Witteveen-Kolk Syndrome: Two Patients With Craniofacial Features and Novel SIN3A Variants.

Witteveen-Kolk syndrome (WKS) is a rare autosomal dominant neurodevelopmental disorder caused by pathogenic SIN3A variants, with diagnosis complicated by variable physical findings. We describe two unrelated patients with de novo SIN3A variants identified through trio whole exome sequencing, expanding the phenotype of WKS. Patient 1, a 16-year-old female, presented with bilateral cleft lip and palate, retained primary teeth, and horseshoe kidney: findings not previously reported in WKS. Patient 2, a 5-year-old male, exhibited macrocephaly and obesity, also novel to the WKS phenotype. These fin

Phenotypic spectrum of 1q21.1 copy number variants in 42 children: genotype-phenotype correlations and clinical implications.

UNLABELLED: Chromosome 1q21.1 copy number variants (CNVs) are associated with a broad and variable clinical spectrum encompassing neurodevelopmental, anthropometric, and systemic anomalies. The incomplete penetrance and variable expressivity of these rearrangements pose significant challenges for clinical interpretation and genotype-phenotype correlation. We retrospectively analyzed 42 pediatric patients with 1q21.1 CNVs, identified by chromosomal microarray analysis (CMA), evaluated at a single tertiary center between 2019 and 2025. Patients were classified according to CNV type and localizat

Pathological Priming and Developmental Heterochrony as Organizing Concepts in Human iPSC Models of Autism Spectrum Disorder.

Autism spectrum disorder (ASD) is a heterogeneous neurodevelopmental condition in which diverse genetic risk factors converge on molecular pathways governing early brain development. Human-induced pluripotent stem cell (iPSC) and brain organoid models now permit direct interrogation of these pathways in patient-derived neural tissue and, in a subset of studies, reveal alterations in neural progenitor proliferation, cortical neurogenesis, synaptic maturation, and network excitability. A landmark iPSC study in macrocephalic idiopathic ASD introduced the concept of pathological priming of neural

Resistance to Thyroid Hormone Alpha: Clinical Spectrum, Diagnostic Clues, and Therapeutic Strategies - A Narrative Review.

Resistance to thyroid hormone alpha (RTHα) is a rare, inherited disorder of thyroid hormone action caused by pathogenic variants in the thyroid hormone receptor alpha gene. Unlike resistance to thyroid hormone beta, RTHα typically presents with near-normal thyroid function tests and a non-elevated thyroid-stimulating hormone (TSH), contributing to substantial underdiagnosis. The biochemical hallmark is a low or low-normal free thyroxine (FT4) level with a disproportionately high free triiodothyronine (FT3) level, yielding a reduced FT4:FT3 ratio and often a low or low-normal reverse T3. Clinic

Consensus statement on the management of Achondroplasia in the Middle East.

Achondroplasia is the most common skeletal dysplasia and presents as a lifelong, multi-system condition characterized by disproportionate short stature, rhizomelic limb shortening, macrocephaly with frontal bossing, and a spectrum of neurological, respiratory, orthopedic, and psychosocial complications. Effective management requires anticipatory surveillance, coordinated multidisciplinary care, and early recognition of life-threatening complications. Therapeutic options have expanded in recent years with the introduction of vosoritide, a C-type natriuretic peptide analog that targets the under

Isogenic modeling of 1q21.1 reciprocal CNVs in human ES cells reveals divergent neurodevelopmental trajectories.

Copy number variations (CNVs) in the distal 1q21.1 region, both deletion (1q del) and duplication (1q dup) are associated with various neurodevelopmental and neuropsychiatric disorders such as autism spectrum disorder, intellectual disability, epilepsy, and schizophrenia. Besides common phenotypes, 1q del and 1q dup manifest opposite clinical phenotypes, e.g. microcephaly in 1q del and macrocephaly in 1q dup. However, molecular and cellular mechanisms underlying these phenotypes are still elusive. Here, to identify molecular mechanisms associated with neurodevelopmental phenotypes from the vie

PTEN variant and genetic backgrounds combine to modify cerebellar neuronal differentiation in autism spectrum disorder.

Mutations in the PTEN gene have been implicated in autism spectrum disorders (ASD), particularly among individuals with comorbid macrocephaly. In our previous study, we demonstrated that the PTEN p.Ile135Leu variant, in an ASD-related genetic background dependent fashion, disrupts both cortical neurogenesis and gliogenesis. While abnormal cerebellar development is a recognized feature of ASD, the specific cellular targets and timing of disruptions during cerebellar differentiation and development remain poorly understood. To investigate these aspects, we applied our previously established cere

CHD8 dysregulation in neurodevelopment: Emerging insights into autism pathophysiology.

