Background McCune-Albright syndrome is a rare mosaic disorder caused by postzygotic activating mutations in GNAS , classically presenting with fibrous dysplasia (FD), hyperpigmented macules, and endocrinopathies. Craniofacial FD is common and typically follows a benign course. However, acute symptomatic changes with aggressive imaging features may mimic malignant transformation or secondary lesions such as aneurysmal bone cysts (ABCs), creating significant diagnostic uncertainty. Case Description We report a 20-year-old woman with known MAS and polyostotic FD who presented with a 4-week histor

BACKGROUND: Polyostotic fibrous dysplasia (POFD) is a mosaic skeletal disorder driven by somatic GNAS mutations that constitutively activate Gsα-cAMP signaling, producing fibro-osseous craniofacial deformities. POFD lesions are histologically classified into three subtypes-Type I (connective-tissue predominant), Type II (cancellous-bone predominant), and Type III (sclerotic-bone predominant)-yet the immune microenvironment across these categories remains undefined. This study aims to dissect lymphoid and myeloid heterogeneity and inter-cellular communication networks among distinct POFD histol

Objectives: This study aimed to evaluate phenotypic associations between preoperative alkaline phosphatase (ALP) levels and clinical characteristics, and explore clinical factors associated for postoperative regrowth in craniofacial fibrous dysplasia. Methods: In this retrospective cohort (2003-2024), 71 surgically treated fibrous dysplasia patients were analyzed. Relationships between preoperative ALP (using age-stratified reference ranges) and key phenotypes (age at surgery, onset age, laterality, lesion type) were assessed via nonparametric tests. Associations with postoperative regrowth we

Low-grade fibro-osseous lesions can be challenging to classify. A 33-year-old man presented with synchronous lesions involving the ilium and T2 vertebra. Because of an impending pathologic fracture, he received several doses of denosumab. Initial biopsies showed bland fibro-osseous lesions lacking GNAS or MDM2 alterations and were favored to represent polyostotic fibrous dysplasia. Subsequent iliac curettage revealed a fascicular spindle cell proliferation with subtle atypia and focal ossification, leading to a revised diagnosis of low-grade fibrosarcoma. Re-biopsy of the T2 lesion demonstrate

Fibrous dysplasia (FD) may demonstrate increased 68 Ga-PSMA uptake and mimic skeletal metastases in prostate cancer. We present a 63-year-old man with newly diagnosed prostate cancer who underwent 68 Ga-PSMA PET/CT for staging. Imaging revealed widespread skeletal uptake involving multiple regions, raising suspicion for diffuse metastatic disease. Bone scintigraphy demonstrated a superscan pattern. However, CT showed characteristic benign features, including ground-glass appearance, bone expansion, and preserved cortical integrity. Histopathologic examination confirmed fibrous dysplasia. This

Fibrous dysplasia is one of the most common skeletal lesions. The wide spectrum of clinical manifestations ranges from asymptomatic conditions (typical of monostotic forms) to severe skeletal diseases with deformity and fractures for polyostotic fibrous dysplasia. The classical radiological features include: an osteolytic geographic pattern, ground-glass bone matrix, cortical thinning/cortical scalloping, bone deformities and enlargement, concavity of margins (evaluated with MRI), and cystic areas (MRI). All the bones can be affected, and the proximal femur is the most common one (about 30% of

INTRODUCTION AND IMPORTANCE: Polyostotic fibrous dysplasia (PFD) is a rare, benign skeletal disorder characterized by the replacement of normal bone with fibro-osseous tissue. Craniofacial involvement often results in progressive deformities, functional impairments, and aesthetic concerns. Surgical intervention aims to remove diseased bone and restore structure and function, employing techniques like vascularized bone flaps, autografts, and cryopreserved bone. This report presents a case utilizing cryopreservation combined with vascularized fibula flap for craniofacial reconstruction.

