Optimising Fronto-Orbital Distraction for Management of Unicoronal Craniosynostosis.

PURPOSE: Unicoronal craniosynostosis (UCS) involves premature fusion of the left or right coronal suture, resulting in skull deformities and facial asymmetry. Fronto-orbital distraction (FOD) has proven effective for surgically correcting UCS; however, determining optimal surgical parameters (e.g. distraction angle, length, and distractor number) remains challenging. This study aimed to optimise FOD for UCS using finite element (FE) analysis and simulated surgical outcomes. METHODS: We developed a patient-specific FE model using pre- and postoperative computed tomography (CT) scans from a 23-

To Stave or Not to Stave: Outcomes of Barrel Stave Osteotomies in Strip Craniectomy for Sagittal Craniosynostosis.

BACKGROUND: Endoscopic strip craniectomy (ESC) with postoperative helmet molding is an accepted treatment for sagittal craniosynostosis. Barrel stave osteotomies (BSO) with immediate greenstick outfracture can be added to ESC for immediate cranial expansion. BSO is hypothesized to improve cranial remodeling, but its benefits and complications remain debated. This study compares operative and morphologic outcomes in patients with sagittal craniosynostosis treated with ESC with and without BSO. METHODS: A retrospective review of patients with isolated sagittal craniosynostosis treated with endo

En-bloc posterior vault distraction including the foramen magnum for Chiari I malformation in severe syndromic craniosynostosis: First cases and literature review.

This case series describes two patients with severe Crouzon syndrome and persistent Chiari I malformation. Initial treatment with standard posterior vault distraction osteogenesis and frontoorbital advancement failed to resolve the patient's symptoms. One patient developed severe syringomyelia and myelopathy refractory even to standard suboccipital decompression. Given the persistent posterior fossa constriction, an en-bloc posterior vault distraction incorporating the foramen magnum was performed over 21 days, achieving an expansion of 20 mm. This approach resulted in complete symptom resolut

Isolated frontosphenoidal suture synostoses: report of two cases and literature review.

INTRODUCTION: Anterior plagiocephaly is most commonly attributed to unilateral coronal synostosis. However, isolated premature fusion of the frontosphenoidal suture (FSC) represents a rare and often misdiagnosed cause due to its overlapping clinical presentation. Accurate diagnosis is essential, as treatment strategies and prognoses differ significantly. OBJECTIVES: To present a comprehensive literature review of FSC and to report two novel clinical cases-one managed with open fronto-orbital advancement and the other with endoscopic suturectomy. METHODS: A systematic review of reported FSC c

Combined Long-Read Genome and Transcriptome Sequencing Establishes Novel Variants in MEGF8 as the Cause for Carpenter Syndrome Type 2.

Carpenter syndrome type 2 (CRPT2) is a rare autosomal recessive disease mainly characterized by craniosynostosis and polysyndactyly. CRPT2 is the rarer subtype of Carpenter syndrome (CRPTS) and is caused by biallelic variants in the multiple epidermal growth factor-like domains 8 gene (MEGF8). Due to its rarity and phenotypic overlap with other craniosynostosis syndromes, definitive molecular diagnosis of CRPT2 can be challenging. Here, we describe a proband with CRPT2 carrying compound heterozygous variants in MEGF8: one de novo variant disrupting splicing and a maternally inherited missense

Surgical Management of Nonsyndromic Scaphocephaly: A Review and a Standardized Cranial Vault Remodeling Technique.

BACKGROUND: Premature sagittal suture fusion causes scaphocephaly, the most prevalent type of nonsyndromic craniosynostosis. There is no consensus on the optimal surgical approach, despite several corrective techniques having been described. The results of a standardized open cranial vault remodeling (CVR) technique with simplified osteotomy planning, intended to address the phenotypic characteristics of sagittal synostosis, are presented in this study. METHODS: Between 2013 and 2023, 22 consecutive patients with moderate-to-severe scaphocephaly who had total CVR using a reproducible techniqu

Craniosynostosis: Imaging and Surgical Implications.

