Mastectomy and sentinel lymph node biopsy in a patient with left-sided breast cancer and Poland syndrome.

This case focuses on breast cancer surgery in the setting of Poland syndrome (PS), a rare congenital condition characterised by unilateral absence or hypoplasia of the pectoral muscles. The literature describing breast cancer in patients with PS is limited. We present a case of an elderly woman with left-sided PS in whom breast cancer was incidentally detected during routine surveillance imaging for malignant melanoma. Imaging and biopsy confirmed a small invasive carcinoma, and the patient underwent mastectomy with sentinel lymph node biopsy. Intraoperatively, the absence of the pectoralis ma

The Fetal Effects of Chorionic Villus Sampling.

BACKGROUND: Physical examinations have been essential in the delineation of the spectrum of teratogenic effects of teratogens, such as valproate, isotretinoin and thalidomide, as well as chorionic villus sampling (CVS). CVS became an alternative to amniocentesis in the 1980s, because results were provided earlier in pregnancy. In 1991, a dramatic report of 5 CVS-exposed infants, out of 289 exposed, with severe limb and craniofacial defects raised the question of a significant risk, particularly when the procedure was carried out before 70 days of gestation. Some epidemiologic studies using dat

A Sustainable Natural-Rubber IoT Smart Insole for Remote Body-Load Monitoring: An Observational Gait Comparison in Flexible Flatfoot.

Flexible flatfoot (pes planus) alters lower-limb biomechanics and plantar-pressure distribution, raising the risk of pain and injury. Laboratory gait analysis with optical motion capture and force plates is the reference standard but is costly, space-constrained, and ecologically limited. We present the design, fabrication, and validation of a low-cost, sustainable smart insole for Internet-of-Things (IoT) remote body-load monitoring. The device pairs a dual-layer natural-rubber body-a silica-filled sponge-rubber upper for comfort and a carbon-black-reinforced solid outsole for durability-with

Syndrome of the Month: Van den Ende-Gupta Syndrome: Gestalt Diagnosis, Outcomes, and Recommendations.

Van den Ende-Gupta syndrome (VDEGS) is an autosomal recessive disorder involving self-resolving joint contractures that are present since birth. Because of characteristic facial features, clinical diagnosis may be suspected by experienced clinicians or identified using facial recognition-based software. Confirmation by identification of disease-causing biallelic variants in SCARF2 can be helpful to definitively provide a clinico-molecular diagnosis to differentiate VDEGS from other disorders with overlapping phenotypes with blepharophimosis, arachnodactyly, and camptodactyly. Most affected ind

Limb Reconstruction in a Resource-Limited Environment.

Limb reconstruction in a resource-limited environment is a real challenge to health authorities, especially with the increasing number of patients. In the Middle East and low-income countries, the incidence of congenital deformities, complex trauma, war injuries, and posttraumatic complications is high. It is important to be guided by the best possible scientific evidence concerning the patient's opinion and available resources. Amputation is not accepted as a treatment option even in neglected complex cases. Therefore, new classifications were developed and current management methods were mod

Soft tissue abnormalities in the congenital limb malformation radial dysplasia (RD): Their clinical impact and treatment significance.

The defining features of the upper limb congenital anomaly, radial dysplasia (RD), involve both skeletal and soft tissue malformation, including muscle, vascular, and neural structures, with varying degrees of severity. Herein, we summarise the range of soft tissue anatomy malformations that have been described in RD, with the objective of better understanding their aetiology and clinical significance. Changes to muscle anatomy can include hypoplasia and fusion; vascular defects often involve persistent median arteries and absent radial arteries. There are also consistent differences in neural

Dual rare genetic variants: case report of a child with SBIDDS syndrome and citrullinemia type 1.

BACKGROUND: The arginine methyltransferase 7 (PRMT7) gene plays a role in signal transduction and protein interactions and negatively regulates neuronal differentiation. Pathogenic variants of PRMT7 cause SBIDDS syndrome (Short stature, brachydactyly, intellectual developmental disability, and seizures). PRMT7 has been shown to interact with the argininosuccinate synthetase (ASS1) gene; biallelic pathogenic variants of ASS1 are associated with citrullinemia type 1. We report a patient with biallelic variants for SBIDDS syndrome and citrullinemia type 1. CASE PRESENTATION: The child exhibits se

Effects of Orthotic Insoles on Gait Biomechanics in Runners With Flatfoot Under Different Gait Loads.

