Anesthesia for Pentalogy of Cantrell with Surgical Repair of Tetralogy of Fallot Along with Absent Diaphragm: A Case Study.

Pentalogy of Cantrell compiles the union of five anomalies including defects of the midline abdominal wall, lower sternum, anterior diaphragm, diaphragmatic pericardium, and any form of intra-cardiac pathology 1. The intra-cardiac defects associated commonly are ventricular septal defects, followed by atrial septal defects, Tetralogy of Fallot, and pulmonary stenosis 2. It is also a rare syndrome with an estimated incidence of 1:65,000 to 200,000 live births.3 4 Here in we present a case of a 4-year-old male patient, diagnosed with Pentalogy of Cantrel was referred to our institute with chief

Late prenatal diagnosis of complete pentalogy of Cantrell.

Cantrell's pentalogy is a rare syndrome associated with a midline embryological defect involving a series of malformations: anomalies of the lower sternum, anterior diaphragm, heart, and anterior abdominal wall. It can be classified as complete, probable or partial, but the most important thing is to describe and understand the anomalies involved. We describe a case of a late diagnosis of Cantrell's pentalogy at 35 weeks and 5 days of pregnancy in a woman from the interior of Pará state, an Amazon Brazilian region. Fetal echocardiography confirmed the diagnosis of Cantrell's pentalogy with tet

Delayed Sternal Closure Following Complex Cardiac Surgery in Neonates.

BackgroundDelayed sternal closure (DSC) is a well-established management strategy following complex congenital cardiac surgery that is used to mitigate postoperative hemodynamic and respiratory instability. It is mostly used in neonates requiring prolonged cardiopulmonary bypass (CPB), long aortic cross-clamp times, or deep hypothermic circulatory arrest who are predisposed to myocardial edema or bleeding. Our study evaluates morbidity and mortality after DSC in neonates including superficial and deep sternal wound infections, along with requirement for surgical debridement.MethodsRetrospectiv

Routine Primary Sternal Closure After the Norwood Procedure.

Background: Delayed sternal closure (DSC) is frequently utilized to facilitate the recovery of myocardial function and edema following the Norwood procedure. At our institution, most patients undergo primary sternal closure (PSC), unless specified high-risk characteristics are present. We sought to analyze the outcomes of our approach. Methods: A retrospective review was performed of patients who underwent the Norwood procedure from 2017 to 2022. Patients were divided into two groups-PSC and DSC. Baseline demographics, clinical characteristics, and perioperative details were compared. The prim

Prenatal diagnosis and management challenges of Pentalogy of Cantrell at term in a resource-constrained setting: A rare case report.

INTRODUCTION AND IMPORTANCE: Pentalogy of Cantrell (POC) is a rare and often fatal congenital malformation characterized by a midline developmental defect involving five anatomical structures: the lower sternum, anterior diaphragm, diaphragmatic pericardium, abdominal wall, and heart. This condition presents significant diagnostic and therapeutic challenges, particularly in resource-limited settings where advanced imaging and surgical interventions are limited. Early diagnosis, supportive care, and strategic surgical planning with a multidisciplinary team are all key components in managing pat

Reliability of external caliper-based measurements for identifying severe pectus excavatum: Results from a multi-institutional regional consortium.

BACKGROUND: Historically, methods for quantifying the severity of pectus excavatum (PE) have involved cross-sectional imaging. An external caliper-based metric, the Modified Percent Depth (MPD), has been described as a viable alternative to radiographic-based measurements such as the Pectus and Correction Indices for identifying severe PE in the clinic setting. The purpose of this study was to evaluate the reproducibility of the MPD among PE patients evaluated within a regional multicenter consortium of children's hospitals. METHODS: This was a prospective observational study of PE patients a

Transesophageal echocardiography diagnosis of dextrocardia during cardiac arrest guided optimal chest compression location: A case report.

The incorporation of transesophageal echocardiography (TEE) into cardiac arrest management overcomes several limitations inherent to transthoracic echocardiography, while producing higher-quality images. Among other advantages, TEE can evaluate chest compression quality and location during ongoing compressions. AHA/ACLS guidelines recommend hand placement for compressions over the lower half of the sternum in order to compress the left ventricle (LV). However, literature demonstrates that chest compressions do not align with the LV over half the time, but rather lie over the left ventricular o

Congenital sternal cleft in a DSH cat: A case report.

The sternum, essential for protecting intrathoracic structures and enabling thoracic stability and breathing motion, is a vital part of small animal thoracic anatomy. Evaluating the sternum is crucial in small animal practice, with typical anatomy including eight to nine sternebrae and species-specific variations. Congenital sternal anomalies, such as pectus excavatum, pectus carinatum, and sternal cleft, though rare, can have significant clinical implications. Sternal cleft, characterized by incomplete fusion of sternebrae, is particularly uncommon in veterinary literature, with limited docum

Innovative surgical repair of severe pectus carinatum using 3D surgical planning and advanced rib fixation techniques.

This case describes an early adolescent boy with severe pectus carinatum, managed with a complex surgical approach after bracing was deemed unfeasible. A multidisciplinary team collaborated, using advanced three-dimensional (3D) imaging, digital segmentation and a patient-specific 3D-printed model to plan and rehearse the surgery. The operation involved sternum repositioning and rib plating with polyether ether ketone plates. The procedure resulted in favourable chest stability and minimal blood loss. Postoperatively, the patient's pain was well-controlled, and he was discharged on day 7. The
