Craniocervical instability after inadvertent neck hyperextension in Ehlers-Danlos syndrome: a retrospective case series and literature review.

BACKGROUND: Patients with hypermobile Ehlers-Danlos Syndrome (hEDS) are at risk of developing craniocervical instability (CCI). Iatrogenic causes of CCI in hEDS are underreported. We describe a case series of hEDS patients who developed CCI from routine neck hyperextension during common procedures (e.g., intubation for anesthesia, dental work). METHODS: We conducted a retrospective case series of 8 adults (≥ 18 years old) with hEDS (2017 diagnostic criteria) who had no preoperative CCI symptoms but developed new CCI symptoms after a surgical procedure that persisted for at least 5 years. Cases

Implications of Dermatologic Disorders in Facial Cosmetic Surgery: A Systematic Review.

BACKGROUND: Invasive and minimally invasive cosmetic procedures in patients with autoimmune or inflammatory skin disorders can carry an increased risk of surgical site infection, disease exacerbation, atrophic/hypertrophic scarring, or suboptimal cosmetic results. As facial procedures continue to rise in popularity, increasing complications are being reported in association with underlying dermatologic disease. In this review, we aim to bring attention to these dermatologic conditions and the potential challenges that they pose to the facial plastic surgeon. METHODS: A comprehensive literatur

When Phenotype Outspeaks Genotype: Uncommon Vascular Anomalies in Suspected Hereditary Hemorrhagic Telangiectasia Despite Negative Genetic Testing.

Hereditary hemorrhagic telangiectasia (HHT) is an inherited vascular disorder characterized by abnormal blood vessel formation involving the skin and visceral organs. Establishing the diagnosis can be challenging, particularly in the absence of classic clinical manifestations or confirmatory genetic findings. We report the case of a 42-year-old woman who presented with acute epigastric pain and was found to have extensive vascular abnormalities involving the liver, lungs, aorta, and mesenteric circulation. She had no history of recurrent epistaxis, mucocutaneous telangiectasias, or family hist

The co-existence of Ehlers-Danlos syndrome and postural orthostatic tachycardia syndrome: A systematic review of the literature.

There is a growing body of literature evaluating both postural orthostatic tachycardia syndrome (POTS) and Ehlers-Danlos syndrome (EDS). We conducted a systematic review to evaluate what is currently known about the co-existence of both conditions. A search of MEDLINE and EMBASE was performed in December 2025 and data were collected in tables and pooled to determine the prevalence of POTS in EDS and EDS in POTS. A total of 30 studies were included with 8421 patients with EDS and 12,983 patients with POTS. The average age across 16 studies that reported mean age was 33.3 years. POTS in patients

Obstructive sleep apnea and CPAP efficacy in patients with Hypermobile Ehlers-Danlos syndrome and hypermobility spectrum disorder: a case-control study.

PURPOSE: Hypermobile Ehlers-Danlos syndrome (hEDS) and Hypermobility Spectrum Disorder (HSD) are hereditary tissue disorders existing on a spectrum associated with insomnia and obstructive sleep apnea (OSA). Oropharyngeal abnormalities in hEDS/HSD may exacerbate OSA; this study investigates if continuous positive airway pressure (CPAP) adequately decreases daytime sleepiness in hEDS/HSD patients with OSA. METHODS: A single institution retrospective review was conducted from 2020 to 2024. Adults with hEDS/HSD and OSA (cases) were matched based on sex, age, race, and body mass index (BMI) to ad

Shared autonomic phenotype of long COVID and myalgic encephalomyelitis/chronic fatigue syndrome.

INTRODUCTION: Long COVID and myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) are relatively common and disabling multisystem disorders that share overlapping features, including post-infectious onset and similar clinical manifestations such as brain fog, fatigue, muscle pain, and dysautonomia with orthostatic intolerance. These similarities suggest that Long COVID and ME/CFS may share common pathophysiological mechanisms, though the underlying mechanisms remain poorly understood, partly due to the difficulty in quantifying many of the symptoms. MATERIALS AND METHODS: This retrospe

Sex differences in aortopathy, arteriopathy, and mortality in vascular Ehlers-Danlos syndrome.