McCune- Albright syndrome (MAS) is a rare disorder characterized by postzygotic somatic activation mutation of GNAS, leading to constitutively active Gsα and ligand-independent signaling of the Gs-coupled protein receptor. Polyostotic fibrous dysplasia, café-au-lait macules, and endocrine hyperfunction, commonly gonadotropin-independent precocious puberty (GIPP), are the three clinical hallmarks of this condition. Mosaicism results in a wide range of clinical presentations. In this case series, we present three cases of MAS with varied presentations. Case 1 describes a 9-year-old boy with recu

INTRODUCTION AND IMPORTANCE: McCune-Albright syndrome (MAS) is a rare, benign mosaic syndrome. Limb length discrepancy and deformity are common, and the new bone formed following corrective osteotomy is dysplastic, indicating that recurrence of deformity should be anticipated.
PRESENTATION OF CASE: A 15-year-old boy presented with left leg bowing following a previously healed femoral fracture with a persistent deformity. His parents reported early pubic and axillary hair development at the age of eight and a rapid growth spurt by the age of 12. Radiographs showed a 42.6° lateral bowing of the

Fibrous dysplasia/McCune-Albright Syndrome (FD/MAS) is a rare skeletal disorder associated with significant pain and symptom burden. Pain in FD/MAS has been predominantly attributed to bone fracturing or fibrous tissue changes within the skeletal system. However, recent evidence indicates that factors beyond skeletal disease burden may contribute to pain in FD/MAS, including central nervous system alterations. Further, the type of pain experienced by patients (eg, constant, flare, radiating, and local) has yet to be fully characterized, limiting the development of individualized treatment appr

We report a rare case of a 60-year-old male patient with X-linked hypophosphatemic rickets (XLH) caused by a PHEX gene mutation complicated with polyostotic fibrous dysplasia (FD). The patient presented with bilateral lower limb deformity for 59 years and recurrent fractures for 30 years. Physical examination revealed short stature (113 cm), multiple skeletal deformities, and limited joint mobility. Laboratory investigations showed persistent hypophosphatemia (0.43 mmol/L), elevated alkaline phosphatase (262 U/L), and increased urinary phosphorus excretion. Imaging studies demonstrated both ty

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or

McCune-Albright syndrome is characterized by a triad of café au lait macules, polyostotic fibrous dysplasia (FD), and endocrine abnormalities. Bilateral and extensive presentation of FD is rarely reported. We report an interesting case of 24-year-old woman presenting with multiple episodes of slip and fall, sustaining multiple fractures with history of precocious puberty and skin complaints. On evaluation with 99m Tc-MDP bone scan, all the long bones, ribs, pelvic bones, frontal bone, and maxilla showed deformities and increased osteoblastic activity, suggesting extensive polyostotic FD. Thus

Fibrous dysplasia (FD) is a benign medullary fibro-osseous anomaly that compromises the mechanical strength of bones, especially the long bones that bear strong mechanical stresses. It can lead to an inability to remodel immature bone into mature lamellar bone, resulting in inappropriate bone alignment in response to mechanical stresses. This case study describes a rare case of polyostotic FD presenting with an epiphyseal lesion of the proximal femoral head in its weight-bearing zone, accompanied by an unconventional femoral malrotation. The present case leads us to recommend that clinicians s

Fibrous dysplasia/McCune-Albright syndrome (FD/MAS) is a rare condition caused by a mutation in the GNAS locus. Apart from endocrinopathies, some cases are characterized by excessive fibroblast growth factor 23 (FGF23) production from abnormal fibro-osseous tissue in FD lesions, resulting in increased renal phosphate excretion. We present a girl with FD/MAS and severe skeletal burden, evidenced by the presence of polyostotic fibrous dysplasia, which was complicated with bone fractures. She also had hyperthyroidism and GnRH-independent precocious puberty. She received zoledronic acid infusions

BACKGROUND: Noonan syndrome (NS) is a developmental malformation condition in the RASopathies group, characterized by variable clinical and molecular features. The syndrome is genetically heterogeneous, with the most frequent mutation found in approximately 50% of cases occurring in the PTPN11 gene. NS is reported to be associated with neurogenic, hematopoietic, melanocytic and other visceral malignancies, but osteosarcoma development has not been reported in association with NS.
CASE REPORT: Therefore, we report the first case of a male with NS who developed osteosarcoma of the mandible in a
