Male Goltz Syndrome due to Postzygotic PORCN Mosaicism: A Case Report.

Goltz syndrome, also known as focal dermal hypoplasia (FDH), is a rare X-linked dominant genetic disorder caused by loss-of-function mutations in the PORCN gene, which is crucial for Wnt protein secretion and signaling during embryonic development. The syndrome primarily affects females, as hemizygous males with pathogenic variants in the gene typically die in utero, while surviving males are usually mosaic for the variant. Patients typically present with diverse phenotypic features, including dermal hypoplasia, musculoskeletal abnormalities, ocular malformations, and dental defects, often dis

Syndactyly.

Syndactyly is a common congenital limb malformation caused by failure of formation or differentiation of an unspecified axis of the hand plate. Syndactyly has a wide range of phenotypes and is classified based on the structures involved, the length of the digit involved, and whether it occurs in isolation or within the setting of a syndrome. Surgery is the mainstay of treatment with the primary goal of restoring form and function by creating an adequate web space and providing sufficient skin coverage after digit separation. Skin flaps with or without the use of skin grafts are the primary tec

LRP4-Related Congenital Myasthenic Syndrome: Clinical, Pathophysiological, and Therapeutic Aspects.

Congenital Myasthenic Syndrome represents a complex and heterogeneous group of inherited neuromuscular disorders, which result from variants in genes involved in different pathophysiological mechanisms related to the neuromuscular junction. LRP4 (Low-density lipoprotein receptor-related protein 4) represents one of the most important proteins involved in this complex signaling pathway, acting in a complex with agrin and Muscle Skeletal Receptor Tyrosine Kinase (MuSK) proteins. LRP4 became known to most neurologists due to the description of anti-LRP4 antibody-related Myasthenia Gravis. There a

Clinical and Genetic Spectrum of Filippi Syndrome: A Systematic Review of Published Case Reports and Case Series.

BACKGROUND AND AIMS: Filippi syndrome is a very rare autosomal recessive craniodigital disorder primarily caused by mutations in the gene CKAP2L, characterized by syndactyly, microcephaly, growth retardation, distinctive craniofacial features, and intellectual disability. Because of its rarity and phenotypical overlap and related craniodigital disorders, its full clinical and genetic spectrum remains poorly defined. This systematic review aimed to synthesize the published literature on the clinical presentation, genetic findings, and reported management of Filippi syndrome and Filippi-like ove

Cornelia de Lange syndrome in an extremely low birth weight infant: a case report.

Cornelia de Lange syndrome (CdLS) is a rare genetic disorder characterized by distinctive facial features, fetal growth restriction, intellectual disability, and hirsutism. Few reports have described extremely low birth weight (ELBW) infants with CdLS. We report an ELBW infant diagnosed with CdLS. The infant was delivered by emergency cesarean section at 34 weeks and 1 day of gestation, weighing 994 g (-3.7 SD score). A diagnosis of CdLS was suspected based on dysmorphic features, including forehead hirsutism, cleft palate, long philtrum, wide nostrils, syndactyly of toes, femoral hirsutism, a

Case Report: Frostbite in the tropics: abdominal pedicled flap reconstruction to prevent the metacarpal hand.

The reconstructive salvage of a severely frostbitten hand in a tropical setting presents a unique challenge. We report a 59-year-old male who sustained extensive frostbite to the face, ears and bilateral hands following a Mount Everest ascent. Hyperbaric oxygen therapy (HBOT) was initiated during demarcation to optimise tissue viability. Following demarcation, dry gangrene of the right hand required amputation, revealing viable 1.5-2 cm proximal phalangeal stumps in all five digits. To avoid a non-functional metacarpal hand, reconstruction aimed to preserve digital length and metacarpophalange

[Ocular manifestations of Apert syndrome: a case report].

A 10-year-old boy with bilaterally reduced palpebral fissure opening since early childhood presented to the Department of Ophthalmology, Shanghai Ninth People's Hospital, Shanghai Jiao Tong University School of Medicine. The diagnosis of Apert syndrome was based on syndactyly of the fingers and toes, midfacial hypoplasia, proptosis, hypertelorism, and other craniofacial dysmorphic features, and was further supported by molecular genetic testing identifying a heterozygous missense variant in FGFR2, c.758C>G(p.P253R). Goltz-Gorlin Syndrome: A Case Report and Literature Review.

Goltz-Gorlin syndrome, also known as focal dermal hypoplasia (FDH), is an uncommon X-linked dominant mesoectodermal dysplasia that predominantly affects females, with most nonmosaic males presumed to be nonviable. The disorder arises from abnormal development of mesodermal and ectodermal tissues and manifests as a broad spectrum of multisystem abnormalities. It is characterized by distinctive cutaneous findings and extracutaneous involvement involving the ocular, dental, skeletal, GI, genitourinary, cardiovascular, and central nervous systems. We report the case of a 49-year-old woman who pres

The real-world safety profile of amitriptyline: A comprehensive pharmacovigilance analysis based on the large-scale FDA adverse event reporting system database.