Autism is considered a multifaceted neurodevelopmental condition, marked by impairments in social interaction, communication, and repetitive behaviours. Recent genetic research has highlighted mutations in the CHD8 (Chromodomain Helicase DNA-binding Protein 8) gene as one of the most significant risk factors associated with ASD. CHD8 encodes a chromatin remodeler protein that regulates gene expression by altering chromatin accessibility. This review aims to comprehensively review current findings on CHD8 mutations, with a focus on the gene's role in brain development, synaptic function, and AS

Brain organoid models of SZT2-related disease reveal an overproduction of outer radial glial cells through mTORC1 activation.

Biallelic loss-of-function variants of Seizure Threshold 2 (SZT2) cause neurodevelopmental diseases with developmental delay, epilepsy, and macrocephaly. SZT2 forms the KICSTOR complex, which represses the mechanistic target of rapamycin complex 1 (mTORC1) amino acid-sensitive pathway. SZT2 dysfunction is thought to cause abnormal activation of the mTOR pathway, underlying the pathogenesis of SZT2-related diseases. We previously reported constitutive activation of mTORC1 in lymphoblastoid cell lines derived from patients with SZT2-related disease. However, the impact of SZT2 dysfunction on hum

Prenatal Diagnosis of Malformations of Cortical Development: A Review of Genetic and Imaging Advances.

Malformations of cortical development (MCD) are a group of neurodevelopmental disorders caused by abnormalities in cerebral cortex development, leading to conditions such as intellectual disability and refractory epilepsy. The prenatal phenotypes of MCD are complex and non-specific, complicating accurate diagnosis and prognosis assessment. Genetic testing, particularly chromosomal microarray analysis (CMA) and whole-exome sequencing (WES), has become an important tool for prenatal diagnosis. This review synthesizes current research on prenatal MCD, focusing on the integration of imaging and ge

Macrocephaly and Characteristic MRI Findings as Early Clues to a Hereditary Overgrowth Syndrome.

Macrocephaly may be the presenting feature of underlying genetic conditions in childhood. We report a five-year-old boy with persistent macrocephaly above +3 standard deviations since infancy and subtle facial dysmorphisms. Neurological examination revealed hypotonia, a wide-based gait, and fine and gross motor difficulties. Developmental assessment confirmed psychomotor delay without features of autism spectrum disorder. Brain MRI revealed multiple enlarged perivascular spaces involving the bilateral subcortical white matter and the corpus callosum, as well as callosal thickening. Genetic tes

AKT is necessary for neuronal hypertrophy and epilepsy caused by Pten knockout.

Phosphatase and tensin homolog on chromosome 10 (PTEN) is a key negative regulator of the AKT/mTOR signaling pathway. Mutations in PTEN are highly implicated in Autism Spectrum Disorder (ASD), epilepsy, congenital hydrocephaly, and macrocephaly. While the conditional genetic knockout of Pten in murine neurons results in hypertrophy, increased migration, excitatory synaptogenesis, hyperexcitability, and epileptiform activity, the specific downstream signalling mediators of these pathologies remain to be fully elucidated. Using retroviral-mediated genetic manipulation of individual neurons withi

A Depolarizing Leak in Sodium Bicarbonate Cotransporter NBCe1 Causes Brain Edema.

OBJECTIVES: SLC4A4 encodes electrogenic sodium bicarbonate cotransporter NBCe1, prominently expressed in kidney and brain. Recessive loss-of-function variants in SLC4A4 cause proximal renal tubular acidosis, no brain edema. In the brain, NBCe1 is expressed by astrocytes, where it regulates pH and mediates astrocyte volume changes. Here we describe a novel dominant variant in SLC4A4 in patients with brain edema and investigate how it affects NBCe1 function. METHODS: Genetic studies identified a novel gene variant in three unrelated pediatric patients with the same MRI pattern of cerebral subco

Chd8 haploinsufficiency leads to molecular layer heterotopias and age-dependent cortical expansion.

Mutations in the chromatin remodeler CHD8 are associated with autism and macrocephaly. While mouse models of Chd8 haploinsufficiency recapitulate brain overgrowth, the specific cellular mechanisms and developmental timing that lead to these anatomical abnormalities remain poorly understood. Here, we conducted 3D imaging of Chd8 V986*/+ mouse brains using magnetic resonance imaging followed by tissue clearing and cellular resolution light-sheet microscopy across embryonic and postnatal developmental stages. We found that brain overgrowth occurs postnatally, driven by an expansion of oligodendro
