Juvenile xanthogranuloma (JXG) of the central nervous system (CNS) is a rare non-Langerhans cell histiocytosis. CSF1R mutations have been reported for peripheral JXG, but not in CNS JXG. A 3-month-old male presented with fever, lymphadenopathy, and macrocephaly with bulging fontanelles. Imaging demonstrated multiple dural-based masses causing obstructive hydrocephalus. The patient underwent right frontal endoscopic third ventriculostomy, choroid plexus cauterization, and craniotomy for resection and tissue diagnosis. Pathology was consistent with JXG, and NGS identified a somatic CSF1R mutatio

BACKGROUND/OBJECTIVES: Post-hemorrhagic hydrocephalus (PHH) is a major complication of extreme prematurity associated with significant neurodevelopmental morbidity. Although post-hemorrhagic ventricular dilatation (PHVD) is a recognized consequence of intraventricular hemorrhage (IVH), progression from an apparently low-grade IVH to severe shunt-dependent disease with imaging features suggestive-but not diagnostic-of a non-communicating hydrocephalus pattern is uncommon and presents important diagnostic and management challenges. We report such a case.

Bannayan-Riley-Ruvalcaba syndrome is a rare PTEN-related disorder characterized by macrocephaly, intellectual disability, autism, hamartomatous intestinal polyps, and pigmentation of the glans penis. We describe a novel pathogenic variant of PTEN, c.48del (p.Gln17LysfsTer7), along with the first description of hepatic glycogenosis and steatosis in a child with BRRS. We proposed that the mutation likely led to a release of the inhibition of the insulin signaling pathway in the liver, leading to increased glycogen production and de novo synthesis of fatty acids, as previously shown in mice. Our

BACKGROUND: Imagawa-Matsumoto syndrome (IMMAS) is a rare overgrowth disorder caused by heterozygous loss-of-function variants in SUZ12, which encodes a core subunit of the Polycomb Repressive Complex 2 (PRC2). To date, fewer than 20 cases have been reported, and the molecular mechanisms underlying PRC2 dysfunction in IMMAS remain incompletely understood.
METHODS: Trio-based whole-exome sequencing (Trio-WES) was performed in a Chinese family presenting with overgrowth and dysmorphic features. The candidate variant was validated by Sanger sequencing and segregation analysis. AlphaFold 3 was use

Background/Objectives: Chromosomal microarray analysis (CMA) is widely used in the genetic evaluation of autism spectrum disorder (ASD), yet its diagnostic contribution in clinically nonsyndromic ASD, particularly in Brazilian populations, remains insufficiently characterized. This study aimed to determine the diagnostic yield of CMA and characterize clinically relevant copy number variants (CNVs) in a cohort of individuals with clinically nonsyndromic ASD from southern Brazil. Methods: In this observational cohort study, 215 individuals with clinically diagnosed ASD underwent high-resolution

Vein of Galen malformation (VoGM) is a rare congenital vascular anomaly. It represents a well-recognized extracardiac cause of high-output cardiac failure in the neonatal period. We report a case of a 3.5-month-old boy who presented for routine vaccination and was incidentally found to have macrocephaly (head circumference 44.5 cm, >99th percentile) with early sun-setting eyes. He was otherwise well-appearing with a normal neurological examination. Bedside cranial ultrasound revealed bilateral lateral and third ventricular dilation with a cystic mass compressing the aqueduct, and brain MRI con

Hypochondroplasia is a skeletal dysplasia caused by pathogenic variants in FGFR3 and characterized by disproportionate short stature and relative macrocephaly. Diagnostic uncertainty remains common, particularly in early childhood and in individuals with mild or atypical presentations, leading to delayed diagnosis, inconsistent management, and challenges in counselling and care planning. Individuals can be affected by medical complications and psychosocial consequences and have unmet needs for multidisciplinary care. To address these unmet needs, an international, multidisciplinary panel of ex

Osteopetrosis is an uncommon inherited skeletal condition arising from impaired osteoclast activity, with its autosomal recessive forms being linked to mutations in the TNFRSF11A gene. This study reports on a three-year-old Chinese girl presenting with recurrent fractures, short stature, and macrocephaly. Radiological examinations revealed diffusely elevated bone mineral density and constricted medullary spaces. Meanwhile, whole-exome sequencing detected compound heterozygous alterations in the TNFRSF11A gene: c.1567?+?2T>A inherited from the father and c.630C>G from the mother. The child unde

INTRODUCTION: Hypochondroplasia (HCH) is mild, often under-recognized skeletal dysplasia caused by FGFR3 variants. It is characterized by disproportionate short stature and subtle radiographic findings, which can delay diagnosis. This study evaluated genetic, endocrine, and radiographic findings in patients with HCH.
METHODS AND RESULTS: We performed retrospective, single-center review of individuals with molecularly confirmed FGFR3 variants. Diagnosis was established by clinical exome and/or Sanger sequencing, and anthropometric, dysmorphologic, radiologic, and molecular data were extracted

