Cellular homeostasis is maintained by deubiquitination, which plays critical roles in DNA damage repair, cell cycle progression, signal transduction, transcriptional regulation, and autophagy. The deubiquitination (DUB) module within the Spt-Ada-Gcn5 acetyltransferase (SAGA) complex is encoded by the ATXN7L3 gene. Disease-causing variants in ATXN7L3 have been associated with an autosomal dominant neurodevelopmental disorder characterized by global developmental delay, hypotonia, speech impairment, and distinctive facial dysmorphism, known as Harel-Tora neurodevelopmental syndrome (OMIM #621377

BACKGROUND: Bardet-Biedl syndrome (BBS) is a rare autosomal recessive ciliopathy with multisystem involvement. While BBS1 mutations are common globally, population-specific genetic patterns and phenotype severity vary. This study aimed to investigate genotype-phenotype correlations in an Indian cohort.
METHODS: In this single-centre, observational cohort study, individuals meeting Beales' clinical criteria for BBS underwent next-generation sequencing (NGS). Phenotypic features were correlated with underlying genotypes, with particular attention to the type of mutation (truncating vs. non-trun

SLC20A1 encodes the ubiquitously expressed phosphate transporter PiT1, a protein with roles extending beyond phosphate homeostasis to include regulation of proliferation, differentiation, apoptosis, and embryonic development. While heterozygous SLC20A1 variants have been associated with urinary tract malformations, the impact of biallelic loss-of-function was unknown. We report the first human case of a biallelic homozygous predicted loss of function variant in SLC20A1 (c.674_675delAA; p.K225TfsX34) in a child with multiple congenital anomalies including tetralogy of Fallot, unilateral renal a

BACKGROUND: Medication safety studies in pregnancy typically focus on selected or composite outcomes (eg, any malformation) to test specific hypotheses or verify suspected safety signals, potentially overlooking or masking other clinically significant associations (eg, individual malformations or malformation types).
OBJECTIVE: To conduct a comprehensive, systematic screening for potential teratogenic safety alerts associated with first-trimester exposure to individual antipsychotics, using a tree-based scan statistic (TBSS) approach for simultaneous evaluation of a broad range of specific ma

INTRODUCTION: Although radial polydactyly is a common congenital hand malformation, its co-occurrence with thumb hypoplasia is rare, as the two are seen as opposite ends of a developmental spectrum. Radial polydactyly with thumb hypoplasia poses a significant diagnostic challenge, often leading to under-recognition and -treatment of the hypoplastic component. This study reviews our experience with the comprehensive management of this complex association.
METHODS: This retrospective study reviewed 13 cases (11 males) with a mean age of 28.2 months. Radial polydactyly with thumb hypoplasia case

Polydactyly occurs in 1 in 1000 live births and is one of the most common congenital anomalies of the hands and feet, according to a thorough literature review. It may be an isolated anomaly, part of a syndromic condition, or associated with other congenital disorders. Clinical presentation can range from a fully functional extra digit to a small, underdeveloped appendage. Different genetic and embryological factors affect limb development, resulting in diverse phenotypic presentations in affected individuals. We present a rare case of type IIB radial polydactyly, discuss its treatment and com

INTRODUCTION: The diverse morphology of proximal phalangeal thumb polydactyly complicates a systematic understanding of soft tissue abnormalities associated with postoperative zigzag deformity. This study aimed to characterize intraoperative anatomical abnormalities of the flexor pollicis longus tendon and its sheath using the concept of the duplication range.
METHODS: Patients with proximal phalangeal thumb duplication who underwent primary surgery and had high-quality intraoperative records with images of tendon and sheath anatomy were included. Duplicated thumbs were classified as articula

Parental consanguinity can increase the risk of selected congenital anomalies, but its population impact depends on local marriage patterns, ancestry structure, and access to reproductive and preventive health services. We compare the prevalence of congenital anomalies in Coro (Falcón State, Venezuela) with other ECLAMC hospitals and to quantify the contribution of parental consanguinity and reproductive/sociodemographic factors to anomalies with excess prevalence in Coro. Hospital-based surveillance data from ECLAMC (2002-2024) were analyzed. Prevalence (per 10,000 births) and 95% confidence

