Microvascular great toe transfer for congenital absence of the thumb.

Two cases of microvascular great toe transfer for congenital absence of the thumb in children are reported. A rationale and operative details are discussed. A long term follow up is needed, but at this stage the results are most encouraging. There has been no interference with gait.

Anatomical analysis of the developmental effects of aneuploidy in man--the 18-trisomy syndrome: II. Anomalies of the upper and lower limbs.

We report the anatomical variations of the limbs in eight infants with the trisomy-18 syndrome that were dissected and studied in detail. In each case, the upper limbs showed defects which further define the specific influence of this aneuploidy on the development of its preaxial (radial) component, and the tendency towards reduction defects. Abnormalities included muscle variations concentrated along the radial margin of the forearm and hand, the absence of the definitive musculocutaneous nerve in all of the limbs, and reductions of the radial artery in four of the bodies. Pathogenetic mechan

Congenital absence of teeth: a review with emphasis on inheritance patterns.

Congenital absence of teeth is a heritably phenomenon probably most often passed to each generation by an autosomal dominant pattern with incomplete penetrance and variable expressivity. Correlation of hypodontia with systemic disease leads to the hypothesis that this frequent dental anomaly may in some cases be a microform of systemic ectodermal dysplasia.

Osteochondritis dissecans and brachymesophalangia: a hereditary syndrome.

A kindred with an unusual congenital hand and foot anomaly is presented. It is an autosomal dominant hereditary disorder characterized by brachymesophalangia, slight clinodactyly, deformed metacarpal heads, osteochondritis dissecans in metacarpophalangeal joints, absence or hypoplasia of the ulnar styloid processes, and brachyhypophalangia in the feet.

Limb anomalies in chromosomal aberrations.

This survey shows that there are at least 6 autosomal and 2 gonosomal aberrations which may produce specific types of limb anomaly in 30%-80% of cases. The "expressivity" of these anomalies covers a wide range within the morphogenetic pattern. No entirely specific malformation type is seen. The most unusual malformation, aplasia of the thumbs with proximal synostosis of the 4th aand 5th metacarpals, is seen in 13q- (13r) but the precise cytogenetic basis is not clear. Aplasia of the thumb associated with synostosis of the 4th and 5th metacarpals was occasionally described before (unilateral [1

Medical sculpture: a valuable aid to patient rehabilitation.

Silicone rubber is the most satisfactory material for cosmetic prostheses. It has a soft, lifelike texture, is easily worked and easily colored and is strong enough to withstand frequent application and removal. Examples of patients referred to the medical sculptor include those with congenital defects of one or both ears, loss of the nose or an eye and orbit due to cancer surgery, loss or absence of fingers or parts of the hand and those who have had radical breast surgery.

A dermatoglyphic study of autistic patients.

Dermatolglyphic patterns of autistic children were compared to those of control subjects matched socioeconomically and by age and sex. Analysis of dermal ridge patterns and ridge counts resulted in significant differences between the 78 autistic and 78 normal children. Differences from normal expectation in the autistic children were most apparent in the reduced number of whorls and increased number of arches, in lowered ridge counts, and in less distinctness in formation of ridge line. The number of dermal ridges on autistic hands was much lower for each finger as well as for the total hand.

Pollicization.

Congenital absence or traumatic amputation of the thumb is a major disability profoundly disturbing the function of the hand, as the thumb represents an estimated 50% of total hand function. The methods for thumb reconstruction utilizing the index finger were developed some twenty years ago, and progress has been made in the last several years with emphasis on intrinsic muscle reconstruction as emphasized by Buck-Gramcko. The method, technique and anatomy of index pollicization is discussed, and representative cases presented for congenital absence of the thumb and for traumatic amputation of

Congenital absence of the flexor pollicis longus--a case report.

Congenital absence of the flexor pollicis longus, associated with hypoplasia of the thumb, is an unusual anomaly. A case of congenital absence of the flexor pollicis longus without hypoplasia or aplasia of the thumb and thenar muscles, which is described, is very rare. Surgical treatment for the transfer of the ring finger flexor superficialis tendon to provide the thumb interphalangeal joint with flexion resulted in a significant improvement in pinch.

Absence of distal interphalangeal fold causing difficulty in extending fingers.

A 13-year-old girl sought medical advice, saying that for two years it had been increasingly difficult for her to extend her little finger. An examination revealed that all her fingers, with the exception of her thumbs, had no interphalangeal fold. Her mother had less pronounced signs of the same type. This abnormality seemed to be the result of an autosomal gene with dominant transmission.

Abductor digiti quinti opponensplasty.

Fifteen neurovascular pedicle transfers of the abductor digiti quinti to the base of the proximal phalanx of the thumb were performed: seven for laceration of the median nerve, seven for neurological disease, and one for congenital absence of the thenar muscles. Fourteen of the transfers were viable, and twelve gave excellent results. The three failures all had had inadequate release of contractures of the thumb. This transfer offers the following advantages over other opponensplasties: (1) an intrinsic muscle with correct amplitude and direction of pull replaces intrinsic muscles; (2) the ten

Congenital anomaly of the thumb: absent intrinsics and flexor pollicis longus.

Congenital absence of the median-innervated intrinsic muscles and flexor pollicis longus was seen in varying degrees of severity in eleven hands of eight patients. Three members were involved in one family in which the anomaly was probably an autosomal dominant. There were no chromosomal abnormalities. Surgical treatment of seven hands, including release of the adduction contracture and transfer of the ring-finger superficialis tendon to provide opposition and to reinforce the ulnar collateral ligament of the metacarpophalangeal joint, resulted in significant improvement in pinch and grasp.

[Poland's syndrome. Clinical and genetic studies; physiopathologic considerations].

Study of malformations of the hand makes possible the definition of three forms of progressive severity in the Poland syndrome, in which there is unilateral symbrachydactylie associated with homolateral aplasia of the pectoralis major. The three forms share the predominance of involvement of the 2nd, 3rd and 4th fingers. The existence of alterations in palmar creases and dermatoglyphia of the involved side alone confirms the absence of any lesion, even infra-clinical, of the other hand. Genetic study favours a pluri-factorial inheritance. Topographical data and the comparative study of vascula

Congenital absence of the pectoral muscles. A review of twenty-five patients.

Twenty-five patients with unilateral partial absence of the pectoral major muscle were reviewed to determine the presence of fetal insult, associated anomalies, neoplasia, and prognosis. Fifteen had ipsilateral synbrachydactyly (the Poland Anomaly) which varied in severity from minimal findings to nearly complete absence of the hand. The pattern of abnormality was remarkably uniform with maximal involvement of the midhand and least involvement of the thumb and small finger. The etiology of this condition remains unknown although fetal insult is suggested in some cases. Although reports in the