Introduction and Clinical Significance: Skeletal dysplasias comprise a genetically heterogeneous group of disorders with substantial phenotypic overlap, often complicating diagnosis. Clinical exome sequencing (CES) can facilitate molecular diagnosis in children with unexplained disproportionate short stature. Case Presentation: An 8-year-old boy presented with severe short stature (-3.24 SDS), brachydactyly, relative macrocephaly, broad nasal bridge, and mild calf hypertrophy. Endocrine evaluation confirmed growth hormone deficiency (GHD). Following negative SHOX testing, CES identified a hete

Robinow syndrome has characteristic craniofacial and dental features and can be caused by gain- or loss-of-function variants in Wnt family member 5A (WNT5A) non-canonical signaling. The craniofacial and dental manifestation of Robinow syndrome is heterogenous, as is the effect of altered Wnt5a in animal models. The relationship between Wnt5a and craniofacial and dental phenotypes is not fully understood. To investigate the role of Wnt5a in bone during craniofacial and dental development, we utilized a Wnt5a conditional loss-of-function (LOF: Wnt5afl/fl;Ctskcre ) and a Wnt5a conditional gain-of

Megalencephalic leukoencephalopathy with subcortical cysts (MLC) is a rare leukodystrophy characterized by early-onset macrocephaly, white matter edema, seizures, and motor and cognitive decline. Approximately 25% of MLC patients carry HEPACAM pathogenic variants, many of which are dominant missense variants causing remitting MLC Type 2b. HEPACAM encodes hepatic and glial cell adhesion molecule (hepaCAM), also known as GlialCAM, an astrocyte-enriched transmembrane protein with important roles in astrocyte territory establishment, gap junction coupling, branching organization, synaptic function

BACKGROUND: Polyhydramnios, megalencephaly, and symptomatic epilepsy (PMSE) syndrome is a rare autosomal recessive mTORopathy caused by biallelic STE20-related kinase adaptor alpha (STRADA) loss-of-function variants. Animal models demonstrate that in utero mechanistic target of rapamycin (mTOR) inhibition can prevent cortical dyslamination, suggesting a prenatal therapeutic window.
METHODS: A gravida 3 para 2 patient presented at 32+2 weeks with medication-refractory preterm labor and severe polyhydramnios. Fetal magnetic resonance imaging (MRI) demonstrated macrocephaly and an enlarged cavum

Pathogenic variants in the Ryanodine Receptor 1 (RYR1) gene represent the most common cause of congenital myopathy. The severity of RYR1-related myopathy presenting in the neonatal period is quite variable, ranging from a perinatal lethal type to a benign late-onset form with progressive improvement. We report a newborn infant who presented at birth with severe hypoxic ischemic encephalopathy (HIE), underwent therapeutic hypothermia (TH), and was later diagnosed with RYR1-related myopathy. Exome sequencing identified a de novo variant of uncertain significance (VUS) in the RYR1 gene (c.14564T>

UNLABELLED: The sirolimus-eluting stent (CYPHER( trade mark )) is a metal stent coated with 140 micro g/cm(2) of sirolimus blended with synthetic polymers. After stent implantation, sirolimus is slowly released causing localized cytostatic inhibition of proliferation of vascular smooth muscle cells in the peri-stent arterial wall over a period of about 1 month. Only minimal amounts of sirolimus enter the bloodstream and these appear to be insufficient to be of clinical relevance. In clinical trials that evaluated single de novo lesions and in-stent restenosis in patients with coronary artery d

Osteoarthritis (OA) is condition which poses a main concern to the aging population and its severity is expected to increase with the increasing life expectancy. In the future, several possible targets for OA treatment need to be defined. Dickkopf‑related protein 3 (DKK3) is an atypical member of the Wnt‑antagonistic dickkopf‑related protein (DKK) family. The availability of research into the role of DKK3 in the abnormal remodeling of subchondral bone in human knee joints is currently limited. Thus, the aim of the present study was the evaluation of DKK3 expression in the abnormal bone remodel

Transcription factors from the family of Signal Transducers and Activators of Transcription (STAT) are activated by numerous cytokines. Two members of this family, STAT5A and STAT5B (collectively called STAT5), have gained prominence in that they are activated by a wide variety of cytokines such as interleukins, erythropoietin, growth hormone, and prolactin. Furthermore, constitutive STAT5 activation is observed in the majority of leukemias and many solid tumors. Inactivation studies in mice as well as human mutations have provided insight into many of STAT5's functions. Disruption of cytokine