The purpose of this study was to quantify the biomechanical performance of the medial longitudinal arch (MLA) in the normal foot (NF), flatfoot (FF), and flatfoot with orthotic insoles (FF + insole) under different gait loads to identify their movement response strategies. Male runners with NF and flexible FF were instructed to walk and run using a rearfoot strike pattern. Subsequently, the FF group underwent the same test again while wearing orthotic insoles. The Qualisys capture system and Kistler force platform were synchronized to collect kinetic and kinematic data. A mixed-design two-way

Accelerated versus standard ponseti method for idiopathic clubfoot: A systematic review and meta-analysis of efficacy and safety.

BACKGROUND: The Ponseti method is the gold standard for managing congenital idiopathic clubfoot. Recent modifications, such as the accelerated Ponseti protocol, aim to shorten treatment duration without compromising outcomes. However, evidence comparing its efficacy and safety to the standard protocol remains inconclusive. OBJECTIVE: To compare the efficacy and safety of the accelerated versus standard Ponseti method in the treatment of congenital idiopathic clubfoot. METHODS: Eligible studies included randomized controlled trials and prospective comparative studies involving patients with i

Plantar pressures in symptomatic and asymptomatic flexible flatfeet: how do they differ?

As radiographic and clinical parameters have not been found to consistently correlate with pain and disability in adolescents with flexible flatfoot deformities, there is no consensus as to why some flexible flatfeet become bothersome and others do not. The purpose of this study was to assess pedobarographic differences between adolescents with symptomatic flexible flatfoot (SFF) and those with asymptomatic flatfeet (AFF) deformities. A retrospective review of a Foot and Ankle registry identified 59 adolescents (64 feet) with SFF who underwent plantar pressure analysis. Normalized contact area

Explainable deep learning-based multiclass classification of foot radiographs into normal, plantar fasciitis, and flatfoot.

BACKGROUND: The medial longitudinal arch plays a critical role in foot biomechanics, and its abnormalities are associated with conditions such as flatfoot and plantar fasciitis. Early and accurate diagnosis of these disorders is clinically important, but plain radiographs have limitations in visualizing soft tissue pathology. PURPOSE: This study aimed to develop and interpret a deep learning model capable of classifying foot radiographs into normal, plantar fasciitis, and flatfoot categories. METHODS: A DenseNet-121 architecture was trained on 9500 synthetic lateral foot X-ray images from th

Clinical and radiographic efficacy of subtalar screw arthroereisis in the treatment of pediatric flexible flatfoot.

BACKGROUND: Subtalar arthroereisis has been reported to be an effective treatment technique for flexible flatfoot (FF) in children. Although many devices for this procedure exist, arthroereisis using screws is still globally used. Therefore, this study aims to revisit subtalar screw arthroereisis (SSA) and investigate its outcomes. METHODS: We retrospectively reviewed 353 flexible flatfeet in 178 pediatric patients who underwent SSA between 2007 and 2020. Clinical and radiological assessments were conducted pre-implantation and pre-explantation. Radiographic angles were measured to quantify c

Successful Haplo-Hematopoietic Stem Cell Transplantation for Juvenile Myelomonocytic Leukemia in a Child With Underlying Thrombocytopenia-Absent Radius Syndrome: A Unique Case.

BACKGROUND: Thrombocytopenia-absent radius (TAR) syndrome is a rare congenital disorder characterized by bilateral radial aplasia with preserved thumbs and early-onset thrombocytopenia. While hematologic and skeletal abnormalities define the condition, its association with hematologic malignancies is extremely rare, with only a few reported cases of leukemia. Juvenile myelomonocytic leukemia (JMML) is an uncommon pediatric myelodysplastic/myeloproliferative neoplasm frequently linked to RAS pathway mutations. To our knowledge, JMML has not previously been reported in association with TAR syndr

Virtual 3D landmark palpation in clubfoot: feasibility of an innovative standardized morphometric protocol. A preliminary study.

BACKGROUND: Reliable morphological assessment remains a clinical challenge in congenital clubfoot management, as current clinical scoring systems fail to capture the complex three-dimensional anatomy required for precise evaluation and treatment planning. Virtual palpation of anatomical landmarks offers a standardized, objective approach to define bony axes, but requires rigorous validation to become clinically applicable. METHODS: 3D models (dried clubfoot and normal foot) were reconstructed from segmented CT scan datasets using image segmentation. For reproducibility purpose, thirty-two ana