Craniosynostosis is characterized by the early fusion of cranial sutures, leading to abnormal skull shapes, potentially increased intracranial pressure (ICP), and, in some cases, neurocognitive challenges. Craniosynostosis may occur in isolation or as part of a genetic syndrome, with the majority of cases being nonsyndromic. Genetic mutations, such as those affecting the FGFR and TWIST genes and numerous signaling pathways, play a pivotal role in the etiopathogenesis of craniosynostosis. Secondary craniosynostoses related to systemic diseases and bone dysplasia are also common. Imaging is inte

Digital Planning and Patient-Specific Implants Facilitate Accurate Conventional Sub-Cranial Le Fort III Advancement.

Syndromic craniosynostosis is characterized by multisuture fusion, midface hypoplasia, and, frequently, extracranial anomalies. In these patients, transcranial monobloc/facial bipartition or subcranial Le Fort III osteotomy is typically used to achieve total midface advancement. Presented here is a case of conventional subcranial Le Fort III osteotomy in a 15-year-old patient with Apert syndrome using digital planning and patient-specific implants. The accuracy of this procedure was assessed by digitally overlaying the postoperative computed tomography scan onto the preoperative surgical plan.

Association between craniosynostosis and phospholipid metabolism: Insights from single-cell and transcriptomic analysis.

Craniosynostosis is a common defect in the craniofacial structure of children, yet its pathogenesis remains unclear. Current research focuses mainly on bone-related genetic mutations, with less emphasis on lipid metabolism disorders. Hence, it is imperative to investigate the underlying mechanisms from various perspectives. We investigated the association between craniosynostosis and phospholipid metabolism through a multi-omics approach. This included Mendelian randomization (MR) to assess causal relationships of lipid traits, transcriptomic differential analysis of clinical samples, and sing

Diagnostic Accuracy of Craniosynostosis in Japan: A Survey Using Three-Dimensional Physical Models.

Craniosynostosis is the premature fusion of cranial sutures, which interferes with normal brain development and requires surgical treatment. By contrast, positional cranial deformity primarily results from external pressure exerted by the sleeping position and is usually managed conservatively. Accurately differentiating between these 2 conditions is essential. In Japan, the use of helmet therapy to treat positional cranial deformities has rapidly expanded, particularly among pediatricians. However, diagnoses based solely on visual inspection and palpation are limited. The aim of this study wa

Analysis of Lateral Ventricular Volume Changes in Patients With Nonsyndromic Sagittal Craniosynostosis and Slit Ventricle Syndrome Following Posterior Vault Distraction.

PURPOSE: The purpose of this case series was to investigate the impact of posterior vault distraction osteogenesis (PVDO) on lateral ventricle volume in 2 patients presenting with nonsyndromic sagittal craniosynostosis and concurrent slit ventricle syndrome (SVS). PVDO may benefit patients with SVS by reducing postoperative shunt revisions and other complications related to elevated intracranial pressure. However, comprehensive volumetric analyses of the lateral ventricles and their implications in predicting long-term outcomes for SVS within the broader context of craniosynostosis have yet to

Relationship Between Serum Nell-1 Level and Bone Geometry, Bone Microarchitecture in Chinese Postmenopausal Women.

PURPOSE: Neural EGF-like 1 (Nell-1), originally implicated in craniosynostosis, has been identified as a key regulator in osteogenic processes. While preclinical data were encouraging, clinical studies correlating serum Nell-1 levels with osteoporosis remain scarce. This study aims to investigate the relationship between circulation Nell-1 level and bone turnover markers, bone mineral density (BMD), bone microstructure, muscle strength, fall risk, and fractures in Chinese postmenopausal women. METHODS: Serum Nell-1 levels were measured in 123 Chinese postmenopausal women. Muscle function was

Surgical Timing and Postoperative Outcomes of Craniosynostosis Repair by Race When Stratified by Suture Type.