OBJECTIVE: Vascular Ehlers-Danlos syndrome (VEDS) is a rare autosomal dominant disorder due to pathogenic alterations in type III collagen structure and production, which results in increased risk of aortopathy and arteriopathy. Given the documented sex-related differences in aortic and other arterial aneurysms and dissections in non-VEDS populations, the potential influence of sex on the incidence of aortopathy, arteriopathy, and mortality in individuals with VEDS was examined. METHODS: A cross-sectional analysis on a cohort of 557 individuals with VEDS with COL3A1 pathogenic variants was pe

A rare case report of vascular Ehlers-Danlos syndrome as a pulsatile neck mass with an internal carotid artery aneurysm.

Vascular Ehlers-Danlos syndrome, also known as Ehlers-Danlos syndrome type IV, is a rare inherited connective tissue disorder caused by autosomal-dominant pathogenic variants in the COL3A1 gene. Defective type III collagen results in marked fragility of arteries and hollow organs, predisposing affected individuals to spontaneous aneurysms, dissections, and rupture. We report the case of a 35-year-old Bangladeshi woman who presented with a gradually enlarging right-sided pulsatile neck mass accompanied by intermittent headache. Physical examination revealed a soft pulsatile cervical swelling wi

Tissue vulnerability revealed by ultrastructural skin analysis as a surrogate for arterial integrity in two cases of vascular Ehlers-Danlos syndrome.

Clinical presentation and operative outcomes in vascular Ehlers-Danlos syndrome (VEDS) are heterogeneous. We present two patients with genetically confirmed VEDS who exhibited markedly different iliac pathology phenotypes. One ruptured a common iliac artery at a near-normal diameter and the other tolerated a large aortoiliac aneurysm without rupture. The Aortic and Arterial Vulnerability Spectrum is a conceptual framework that characterizes aortic and arterial failure (dissection/rupture) along a continuum of underlying tissue vulnerability, integrating extracellular matrix (ECM) ultrastructur

Gastrointestinal involvement in Ehlers-Danlos syndrome classical-like type 2 associated with a novel AEBP1 splice-site variant.

Ehlers-Danlos syndrome classical-like type 2 (clEDS2) is a rare autosomal recessive connective tissue disorder caused by biallelic loss-of-function variants in the gene encoding adipocyte enhancer-binding protein 1 (AEBP1). While cutaneous and skeletal manifestations are commonly observed, gastrointestinal complications, including bowel rupture, have been reported only rarely, and their histopathological basis remains poorly characterized. Here, we report findings of molecular investigations, gastrointestinal histopathological evaluation, and long-term clinical follow-up in the 16th reported p

Unmasking Ehlers-Danlos Syndrome Through Reduction Mammaplasty: An Illustrative Case and a Systematic Literature Review.

Ehlers-Danlos syndrome (EDS) is characterized by joint hypermobility, skin hyperextensibility, and tissue fragility, which may worsen symptomatic macromastia. Concerns about elevated surgical risk have historically discouraged elective procedures in EDS patients; however, postoperative complications may themselves prompt an EDS diagnosis, as illustrated by our case. This relationship prompted the conduct of a systematic review examining outcomes of reduction mammaplasty in EDS. A 37-year-old woman underwent bilateral inferior pedicle reduction mammaplasty for symptomatic mammary hyperplasia. P

Pulmonary hemorrhage as an early clue: an integrated clinical-imaging-genetic diagnostic insight for vascular Ehlers-Danlos syndrome.

BACKGROUND AND AIMS: Vascular Ehlers-Danlos syndrome (vEDS) is a rare genetic disorder characterized by connective tissue fragility; however, respiratory manifestations such as pulmonary hemorrhage and spontaneous pneumothorax are frequently overlooked during early diagnosis. This study aimed to comprehensively characterize pulmonary features, particularly pulmonary hemorrhage, in vEDS patients and develop an integrated clinical-imaging-genetic diagnostic strategy to facilitate timely recognition and reduce diagnostic delays. RESULTS: A single-center retrospective cohort study was conducted at
