INTRODUCTION: The "terrible tetrad" of the elbow - simultaneous posterior ulno-humeral dislocation, comminuted radial head fracture with dislocation, coronoid process fracture, and proximal ulnar fracture - is an injury pattern that simultaneously fulfills criteria for both the terrible triad and a Monteggia-type fracture-dislocation. Fewer than fifteen cases have been documented worldwide. We present two cases managed by different surgeons at different institutions and propose the formal adoption of the term "terrible tetrad."
CASE REPORT: Case 1: A 50-year-old male painter sustained a right

INTRODUCTION: Osteogenesis imperfecta (OI) is characterized by bone fragility with frequent fractures, especially in children. Some studies have used peripheral-QCT (pQCT) to examine bone density in children with OI and one cross-sectional study used HR-pQCT in 9 children with OI.
MATERIALS AND METHODS: We compared bone density, microstructure, and strength changes over 1 yr at the metaphysis and diaphysis of the radius and tibia, as well as lumbar spine (LS) bone mass and area, in 20 children with OI under bisphosphonate treatment and 20 age- and sex-matched controls. For the double-stack me

CONTEXT: Osteogenesis imperfecta (OI) is a genetic disorder characterized by increased bone fragility. Despite this, the use of antifracture drugs (AFD) among people with OI in the United States is not known.
OBJECTIVE: This study examined the rates and types of AFD use in OI and whether use patterns varied with age, sex, and clinical severity.
METHODS: We conducted a cohort study using medical and prescription claims data from January 1, 2011, to December 31, 2022, in MarketScan and from January 1, 2006, to December 31, 2021, in a 5% Medicare sample. Durable medical equipment claims for lon

Hypophosphatasia is a rare metabolic bone disorder that is often misdiagnosed. We present the case of a middle-aged woman initially misdiagnosed with rickets and later as osteogenesis imperfecta and treated with zoledronate, after which she developed atypical femoral fractures. After switching to teriparatide therapy, her bone mineral density improved significantly. This case underscores that persistently low alkaline phosphatase with fragility or atypical fractures should prompt evaluation for hypophosphatasia and that antiresorptives (eg, bisphosphonates) may precipitate atypical fractures i

Rare genetic skeletal disorders (RGSDs) encompass a heterogeneous group of hundreds rare conditions affecting the skeletal system. The rarity of these disorders, phenotypic and genetic diversity, combined with the limitations of conventional cellular and animal RGSD models, have hindered progress in understanding their pathophysiology and developing effective therapies. However, the latest advances in stem cell and bone tissue engineering techniques offer transformative opportunities in investigation of RGSD, particularly through bone organoids that enable disease modeling within a precision m

Osteogenesis Imperfecta (OI) is a rare hereditary brittle bone disorder typically caused by COL1A1 and COL1A2 variants impairing type I collagen. However, gross deletions involving COL1A1 are uncommon. Here, we report a family with type I OI harboring a 101-kbp deletion encompassing COL1A1, identified through whole genome analysis. Affected individuals presented mild phenotypes. Breakpoint analysis revealed a 5-bp microhomology-mediated end joining involving an Alu element. This report expands the understanding of genetic mechanisms underlying OI.

The use of locking plates with intramedullary rodding in children with osteogenesis imperfecta reduces rotational and longitudinal instability. However, adjunctive locking plates do not allow early weight-bearing. We explored the outcomes of intramedullary rodding combined with external fixation or titanium threaded wires (ThW). The study included osteogenesis imperfecta children who underwent surgery with intramedullary telescopic rods and adjunctive fixation with external fixation or ThW. The timing of and reason for implant failure were primary outcome variables. Our cohort consisted of 62

PURPOSE: Telescoping intramedullary rods have transformed the management of long bone deformity and fragility fractures in children, yet complications and revision surgery remain common. This systematic review synthesizes outcomes across telescoping systems to evaluate complication and revision rates, telescoping success, and design evolution to guide implant selection and surgical decision-making in pediatric orthopedics.
METHODS: A search was conducted through July 2025 following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Studies reporting outcomes of

