STUDY DESIGN: Retrospective case series.
OBJECTIVES: To report the complications and outcomes of pediatric revision spine surgery and review the surgical indications and treatment methods.
BACKGROUND SUMMARY: Recent reports have shown the necessity of revision spine surgery for pediatric deformities. Many reports have shown the result of revision surgery for pediatric spinal deformity.
MATERIALS AND METHODS: We conducted a retrospective review of 638 patients with pediatric spinal deformity who were treated with surgery from 1996 to 2007. We identified 50 patients who underwent revision spi

STUDY DESIGN: Retrospective clinical study.
OBJECTIVE: The aim of this study was to evaluate retrospectively the safety and efficacy of 1-stage surgical treatment of 45 consecutive patients, who had progressive congenital spinal deformity associated with split spinal cord malformation (SSCM).
SUMMARY OF BACKGROUND DATA: For correction of progressive congenital spinal deformity with SSCM, it has been reported that all SSCM should be operated on before any orthopedic intervention, and then surgery for correction and stabilization of the spinal deformity should be performed 3 to 6 months later.

BACKGROUND: Expansion thoracoplasty (ET) is claimed to be a spine-sparing procedure because of the fact that the spine is not exposed directly and intervention toward the spinal column is not performed. It is also recommended in cases of primary spine deformities without rib fusion/aplasia and when the primary problem is not in the thorax itself. The aim of this study was to report spontaneous spinal fusion after multiple thoracic distractions in patients with congenital thoracospinal deformities who have undergone ET in early childhood.
METHODS: In our clinic, between May 2003 and March 2010

OBJECTIVES: To discuss the controversies, variability of opinion and approach, and special considerations in pediatric Chiari malformation.
METHODS: PubMed-based literature review with focused discussion on clinically relevant pediatric issues.
RESULTS: Compared to adults, children with Chiari more often present with congenital anomalies, scoliosis, and deficits in oropharyngeal control. Those with a syrinx present more with scoliosis than with deficits in sensation or pain. Surgical intervention has varied widely in its level of invasiveness though there has been a trend towards less invasi

BACKGROUND: Diastematomyelia is a rare congenital malformation of the spinal cord, which belongs to the group of occult spinal dysraphisms. This disorder consists in the separation of the spinal cord into two parts in the sagittal plane (hemicords). Diastematomyelia may coexist with other spinal dysraphisms, such as myelomeningocele, meningocele, spinal lipoma, neuroenteric cysts or dermal sinuses and vertebral abnormalities, such as hemivertebrae, butterfly vertebrae or scoliosis.
CASE REPORT: We reported a case of a woman with a congenital defect of the spinal cord, in the form of diastemat

We developed a modeling procedure using CT scans or MRI data for exploring the bony and lymphatic canals of vestibular patients. We submitted 445 patients with instability and spatial de-orientation to this procedure. Out of the 445 patients, 95 had scoliosis, some of them, because malformations were suspected also had CT-scan modeling and functional tests. We focused on a never described, abnormal connection between the lymphatic lateral and posterior canal (LPCC) with a frequency of 67/445 (15%). In the scoliosis subgroup, the frequency was 52/95 (55%). Three scoliotic patients had CT scans.

A wide variety of neuromuscular diseases affect children, including central nervous system disorders such as cerebral palsy and spinal cord injury; motor neuron disorders such as spinal muscular atrophy; peripheral nerve disorders such as Charcot-Marie-Tooth disease; neuromuscular junction disorders such as congenital myasthenia gravis; and muscle fiber disorders such as Duchenne's muscular dystrophy. Although the origins and clinical syndromes vary significantly, outcomes related to musculoskeletal complications are often shared. The most frequently encountered musculoskeletal complications o

A variety of treatments has been described in the literature for the treatment of HV. We report the results of early surgical anterior instrumented fusion with partial preservation of the HV and posterior non-instrumented fusion in the treatment of progressive congenital scoliosis in children below the age of six. Between 1996 and 2006, 31 consecutive patients with 33 lateral HV and progressive scoliosis underwent short segment fusions. Mean age at surgery was 2 years and 10 months. Mean follow-up period was 6.1 years. The major scoliotic curve improved from 41 degrees preoperatively to 17 deg