Despite its established efficacy, the long-term safety profile of amitriptyline among broad patient populations remains insufficiently characterized. We thoroughly characterized the real-world safety profile of amitriptyline through a systematic analysis of the FDA Adverse Event Reporting System (FAERS). Adverse events were collected from Q1 2004 to Q1 2026, and four disproportionality algorithms-reporting odds ratio (ROR), proportional reporting ratio (PRR), Bayesian confidence propagation neural network (BCPNN), and multi-item gamma Poisson shrinker (MGPS)-were employed to identify safety si

Guillouet-Gordon Syndrome With Severe Microtia in an Infant With a Homozygous Variant in MED16.

Bi-allelic MED16 variants underlie the recently described MEDopathy termed Guillouet-Gordon syndrome (GGNS, MIM #621220), characterized by neurodevelopmental and structural anomalies. To date, 27 individuals with GGNS have been reported, collectively harboring 28 distinct compound heterozygous or homozygous MED16 variants. Here we present a male infant, born to unrelated parents, presenting with multiple congenital anomalies overlapping with the established GGNS phenotypic spectrum. Specifically, the clinical presentation included structural brain defects and neurodevelopmental deficits alongs

Grange-Like Phenotype Associated With an RNF213 Pathogenic Variant: Expanding the Vasculopathy Spectrum.

Grange syndrome, caused by biallelic loss-of-function variants in YY1AP1, is characterized by multivascular stenoses, renovascular hypertension, brachydactyly, syndactyly, and mild cognitive impairment. Although RNF213 variants are typically associated with Moyamoya disease and systemic arterial stenosis, evidence suggests that pathogenic RNF213 variants may contribute to a broader vascular phenotype. We describe a 15-year-old girl with Moyamoya vasculopathy presenting with bilateral renal artery stenosis, brachydactyly, and intellectual disability. Her history included left-arm weakness and s

Characterising skull pathologies in the Sost-deficient mouse for preclinical evaluation of sclerosteosis treatments.

Facial paralysis, hearing loss and potentially lethal raised intracranial pressure are primary symptoms of sclerosteosis, an ultra-rare, autosomal recessive high bone mass (HBM) condition that predominantly manifests in skull pathologies. Non-invasive treatment remains an unmet clinical need. Recently, PORCN inhibition has been shown to reduce bone mass in young Sost-deficient (Sost-/-) mice. In this paper, we characterised sclerosteosis skull pathologies at later adult stages using a Sost-/- mouse model, with a view to drug efficacy evaluation. This revealed that male and female 9-month-old S

Deep Phenotyping and Molecular Elucidation of a New Syndrome: Ectodermal Dysplasia Caused by IRF6 Variants.

The diagnosis of an ectodermal dysplasia (ED) is often made by dermatologists. Some of the more than 50 distinct ectodermal dysplasias, however, are still largely unknown and their pathogenesis is poorly understood. Since we recently discovered that variants of the Interferon Regulatory Factor 6 (IRF6) gene IRF6 may cause ED, we have further investigated the link between this gene and maldevelopment of tissues originating from the embryonic ectoderm. Disease characterization in two previously reported subjects and one newly identified patient (mosaic status) included systematic clinical and ge

Phenotypic and Molecular Features of a Large ODDD Family: Expanding the Spectrum of CX43-Related Disorder.

We present a family of five siblings who came to our attention with a clinical and radiological diagnosis of familial hypomyelinating leukodystrophy. Despite brain white matter abnormalities being present in all siblings, the clinical phenotype was variable: the three brothers presented with a clear-cut late-onset spastic paraplegia, whereas the two sisters displayed only mild pyramidal signs. Molecular analysis revealed a single relevant variant shared by all affected siblings, namely the likely pathogenic variant c.659C>T (p.Ser220Phe) in the GJA1 gene. Variants in this gene are generally as

Multiple strategies to improve the therapeutic efficacy of mesenchymal stem cell‑derived exosome transplantation in ischemic stroke (Review).

UNLABELLED: Methotrexate (MTX) is a class of antifolate chemotherapeutics and immunosuppressants that are effectively used in the treatment regimen of cancer and autoimmune diseases. In the current MASLD era, methotrexate-induced hepatotoxicity must be understood in the context of host metabolic dysfunction, obesity, diabetes, and alcohol use that amplify oxidative and inflammatory liver injury. But its administration is being restricted due to hepatic toxicity, which increases asymptomatic in transaminases and progresses to fibrosis. This review examines the complex mechanisms underlying MTX-
