Access to praziquantel in Italy: persistent gaps and ongoing responses amid a Europe-wide shortage.

In Italy, praziquantel (PZQ) is not registered for human use and can be imported from European countries where it is authorized, according to a specific ministerial decree. Recent Europe-wide shortages, however, have made this route impractical, resulting in the unavailability of this essential medicine. This editorial summarizes historical gaps in supply, the actions undertaken, and the interim solutions implemented to restore PZQ access in Italy. The Italian experience highlights broader challenges related to essential medicines and underscores the need for coordinated national and European

Cecal schistosomiasis with perforation: case report and literature review.

Schistosomal cecum involvement is an extremely rare condition in endemic areas like Sudan. We report a 17-year-old male presenting with acute abdomen diagnosed preoperatively as perforated appendicitis. Emergency surgery revealed purulent peritonitis, a cecal mass, and a large cecal perforation and right hemicolectomy was done. Histopathology revealed viable schistosomal ova and granulomas in the cecum. Post-operative management included anthelmintic therapy. This case presented the third case of large bowel schistosomiasis in Sudan, emphasizing the need for a high index of suspicion for paras

Acute Schistosomiasis in a Traveler Returning From St. Croix, United States Virgin Islands: A Case Report.

In this case report, we describe a patient who presented to their primary care provider with abdominal pain after returning from travel to the Caribbean island of St. Croix. The patient reported two weeks of epigastric abdominal pain, dyspepsia, nausea, and night sweats. Following an initial evaluation that revealed eosinophilia with unclear etiology, they were referred to an infectious disease clinic. Through a targeted infectious evaluation and workup, the patient was diagnosed with acute schistosomiasis syndrome and was treated with praziquantel. Treatment resulted in the complete resolutio

Comparative mitogenomics of freshwater snails of the genus Biomphalaria (Gastropoda: Planorbidae) and new insights into mitochondrial genome evolution in Hygrophila.

BACKGROUND: Freshwater snails of the genus Biomphalaria include several species of major public health importance as intermediate hosts of schistosomiasis and represent a key group for evolutionary and taxonomic research within Hygrophila. However, complete mitochondrial genomes are available for only a limited number of Biomphalaria species, restricting comparative assessments of mitogenome evolution and the ability of mitogenomic data to resolve contentious relationships within Planorbidae and across Hygrophila. Here, we generate and analyse six complete mitochondrial genomes from four Neotr

Schistosomiasis-Associated Pulmonary Arterial Hypertension in Brazil: A Call to Action for Enhanced Diagnostic Recognition-2025 Pulmonary Vascular Research Institute Infection in Pulmonary Vascular Disease Consortium Perspectives.

Schistosomiasis-associated pulmonary arterial hypertension (Sch-PAH) is one of the most frequent etiologies of pulmonary arterial hypertension in Brazil and globally. The disease is a complication of hepatosplenic schistosomiasis characterized by portal hypertension, egg deposition, and granuloma formation in the pulmonary arteries leading to inflammation, vascular remodeling, increased pulmonary vascular resistance, and progressive right heart failure. During the Infection in Pulmonary Vascular Disease (iPVD) consortium meeting during the Pulmonary Vascular Research Institute (PVRI) annual me

Schistosomiasis japonicum in Indonesia: Progress and Surveillance Needs in Verge-of-Elimination Settings.

Schistosomiasis japonicum transmission in Indonesia has declined substantially over recent decades, placing it in the last miles of elimination in the Western Pacific Region. As programmes transition from control to interruption of transmission, surveillance systems must be capable of detecting residual transmission. This study synthesised routine epidemiological data from 2015 to 2025 to assess Indonesia's readiness for elimination and to identify key surveillance gaps in near-elimination settings. Descriptive quantitative analysis was conducted using national surveillance data from two endem

Schistosomiasis in Western Lake Turkana, Kenya: An Exploratory Serosurvey and Validation of Dried Blood Spots for Field Studies.

