Obstetrical, perinatal, and children's health outcomes following fresh embryo transfer after extended embryo culture.

STUDY QUESTION: Does extended embryo culture (EEC) associate with an increased risk of obstetrical, perinatal, or children's health complications? SUMMARY ANSWER: After thorough adjustment, EEC was not associated with widespread increased risks, although a moderate excess risk persisted for a few specific outcomes, notably cardiac anomalies, whereas reduced risks were observed for gestational diabetes, small birthweight, and musculoskeletal-limb anomalies. WHAT IS KNOWN ALREADY: EEC is increasingly used in IVF cycles. While blastocyst transfer (day-5/6) often improves birth rates, concerns r

Validation of Claims-Based Algorithms for Identifying Congenital Urinary Tract, Genital, Gastrointestinal, and Musculoskeletal Malformations.

AIM: Congenital malformations are important outcomes when evaluating medication safety in pregnancy. However, the accuracy of claims-based algorithms for identifying organ-specific malformations remains understudied. We validated algorithms for four malformation groups: urinary tract, genital, gastrointestinal, and musculoskeletal. MATERIALS AND METHODS: Using the Mass General Brigham (MGB, 2007-2020) and Stanford Medicine (2016-2023) databases, we identified infants with potential malformations of interest based on diagnosis and procedure codes within 90 days of birth. A total of 150 cases w

Bone Phenotype in Autosomal Dominant Polycystic Kidney Disease.

Autosomal dominant polycystic kidney disease (ADPKD) is a systemic ciliary disorder, yet the impact on bone health remains insufficiently defined. Given the expression of polycystins in osteocytes, osteoclasts and extracellular matrix, a disease-specific bone phenotype has been hypothesized. This study aimed to comprehensively characterize the bone phenotype in ADPKD across stages of chronic kidney disease (CKD) and Mayo Imaging Classification (MIC). In this cross-sectional study, 81 patients with ADPKD, 15 with CKD of other etiologies, and 21 healthy individuals were included. Bone phenotype

Isolated frontosphenoidal suture synostoses: report of two cases and literature review.

INTRODUCTION: Anterior plagiocephaly is most commonly attributed to unilateral coronal synostosis. However, isolated premature fusion of the frontosphenoidal suture (FSC) represents a rare and often misdiagnosed cause due to its overlapping clinical presentation. Accurate diagnosis is essential, as treatment strategies and prognoses differ significantly. OBJECTIVES: To present a comprehensive literature review of FSC and to report two novel clinical cases-one managed with open fronto-orbital advancement and the other with endoscopic suturectomy. METHODS: A systematic review of reported FSC c

Association between craniosynostosis and phospholipid metabolism: Insights from single-cell and transcriptomic analysis.

Craniosynostosis is a common defect in the craniofacial structure of children, yet its pathogenesis remains unclear. Current research focuses mainly on bone-related genetic mutations, with less emphasis on lipid metabolism disorders. Hence, it is imperative to investigate the underlying mechanisms from various perspectives. We investigated the association between craniosynostosis and phospholipid metabolism through a multi-omics approach. This included Mendelian randomization (MR) to assess causal relationships of lipid traits, transcriptomic differential analysis of clinical samples, and sing

It's not about the dent: Anteroposterior insufficiency in pectus excavatum.

BACKGROUND: Pectus Excavatum (PEx) is an anterior chest wall deformity associated with sternal depression. Minimal invasive repair of PEx (MIRPE) and open pectus repair depend on anterior ribs to elevate the sternal defect. Many PEx patients, however, have inadequate anteroposterior (AP) distance to support a complete repair. The quantity and frequency of AP insufficiency (API) in a PEx surgical population is reported. METHODS: A retrospective analysis of 100 pectus excavatum (PEx) patients with a CT or MRI Haller Index >3.2 was performed. Demographics, Haller Index (HI), Correction Index (CI

Multi-Level Genomic and Computational Analyses Identify a Novel IFT122 Variant Associated With Cranioectodermal Dysplasia 1 in a Consanguineous Saudi Family.

BACKGROUND: Cranioectodermal dysplasia (CED) is a rare autosomal recessive ciliopathy characterized by craniofacial, skeletal, and ectodermal anomalies. Significant phenotypic heterogeneity often results in clinical overlap with other skeletal dysplasias, including Robinow syndrome. In consanguineous populations, the presence of multiple rare variants can further complicate the molecular diagnosis. This study aimed to clarify the genetic basis of a complex syndromic presentation in a consanguineous Saudi family exhibiting features suggestive of both disorders using an integrated phenotypic, ge

Pseudo-osteoporosis on bone densitometry due to congenital spinal dysraphism.

A patient in his late adolescence was referred for evaluation of low and apparently declining spinal bone density on dual-energy X-ray absorptiometry (DXA). Review of the DXA images revealed normal bone mineral density at other body sites but exceedingly low lumbar spine Z-scores which appeared to get progressively lower over time. The DXA result was explained by congenital spinal dysraphism with absent posterior spinous elements, rather than true low bone mineral content. Recognition of this structural artefact prevented unnecessary interventions. The case underscores the importance of correl

Shelf acetabuloplasty restores native rotational motion in a cadaveric acetabular dysplasia model.

