings, she was diagnosed with Paley Va (Jones Ia) axial longitudinal intercalary deficiency of the right leg as the entire tibia was absent. Consequent to cultural non-acceptance of amputation by the parents, her management involved staged procedures. The initial surgery aimed to achieve fusion between the talus and the distal end of the fibula after soft tissue release and fibular shortening. In the subsequent surgery, Weber patel

BACKGROUND: Cancer remains the second leading cause of death in the state of Alabama and disproportionately affects communities that are rural, of lower socioeconomic status, and individuals with high-risk, predisposing conditions such as obesity, cardiovascular disease, advanced age, and diabetes. One method to address the increased burden of cancer is to expand the cancer research and clinical workforce. The Roadmap for America's Cancer Explorers for the 21st Century (RACE21) is a National Cancer Institute-funded Youth Enjoy Science program at the University of Alabama at Birmingham (UAB) an

BACKGROUND: Coffin-Siris syndrome 12 (CSS12) is a recently described neurodevelopmental disorder caused by heterozygous pathogenic variants in BICRA, a gene encoding a core subunit of the non-canonical BAF (ncBAF) chromatin-remodeling complex. The condition is characterized by developmental delay, hypotonia, hypertrichosis, and joint laxity. However, long-term data remain limited, and systemic manifestations are incompletely defined.
CASE DESCRIPTION: We report a 22-year-old male with a de novo BICRA frameshift variant, c.2479_2480delinsA (p.Ala827Thrfs*15), previously included in the origina

BACKGROUND: Here, we present the first reported case of a child presenting with unprovoked deep vein thrombosis (DVT) due to the unique combination of May-Thurner syndrome, protein S deficiency and lupus anticoagulant. Of interest to the clinician and scientific reader alike is that this one-of-a-kind interplay between mechanical obstruction and hypercoagulability caused a very extensive blood clot which extended from the left popliteal to the femoral and iliac veins. A 12-month course of warfarin with a target INR value of 2.5 was used to treat the DVT which would be of use to note for any fu

Krabbe disease, also known as globoid cell leukodystrophy, is a rare lysosomal disorder affecting the white matter of the central and peripheral nervous systems. It is characterized by neurodegeneration, with infantile-onset Krabbe disease as the most common form, typically diagnosed within the first year of life and associated with high morbidity and mortality. This autosomal recessive disease is caused by mutations in the GALC gene, which encodes the lysosomal enzyme β-galactocerebrosidase (GALC). This study focuses on examining the structural effects of 4 GALC variants (Gly59Arg, Ser68Phe,

BACKGROUND: Coffin-Siris syndrome type 2 (CSS2), caused by ARID1A mutations, is characterized by neurodevelopmental delay and recurrent infections. The ARID1A p.Ala1077Glu variant has been linked to CSS2, but the immune mechanisms underlying infection susceptibility remain poorly understood.
METHODS: The variant was identified by trio-based whole-exome sequencing and confirmed by Sanger sequencing. Immune status was assessed via serum immunoglobulins, complement measurements, and flow cytometric immunophenotyping. RNA sequencing was performed on PMA-stimulated PBMCs from the patient and healt

May-Thurner Syndrome (MTS) with acute iliofemoral deep vein thrombosis (IFDVT) carries high risks of limb ischemia and post-thrombotic syndrome (PTS). Traditional treatments like catheter-directed thrombolysis have limitations, including high major bleeding risk and prolonged hospital stays. A retrospective cohort study included 52 patients with MTS-related acute IFDVT (one-stop group: 27, Angiojet mechanical thrombectomy + simultaneous iliac vein stenting; traditional group: 25, catheter-directed thrombolysis alone or with delayed stenting). Main outcomes were 24-hour limb circumference edema

Ehlers Danlos and related hypermobility syndromes underly chronic, recurrent, and multiple-site pain in a small number of persons. By making the diagnosis, clinicians can better guide the treatment of the presenting problem. The diagnosis can help a patient make sense of their recurrent challenges as well. By inquiring about numerous associated conditions, ranging from postural orthostatic tachycardia syndrome to autism spectrum and attention deficit issues, the clinician may help the patient deal with other challenges. Detection can be easy-primarily observation of flexibility in the thumb, f

Madelung deformity is a rare congenital wrist deformity characterized by growth disturbance of the volar-ulnar aspect of the distal radial physis, leading to progressive wrist deformity, pain, and stiffness. Madelung deformity is frequently associated with genetic conditions such as Leri-Weill dyschondrosteosis and Turner syndrome, and more recently, mutations in the GNAS gene have been implicated. Diagnosis is based on clinical findings and radiographic criteria, with plain radiographs demonstrating characteristic volar-ulnar radial curvature, positive ulnar variance, and carpal subluxation.

Myhre syndrome (MS) is a connective-tissue disorder within the acromelic dysplasia spectrum. It is characterized by congenital craniofacial, skeletal, cutaneous anomalies, respiratory, cardiovascular along with intellectual disability, deafness, and progressive fibrosis. MS is caused by germline variants in the transcriptional co-regulator SMAD4 (mothers against DPP homolog 4). SMAD4 is a key intracellular mediator of signal transduction and participates in several pathways, notably canonical TGFβ and BMP signaling. These pathways play central roles in developmental and homeostatic processes.

OBJECTIVES: Coffin-Siris syndrome (CSS) is predominantly attributed to variants in ARID1B gene, however, the molecular pathways connecting ARID1B to myelination and neural development are not well elucidated.
METHODS: We employed ARID1B knockdown in SK-N-SH cells coupled with transcriptomic analysis to identify downstream targets. We conducted quantitative Real-Time PCR, Western blotting, chromatin immunoprecipitation (ChIP) and co-immunoprecipitation (Co-IP) assays to investigate the regulatory mechanisms of ARID1B. The clinical phenotypes of three CSS patients with pathogenic ARID1B variant
