[Molecular diagnosis of non-syndromic craniosynostosis: new challenges for real clinical practice].

Craniosynostoses (CS) are a congenital malformation caused by absence or premature closure of skull sutures. This leads to deformation and impaired growth of skull. Traditionally, syndromic and non-syndromic forms are distinguished. For a long time, it was believed that molecular genetic mechanisms are significant exclusively for syndromic CS. However, clear data on involvement of somatic and germinal mutations in pathogenesis of non-syndromic forms were accumulated in recent years. Currently, there are no unified clinical guidelines defining the strategy for molecular genetic diagnosis for no

Addressing unilateral lambdoid craniosynostosis using endoscopic suturectomy and barrel-stave osteotomies: surgical technique and literature review.

PURPOSE: Unilateral lambdoid craniosynostosis (ULC) is a rare cranial deformity requiring intervention to correct asymmetry of the posterior skull. Conventional open remodeling and distraction osteogenesis offer robust expansion but carry risks of higher blood loss, hardware complications, or reoperation. We report a novel, minimally invasive technique combining endoscopic lambdoid suturectomy with barrel-stave osteotomies, supplemented by a literature review. CASE DESCRIPTION: A 2-month-old healthy male presented with classic left ULC features, including occipitoparietal flattening, ipsilate

Short-term outcomes after suturectomy and cranial remodeling orthosis treatment for craniosynostosis in infants 5 months and older.

OBJECTIVE: Suturectomy followed by cranial remodeling orthosis (CRO) is traditionally performed in infants younger than 4 months. The authors aimed to evaluate the effectiveness of this technique in patients aged 5 months and older. METHODS: A single-institution retrospective chart review of all patients undergoing nonendoscopic suturectomy for craniosynostosis between January 2018 and June 2023 was performed. Demographic and operative data and remodeling outcomes assessed by cranial measurements from head scans were analyzed. Patients were stratified into 2 groups based on age: age ≥ 5 month

Targeting Piezo1/2 ion channels for modulation of the lymphatic system: mechanisms, applications, and therapeutic prospects.

In recent years, the role of mechanosensation in lymphatic system development, functional homeostasis, and disease progression has received increasing attention. As key mechanosensitive receptors in lymphatic endothelial cells, Piezo1/2 ion channels respond to biomechanical stimuli such as fluid shear stress and interstitial pressure, thereby regulating calcium influx and downstream signaling pathways. These channels play pivotal roles in physiological processes, including lymphatic network formation, valve development, and the maintenance of meningeal lymphatic function. This review systemati

Pulse pressure variation demonstrates dose-dependent response to volume resuscitation in pediatric craniosynostosis repair.

BACKGROUND: Restoring intravascular volume in the face of hemorrhagic blood loss is a fundamental skill critical to the practice of anesthesiology. Predictors of fluid responsiveness in pediatric patients remain a challenge. Pulse pressure variation (PPV) is a valuable predictor of volume responsiveness in adults, but the utility in pediatric resuscitation remains controversial. Surgical correction of craniosynostosis occurs early in childhood and is commonly associated with significant, rapid blood loss intraoperatively. This study examines the relationship between blood product boluses with

Biomechanical strain patterns in Meckel's cartilage underlie mandibular variation in FGFR2 mouse models.

Sequence variations in fibroblast growth factor receptor 2 (FGFR2) are associated with craniosynostosis syndromes, disrupting craniofacial development, yet early mandibular alterations remain poorly characterized. This study examined Meckel's cartilage (MC) and mandibular morphology in mouse models of Apert (Fgfr2+/P253R) and Crouzon (Fgfr2cC342Y/+) syndromes using Euclidean distance matrix analysis and finite element modeling at embryonic days 14.5 and 16.5. Apert mice exhibited significant MC shortening and elevated, widespread strain in the growth center area correlating with mandibular sho

Intraoperative and radiographic indicators of extraocular muscle aberrancy in syndromic craniosynostosis.