BACKGROUND: Prompt surgical repair of craniosynostosis has been recommended to reduce neurological sequelae and morbidity. Socioeconomic disparities may affect surgical timing, but their effect has not been adequately separated from clinical factors in prior database studies. This study incorporates new diagnostic codes to investigate the effect of race on surgical timing and postoperative outcomes while stratifying for comorbidities and suture involvement. METHODS: All craniosynostosis repair surgeries with specified suture involvement were identified in the 2023 and 2024 National Surgical Q

Divergent Mechanisms of Cranial Suture Ossification in Normal Development and Pathologic Fusion.

Cranial sutures are dynamic growth sites that balance bone growth with mesenchymal patency to accommodate cranial expansion during development. While intramembranous ossification has traditionally been considered the default mechanism of suture fusion, accumulating evidence demonstrates that endochondral pathways might also play a significant role under both physiological and pathological conditions. In this review, we contrast normal developmental ossification processes with premature fusion in craniosynostosis, integrating histological, molecular, and imaging data. We highlight the context-d

Endoscopic-assisted strip craniectomy using a diamond drill bit attached to a dural retractor: a technical paper.

BACKGROUND AND IMPORTANCE: Minimally invasive techniques have largely replaced open cranial vault remodeling as the preferred treatment approach for infants with single-suture craniosynostosis. We describe a refined method for minimizing blood loss during strip craniectomy, using a high-speed drill with a diamond burr and a novel attachable dural retractor. The diamond drill bit generates localized heat that cauterizes the bone edges, thereby potentially reducing bleeding, while the dural retractor is designed to limit heat dissipation and protects the dura from thermal and mechanical injury.

Mechanical bone loading effects on morphology and mechanobiology in the coronal suture of Crouzon mice.

Craniosynostosis is a congenital condition characterized by the premature fusion of the craniofacial sutures. The Crouzon mouse (Fgfr2cC342Y/+) is a well-established model of this condition which shows premature fusion of the coronal suture. Our group has recently shown that postnatal, cyclic loading can potentially rescue the coronal suture and normalize skull morphology in Crouzon mice. This study aimed to investigate the underlying biological mechanism of the treatment. Wild-type (WT) and Crouzon (MUT) mice underwent in vivo loading sessions. Loading did not significantly affect skull shape

Public Insurance Is Associated With Increased Emergency Visits After Craniosynostosis Surgery.

INTRODUCTION: Postoperative emergency department (ED) visits are common across various procedures, often due to surgical complications, pain, and patient uncertainty about normal surgical recovery. While issues such as infections or dehiscence require acute medical management in the ED setting, many visit reasons are driven by manageable postoperative symptoms like fever or swelling. In this study, we aim to identify the most common reasons for acute care visits within 30 d following craniosynostosis surgery, as to inform postoperative care and address health-care utilization. METHODS: A retr

What is the impact of age at surgery on perioperative medical care in sagittal craniosynostosis ?

Choice of timing for surgical correction of scaphocephaly varies considerably from one surgeon to another. We evaluated the impact of age at the time of surgery on blood loss and transfusion, and length of stay in hospital. We compared 2 homogeneous populations of children operated on by the same surgeon between 2011 and 2021. Younger patients (group 1, under 6 months of age, n = 60) underwent a "H-shaped" 5 cm craniectomy between the coronal and lambdoid sutures. Older patients (group 2, aged over 7 months, n = 34) had a cranial vault remodeling with frontal and parietal translation of cranio

Post-Traumatic Sagittal Craniosynostosisin Early Childhood: A Rare Case of Acquired Scaphocephaly.

Craniosynostosis typically presents as a congenital condition. Acquired forms secondary to childhood cranial trauma remain exceptionally rare in medical literature. We report the case of a 2-year-old female patient who developed progressive scaphocephaly following a head injury at 5 months of age. Clinical and radiological examinations confirmed an acquired sagittal craniosynostosis. The patient successfully underwent surgical reshaping of the cranial vault to correct the deformity. At the 1-year post-operative follow-up, excellent cranial remodeling, stable esthetic results, and normal psycho