INTRODUCTION/OBJECTIVES: Developmental dysplasia of the hip is associated with inferior clinical outcomes when treating concomitant intra-articular pathologies. Shelf acetabuloplasty has been proposed as a less invasive alternative to periacetabular osteotomy for developmental dysplasia of the hip; however, there is a lack of biomechanical studies regarding its efficacy. We hypothesized that a cadaveric dysplasia model would cause increased range of motion and lateral femoral head translation compared to native, and that shelf acetabuloplasty would restore native biomechanics. METHODS: Ten ca

Bilateral cervical ribs forming pseudoarthrosis with the first ribs co-occurring with an aberrant right subclavian artery.

While anatomical variations in both aortic arch branching and rib cage development are considered relatively common, compared to various other anomalies, their coexistence seems to be underreported. We present a case report of a 70-year-old Polish woman with bilateral complete cervical ribs forming a pseudoarthrosis with the superior surface of the first ribs, and an aberrant right subclavian artery (arteria lusoria) with a retroesophageal course. We believe that the frequency of their co-occurrence may be higher than would be expected based on statistical probability derived from their preval

Morphological features and long-term outcomes after periacetabular osteotomy for borderline hip dysplasia.

PURPOSE: The role of periacetabular osteotomy (PAO) in symptomatic borderline hip dysplasia (borderline HD) remains controversial because the lateral centre-edge angle may not reflect global dysplastic morphology. We examined whether selected borderline dysplastic hips showed additional dysplastic features and compared mid- to long-term outcomes with frank hip dysplasia (frank HD). METHODS: This retrospective comparative cohort study included 33 patients (35 hips) with borderline HD (18° ≤ lateral centre-edge angle < 25°) and 319 patients (357 hips) with frank HD (lateral centre-edge angle <

Embryonic exposure to bisphenol A glycidyl methacrylate induces craniofacial cartilage malformation via NLR signaling pathway in zebrafish.

Bisphenol A (BPA) can combine with glycidyl methacrylate to form bisphenol A glycidyl methacrylate (Bis-GMA). Bis-GMA is an important industrial raw material and a significant component of dental resin materials. Although Bis-GMA is widely utilized, its effects on neural crest cells and the development of embryonic maxillofacial cartilage remain insufficiently reported. This study evaluated the effect of Bis-GMA on cranio-maxillofacial cartilage using zebrafish as a model organism. Six to 120 h after fertilization, zebrafish embryos exposed to 0.8, 1.6 or 2.4 mg/L Bis-GMA presented with cranio

Deleterious NKAP Mutations Are Associated with Musculoskeletal Abnormalities in Hemizygous Males and Skewed X Chromosome Inactivation in Heterozygous Females.

NKAP (NF-kappa-B-activating protein) is a ubiquitously expressed nuclear protein involved in multiple biological processes. Males with missense NKAP mutations have been reported to present with marfanoid features and behavioral and musculoskeletal abnormalities. We have previously reported that a disruptive NKAP mutation resulted in extremely skewed X chromosome inactivation (XCI), leading to phenotypic manifestation of hemophilia A (HA) in a HA carrier. In this study, with the aim of exploring the phenotypic manifestations of deleterious NKAP mutations in males, as well as their involvement i

Two Unrelated Families With Noncoding Duplications Upstream of MSX2 Refine the Critical Regulatory Region Likely Involved in Cranial Bone Development and a Cleidocranial Dysplasia-Like Phenotype.

Cleidocranial dysplasia (CCD) is a genetic disorder characterized by delayed cranial suture closure, hypoplastic clavicles, and dental anomalies, with varying severity. Most cases are linked to RUNX2 variants; however, rare CCD-like phenotypes can arise from other genetic alterations, including variants in MSX2, a critical skeletal development gene. Here, we describe two patients with persistent anterior fontanelles and microduplications upstream of MSX2, a region previously associated with CCD-like phenotypes. Microarray analysis revealed narrower duplication ranges in these patients than in

Systematic review of patella cubiti: Does it really exist?

BACKGROUND: Accessory bones located dorsally to the elbow joint are exceedingly rare and remain poorly defined. These include the patella cubiti, os supratrochleare posterius, and os sesamoideum tricipitale, which are frequently misdiagnosed as traumatic or pathological entities such as congenital pseudarthrosis, persistent epiphysis, and non-unions. The present study aims to systematically analyse documented cases of patella cubiti, establish diagnostic criteria, and differentiate these conditions from other pathologies. METHODS: A systematic review of the literature was conducted, examining

ZNF827 pleiotropic cardiovascular risk locus involves regulation by nuclear factor-1.

Spontaneous coronary artery dissection (SCAD) is a form of myocardial infarction that predominantly affects middle-aged women, caused by the spontaneous onset of an intramural hematoma leading to heart ischemia. SCAD genetic risk loci, such as the ZNF827 locus on chromosome 4, were previously associated with the risk for coronary artery disease, systolic blood pressure, and ascending aortic diameter variability, but the molecular processes driving these genetic associations are unknown. In the present study, we demonstrated that these genetic associations were colocalized and could all be expl

Evaluation of femoro-epiphyseal acetabular roof index for assessing hip instability in hip disorders in female patients.

INTRODUCTION: The Femoro-Epiphyseal Acetabular Roof (FEAR) index is a proposed radiographic marker of hip instability, particularly in borderline developmental dysplasia of the hip (BDDH). Its diagnostic value in other hip morphologies and its association with patient-reported outcomes (PROMs) remain unclear. This study evaluated the FEAR index for assessing hip instability across different acetabular morphologies by analyzing its associations with radiographic measures obtained from standard anteroposterior pelvic and false-profile radiographs, dynamic ultrasonography, and PROMs. MATERIALS AN