INTRODUCTION: Type B ulnar polydactyly is a common congenital hand anomaly. Treatment options include suture ligation, vascular clip ligation and excision under local or general anaesthesia. However, the optimal treatment remains unclear, and evidence on parental perspectives is limited. This review investigated treatment outcomes for type B ulnar polydactyly, in combination with parental satisfaction and preferences.
METHODS: A systematic search of Embase, Pubmed, Medline, Web of Science and Cochrane Central Register of Controlled Trials was conducted up to March 2025 for studies reporting p

Short rib-polydactyly syndrome (SRPS), with or without polydactyly, encompasses a range of autosomal recessive skeletal dysplasias characterized by shortened limbs, narrow thorax, and visceral abnormalities. Accurate genetic testing is crucial for the diagnosis and treatment of different clinical subtypes. This study investigates gene variants associated with the clinical features in a Chinese family affected by short rib-polydactyly syndrome (SRPS), DYNC2I1-related. A retrospective analysis of two adverse pregnancy histories in the family was conducted using genetic testing methods, including

Background: The PITX1 gene encodes a transcription factor that plays a crucial role in the development of the lower limbs, pelvis, and structures derived from the first branchial arch. Pathogenic variants in PITX1 are associated with a limited spectrum of rare disorders, including congenital talipes equinovarus with or without long bone anomalies and/or mirror-image polydactyly, and Liebenberg syndrome. In 2020, a novel clinical phenotype, Mandibular-Pelvic-Patellar (MPP) syndrome, resulting PITX1 missense variants, was proposed. Case presentation: We report the fourth documented case of MPP s

INTRODUCTION: Inherited retinal diseases (IRDs) may present as an isolated ocular condition or as part of multisystem disorders, such as Bardet-Biedl syndrome (BBS). Several BBS-related manifestations are age-dependent and variably expressed. Therefore, patients with early or subtle extra-ocular features may be incorrectly diagnosed as having non-syndromic IRD. There are two diagnostic frameworks for BBS: a phenotype-based, Beales-derived approach and a genotype-first approach, as recommended by the InterEuropean Reference Networks (ERNs) criteria. Here, we aimed to conduct comprehensive evalu

BACKGROUND: One in 500 children in the United States is born with polydactyly. Type B ulnar polydactyly excision is often performed in the operating room (OR) but can also be conducted in an office setting. This study compares costs for patients/families and healthcare systems between these settings.
METHODS: A retrospective review from January 2019 to December 2023 included patients who underwent type B ulnar polydactyly excision. Patients were categorized into in-office or OR groups. Patient demographics and financial data were analyzed using independent-samples t tests, χ2 tests, and analy

BACKGROUND: Polydactyly is a common congenital limb anomaly characterized by supernumerary digits, most frequently involving the hands or feet. Foot polydactyly is traditionally classified into preaxial, postaxial, and central types, with postaxial forms being the most common and central forms the rarest. Heptadactyly, defined as the presence of seven digits in a single limb, represents an exceptionally rare and severe expression within this spectrum, most often described as central duplication. Isolated unilateral heptadactyly of the foot with combined preaxial and postaxial duplication is ex

What's already known about this topic? ◦. Pathogenic PITX1 variants, including deletions and missense mutations, give rise to a syndromic phenotype characterized by clubfoot, tibial hemimelia, and preaxial polydactyly. ◦. To date, no previously reported case of PITX1 variant or deletion has featured upper‐limb involvement. What does this study add? ◦. The PITX1 p.Ala123Thr variant is associated with a markedly distinct clinical presentation involving anomalies of both upper and lower limbs. ◦. Our cases expand both the phenotypic and genotypic spectrum of the PITX1‐related limb malformations.