BACKGROUND: Strabismus is common in syndromic craniosynostosis, often attributed to abnormal extraocular muscle (EOM) trajectory, and orbital anatomy. Absent EOMs have been reported, but the spectrum of primary dysgenesis remains poorly understood. The purpose of this study was to characterize the range of EOM dysgenesis in a large cohort that had undergone strabismus surgery. METHODS: The medical records of patients with syndromic craniosynostosis treated at Boston Children's Hospital (2010-2023) were reviewed retrospectively to identify all cases with neuroimaging and history of strabismus

Blood Transfusion Rates in Non-syndromic Craniosynostosis: A Proportion Meta-analysis of Open Versus Minimally Invasive Techniques.

OBJECTIVES: Premature suture fusion in non-syndromic craniosynostosis often requires surgery to permit normal brain growth. To minimize blood transfusion requirements, minimally invasive surgery (MIS) has been proposed. This study will pool the prevalence of blood transfusion in non-syndromic craniosynostosis surgeries, comparing open surgery (OS) with MISs. METHODS: A systematic review and proportion meta-analysis was conducted in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines and registered in PROSPERO (CRD42024604632). Witteveen-Kolk Syndrome: Two Patients With Craniofacial Features and Novel SIN3A Variants.

Witteveen-Kolk syndrome (WKS) is a rare autosomal dominant neurodevelopmental disorder caused by pathogenic SIN3A variants, with diagnosis complicated by variable physical findings. We describe two unrelated patients with de novo SIN3A variants identified through trio whole exome sequencing, expanding the phenotype of WKS. Patient 1, a 16-year-old female, presented with bilateral cleft lip and palate, retained primary teeth, and horseshoe kidney: findings not previously reported in WKS. Patient 2, a 5-year-old male, exhibited macrocephaly and obesity, also novel to the WKS phenotype. These fin

Phenotypic spectrum of 1q21.1 copy number variants in 42 children: genotype-phenotype correlations and clinical implications.

UNLABELLED: Chromosome 1q21.1 copy number variants (CNVs) are associated with a broad and variable clinical spectrum encompassing neurodevelopmental, anthropometric, and systemic anomalies. The incomplete penetrance and variable expressivity of these rearrangements pose significant challenges for clinical interpretation and genotype-phenotype correlation. We retrospectively analyzed 42 pediatric patients with 1q21.1 CNVs, identified by chromosomal microarray analysis (CMA), evaluated at a single tertiary center between 2019 and 2025. Patients were classified according to CNV type and localizat

Parent-Reported Health-Related Quality of Life at 3 Years of Age in Children Treated for Sagittal or Metopic Synostosis.

Early surgical intervention is recommended for sagittal (SS) and metopic (MS) synostosis to correct head shape and reduce the risk of later social stigmatization. However, the impact of craniosynostosis (CS) and its surgical treatment on health-related quality of life (HRQoL) in early childhood remains insufficiently understood. To evaluate parent-reported HRQoL at 3 years of age in children treated for SS or MS. At the 3-year follow-up, parents of children treated for SS or MS completed the Pediatric Quality of Life Inventory (PedsQL) version 2 to 4 years. Age- and nationality-matched healthy

Pathological Priming and Developmental Heterochrony as Organizing Concepts in Human iPSC Models of Autism Spectrum Disorder.

Autism spectrum disorder (ASD) is a heterogeneous neurodevelopmental condition in which diverse genetic risk factors converge on molecular pathways governing early brain development. Human-induced pluripotent stem cell (iPSC) and brain organoid models now permit direct interrogation of these pathways in patient-derived neural tissue and, in a subset of studies, reveal alterations in neural progenitor proliferation, cortical neurogenesis, synaptic maturation, and network excitability. A landmark iPSC study in macrocephalic idiopathic ASD introduced the concept of pathological priming of neural

Resistance to Thyroid Hormone Alpha: Clinical Spectrum, Diagnostic Clues, and Therapeutic Strategies - A Narrative Review.

Resistance to thyroid hormone alpha (RTHα) is a rare, inherited disorder of thyroid hormone action caused by pathogenic variants in the thyroid hormone receptor alpha gene. Unlike resistance to thyroid hormone beta, RTHα typically presents with near-normal thyroid function tests and a non-elevated thyroid-stimulating hormone (TSH), contributing to substantial underdiagnosis. The biochemical hallmark is a low or low-normal free thyroxine (FT4) level with a disproportionately high free triiodothyronine (FT3) level, yielding a reduced FT4:FT3 ratio and often a low or low-normal reverse T3.