BACKGROUND: Osteogenesis imperfecta (OI) is a hereditary disorder primarily caused by mutations in COL1A1 or COL1A2, leading to bone fragility and deformities. Although numerous pathogenic variants have been identified, novel mutations in specific populations remain underreported, complicating diagnosis and genetic counseling.
METHODS: A Chinese family with mild type I OI was recruited. Whole-exome sequencing and Sanger sequencing were used to identify and validate a novel splice-site variant in COL1A1. Functional effects were assessed using two minigene constructs (pcMINI-COL1A1 and pcMINI-N

The developmental dysfunction of cartilage progenitor cells (CPCs) causes dwarfism. Radical therapies for dwarfism remain underdeveloped. Recently, the therapeutic benefits of extracellular vesicles (EVs) released from human exfoliated deciduous tooth-derived stem cells (SHED) have been investigated for their potential to restore disease-target cells. However, the effects of EVs on human CPCs for the treatment of dwarfism remain unclear. We investigated the impact of EVs on cell proliferation, telomerase activity, telomerase reverse transcriptase (TERT) expression, cell cycle, and extracellula

INTRODUCTION: Skeletal dysplasias are genetically heterogeneous disorders characterized by high diagnostic complexity and major perinatal impact. Under current diagnostic guidelines, chromosomal microarray remains the recommended first-tier test for fetal anomalies. However, in suspected skeletal dysplasias ‑ where sequencing provides a markedly higher diagnostic yield ‑ starting with chromosomal microarray testing may delay diagnosis. Recent advances in next-generation sequencing, along with the 2022 International Society of Skeletal Dysplasias classification, now allow for earlier and more p

Osteogenesis imperfecta (OI) is a rare genetically heterogeneous connective tissue disorder primarily characterized by bone fragility, along with additional features such as blue sclerae, short stature, and dentinogenesis imperfecta. Although OI mainly presents an autosomal dominant inheritance pattern, over the past two decades, at least 17 genes have been implicated in autosomal recessive OI. In this study, we identified a novel homozygous non-coding variant, c.-167C > T, in the 5' untranslated region (5'UTR) of the SEC24D gene in a family with three affected individuals exhibiting a severe

OBJECTIVE: Considering the need for specialized care in Osteogenesis Imperfecta (OI), we investigated whether the use of a digital platform could optimize treatment and improve patients' quality of life.
METHODS: A guidance and data collection app was developed, complemented by a YouTube channel with physiotherapy instructions. The interventional clinical trial without a control group was carried out for 12 months with patients with OI. Patient functionality was assessed using the Functional Independence Measure (FIM) scale. Patients were monitored via telemedicine, receiving remote multidisc

Type I collagen is the main structural protein in vertebrates and undergoes extensive post-translational modification (PTM) during biosynthesis. Prolyl-3-hydroxylase 1 (P3H1) catalyzes collagen prolyl-3-hydroxylation and functions as a collagen chaperone. Loss of P3H1 causes osteogenesis imperfecta, and P3H1 is consistently upregulated in idiopathic pulmonary fibrosis. However, the full impact of P3H1 deficiency on the collagen biosynthesis machinery, including PTMs, is not known. Here, we comprehensively investigated the consequences of P3H1 deficiency in two independent models: type I collag

Osteogenesis imperfecta (OI) is a rare heritable connective tissue disorder characterized by bone fragility and heterogeneous clinical presentation. Mild phenotypes may remain undiagnosed for prolonged periods, particularly when clinical manifestations are subtle or overlooked. We report the case of a 19-year-old man with a history of recurrent low-energy fractures since the age of eight. The patient sustained multiple fractures involving long bones, associated with progressive deformities of the lower extremities. Notably, no etiological workup had been performed during childhood. On admissio

During the conversion of fibroblasts into induced pluripotent stem cells (iPSCs), cellular metabolism shifts from oxidative phosphorylation toward glycolysis; however, how functional and structural mitochondrial adaptations are coordinated remains incompletely understood. Here, we examined selected aspects of mitochondrial function, targeted gene expression, and ultrastructure in fibroblasts and iPSCs derived from healthy donors and patients with osteogenesis imperfecta (OI). Targeted gene expression analysis was performed in all four cell types, while functional and ultrastructural assays foc
