Gastroparesis syndrome following thoracoscopic diaphragmatic hernia repair in a pediatric patient: a case report and literature review.

We present a rare case of postoperative gastroparesis syndrome (PGS) in a 10-year-old girl following thoracoscopic repair of congenital diaphragmatic hernia (CDH). The patient presented to our institution with a 20-day history of intermittent abdominal pain and vomiting. Thoracoabdominal computed tomography revealed a posterolateral congenital diaphragmatic hernia. Emergency thoracoscopic diaphragmatic repair was performed, after which the patient developed gastroparesis syndrome. The child was given continuous nasogastric suction, prokinetic therapy with domperidone, and acupressure massage a

Evolution of minimally invasive surgery for congenital diaphragmatic hernia: a nationwide multicenter retrospective cohort analysis in Japan.

BACKGROUND: Minimally invasive surgery (MIS) for congenital diaphragmatic hernia (CDH) has previously been associated with higher recurrence rates and uncertain indications compared with open surgery. Since 2017, the Japanese Congenital Diaphragmatic Hernia Study Group (JCDHSG) has implemented a standardized national protocol, potentially improving perioperative management and surgical outcomes. METHODS: This multicenter retrospective study analyzed 1,081 infants who underwent definitive CDH surgery at 15 tertiary centers in Japan between 2007 and 2023. Patients were categorized by surgical a

Correlation of Oxygen Saturation Index with Oxygenation Index in Congenital Diaphragmatic Hernia: in A Secondary Analysis of a Randomized Clinical Trial.

OBJECTIVE: To assess accuracy of prediction of oxygenation index (OI=mean airway pressure*FiO2*100÷PaO2) with a non-invasive alternative, oxygen saturation index (OSI= FiO2*100÷SpO2) in a prospectively collected multicenter cohort of children with congenital diaphragmatic hernia (CDH) with substantial degree of hypoxemia (OI≥10 or OSI≥5). STUDY DESIGN: This secondary analysis of the Milrinone in CDH Trial studied 61 subjects with OI≥10 or OSI≥5 at randomization. Prospectively collected arterial blood gas (ABG) data with PaO2 and preductal and postductal SpO2 were used to compare all OI and pr

Bivalirudin use and Outcomes in Neonates With Congenital Diaphragmatic Hernia Managed With Extracorporeal Life Support.

OBJECTIVE: To examine national trends in bivalirudin use among neonates with congenital diaphragmatic hernia (CDH) supported with extracorporeal membrane oxygenation (ECMO) and to evaluate its association with mortality and morbidity. SUMMARY BACKGROUND DATA: Preliminary data suggest that first-line anticoagulation of the ECMO circuit with bivalirudin, a direct thrombin inhibitor, may improve outcomes in neonates with CDH. METHODS: This multicenter retrospective cohort study used data from 48 United States children's hospitals from January 1, 2016, to December 31, 2023. The analytic cohort i

The first reported coexistence of hepatopulmonary fusion and surgically palliated hypoplastic left heart syndrome.

Hepatopulmonary fusion is a rare defect describing the physical connection of liver and lung. A neonate with hypoplastic left heart syndrome arrived at the hospital with concern for congenital diaphragmatic hernia, later found to have hepatopulmonary fusion. In the coming months, he underwent a series of operations, including ligation of the hepatopulmonary fusion and the first two stages of surgical palliation of hypoplastic left heart syndrome. He ultimately died of septic shock at 15 months of age.

[Thoracoscopic treatment for congenital diaphragmatic hernia and oesophageal atresia].

Thoracoscopic surgery is an alternative to open repair of congenital diaphragmatic hernia and oesophageal atresia. Studies show faster recovery compared with open approaches. Neonatal thoracoscopy was initially associated with longer operative times and higher complication rates. However, risks have declined with greater expertise, improved techniques, and careful patient selection. Neonatal anaesthetic management remains a demanding task and requires specialised skills. Thoracoscopy is now a safe option, though open surgery remains necessary in selected cases.

Predictors of challenging initial intubation and association with outcomes in congenital diaphragmatic hernia.

OBJECTIVE: To assess for differences in congenital diaphragmatic hernia patient characteristics between those requiring single or multiple intubation attempts, and to determine the relationship between number of intubation attempts and time to intubation with outcomes. STUDY DESIGN: This was a retrospective cohort study of 205 infants with congenital diaphragmatic hernia born at Texas Children's Hospital. Patient demographics and severity numbers were compared to number of laryngoscopies as a surrogate for intubation attempts. Number of laryngoscopies and time to intubation were compared to c

Association between vasoactive-inotropic score, clinical outcomes, defect size and laterality in congenital diaphragmatic hernia.

OBJECTIVE: To assess whether vasoactive-inotropic score (VIS) during early life was predictive of outcomes for infants with congenital diaphragmatic hernia and to determine whether VIS differed by defect size and laterality. STUDY DESIGN: Retrospective single-center cohort study of 206 infants with CDH. VIS was calculated at 6, 12, 24, 48 and 72 h of life as well as at repair, 24 h and 48 h-post repair. RESULT: Higher VIS scores at all time points were associated with an increased risk of mechanical ventilation at 28 days of life (p < 0.02). VIS in the first 24 h was associated with an incre

Maternal Neighborhood Characteristics Are Associated with Left Heart Disease, Extracorporeal Membrane Oxygenation, and Mortality in Congenital Diaphragmatic Hernia.

INTRODUCTION: In congenital diaphragmatic hernia (CDH), infants with left heart disease are at the highest risk of extracorporeal membrane oxygenation (ECMO) and mortality, but the association between maternal-fetal environmental characteristics and these adverse postnatal outcomes is unknown. METHODS: Maternal-fetal dyads with CDH who were enrolled in a single-center birth defects biorepository and also in comprehensive postnatal echocardiographic studies from 2019 to 2023 were included in a retrospective analysis. Geocoded census tract was used to generate maternal neighborhood characterist

Urinary proteomics identifies unique signatures in infants with congenital diaphragmatic hernia and tetralogy of Fallot.

BACKGROUND: Serum biomarkers of right ventricular (RV) fibrosis associate with adverse outcomes in right heart conditions. Urinary protein profiling may be preferable in critically ill infants, but its feasibility is unknown. We sought to characterize the urinary proteome in conditions affecting the RV-Tetralogy of Fallot (TOF) and congenital diaphragmatic hernia (CDH)-and to explore the impact of surgery. METHODS: Urine from 53 infants (19 CDH, 17 TOF, 17 controls) was analyzed in this prospective single-center pilot study. Protein detection and quantification were performed by mass spectrom

Long-term outcomes after operative versus conservative management of congenital thoracic malformations: a propensity-matched cohort study.

PURPOSE: Management of asymptomatic congenital thoracic malformations (CTMs) remains controversial. Early resection may prevent infection or malignancy and promote compensatory lung growth, but its long-term impact is unclear. This study compared long-term cardiorespiratory and psychosocial outcomes after operative versus conservative management of CTMs and evaluated open versus thoracoscopic surgery. METHODS: Children with congenital pulmonary airway malformation, pulmonary sequestration, or congenital lobar emphysema treated at a tertiary center between 2000 and 2023 were identified retrosp
