Congenital absence of the radius: the occupational therapist and a new orthosis.

Congenital absence of the radius (radial club hand) is a relatively common and disabling limb deficiency characterized by dislocation of the hand and carpus from the single forearm bore, the ulna. An understanding of the clinical-pathologic anatomy and consequent disruption of hand mechanics enables the orthopedic surgeon and the occupational therapist to formulate a team approach to this birth defect that can result in maximum functional capability for the child. The role of the occupational therapist and a specific therapy program for early stimulation and achievement of hand function are de

Congenital anomaly of the thumb: absent intrinsics and flexor pollicis longus.

Congenital absence of the median-innervated intrinsic muscles and flexor pollicis longus was seen in varying degrees of severity in eleven hands of eight patients. Three members were involved in one family in which the anomaly was probably an autosomal dominant. There were no chromosomal abnormalities. Surgical treatment of seven hands, including release of the adduction contracture and transfer of the ring-finger superficialis tendon to provide opposition and to reinforce the ulnar collateral ligament of the metacarpophalangeal joint, resulted in significant improvement in pinch and grasp.

[Poland's syndrome. Clinical and genetic studies; physiopathologic considerations].

Study of malformations of the hand makes possible the definition of three forms of progressive severity in the Poland syndrome, in which there is unilateral symbrachydactylie associated with homolateral aplasia of the pectoralis major. The three forms share the predominance of involvement of the 2nd, 3rd and 4th fingers. The existence of alterations in palmar creases and dermatoglyphia of the involved side alone confirms the absence of any lesion, even infra-clinical, of the other hand. Genetic study favours a pluri-factorial inheritance. Topographical data and the comparative study of vascula

A method for reconstruction of the central slip of the extensor tendon of a finger.

Congenital absence of the proximal interphalangeal joint extensor mechanism was corrected by using the lateral band and attached intrinsics of an adjacent finger. This technique has worked well in 3 cases.

Congenital absence of the pectoral muscles. A review of twenty-five patients.

Twenty-five patients with unilateral partial absence of the pectoral major muscle were reviewed to determine the presence of fetal insult, associated anomalies, neoplasia, and prognosis. Fifteen had ipsilateral synbrachydactyly (the Poland Anomaly) which varied in severity from minimal findings to nearly complete absence of the hand. The pattern of abnormality was remarkably uniform with maximal involvement of the midhand and least involvement of the thumb and small finger. The etiology of this condition remains unknown although fetal insult is suggested in some cases. Although reports in the

Congenital absence of flexor pollicis longus and flexor pollicis brevis: a case report.

A case of the absence of the flexor pollicis longus and flexor pollicis brevis alone, a congenital anomaly of exceedingly rare occurrence, is reported. Literature relevant to the absence of muscles and tendons of the hand is reviewed and discussed.

Family study of inherited syndrome with multiple congenital deformities: symphalangism, carpal and tarsal fusion, brachydactyly, craniosynostosis, strabismus, hip osteochondritis.

A syndrome of brachydactyly (absence of some middle or distal phalanges), aplastic or hypoplastic nails, symphalangism (ankylois of proximal interphalangeal joints), synostosis of some carpal and tarsal bones, craniosynostosis, and dysplastic hip joints is reported in five members of an Italian family. It may represent a previously undescribed autosomal dominant trait.

Radial club hand with absence of the biceps muscle treated by centralisation of the ulna and triceps transfer. Report of two cases.

Two children with radial club hand and absence of the biceps muscle were treated by centralisation of the ulna into the carpus and triceps transfer. The two operations were performed only a short time apart so that the period between the procedures could be used to stretch the triceps and to enable the children to adapt to an altered position of the wrist and to mobility of the elbow at one step and following a single period of plaster immobilisation. It is very likely that function is better than it would have been had the condition remained untreated. Before operation the children had only a

[Total syndactylism with mesomelic shortening of the arm, radioulnar and metacarpal synostoses and disorganization of the phalanges ("cenani syndactylism") (author's transl)].

An additional case "Cenani syndactylism" in a 7 years-old female child was described. With this autosomal recessive inherited disease, the radius and ulna are severly shortened and generally fused with each other. The metacarpal bones are, for the most part, synostosized. The phalanges are badly disorganized. Usually no fingers can be recognized externally. The alterations in the feet are far less pronounced. Syndactylism and ray reduction are frequent. A similar, but apparently independent, case with radioulnar synstostoses, reduction, dysplasia and fusion of the metacarpal bones but less pro

Congenital stapes fixation, symphalangism and syndactylia.

An autosomal dominant hereditary syndrome is described consisting of congenital deafness, characteristic finger and toe deformities, with absence of the proximal interphalangeal joint together with syndactylia. The syndrome has been observed in 5 members of a Danish family, and the study confirms the autosomal dominant trait.

Congenital limb anomalies:frequency and aetiological factors. Data from the Edinburgh Register of the Newborn (1964-68).

This paper reports a detailed follow up of 156 patients with structural upper and lower limb anomalies, obtained from the Edinburgh Register of the Newborn (a registration of 52,029 consecutive births between 1964 and 1968). At the time of this survey the patients were between 4 and 9 years of age. The Register had noted probably 96% of all children born with limb defects in Edinburgh between 1964 and 1968. It was found that the diagnosis was completely accurate in only 26% of cases, and partially so in a further 42%. The frequency of each anomaly has been noted, subdivided into isolated anoma