BACKGROUND: Schistosomiasis remains a significant neglected tropical disease in Kenya, but its presence in the western/northern Lake Turkana region is poorly characterised. We conducted an exploratory serosurvey to assess evidence of Schistosoma spp. exposure and a diagnostic accuracy study to evaluate dried blood spots (DBSs) for field serology. METHODS: We performed a cross-sectional survey in adults (≥18 years) from six communities in the western/northern and shoreline area of Turkana Lake, excluding individuals with >6 months of residence in other Kenyan endemic areas. Capillary blood was

Progressive IL-13 signaling promotes inflammation-associated liver fibrosis via IL-13RA2/MEK/ERK activation in Schistosoma japonicum infection.

Progressive IL-13 signaling is closely associated with liver fibrosis. Among fibrotic diseases, liver fibrosis induced by Schistosoma japonicum in advanced schistosomiasis is the primary driver of portal hypertension, which is the leading cause of mortality in affected patients. RNA sequencing analysis of human hepatic stellate cells revealed that IL-13RA2 was markedly upregulated in activated hepatic stellate cells and enriched in the cytokine-receptor interaction pathways, suggesting a potential role for IL-13RA2 in hepatic stellate cell activation and fibrosis progression. Based on these fi

Selexipag for the treatment of schistosomiasis-associated pulmonary arterial hypertension: the SELSCH study.

BACKGROUND: Schistosomiasis-associated pulmonary arterial hypertension (Sch-PAH) is one of the most prevalent forms of PAH in Brazil. However, no prospective data are available for evaluating the efficacy of PAH drugs in the treatment of Sch-PAH. Does selexipag improve pulmonary vascular resistance (PVR) in Sch-PAH patients? METHODS: The SELSCH study was a single-centre, single-arm, open-label prospective study with the aim of evaluating selexipag for the treatment of Sch-PAH. The primary end-point was a reduction in PVR at week 16. RESULTS: 20 Sch-PAH patients completed the SELSCH study. 20

Pharos: a mobile GIS application for network assessment and ground-truth mapping to support mHealth study design in low-resource settings.

Mobile health (mHealth) is a promising tool for improving healthcare access, particularly in low-resource settings. However, limited mobile accessibility and poor connectivity remain significant barriers to implementing mHealth interventions in these regions. To address these challenges and support the development and scalability of mHealth studies, we developed Pharos, a mobile GIS application designed to assess network coverage and facilitate ground-truth mapping. Pharos autonomously measures spatiotemporal variations in mobile network signal strength and enables precise mapping of critical

Predation preference of the crayfish Procambarus clarkii on schistosomiasis transmitting snails: influence of snail species, infection status, and size.

BACKGROUND: Schistosomiasis control relies largely on preventive chemotherapy; however, rapid reinfection persists in many endemic settings due to continued transmission by freshwater snail intermediate hosts. Biological control using natural predators has been proposed as a complementary strategy to reduce vector snail populations. The invasive North American crayfish Procambarus clarkii, which preys on schistosome-transmitting snails, represents a promising candidate. However, the ecological and biological factors influencing predator-prey interactions between crayfish and intermediate snail

Non-coding RNAs in schistosome reproductive maturation.

In schistosomes, pathology and transmission are primarily driven by sustained egg deposition rather than by the mere presence of adult worms, and female reproductive competence is therefore regarded as a central issue in schistosome biology. Because the establishment and maintenance of female reproductive maturity require prolonged pairing with a male, this process is better understood as the establishment and maintenance of a pairing-dependent developmental state than as a simple morphological consequence of mating. Although this question has traditionally been addressed through protein-codin

Evaluation of the diagnostic capacity for neglected tropical diseases in a refugee settlement in Uganda: a cross-sectional study.

BACKGROUND: Neglected tropical diseases (NTDs) pose a substantial global public health challenge, particularly in vulnerable refugee populations. In Uganda, approximately 1.8 million refugees residing in settlements nationwide, including over 145,000 in Nakivale alone, are at risk, yet primary healthcare (PHC) facility capacity within refugee settlements to effectively manage these diseases remains underexplored. This study assessed infrastructure, diagnostic capabilities, and resource availability for NTD management in health facilities across Nakivale Refugee Settlement, Uganda. METHODS: A