Klippel-Feil syndrome (KFS) is a congenital disorder of spinal segmentation distinguished by the bony fusion of anterior/cervical vertebrae. Scoliosis, mirror movements, otolaryngological, kidney, ocular, cranial, limb, and/or digit anomalies are often associated. Here we report mutations at the GDF6 gene locus in familial and sporadic cases of KFS including the recurrent missense mutation of an extremely conserved residue c.866T>C (p.Leu289Pro) in association with mirror movements and an inversion breakpoint downstream of the gene in association with carpal, tarsal, and vertebral fusions. Tethered cord syndrome (TCS) and the Chiari malformation (CM) are generally related to congenital malformations, but both entities can be acquired.TCS often presents with progressing sensory and motor symptoms that can be attributed to increased tension of the spinal cord. Fixation of the spinal cord can occur congenitally (primary TCS) or in association with other intraspinal pathologies or postoperative scarring (secondary TCS). Exact diagnosis in the adult can be difficult if symptoms are interpreted as being related to degenerative disorders of the spine. In the presence of neurological sy

We present a series of seven patients who were previously diagnosed with Proteus syndrome, but who do not meet published diagnostic criteria for this disorder and whose natural history is distinct from that of Proteus syndrome. This newly recognized phenotype comprises progressive, complex, and mixed truncal vascular malformations, dysregulated adipose tissue, varying degrees of scoliosis, and enlarged bony structures without progressive bony overgrowth. We have named this condition congenital lipomatous overgrowth, vascular malformations, and epidermal nevi (CLOVE syndrome) on a heuristic bas

Neurofibromatosis type 1 (NF-1) is a multisystemic disease. It may manifest as abnormalities of the nervous tissue, bones, soft tissue, and skin. The manifestations of NF-1 vary from person to person and range from subclinical to severe. Individuals who carry the gene eventually exhibit some clinical feature of the disease. The penetrance for NF-1 nears 100% during adulthood. Skeletal abnormalities are common in NF-1, with most patients presenting with some type of bony dysplasia. The orthopedic complications usually appear early. They include spinal deformities, such as scoliosis or kyphosis,

We report a case of congenital lumbosacral limb duplication at the pelvis, lower limb, and foot of a 4-year-old boy. The patient presented with a supernumerary lower limb joined at the lumbosacral region by a bony connection. A pseudo-navel and a pseudo-penis were found on the supernumerary limb and foot duplication was noted at the end. Radiography and a 3-dimensional computed-tomography reconstruction revealed duplication of an inversely oriented pelvis dorsal to the normal pelvis, semi-vertebrae in T7 to T9, thoracolumbar scoliosis, lumbosacral vertebral dysplasia, and dislocation of the le

This study evaluates the effectiveness of hemivertebra excision in children via simultaneous anterior and posterior operative exposures. This study was a retrospective review of patients treated with this technique over a 5-year period with minimum 2-year follow-up. Eighteen patients were studied, all of whom had undergone hemivertebra excision and deformity correction using unilateral compression instrumentation. The average age of the patients at the time of operation was 3 years 2 months and the average preoperative coronal curve measured 40 degrees. There were no neurologic complications.

BACKGROUND DATA: Our purpose was to describe the deformities, natural history, and course of treatment in chondrodysplasia punctata, a skeletal dysplasia characterized by stippled epiphysis in infancy, short stature, and progressive kyphoscoliosis.
STUDY DESIGN: We identified 20 patients (15 female, 5 male) with chondrodysplasia punctata and spinal abnormalities.
RESULTS: The spinal deformity was rarely present in the neonatal radiographs but typically appeared during the first year. We identified three patterns of spinal deformity. The first was a C2 dentocentral synchondrosis separation, w

Significant spinal deformity is particularly common in nonambulatory patients with myelodysplasia. Progressive deformity may be caused by congenital anomalies, paralytic collapse, hip contractures, or spinal cord tethering. Existing or projected functional impairment should be the principle indication for treatment. Surgical treatment is complicated by poor soft tissue coverage, associated contractures, lack of sensation, weak bone, and absence of posterior elements. Successful fusion can be achieved by circumferential (anterior and posterior) fusion and current rigid segmental instrumentation

BACKGROUND: Hamartomatous dysplasias with hemihypertrophy are a part of several congenital diseases, sometimes difficult to classify when their symptomatology is relatively poor.
CASE REPORTS: Case 1. A girl, 11 year-old, suffered from gigantism of the feet, left hemihypertrophy of the limbs, scoliosis and kyphosis. X-rays showed frontal hyperostose, bony and soft tissues overgrowth involving the left limbs and pelvis with hip dislocation and coxa valga. The vertebrae were hypertrophic and dysplastic. No tumor could be found. Case 2. A girl, 18 year-old, suffered from left hemihypertrophy of

Cervical diastematomyelia, a rare anomaly, was found in a 14-year-old girl with congenital scoliosis. Metrizamide myelography and metrizamide CT demonstrated anomalies of the cervical spine and a split cervical cord within a single dural tube, without the presence of a fibrous, cartilaginous, or bony spur.
