Altered body composition in children and adolescents with achondroplasia.

INTRODUCTION: Achondroplasia is characterized by disproportionate short stature, yet little is known about how growth patterns and body composition differ from peers during childhood and adolescence. We aimed to compare anthropometric variables and detailed body composition profiles between children and adolescents with achondroplasia and age-matched controls. METHODS: In this cross-sectional study, anthropometry and body composition were assessed in participants aged 6-18 years using air displacement plethysmography (BOD POD). Height, weight, BMI, fat mass (FM), fat mass index (FMI), fat-fre

A Case of Inadvertent Overdose of Vosoritide Injection in a Three-Month-Old Infant With Achondroplasia.

Vosoritide, a recombinant C-type natriuretic peptide analog that antagonizes fibroblast growth factor receptor 3 (FGFR3) signaling, is recommended for early initiation in infants with achondroplasia. However, clinical safety data for infants under six months remain limited, and concerns persist regarding its potential to induce symptomatic hypotension. We present a case of a three-month-old female infant with genetically confirmed achondroplasia (FGFR3 p.Gly380Arg) who accidentally received an inadvertent double dose of vosoritide (0.24 mg; 53 μg/kg) during home self-injection therapy. This pa

T1-T12 and T1-S1 Lengths at Maturity in Patients With Skeletal Dysplasia.

BACKGROUND: Growing-spine surgery aims to promote thoracic growth in skeletally immature patients by achieving adequate T1-T12 length. Surgical decision-making often relies on assumptions regarding spinal length thresholds and their relationships to cardiopulmonary function. This study aimed to characterize thoracic (T1-T12) and total spine (T1-S1) lengths in skeletally mature patients with skeletal dysplasias, with a primary focus on achondroplasia, and to evaluate whether these measurements were associated with cardiopulmonary comorbidities. METHODS: A retrospective review was conducted of

Genetic Skeletal Disorders with Defects in Glycosaminoglycan Biosynthesis.

BACKGROUND: Proteoglycans are a major component of the connective tissue matrix, which consists of a core protein and covalently attached glycosaminoglycan (GAG) chains, which are highly sulfated polysaccharides with a tetrasaccharide linker for the core protein attachment. Impaired synthesis or degradation of GAG causes genetic disorders. In the 1950s, deficient lysosomal GAG degradation was discovered in mucopolysaccharidoses. In the 1990s, a defective enzyme for GAG synthesis was implicated in a variant of Ehlers-Danlos syndrome and an impaired GAG sulfation in diastrophic dysplasia. Newer

Surgical Correction of Thoracolumbar Kyphosis in Achondroplasia: Complications, Pitfalls, and Reflections on the Pursuit of Maximal Realignment in View of Correction Leading to Functional Disability.

Background: Achondroplasia, the most common genetic dwarfism caused by the FGFR3 mutation (autosomal dominant, 80% de novo), results in a disproportionately short stature. Thoracolumbar kyphosis (TLK), combined with characteristic spinal canal stenosis, increases the risk of symptomatic compression, yet the literature lacks clear thresholds for symptom onset or progressive deformity angles. Methods: A 16-year-old female with achondroplasia presented with rapidly progressive kyphosis despite conservative management (bracing and therapy). Over six months, she developed neurogenic claudication; b

Breathing challenges in achondroplasia: a comprehensive review of pediatric respiratory complications.

Achondroplasia is the most common skeletal dysplasia, characterized by disproportionate short stature and associated craniofacial, musculoskeletal, and neurological abnormalities. Respiratory complications are frequent and represent a major contributor to morbidity and mortality. Sleep-disordered breathing (SDB), encompassing obstructive (OSA) and central sleep apnea (CSA), is the most prevalent respiratory complication with pediatric prevalence reported between 22% and 93%. OSA results primarily from multilevel upper-airway obstruction related to craniofacial abnormalities and adenotonsillar

Diagnosis of hypochondroplasia: international Delphi consensus recommendations.

Hypochondroplasia is a skeletal dysplasia caused by pathogenic variants in FGFR3 and characterized by disproportionate short stature and relative macrocephaly. Diagnostic uncertainty remains common, particularly in early childhood and in individuals with mild or atypical presentations, leading to delayed diagnosis, inconsistent management, and challenges in counselling and care planning. Individuals can be affected by medical complications and psychosocial consequences and have unmet needs for multidisciplinary care. To address these unmet needs, an international, multidisciplinary panel of ex

From FGFR3 Hyperactivation to Disease-Modifying Therapy in Pediatric Achondroplasia: Molecular Mechanisms, Clinical Evidence, and Emerging Treatments.

BACKGROUND/OBJECTIVES: Achondroplasia is the most common genetic skeletal dysplasia associated with disproportionate short stature and is primarily caused by gain-of-function variants in the fibroblast growth factor receptor 3 (FGFR3) gene. Constitutive FGFR3 activation disrupts growth plate homeostasis and endochondral ossification through complex alterations in chondrocyte proliferation, differentiation, hypertrophy, extracellular matrix organization, and intracellular signaling. The increasing understanding of these mechanisms has enabled the transition from exclusively supportive managemen

A phase 3, randomized, double-blind, placebo-controlled, multicenter study to evaluate the efficacy and safety of vosoritide in children with hypochondroplasia: CANOPY HCH-3 study design.

BACKGROUND: Hypochondroplasia is a skeletal dysplasia characterized by disproportionate short stature that is caused by gain-of-function variants in the fibroblast growth factor receptor 3 gene (FGFR3), which negatively regulates endochondral bone growth. Current treatments are based on symptom management; there are no treatments targeting the signaling pathways that underlie hypochondroplasia. Vosoritide, a C-type natriuretic peptide analog that counteracts overactive FGFR3 signaling to stimulate endochondral bone growth, is approved for the treatment of achondroplasia in children.
