BACKGROUND: The past decade has been marked by the progressive integration of artificial intelligence (AI), especially in the form of machine learning and deep learning. Pediatric orthopaedics remains an underexplored area in AI, especially in the context of congenital deformities such as clubfoot.
OBJECTIVE: A scoping review would help organize the various uses of AI in this domain by identifying studies and categorizing them based on the technique used and their applications, exposing gaps, and informing future studies to support AI integration in pediatric care.
METHODS: A Preferred Repor

Flatfoot (pes planus) is a common foot deformity associated with altered lower-extremity muscle activity during gait. Conventional foot orthoses and insoles can provide arch support but have limitations related to comfort, adherence, and everyday usability. This study evaluated the immediate effects of a thermoplastic polyurethane (TPU) arch pad sock on lower-extremity muscle activity in adults with flexible flatfoot. Twenty adults with flexible flatfoot participated. Surface electromyography signals were recorded bilaterally from the tibialis anterior (TA), peroneus longus (PL), medial gastro

BACKGROUND: Pediatric polydactyly is a relatively common congenital condition of hand and foot, affecting physical and mental development of children. Hand polydactyly has been brought into focus recently. By contrast, the studies on foot polydactyly are limited. Among the types, there is little international research on preaxial foot polydactyly since it is a relatively rare type, and the domestic research on it is fewer. The purpose of our study is to propose a new classification for guiding operation.
METHODS: We collected all patients with preaxial foot polydactyly who had preoperative ra

BACKGROUND: Calcaneocuboid (CC) joint subluxation is a recognized complication after calcaneal lengthening osteotomy for pediatric idiopathic flexible flatfoot, and temporary pin fixation is often performed to prevent it. This study compared postoperative radiologic outcomes at 2 years of single- and double-pin fixation of the CC joint.
METHODS: We retrospectively reviewed 50 feet from 30 patients aged 9 to 15 years who underwent calcaneal lengthening osteotomy for idiopathic flexible flatfoot with a follow-up of at least 2 years. Fourteen feet (28%) underwent single-pin fixation, and 36 (72%

BACKGROUND: Progressive collapsing foot deformity involves failure of both static and dynamic medial arch stabilizers. The relative timing and interaction between posterior tibial tendon dysfunction and ligamentous attenuation remain unclear. This study investigates whether the posterior tibial tendon can compensate for progressive ligament failure in distributing pressure under the foot in a cadaveric progressive collapsing foot deformity model at balanced stance.
METHODS: Eight cadaveric lower limbs underwent sequential ligament transections replicating progressive collapsing foot deformity

PURPOSE: This study evaluated tibiofemoral loading and medial meniscal stress distribution in individuals with flexible flatfoot (FFF) during walking under different foot progression angle (FPA) conditions.
METHODS: This study analyzed the gait of 28 FFF patients (16 males, 12 females) under three FPA conditions (neutral, toe-in, toe-out). Kinematic (Vicon) and kinetic (Kistler) data were used to estimate tibiofemoral forces in OpenSim. Subsequently, joint angles and muscle forces at peak tibiofemoral forces were used to drive a finite element (FE) model of the knee, enabling the comparison o

This case report details a neonate diagnosed with arthrogryposis multiplex congenita (AMC), specifically the amyoplasia subtype, presenting with bilateral talipes equinovarus, elbow contractures, ulnar deviation of the wrists and clinodactyly. Additional clinical findings included mild developmental dysplasia of the hips and a small atrial septal defect. Genetic analysis revealed heterozygous variants of uncertain significance in the SYNE1 and CHD4 genes, both of which were inherited from asymptomatic parents. While SYNE1 is associated with autosomal recessive AMC type 3 and CHD4 with Sifrim-H

INTRODUCTION: Pallister-Hall syndrome (PHS) is a rare autosomal dominant disorder caused by pathogenic variants in GLI3 gene. It is characterized by variable combinations of hypothalamic hamartoma, polydactyly, bifid epiglottis, renal and genitourinary abnormalities, imperforate anus, and pituitary dysfunction. Most cases are recognized during infancy or childhood; adult presentation is uncommon and hypoglycemia is not an established manifestation of PHS.
CASE DESCRIPTION: A 26-year-old man with obesity and well-controlled hypertension was referred for evaluation of recurrent fasting and post

UNLABELLED: Crisponi/Cold-Induced Sweating Syndrome Type 1 (CS/CISS1) is an ultra-rare genetic disorder caused by biallelic variants in the cytokine receptor-like factor-1 (CRLF1) gene and is associated with several comorbidities. Comprehensive data on the respiratory phenotype of this condition are lacking despite being associated with high mortality in infancy, often due to severe respiratory complications like aspiration pneumonia and sudden death related to hyperthermic crises. Our study investigates respiratory complications and lung ultrasound characteristics, enhancing the comprehension

Arthrogryposis multiplex congenita (AMC) presents complex therapeutic challenges in upper limb reconstruction, in which establishing an optimal positioning-reaching-grasping axis is critical for patient independence. However, clinical evidence is often limited by low-level data and significant phenotypic heterogeneity, leaving management strategies fragmented. This review synthesizes current evidence to provide a strategic framework for managing complex upper extremity deformities in AMC. A selective literature search was conducted via the MEDLINE database in April 2026, focusing on advancemen

Adams-Oliver syndrome (AOS) is characterized by scalp aplasia cutis congenita (ACC) and terminal transverse limb defects (TTLD). Autosomal recessive AOS type 2 caused by DOCK6 variants is usually accompanied by central-nervous-system and ocular anomalies. We report two unrelated children who each carried compound heterozygous DOCK6 variants. Both presented with variable degrees of global developmental delay and visual impairment, but neither showed ACC or TTLD. Our observations highlight the phenotypic variability in DOCK6-related AOS and a possible ascertainment bias in screening patients onl

The authors describe a novel single-stage, nailbed-preserving debulking procedure for macrodactyly used in their unit. This simple technique preserves perfusion and venous drainage of the nailbed complex through a subperiosteal dissection and use of a dermal bridge by de-epithelialization of the proximal area, followed by folding the nailbed flap back onto itself in a pleated fashion. Four toes of 3 patients were treated with this technique, and all achieved a shorter, less bulky appearance with nail preservation at 2-year follow-up. The single-stage pleated nailbed flap is a simple and robust

Myhre syndrome is a rare genetic disorder caused by pathogenic variants in SMAD4. The neurocognitive profile may include intellectual disability and developmental delay across a wide spectrum of domains. Four individuals with Myhre syndrome and psychosis and/or schizophrenia have been reported. We discuss three additional individuals (one briefly mentioned in a review of patients with Myhre syndrome) with SMAD4 pathogenic variants at codon Arg496Cys (two cases) and codon Ile500Val (one case). All had social challenges and variable developmental delays. They presented with acute-onset psychosis

Myhre syndrome (MYHRS) is an ultra-rare, progressive multisystem disorder caused by recurrent heterozygous missense variants in the SMAD4 gene, a central mediator of TGF-β and BMP signaling. Skeletal abnormalities-including postnatal short stature, brachydactyly, thickened calvarium, and craniofacial dysmorphism-are cardinal features, often accompanied by joint contractures and progressive soft tissue fibrosis. Extensive cellular and genetic evidence supports a gain-of-function (GoF) mechanism wherein mutant SMAD4 displays increased protein stability and prolonged nuclear localization, enhanci
