Establishing gastroschisis care: Impact of pediatric surgical services and a dedicated neonatal surgical unit on survival of gastroschisis in Rwanda.

INTRODUCTION: Gastroschisis has historically been fatal in resource-limited settings, where lack of prenatal diagnosis, surgical neonatal care, and parenteral nutrition challenge replication of western care paradigms. We present a Rwandan tertiary center's experience managing gastroschisis and outcomes before and after opening a neonatal surgical unit. METHODS: Patients with gastroschisis admitted to the main referral hospital in Rwanda were included. A neonatal surgery unit opened 3/2020. Data was collected from 1/2016-8/2022. Descriptive, bivariate, and multivariate statistics were performe

Closing/Closed Gastroschisis (CGS): Antenatal Predictors and Surgical Strategies in Cases of Unique Anatomy from a Case Series.

Background: Closing/closed gastroschisis (CGS) accounts for approximately 6% of gastroschisis cases globally. Currently, no consensus exists regarding: antenatal predictors of CGS types, optimal antenatal management (ultrasound screening frequency, indications for early delivery), or standardized surgical strategies tailored to CGS type (staging/timing of procedures, enterostomy necessity/level). Methods: Five neonates with CGS were enrolled and classified according to Perrone's classification: two patients with type B (40%), one with type C (20%), one with type D (20%), one patient was classi

Origin of pan-drug-resistant Acinetobacter baumannii infection in a neonate with gastroschisis: nosocomial or agricultural source? A case report.

BACKGROUND: Pan-drug resistance (PDR) in neonatal surgical cases remains rare and poorly understood. The intersection of congenital anomalies with PDR infections poses unique clinical challenges. CASE PRESENTATION: A Javanese male neonate was delivered vaginally at 36 weeks' gestation from a 23-year-old mother. The neonate was diagnosed with gastroschisis, along with a 2.5-cm gastric perforation and an ileal perforation located 7 cm proximal to the ileocecal junction. Emergency surgery was undertaken, consisting of primary gastric perforation repair and a double-barrel ileostomy. The patient

A socioecological model of skilled birth attendant perspectives on gastroschisis in southwest Uganda.

BACKGROUND: Gastroschisis, a congenital abdominal wall defect, is increasingly reported in sub-Saharan Africa, yet survival remains poor in low-income and middle-income countries, such as Uganda. Barriers to care extend beyond clinical limitations, with societal stigma and cultural beliefs influencing health-seeking behavior. This study explores the sociocultural perspectives surrounding gastroschisis in southwest Uganda using a socioecological model. METHODS: A qualitative study was conducted with midwives and nurses from public and private facilities across 9 districts in Uganda. Participan

Gut microbial differences and function in infants with gastroschisis: a pilot prospective cohort study.

Newborns with gastroschisis hospitalised in the neonatal intensive care unit (NICU) are at risk for a disrupted gut microbiome. Infants with gastroschisis are particularly vulnerable to a dysbiotic microbiome; they require prolonged parenteral nutrition (PN) due to intestinal dysmotility, which often leads to growth faltering (GF). This pilot study's goals were to (1) compare the gut microbiome in infants with gastroschisis to infants admitted to the NICU without congenital anomalies, (2) identify differences in the gut microbiome between infants with gastroschisis requiring prolonged PN and t

Feasibility of neonatal intravenous nutrition for the management of gastroschisis in sub-Saharan Africa.

INTRODUCTION: Parenteral nutrition (PN) is a life-saving intervention for neonates with gastroschisis. As most low and middle-income countries lack access to comprehensive neonatal PN, we evaluated the feasibility and safety of a pragmatic intravenous nutrition strategy for neonates with gastroschisis in sub-Saharan Africa. METHODS: Local protocols for partial parenteral nutritional support and early enteral feeding were developed across the seven pediatric surgery centers participating in the Gastroschisis Interventional Study (n=192). Protocols aimed to provide up to 100 kcal/kg/day using 1

Intra-Abdominal Bowel Dilation in Experimental Gastroschisis is Associated with a Modifiable Transcriptomic Program of Intestinal Dysfunction.

OBJECTIVE: To characterize intestinal transcriptional profiles in gastroschisis, their temporal evolution, and response to fetal intervention. SUMMARY BACKGROUND DATA: Gastroschisis causes significant intestinal dysfunction, with intra-abdominal bowel dilation clinically shown to correlate with worse outcomes. While inflammation and neurovascular impairment have been implicated, genome-wide transcriptional characterization of disease severity remains lacking. METHODS: Using a fetal ovine model of complex gastroschisis, in which all gastroschisis animals demonstrated significant intra-abdomin

GAIN: a multicenter pilot protocol for early initiation of enteral feeds in neonates with simple gastroschisis.

BACKGROUND: Gastroschisis is a congenital abdominal wall defect characterized by severe gut dysmotility. These neonates often face lengthy hospital stays, prolonged parenteral nutrition dependence, and complications such as growth faltering, sepsis, and intestinal failure. Feeding practices for neonates with gastroschisis vary and emphasize extended bowel rest and delayed feeding initiation. Usual practices in determining timing to start feeds include awaiting return of bowel function which typically consists of low Replogle output volume, color changing from bilious to clear and/or passing st

Prenatal therapies to improve outcomes in gastroschisis: a systematic scoping review protocol.

INTRODUCTION: Gastroschisis is a congenital birth defect with a rising incidence. The herniation of intestines into the amniotic cavity leads to prenatal bowel injury, which contributes to significant postnatal morbidity and mortality. Current management focuses on postnatal interventions, though bowel injury begins in utero. Emerging interest in prenatal therapies aims to mitigate this injury and improve outcomes. However, none of these interventions has been widely adopted in clinical practice, highlighting the need for further research and validation. This protocol aims to outline the proce
